모유수유 실천에 미치는 산모의 사회심리적 요인 (Maternal Psychosocial Factors Affecting Breast Feeding Practice)

  • Purpose: This study aimed to elucidate the prevalence of breast feeding and maternal psychosocial predictors affecting the breast feeding practice at six months postpartum. Method: One hundred and sixty one mothers at six months postpartum were conveniently recruited in two public health center in Jeju-city. Subjects completed a well-structured questionnaire regarding the feeding types of the child, maternal self-esteem, the Edinburgh Postnatal Depression Scale (EPDS), husband support, and marital adjustment scale. The data was analyzed using the $X^2-test$, t-test, Pearson correlation coefficients, and logistic regression. Result: The prevalence of breast feeding at six months postpartum was 29.9%. Predictors of breast feeding practice identified by the logistic regression analysis include first feeding type after delivery, marital adjustment, readiness for maternal role and work state. Conclusion: The prevalence of breast feeding practice was lower than WHO desired at six months postpartum. Therefore, the findings support the need for breast feeding education including interventions focused on improving readiness for maternal role and marital adjustment.

까다롭게 먹는 아기의 기질, 어머니의 식행동과 식사 지도 방법의 특성 (The Characteristics of Infants' Temperament, Maternal Feeding Behavior and Feeding Practices in Picky Eaters)

  • The purpose of this study was to determine the characteristics of infants' temperaments, maternal feeding behaviors and feeding practices in picky eaters. Participants were 83 infants (aged 12 - 24 months) from 'A' hospital (Seoul) and 'B' public health center (Kyunggido). Mothers completed questionnaires that assessed their own feeding behavior, feeding practices, infants' temperament and infants' feeding behavior. Picky eaters' demographics were not significantly different from non-picky eaters after adjusting sex and age. The average of thiamin, niacin and vitamin E intakes of picky eaters were below $75\%$ Korean RDA, whereas vitamin A intakes exceed $120\%$ RDA in both groups. Activity level of infants' temperament and disinhibition of maternal feeding behavior in picky eaters were significantly higher than those in non-picky eater. All constructs of infants feeding behavior were significantly associated with certain constructs of infants' temperament, maternal feeding practice and maternal feeding behavior. The pickiness of infants feeding behavior was positively correlated with activity level of infants' temperament, pickiness and disinhibition of maternal feeding behavior and negatively correlated with adaptability of infants' temperament. Findings suggest that maternal feeding behavior and feeding practices as well as infants' temperament should be addressed in nutrition education for picky eaters.

어머니 역할 실습교육이 어머니됨에 미치는 효과 (Effects of Maternal Role Practice Education on Becoming a Mother)

  • Purpose: The purpose of this study was to investigate the effects of education and practice of the maternal role and becoming a mother using the concepts of self-confidence in caring, maternal attachment, and maternal identity. Methods: A non-equivalent control group non-synchronized design was used. Data was collected with the use of self-administered questionnaires from November 2007 to April, 2008. A total of 54 women at a Sanhujoriwon (postpartum care center) participated in the study. The experimental group consisted of 26 women (Nov-Dec), whereas the control group had 28 women (Feb-Apr). Education and practice on the maternal role was given individually and consisted of explanations, demonstrations and practice on breast feeding (day1 to 3), maternal attachment (day 8 to 9), bathing (day 10 to 11) and infant healthcare (day 13 to 14). The experimental group received education and practice on the maternal role while the control group received routine education only. Results: Data collection was done using pre and post questionnaires at admission and fourteen days later. The experimental group showed significant increases in self-confidence in caring (t=-3.31, p=.002) and maternal identity (t=-2.16, p=.036) compared to the control group. Conclusion: Education and practice on the maternal role and being a mother was an effective intervention in increasing self-confidence in caring and maternal identity.

  • Purpose: This study was conducted to assess the breast feeding rate and maternal adaptation of mothers with infants and children in early childhood in a community. Methods: This descriptive study was conducted from November to December 2015, and included a total of 283 mothers of infants and children in early childhood. The data were analyzed using descriptive analysis, the ${\chi}^2-test$, and one way ANOVA. Results: The rates of breast feeding after birth were 76.3% (1 month postpartum), 69.3% (3 months postpartum), 53.4% (6 months postpartum), 32.2% (9 months postpartum), and 22.6% (12 months postpartum). The level of maternal adaptation of subjects was $3.78{\pm}0.54$. Subjects who were breastfed until 9 months postpartum had a higher level of maternal adaptation than those who stopped breast feeding at 1 month postpartum (F=3.926, p<.002). The breast feeding rate of subjects who were educated about breast feeding after childbirth was significantly higher than that of those who did not receive breast feeding education after delivery. Conclusion: To increase the breast feeding rate and maternal adaptation, community health nurses should develop and provide breast feeding programs to mothers soon after childbirth.

  • The survey was performed on 201 cases of mothers who visited pediatric hospitals and public health clinics in Pusan and Kyeong-nam provinces from Dec. 10 to Dec. 30 of 1993. If this study, each infant weighed over 2500gm and between 2 months and 12 months old. The research was statistically analyzed according to the percentage, Pearson correlation, $x^2$-test. The result of this study were as follows ; 1. Feeding method related to the job of mother (p<0.01). 2. Feeding method related to the way of bringing up child(p<0.05). 3. Whether the mother and child shared the same room effected the feeding method (p<0.01). 4. The shape of nipple before breast feeding related to feeding method. 5. The reason for stopping breast feeding related to feeding method(p<0.01). 6. First breast feeding related to period of seperation after delivery(p<0.05). 7. Plan of breast feeding related to birth weight (p<0.05).

워크북 프로그램이 미숙아어머니의 스트레스, 모성역할 자신감, 모유수유 실천에 미치는 효과 (Effects of a Workbook Program on the Perceived Stress Level, Maternal Role Confidence and Breast Feeding Practice of Mothers of Premature Infants)

  • Purpose: The purpose of this study was to identify the effects of a workbook program on percieved stress levels, maternal role confidence and breastfeeding practices of mothers of premature infants. Method: This study employed a quasi-experimental non-equivalent pre-and-post test to compare the two groups. Data was collected from 32 subjects from March 10. to September 5, 2003 at an NICU located in S. city. In the sample, fifteen mothers were in the experimental group and seventeen were in the control group. A workbook program was provided twice during a period from two-three days after the baby's admission to one week after the first intervention. The instruments used were the PSS and self confidence scale. Data was analyzed by means of frequency, SD, $x^2-test$, Fisher's exact test, Mann-Whitney test, and the Wilcoxon signed rank test. Result: There was a significant difference in perceived stress levels between the experimental and control group (U=2.366, p=.018). There was not a significant difference in maternal role confidence between the two groups (U=1.002, p=.316). There was a significant difference in breastfeeding practice between the two groups ($x^2=4.910$, p=.035). Conclusion: It is concluded that a program using a workbook has a positive effect on decreasing the perceived stress level and increasing breastfeeding practice.

영아 어머니의 과도한 수유 관련 요인 (Factors Associated with Pressure to Eat as a Feeding Practice among Mothers with Infants)

  • Purpose: This study aimed to identify factors-both infant-related and maternal-associated with pressure to eat as a feeding practice among mothers with infants. Methods: This study used a cross-sectional design and included 163 mothers of infants aged 2~12 months. Of the 180 self-reported questionnaires that were distributed, 163 (91%) were included in the data analysis. Multiple regression analysis was used to identify the factors associated with pressure to eat as a feeding practice among the mothers. Results: Infant's temperament (β=-.17, p=.035), mother's body mass index (β=-.16, p=.048), and concern about the infant being underweight (β=.30, p=.001) were associated with pressure to eat as a feeding practice among mothers. The explanatory power of these variables in the predictive model was 19.2%. Conclusion: Educational programs should be developed for improving mother's awareness of cues from infants with difficult temperament. In addition, educational interventions regarding the correct evaluation of infant's weight are needed to relieve mother's concern about their infant's being underweight. These interventions might be helpful to reduce the prevalence of pressure to eat as a feeding practice among mothers with infants.

대전지역 영유아의 수유 및 이유실태 조사 (A Study on Infant Feeding and Weaning Practice in Taejon)

  • This study was conducted to investigate the weaning practice of 198 infants in Taejon city in October, 1998. Information on infant feeding and weaning practices were obtained by interviewing mothers in gynecologist and pediatric clinics located in Taejon. The results obtained were as follows : In the survey 37.4% of the infants were breast-feeding, while 31.3% of them were bottle-feeding and 29.3% of them were mixed-feeding. The reason for bottled-feeding was due either to the lack of breast-milk secretion or motner’s job. The educational level of the mother and maternal job affected the feeding methods before weaning. Lower rates of breast-feeding were found among mothers witn a lover level of education. The breast-feeding rate was lower in full-time and part-time job worker groups than in housewife. There was no significant difference in the feeding methods according to family income. Among the subjects, 71.5% of infants began to be weaned within 7 months. The onset of weaning was delayed in the full-time worker group. There was no significant difference in the onset time of weaning according to feeding method, the educational level of the mother, and family income. 60.6% of infants received fruit juice and 29.6% of them received cereals for first-given-supplementary food. ‘For baby’s nutritional status’ was the most common motivation for the onset of weaning. Commercially prepared foods were used more than home-made food for supplementary food.

개별적 모유수유증진 프로그램이 모유수유태도와 모유수유실천에 미치는 효과 (Effect of Breast-Feeding Attitude and Breast-Feeding Practice by Individual Breast-Feeding Promotion Program)

  • 본 연구의 목적은 개별적 모유수유증진 프로그램이 모유수유 태도와 모유 수유 실천에 미치는 효과를 검정하고자 시행되었다. 연구 설계는 비동등성 대조군 전후 시차설계의 유사 실험 연구로, 연구 기간은 2011년 6월 1일부터 9월 30일까지 이었다. 참여자는 일개 모자보건센터에 산전관리를 위해 내원한 임신 32주 이상의 임신부 64명(실험군; 32, 대조군; 32)을 대상으로 하였다. 수집된 자료는 SPSS 프로그램을 이용하여 빈도, 백분율, $x^2$-test와 t-test로 분석하였으며, 결과는 다음과 같다. 개별적 모유수유증진 프로그램을 제공 받은 실험군은 대조군보다 모유수유 태도 점수와 모유수유 실천율이 더 높았고, 시간 경과에 따른 두 그룹 간 모유수유실천 정도는 통계적으로 유의한 차이가 있었다. 결론적으로, 본 연구에서 사용한 개별적 모유수유 증진 프로그램은 모유수유 태도를 변화시키고, 모유수유 실천율 상승을 위한 효과적인 간호중재인 것으로 확인되었다.

어머니의 종합적 식사지도와 유아의 식생활 실천과의 관계 (Associations between maternal comprehensive feeding practices and dietary practices in preschool children)

  • 본 연구는 어머니의 종합적 식사지도와 유아의 식생활 실천과의 관련성을 알아보기 위해 실시되었다. 서울과 경기 지역의 어린이집과 유치원에 재학 중인 만 3세부터 만 5세의 유아의 어머니 227명을 대상으로 CFPQ를 이용하여 어머니의 식사지도 내용을 조사하였으며, 유아의 영양지수 (NQ-P)를 측정하여 식생활 실천 정도를 평가하였다. 유아의 영양지수 중 균형 요인은 어머니의 종합적 식사지도의 모델링, 체중조절을 위한 섭취제한, 균형 및 다양성 권장, 음식강요 및 식사준비 참여와 양의 상관관계를 나타내었다. 유아의 영양지수의 절제 요인은 어머니의 종합적 식사지도의 모니터링, 모델링, 영양정보 전달, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 양의 상관관계를, 정서조절, 음식보상, 아동통제와는 음의 상관관계를 나타내었다. 유아의 영양지수 환경 요인은 어머니의 종합적 식사지도 중 모니터링, 모델링, 영양정보 제공, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 양의 상관관계를 보였으며, 정서조절과는 음의 상관관계가 나타났다. 유아의 식생활 실천정도를 나타내는 지표인 영양지수 각 요인에 대한 어머니의 종합적 식사지도의 영향을 평가하기 위해 다중회귀분석을 실시한 결과, 영양지수 중 균형 요인을 양의 방향으로 유의하게 예측가능한 식사지도 항목은 체중조절을 위한 음식섭취제한, 음식강요, 식사준비 참여였다. 어머니의 식사지도 항목 중 영양지수 절제 요인에 대한 양적 연관성이 있는 요인은 균형 및 다양성 권장, 건강한 식생활 환경이었으며, 음적으로 연관성이 있는 요인은 음식보상, 아동통제였다. 또한 영양지수 환경 요인과 양적 연관성이 있는 어머니의 식사지도 항목은 모니터링, 건강한 식생활 환경이었다. 따라서 어머니는 자녀에게 모니터링, 모델링, 영양정보 전달, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 같은 식사 지도를 실천하고, 정서조절, 음식으로 보상, 아동통제, 음식강요 와 같은 부정적인 식사지도 방법을 지양해야 할 것이다.
