Clinical Efficacy and Safety of Gyebutang Granules Combined with Acupuncture for the Treatment of Knee Osteoarthritis: Protocol for a Multicenter, Randomized, Assessor-blinded, 2-armed Parallel, Controlled Trial

  • Background: Due to the aging population in Korea, knee osteoarthritis (KOA) has become an increasingly common condition. Many patients with KOA prefer analgesics, herbal medicines, acupuncture, or exercise, rather than arthroscopic surgery or a knee replacement. Gyebutang (GB) granules are a herbal extract widely used to treat KOA in traditional Korean medicine, but there is insufficient evidence of its efficacy and safety. Methods: A multicenter, randomized, assessor-blinded, 2-armed parallel, controlled clinical trial has been designed to investigate the efficacy and safety of GB combined with acupuncture for the treatment of KOA. There will be 100 patients with KOA enrolled in the study from 3 traditional Korean medicine hospitals. The participants will be randomly allocated to an experimental group (GB and acupuncture) or a control group (celecoxib and acupuncture) in a 1:1 ratio. Both groups will receive acupuncture treatment once a week for 6 weeks; one group will receive GB and the other will receive celecoxib for the same duration. Results: The primary outcome will be the change of knee osteoarthritic pain, based on scores on a 100 mm visual analog scale. The secondary outcomes will be scores on a numeric rating scale, the Western Ontario and McMaster Universities osteoarthritis index, patient global assessment, European quality of life 5-dimension 5-level scale, and adverse events. Conclusion: The results of this study will provide evidence of efficacy and safety of GB as a treatment for patients with KOA.

Expression and Clinical Significance of mTOR in Surgically Resected Non-small Cell Lung Cancer Tissues: a Case Control Study

  • Aims: Mammalian target of rapamycin (mTOR) is master regulator of the PI3K/Akt/mTOR pathway and plays an important role in NSCLCs. Here we characterized mRNA and protein expression levels of mTOR and its functional associated molecules including PTEN, IGF-1R and 4EBP1 in surgically resected NSCLCs. Methods: Fifty-four patients with NSCLCs who underwent pulmonary resection were included in current study. mRNA levels of mTOR, PTEN, IGF-1R, and 4EBP1 were evaluated by RT-PCR and protein expression of mTOR, PTEN, and IGF-1R by immunohistochemistry (IHC). Association of expression of the relevant molecules with clinical characteristics, as well as correlations between mTOR and PTEN, 4EBP1 and IGF-1R were also assessed. Results: The results of RT-PCR showed that in NSCLCs, the expression level of mTOR increased, while PTEN, 4EBP1 and IGF-1R decreased. Statistical analysis indicated high IGF-1R expression was correlated with advanced clinical stage (stage III) and PTEN expression was reversely associated with tumor size (P=0.16). The results of IHC showed mTOR positive staining in 51.8% of cases, while IGF-1R positive staining was found in 83.3% and loss of PTEN in 46.3%. Protein expression of mTOR was correlated with its regulators, PTEN and IGF-1R, to some extent. Conclusions: Abnormal activation of mTOR signaling, high expression of IGF-1R, and loss of PTEN were observed in resected NSCLC specimens. The poor expression agreement of mTOR with its regulators, PTEN, and IGF-1R, implied that combination strategy of mTOR inhibitors with other targets hold significant potential for NSCLC treatment.

  • 80년대 초 GIS의 개념이 국내에 도입된 후, 1988년 대구광역시를 필두로 지자체의 GIS 구축이 시작되어 국가에서는 GIS 구축의 체계적인 관리의 필요성에 따라 1995년 제1차 국가GIS 기본계획을 수립하고, 2000년 ‘국가지리정보체계의 구축 및 활용 등에 관한 법률’을 제정, 공포하여 각 지자체의 실정에 맞는 GIS 기본계획을 수립하도록 유도하고 있다. 그러나, 바람직한 지자체 GIS 기본계획 작성의 표준이 제시되지 않아 지자체 GIS 기본계획이 개인의 안목이나 타 지자체의 내용을 참조로 만들어져 형식적이고 획일적인 결과물이 산출되고 있다. 그리하여 기존에 수립된 지자체 GIS 기본계획이 일부의 지자체를 제외하고는 해당 지자체의 GIS 구축 사업에 활용되지 못하고 책장속의 책으로 존재하고 있다. 따라서, 본 논문의 목적은 지자체 GIS 기본계획 표준안을 제시하여 향후 해당 지자체에서 GIS 기본계획을 수립할 때, 구성적인 측면의 틀을 제공하여 일정수준을 만족할 수 있는 지자체 GIS 기본계획의 수립을 지원하고자 한다. 지자체 GIS 기본계획 표준(안)을 도출하기 위해서 정보화전략계획과 해외 관련 연구, 국가GIS 기본계획의 검토를 통하여 지자체 GIS 기본계획의 구성요소를 도출하고, 기존지자체 GIS 기본계획의 목차를 중심으로 분석하여, 문제점을 도출하고, 그에 따른 시사점을 제시하여 지자체 GIS 기본계획 표준(안)의 기반이 되도록 하였다. 제안된 지자체 GIS 기본계획 표준(안)은 향후 각 지자체의 활용가치가 높은 GIS 기본 계획의 수립에 도움이 될 수 있도록 하며, 나아가서는 수립된 지자체 GIS 기본계획이 지자체의 성공적인 GIS 구축을 유도하는 가이드의 역할을 수행할 것으로 기대된다.가 있다. 본 연구의 결과를 통해 지방자치단체 GIS 기본계획에 있어 조직 측면의 장기적 비전의 제시가 가능하며 이를 통해 보다 성숙된 GIS 사업의 추진과 효율적인 시스템의 운영이 가능할 것이다.. 이상의 결과를 종합해볼 때, ${\beta}$-glucan은 고용량일 때 직접적으로 또는 $IFN-{\gamma}$ 존재시에는 저용량에서도 복강 큰 포식세로를 활성화시킬 뿐 아니라, 탐식효율도 높임으로써 면역기능을 증진 시키는 것으로 나타났고, 그 효과는 crude ${\beta}$-glucan의 추출조건에 따라 달라지는 것을 알 수 있었다.eveloped. Comparative Proteomic Analysis of Yak Follicular Fluid during Estrus

  • The breeding of yaks is highly seasonal, there are many crucial proteins involved in the reproduction control program, especially in follicular development. In order to isolate differential proteins between mature and immature follicular fluid (FF) of yak, the FF from yak follicles with different sizes were sampled respectively, and two-dimensional gel electrophoresis (2-DE) of the proteins was carried out. After silver staining, the Image Master 2D platinum software was used for protein analysis and matrix-assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF-MS) was performed for differential protein identification. The expression level of transferrin and enolase superfamily member 1 (ENOSF1) was determined by Western blotting for verification analysis. The results showed that 2-DE obtained an electrophoresis map of proteins from mature and immature yak FF with high resolution and repeatability. A comparison of protein profiles identified 12 differently expressed proteins, out of which 10 of them were upregulated while 2 were downregulated. Western blotting showed that the expression of transferrin and ENOSF1 was enhanced with follicular development. Both the obtained protein profiles and the differently expressed proteins identified in this study provided experimental data related to follicular development during yak breeding seasons. This study also laid the foundation for understanding the microenvironment during oocyte development.

The Analysis of Exercise Therapy in Nursing Research (운동중재에 관한 국내 간호학 논문 분석)

  • This study is aimed at analyzing the trend of research on the use of exercise in nursing research, through suggesting directions for future research and implementation of the various exercise therapies. Research studies were taken from dissertations and theses selected from the Academic Society Journals of nursing science, Journals from universities, medical and nursing schools, research institutes in various universities, the Central Journal of Medicine, and the New Medical Journal. The research published between 1970 and 1997 in Korea were analyzed, 51 research were selected. The research are analyzed according to: published time, source of the research, research design, subjects, sample size, dependant variables, exercise therapy, and effect of exercise therapy. The results were as follows: 1. Research on exercise therapy increased rapidly in the 1990's. At this time, 88.2% of research was published. 2. Research areas included: 54.9% non- degree research, 27.5% Doctoral theses, and 17.6% Master's theses. 3. The experimental design included: 66.7% non- equivalent control group pre-test/ post-test design and 29.4% one group pre-test/post- test design. 4. Out of the Subjects: 52.9% were patients with various health problems, and 47.1% were healthy individuals. 5. Sample size included: 52.9% with above 31 subjects, 11.8% with 11~15 and 26~30. 6. Exercise therapy was analyzed by type, intensity, frequency, duration, and period. 1) The Types: Aerobic exercise at 60.8% was the most common, active exercise for muscle strengthening and building made up 21.6%. 2) Exercise with 40~65% intensity comprised 25.5%, 70~85% with 7.8%, and no description of the intensity was 66.7%. 3) Frequency of 3~5 per week was the most common at 78.4%. 4) Duration: 15~60 minutes was the most common length of time at 76.5%. 5) Periods: More than 5 weeks at 82.3% were the most common in their categories. 7. Dependant variables: Psychological response was measured as a dependant variable in 92.2%, Cardio-pulmonary function 88.2%, Body Composition was 86.3%, Physical Response was 60.8%, Lipid Metabolism was 58.8%, Physical Strength was 49.0%, Glucose Metabolism was 25.5%, Activities of Daily Living was 17.6% and others added to be 3.9%. 8. The effect of Exercise Therapy was categorized into 'positive', 'partially positive', and 'no effect' according to dependant variables: Having a positive effect - Glucose Metabolism (93.3%), Physical Response (85.0%), Activities of Daily Living (81.8%), Psychological Response (71.6%), Lipid Metabolism (67.6%), Cardio- pulmonary Function (63.6%), Physical Strength (68.1%), and Body Composition (56.4%). The following suggestions can be made on the above findings: 1. Research findings on Exercise Therapy as a Nursing Intervention need to be described by their elements of type, intensity, frequency, duration and period. 2. Toproperly study the positive effects of Exercise Therapy, there needs to be ⑴ appropriate research design, ⑵ selection criteria for the subjects ⑶ contents of exercise prescription to individuals or groups ⑷ measurement criterion for the dependent variables. 3. Meta-analysis on exercise therapy also needs to be done to analyze and integrate the various results.

Knowledge, Perception, and Attitude related to Personal Protective Equipment of Student Nurses : Acute Respiratory Infections (간호대학생의 개인보호구 관련 인식과 지식 및 태도 : 급성 호흡기 감염병을 중심으로)

  • This study examined the level of knowledge, perception, and attitude related to personal protective equipment (PPE) of acute respiratory infection, and their relationship on nursing students. The participants were 200 nursing students of the 3rd and 4th grade in hospital practice. Data were collected from September 1 to October 15, 2018. The knowledge, perception, and attitude related to PPE were measured by structured questionnaires. Data were analyzed by descriptive statistics, one-way ANOVA, independent t-test, and Pearson correlation. From the results, knowledge score related to PPE was 17.83 out of 20 points, and perception score related to PPE was 3.68 out of 5 points, and attitude score related to PPE was 4.14 out of 5 points. Therefore, knowledge and attitude level were higher than average. Perception level was moderate. And attitude related to PPE was positively correlated with knowledge and perception. These study findings can be used as basic data for developing an effective infection control educational program to enhance the correct knowledge, perception, and positive attitude related to use of PPE for nursing students. Also, it will be necessary to provide nursing students with the equipment and related resources to master the use of PPE.

The Development and Application of Landscape Ecological Planning Model based on Biotop Mapping - A Case study of Large Residential Land Development Area - (비오톱 지도를 토대로 한 경관생태계획 모형개발과 적용 - 대규모 택지개발 예정지를 대상으로 -)

  • Reckless development policy, which has continued during the past several decades, caused landscape damage. In order to solve this problem, it is very urgent to conserve the habitat and to establish systematic database. Recently, various researches related to landscape damage have been conducted actively and the necessity of landscape ecological planning based on the biotop mapping has been issued as one of the solutions. Therefore, the purpose of this study is to select the study area, which is located in Guji-meon, Dalseong-gun, Daegu and will be formed into Daegu Science Park, and then to develop and apply the landscape ecological planning model based on biotop mapping in district unit. The results are as follows. Fist of all, according to the results of biotop classification with field survey and literature review, total 13 biotop and total 63 biotop types were classified. According to the first evaluation with B-VAT, total 19 biotop types were classified I grade including natural river with abundant plants(BA) and so on. While V grade, which has lowest value, was classified 16 biotop types including vegetable garden adjacent to river(BC). Also the second evaluation, we analyzed total 30 areas such as 1a, 1b grades, which had special meaning for the conservation of species and biotop. Next, 2a, 2b, 2c grades were analyzed total 82 areas. Secondly, in the compliance with Ecological landscape planning, we divided landscape planning model into two parts such as setting of improvement goal and detailed implement plan. For instance, setting of improvement goal, we classified 9 parts such as conservation region and parts of detailed implement plan, we classified total 28 planning indicators with 4 view such as the region which should be special protected from nature and landscape. Lastly, with the developed landscape model we applied to research areas, made maps, and differentiate proposals in each region. Specially, the final master plan was made to help understand the contents of detailed landscape planning. This study will be a useful data, which can solve the landscape damage problem systematically and control it landscape-friendly with biotop classification, evaluation, landscape ecological planning model, and application method which we developed.

A Study on Introduction of Accreditation Program for Industrial Hygiene Laboratories in Korea (국내 작업환경측정기관 인정제도 도입을 위한 현황 조사 연구)

  • There are growing needs for improving the general performances of the domestic working environment evaluation organizations. For this purpose, introduction of laboratory accreditation program has been suggested. This study was conducted as a part of this effort. With a questionnaire developed in accordance with the ISO/IEC 17025, the current status of industrial hygiene (IH) laboratories in terms of manpower, management and technical aspects. The results of this study were as follows: 1. The average working staffs' number of the working environment evaluation organizations was $6.8{\pm}3.3$ persons. In addition, 49% of all organizations are run by less than 5 persons. This suggests that manpower of Korean IH laboratories is very limited. 2. IH laboratories surveyed in this study obtained 53% of the points by the international standard. And there is significant correlation between the number of staffing and total scores (P<0.05). 3. The period of work experience is one of the most important factors to determine the working capability. The average year of work experience of the laboratories' directors was $13.5{\pm}5.3$ years. Directors with more wok experiences obtained higher scores on the questions that ask to prove the appropriateness of the research methodology (p<0.05). 4. As for academic qualifications of laboratory directors, 14% had Ph.D., 31% with Master's, 29% with Bachelor's, and 4% had Associate degree. There was significant correlation between the total scores and the general managers' academic background (p<0.05). The 27% of laboratory directors have majored in either industrial hygiene or health, 8% majored in medicine, another 8% majored environmental studies, and 6% majored in chemistry. 5. Only 14% of all IH laboratories surveyed employ directors with Certified Industrial Hygienist licence, 41% have general managers with Certified Associate Industrial Hygienist (Level 1) licence, and 45% of all laboratories either employ directors without relevant qualification or did not respond. When the laboratory manger holds relevant qualification, laboratory health and safety management was better (p<0.05). 6. When compared to the general international standard in terms of the management, and 55% in terms of technological level.

Insulin-like Growth Factor-I Induces FATP1 Expression in C2C12 Myotubes (C2C12 myotube에서 Insulin-like growth factor-I 이 FATP1 발현에 미치는 영향)

  • Fatty acid transporter protein 1 (FATP1) is highly expressed in skeletal muscle and modulates fatty acid uptake and metabolism. However, the influence of insulin-like growth factor-I (IGF-I), a master regulator of skeletal muscle cells, on FATP1 in skeletal muscle cells has not been demonstrated. To investigate the effect of IGF-I on FATP1 and the expression of the IGFBP5 protein, differentiated C2C12 murine skeletal muscle cells were treated with 20 ng/ml of IGF-I at different time points. The results showed that IGF-I increased FATP1 and IGFBP5 protein expression in a time-dependent manner. To determine whether this induction of FATP1 by the IGF-I treatment was regulated pretranslationally, the mRNA level of FATP1 was measured by real-time quantitative PCR. The IGF-I treatment resulted in very rapid induction of the FATP1 mRNA transcript in C2C12 myotubes. FATP1 mRNA increased 169% and 132% after 24 and 48 h of the IGF-I treatment, respectively, and it returned to control levels after 72 h of the treatment, suggesting that the FATP1 gene is regulated pretranslationally by IGF-I in skeletal muscle cells. This is the first evidence that IGF-I can regulate the expression of FATP1. In conclusion, IGF-I induced rapid transcriptional modification of the FATP1 gene in C2C12 skeletal muscle cells and had modulating effects on fatty acid uptake proteins and oxidative proteins.

Analysis of Nursing Studies of Coping Conducted in Korea from 1978 to 1995 (대처 개념의 국내 간호연구 분석)

  • This study was done to analyze the trends of research on coping in Korea, to suggest future direction, for research on coping, and ultimately to contribute to an increase in explanation of adaptation. This article reviewed 79 nursing research papers on coping done since 1978 by examining them according to the period of publication or presentation, research design, type of subjects, measurement instruments, research for a degree or not, range of reliability, and association of coping and related variables. The results are as follows : The number of studies on coping increased rapidly from the mid-1980's and decreased slowly from the mid-1990's. The maority of the studies were surveys, comparative studies, or correlational studies. The subects of the 46 studies were healthy people, while those in the remaining studies were patients with a variety of illnesses. Thirth-eight studies on coping were done for master's thesis, three for dissertion, and 38 were not degrees. The Bell and Jalowiec coping scales have not been used since the early 1990's. In contrast, Lazarus and Folkman's W.C.C.L. has been used increasingly since that time. The reliabilities of the coping scale were reported in 37 cases and the Cronbach's alpha coefficients were .71 to .86. All subjects reported using more problem-oriented coping than emotion-oriented coping in short-term or emotion-oriented coping and healthy groups did more long-term coping. It was difficult to describe consistently the relationship between stress and coping according to the type of coping scale or research subjects, but generally moderate relationships were found. This was due to instrumental problems and no consideration of situational context. The subject group who used more short-term coping and less long-term coping reported poorer mental status, and higher scores in burnout and state anxiey than others. That is, the relationship between stress and adaptation increased the power of explanation with intervening the mediating effect of coping. The association of locus of control, mastery, social support, and self-concept with coping showed positive relationships : those of uncertainty and severity in illness with coping showed negativerelationships ; those of state anxiety and depression with short-term coping were positive, and those of self-esteem with long-term coping or problem-oriented coping were negative. There were significant differences in the scores of types of coping according to religion, level of education, and socio-economic status. That is, Presbyterians and Catholics, those with higher education levels and higher socio-economic status used more long-term or problem-oriented coping. On the basis of the above findings the following recommendations are made : 1. There is a need to test the mediating effect of coping variable in order to clarify the concept. 2. Longitudinal studies are needed to determine the patterns of change in coping strategies when stressful events are encountered. 3. It's necessary to develop a reliable and variable measurement tool for coping. 4. There is a need to identify subscales of coping to increase explanation of variance 5. It's necessary to consider personal, situational, and antecedent variables : the characteristics of subject populations, the natures of illness and treatment situations. 6. The power of explanation of studies designed to identify the stress-adaptation process should be increased using the combination model of process-oriented coping and cognitive-structural model.
