THE ROENTGENOGRAPHIC STUDY OF THE RELATIONSHIP BETWEEN ERUPTION AND CALCIFICATION OF THE MANDIBULAR PERMANENT TEETH IN KOREAN. (한국인 하악 영구치 석회화와 맹출의 상호 관계에 관한 방사선학적 연구)

  • The purpose of this study was to finding out the relationship between the tooth calcification and eruption of the mandibular permanent teeth in Korean. This study was undertaken in 592 children at ages from 3 to 13 years who had good oral condition by means of panoramic roentgenographic analysis. The following results were obtained. 1. The mean ages of crown completion were as follows; Canine 1st. Premolar 2nd. Premolar 1st. Molar Male 6yrs. 4mos. 6yrs. 8mos. 7yrs. 6mos. 7yrs. 6mos. Female 5yrs. 11mos. 6yrs. 5mos. 7yrs. 2mos. 3yrs. 3mos. 2. Each tooth started to move toward occlusion at approximately stage 6 or after crown completion. 3. The highest increment in eruption rate was at about 1/3~1/2 completion of root and ages at 10-11 years in male, 9-10 years in female. 4. Eruption period of both sexes were as follows; Canine: 6-12years 1st. Premolar: 7-12 years 2nd. Premolar: 7-13 years 1st. Molar: 3-7 years 5. The eruption was completed before the root completion. 6. The sequence of eruption and calcification was 1st. Molar-Canine-1st. Premolar-2nd. Premolar in both sexes.

Endodontic management of mandibular first premolar with C-shaped canals by using cone-beam computed tomography and dental operating microscope (CBCT와 치과용 현미경을 이용한 C형 근관을 가지는 하악 제1소구치의 근관치료)

  • Aberrant anatomy of mandibular premolars is very rare in Korean, but aberration can contribute the endodontic failure as it makes difficult to remove the irritants during cleaning and shaping procedure. This case report describes the successful root canal treatment of a rare mandibular first premolar with C-shaped root canal as using a cone-beam computed tomography to understand the internal shape of root canal system and a dental operating microscope to improve the magnification and illumination.

ISOMETRIC BITE FORCE AND ITS RELATION TO CRANIOFACIAL MORPHOLOGY (교합력과 두개안면 형태의 상관관계에 대한 연구)

  • This study was undertaken to grope the correlation of the maximal bite force and tooth-craniofacial structure. The maximal bite force of 76 adult male, aged 18-28 (mean aged: $23.4{\pm}2.2$) years, was estimated and cephalometric headplates were measured, tabulated and statistically analyzed. The results were as follows. 1. 59.61kg of bite force in first molar, 45.38kg in premolar and 17.10kg in central incisor were arranged. 2. The bite force was negatively correlated to genial angle, mandibular plane angle, the angle between occlusal plane and mandibular plane, the angle between palatal plane and mandibular plane, and positively correlated to posterior height of face, length of mandibular body, length of ramus, facial depth in craniofacial structure. 3. The group with strong bite force showed small genial angle, mandibular plane angle, the angle between occlusal plane and mandibular plane, the angle between palatal plane and mandibular plane, and long posterior height of face, length of mandibular body, length of ramus, facial depth. So they manifested the tendency to brachycephalic pattern, on the other hand, the group with weak bite force manifested the tendency to dolichocephalic pattern. 4. There is no correlationships between bite force and mesial inclination of premolar axis in this subject. 5. It is considered bite force have an effect upon craniofacial pattern, especially upon the lower face.

  • Of 633 children, 334 in male and 299 in female, whose ages ranged from 3 to 13 years old, the amounts of development of teeth germ were observed according to ages with oblique lateral cephalogram to study the developing state of teeth germ of mandibular premolars. The results were as followings; 1. The time of actively develop:ng tooth germ was observed about 10 years old both in male and in female. 2. The increasing rate of development of tooth germ a year was higher in female than in male at mandibular 1st premolar and in male than in female at mandibular 2nd premolar. 3. Age of complete tooth germ development was earlier in female than in male at mandibular 1st and 2nd premolars.

  • First case, a one-year-old female Shih Tzu weighing 3 kg with closed transverse fractures of bilateral mandibular body between 2nd and 3rd premolar teeth were treated surgically with bone plates and screws, and cranial luxation of temporomandibular joints were reduced to closed method. Second case, a 8.4 years old male Yorkshire terrier weighing 2.6 kg with closed transverse/oblique fractures of bilateral mandibular body between premolar and molar teeth were treated surgically with bone plates and screws. Radiographic examination revealed that the fractures in two cases fixed well and temporomandibular joint was stabilized. One month after operation, jaw activity was excellent and malocclusion wasn't observed.

  • The author observed the eruption pattern of the mandibular premolars. For this study the author took 406 cases of oblique cephalographs of 6 to 12 year old children. The measurement were with regard to the vertical and horizontal dimensional changes and tooth axis changes to the occlusal plane. The following results were obtained. 1. At stage V the tooth axis of 1st premolar inclined mesially to the occlusal plane, as the tooth erupt toward vertically the angulation changed that at stage X the tooth axis almost right angle to the occlusal plane. 2. The distance from the root apex of premolars to the lower border of mandible were slightly increased to the stage VII, while after stage VII rapid increasing appeared. At stage X the distance of 1st premolar was more longer than 2nd premolar. 3. The distance from the mesial surface of 1st molar to the mesial surface of premolars were decreased slightly to the stage VII, but this distances were decreased rapidly after stage VII. 4. The distance between distal surface of 1st premolar and mesial surface of 2nd premolar was almost constant before stage VIII, at stage IX this distance was little presented because the premolars were contacted.

Observation of an extracted premolar 2.5 years after mineral trioxide aggregate apexification using micro-computed tomography

  • Although numerous studies have been conducted on apexification using mineral trioxide aggregate (MTA), direct observation of extracted human teeth after the procedure has been rarely reported. This case report describes a mandibular premolar treated 2.5 years ago and extracted recently for orthodontic treatment. The tubercle of the right mandibular premolar of a 12-year-old boy with dens evaginatus was fractured and the pulp was exposed. The tooth was diagnosed with pulp necrosis and asymptomatic periapical abscess. During the first visit, copious irrigation was performed with 2.5% sodium hypochlorite. Calcium hydroxide paste was placed as an intracanal medicament. The sinus tract had disappeared at the second visit after 3 weeks. MTA was applied on to the bleeding point as a 4-mm-thick layer, followed by a 3-mm-thick gutta-percha filling and resin core build-up. After 2.5 years, the tooth and three other premolars were extracted for orthodontic treatment. The right and left mandibular premolars were scanned with micro-computed tomography to determine the root shape and canal anatomy. Irregular root growth was observed and the root outline of the right mandibular premolar differed from that of the contralateral tooth. Apexification with MTA leads to the formation of roots with irregular morphology, without any pulpal space.

Late development of a mandibular second premolar

  • In this report, we present the case of a girl with delayed odontogenesis of a lower second premolar for which she was followed up for 8.5 years. Congenital absence of permanent mandibular second premolars was observed at the initial radiographic examination at 8 years and 1 month. One year later, during the treatment period, an unexpected odontogenesis of a right second premolar was diagnosed on follow-up radiography. The original treatment plan was revised and a new plan was successfully implemented. This unusual case showed that the orthodontist's clinical philosophy must be flexible because unexpected situations can arise, especially when treating growing patients.

Use of cone-beam computed tomography and three-dimensional modeling for assessment of anomalous pulp canal configuration: a case report

  • Three-dimensional (3D) reconstruction of cone-beam computed tomography (CBCT) scans appears to be a valuable method for assessing pulp canal configuration. The aim of this report is to describe endodontic treatment of a mandibular second premolar with aberrant pulp canal morphology detected by CBCT and confirmed by 3D modeling. An accessory canal was suspected during endodontic treatment of the mandibular left second premolar in a 21 year old woman with a chief complaint of pulsating pain. Axial cross-sectional CBCT scans revealed that the pulp canal divided into mesiobuccal, lingual, and buccal canals in the middle third and ended as four separate foramina. 3D modeling confirmed the anomalous configuration of the fused root with a deep lingual groove. Endodontic treatment of the tooth was completed in two appointments. The root canals were obturated using lateral compaction of gutta-percha and root canal sealer. The tooth remained asymptomatic and did not develop periapical pathology until 12 months postoperatively. CBCT and 3D modeling enable preoperative evaluation of aberrant root canal systems and facilitate endodontic treatment.

A Case of External Skeletal Fixation for Treatment of Bilateral Mandibular Fracture in a Dog (개에서 외부고정법을 이용한 양측성 하악골절의 치유 일례)

  • A 5-year-old, male mixed breed dog was presented to Gyeongsang National University Animal Hospital for bilateral mandibular open fracture by bite. The dog had loss of the teeth and injury of the oral mucosa. Radio-graphic and blood examinations were conducted. Fracture lines were observed between first premolar and second premolar, but temporomandibular luxation was not observed on radiographs. In serum analysis, elevated levels of CPK and LDH were found. The dog was anesthetized and prepared for aseptic surgery. Acrylic external skeletal fixator with Kirschiner wire was applied. Pin loosening and malalignment of the right fracture line was observed at 7 weeks after surgery. We applied Kirschner-Ehmer fixator and half pins during the second surgical procedure. At 4 weeks after a second surgical procedure, the fixator and pins were removed from mandible. The fractures were healed with no complications.
