  • Bone density is one of the factors in the early failure of dental implants and doctors should make a preoperative assessment of jaw bone density using patient's CT data before dental implant surgery in order to find out whether the patient has osteoporosis and osteopenia. The main goal of this study was to propose a method that based on image processing techniques in order to provide accurate information about where to drill and place an abutment screw of implants in the jaw bone for doctors and reduce human activity for the estimation of the local cancellous bone density of mandible using CT data. The experiment was performed on a computed tomography data of the jaw bone of two different individuals. We assumed that the result of the estimation of jaw bone density depends on the angle of drilling and average HU (Hounsfield Unit) values were used to evaluate the quality of local cancellous bone density of mandible. As a result of this study, we have been developed a toolbox that can be used to estimate jaw bone density automatically and found a positive correlation between the angle of the drill and time complexity but a negative correlation between the diameter of the drill and time complexity.

  • The purpose of this study was to evaluate the detectability of various radiographic techniques for mandibular condylar lesions. Erosive lesion, osteophyte and flattening were formed on the artificial mandibular condyle, and panoramic, transcranial, transorbital radiography, lateral and frontal tomography were taken. The results were as follows; 1. The detectability for erosive lesions was superior in the order of frontal tomography(96%), lateral tomography(78%), transorbital(59%), transcranial(16%) and panoramic(48%) radiography. 2. The location of erosive lesion that showed the highest detectability was the medial third in panoramic, the lateral third in transcranial, the central portion of anteroposterior direction in transorbital, the central portion of mediolateral direction and the posterior third in lateral tomography. Frontal tomography disclosed all erosive lesions except one anterolateral lesion. 3. The detectability of osteophyte was 100% in lateral tomography, 78% in transcranial and 56% in panoramic radiography. 4. For flattening, lateral tomography showed the flattened condyle, but both panoramic and transcranial views showed only decreased bone density without the change of condylar shape.

저칼슘식이와 방사선조사가 백서 악골에 미치는 영향의 실험적 연구 (THE EFFECT OF LOW DIETARY CALCIUM AND IRRADIATION ON MANDIBLE IN RATS)

  • This study was performed to investigate the morphological and structural changes of bone tissues and the effects of irradiation on the mandibular bodies of rats which were fed low calcium diets. In order to carry out this experiment, 160 seven-week old Sprague-Dawley strain rats weighing about 150 gm were selected and equally divided into one normal diet group of 80 rats and one low calcium diet group with the remainder. These groups were then subdivided into two groups, 40 were assigned rats for each subdivided group, exposed to radiation. The Group 1 was composed of forty non-irradiated rats with normal diet, Group 2 of forty irradiated rats with normal diet, Group 3 forty non-irradiated rats with low calcium diet, and Group 4 forty irradiated rats with low calcium diet. The two irradiation groups received a single dose of 20 Gy on the jaw area only and irradiated with a cobalt-50 teletherapy unit. The rats with normal and low calcium diet groups were serially terminated by ten on the 3rd, the 7th, the 14th, and the 21st day after irradiation. After termination, both sides of the dead rats mandible were removed and fixed with 10% neutral formalin. The bone density of mandibular body was measured by use of bone mineral densitometer(Model DPX -alpha, Lunar Corp., U.SA). Triga Mark ill nuclear reactor in Korea Atomic Research Institute was used for neutron activation and then calcium contents of mandibular body were measured by using a 4096 multichannel analyzer (EG and G ORTEC 919 MCA, U.SA). Also the mandibular body was radiographed with a soft X-ray apparatus(Hitex Co., Ltd., Japan). Thereafter, the obtained microradiograms were observed by a light microscope and were used for the morphometric analysis using a image analyzer(Leco 2001 System, Leco Co., Canada). The morphometric analysis was performed for parameters such as the total area, the bone area, the inner and outer perimeters of the bone. The obtained results were as follows: 1. In the morphometric analysis, total area and outer perimeter of the mandibular bodies of Group 3 were a little smaller than that of Group 1. The mean bone width and bone area were much smaller than that of Group 1 and the inner perimeter of Group 3 was much longer than that of Group 1. The total area and outer perimeter of Group 2 and Group 4 showed little difference. The mean bone width and bone area of Group 4 were smaller than that of Group 2 and the inner perimeter of Group 4 was longer than that of Group 2. 2. The remarkable decreases of the number and thickness of trabeculae and also the resorption of endosteal surface of cortical bone could be seen in the microradiogram of Group 3, Group 4 since the 3rd day of experiment. On the 21st day of experiment, the above findings could be more clearly seen in Group 4 than in Group 3. 3. The bone mineral density of Group 3 was lesser than that of Group 1 and the bone mineral density of Group 4 was lesser than that of Group 2 on the 7th, 14th, 21st days. The irradiation caused the bone mineral density to be decreased regardless of diet. In the case of Groups with low calcium diet, the bone mineral density was much decreased on the 21st day than on the 3rd day of experiment. 4. The calcium content in mandible of Group 3 was smaller than that of Group 1 throughout the experiment. roup 4 showed the least amount of calcium content. The irradiation caused the calcium content to be decreased regardless of diet. In the case of Groups with low calcium diet, the calcium content was much decreased on the 21st day than on the 3rd day of experiment. In conclusion, the present study demonstrated that morphological changs and decrease of bone mass due to resorption of bone by low calcium diet, and that the resorption of bone could be found in the spongeous bone and endosteal surface of cortical bone. So the problem of resorption of bone must be considered when the old and the postmenopausal women are taken radiotherapy because the irradiation seems to be accelerated the resorption of osteoporotic bone.

A CASE REPORT OF THE MANDIBULAR INFECTED BUCCAL CYST

  • The author observed a cystic lesion which had new radiographic entities in the 6 years old male patient and obtained the following features. 1. The main clinical feature is the swelling. 2. The lesion is situated on the buccal surface of a erupting mandibular first molar. 3. The affected molar is tilted, so that the apices are adjacent to the lingual plate of the mandible. 4. The affected molar is of normal morphology, caries-free and vital. 5. The radiographic feature is the cystic radiolucency which is extended over the buccal surface of the roots of the affected molar and slightly inferior to the root apices. 6. The dental papilla of the affected molar shows the lamina dura of relatively normal density and definitition. 7. The buccal surface shows the laminated new bone formation. 8. The inferior concave border of the lesion is delineated by a thick and calcified layer of cortical bone. 9. The lesion causes displacement of the developing second molar of which anterior cortex is partially destroyed.

구리당량 영상작성에 의한 골밀도계측방법의 평가 (Assessment of the Measurement Method of the Bone Mineral Density on Cu-Equivalent Image)

  • Purpose : The effects of step numbers of copper wedge and exposure on the coefficient of determination (r²) of the conversion equation to Cu-equivalent image and on the Cu-equivalent value (mmCu) and it's coefficient of variation measured at each copper step and the mandibular premolar area were evaluated. Method: Digital image analyzing system consisted of scanner, personal computer, and a stepwedge with 10 steps of 0.03 mm copper in thickness as reference material was prepared for quantitative assessment of the bone mineral density. NIH image program was used for analyzing images. Results : The film having moderately high film density showed the discrepancy between the real thickness and the measured Cu-equivalent value of each copper step. The Cu-equivalent image was dependent on the determinational coefficient of the conversion equation than the coefficient of variance of the measured value. Conclusion : Obtaining conversion equation with high coefficient of determination and proper film exposure are supposed to be neccessary for quantitative assessment of bone density. Multiple steps in the range of the corresponding copper thickness to the bone density of the area to be measured should be prepared.

Effect of Gum-Chewing on Facial Appearance and Stomatognathic System

  • Purpose: This study was planned to clarify a negative view of chewing gum due to the concern that continuous gum chewing might cause a change in the gonial angle and make the lower facial appearance look square. Materials and Methods: We had 25 adults (13 males and 12 females, with an average of 27.3 years) chew 6 g of gum (spearmint) evenly with both right and left posterior teeth for one hour per day for three months. We then measured their gonial angle, the inclination of occlusal plane, facial height, bone marrow density, and masticatory force before chewing, 1, 2, and 3 month after chewing to verify its significance statistically. Result: The results showed that the gonial angle increased from $122.7^{\circ}$ to $123.3^{\circ}$ (P>0.05), and thus the jaws became slightly slimmer. There was no change in the occlusal plane inclination and facial height. Meanwhile bone marrow density in the mandibular angle and ascending ramus increased from $0.285g/cm^2$ to $0.290g/cm^2$ (P<0.05), and masticatory force also increased by 0.5 kg on the right side and 0.8 kg on the left side (P<0.05). Conclusion: Continuous chewing of gum gives an appropriate exercise effect to the stomatognathic system. As chewing gum has effect on increase bone marrow density without changing the mandibular angle and facial appearance the claim that jaw bone changes to a square jaw through chewing gum is regarded to be groundless.

Cone beam CT를 이용한 비글견 발치창 치유과정에 대한 방사선학적 연구 (Radiologic study of the healing process of the extracted socket of beagle dogs using cone beam CT)

  • Purpose: To longitudinally observe the healing process of extracted socket and the alterations of the residual ridge in healthy adult dogs using cone beam CT (CBCT). Materials and Methods: The mandibular premolars of two beagle dogs were removed and the extraction sites were covered with the gingival tissue. CBCTs (3D X-ray CT scanner, Alphard vega, Asahi Co.) were taken at baseline and at 1 week interval for 12 weeks. Radiographic density of extracted wounds was measured on normalized images with a custom-made image analysis program. The amount of alveolar crestal resorption after the teeth extraction was measured with a reformatted three-dimensional image using CBCT. Bony healing pattern of extracted wound of each group was also longitudinally observed and analyzed. Results: Dimensional changes occurred during the first 6 weeks following the extraction of dogs' mandibular premolars. The reduction of the height of residual ridge was more pronounced at the buccal than at the lingual aspect of the extraction socket. Radiographic density of extracted wounds increased by week 4, but the change in density stabilized after week 6. New bone formation was observed at the floor and the peripheral side of extracted socket from week 1. The entrance of extracted socket was sealed by a hard-tissue bridge at week 5. Conclusion: The healing process of extracted wound involved a series of events including new bone formation and residual ridge resorption.

흰쥐에서 당뇨 유도 후 하악골 골밀도 변화에 관한 연구 (THE STUDY OF BONE MINERAL DENSITY IN THE MANDIBLE OF STREPTOZOTOCIN-INDUCED DIABETIC RATS)

  • DM is a systemic disease with many complications. One of them, diabetic osteopenia is important sequelae and many authors reported reduced bone mass in diabetic rats. However, in mandible, study has been rare because of its anatomical limits. So the objective of this study was to investigate bony change in mandible of diabetic rats. Thirty-two adult rats were used in this study. Half of them were male and female respectively. In sixteen rats, streptozotocin was injected intraperitoneally to induce DM and the serum glucose concentration was checked to ensure the induction of DM prior to the time of sacrifice. At 1, 2, 3, 4, 6, 8, 12, 16weeks, control group and diabetic group rats were sacrificed respectively. And then bone mineral density of mandibles and femurs of the rats was measured using dual energy X-ray absorptiometry(DEXA). In addition serum osteocalcin and urine deoxypyridinoline were measured as markers of bone formation and resoption respectively. Mandibular and femoral bone density in streptozotocin induced rats was decreased with significance statistically after 4 weeks from injection. In mandible, comparing with femur, bone density was moderately decreased. The alveolar bone in mandible was more decreased bone density than the whole body in the mandible From these results, bone mineral density decreased in uncontrolled diabetic group with time, and especially alveolar bone was more destructive in the mandible. So authors think that consideration of reduced bone mineral density is necessary in dental procedure.

Bone remodeling effects of Korean Red Ginseng extracts for dental implant applications

  • Background: The formation of a nanotube layer on a titanium nanotube (N-Ti) plate facilitates an active reaction between bone cells and the material surface via efficient delivery of the surface materials of the dental implant into the tissues. Studies have reported that Korean Red Ginseng extracts (KRGEs) are involved in a variety of pharmacological activities: we investigated whether implantation with a KRGE-loaded N-Ti miniimplant affects osteogenesis and osseointegration. Methods: KRGE-loaded nanotubes were constructed by fabrication on pure Ti via anodization, and MC3T3-E1 cells were cultured on the N-Ti. N-Ti implants were subsequently placed on a rat's edentulous mandibular site. New bone formation and bone mineral density were measured to analyze osteogenesis and osseointegration. Results: KRGE-loaded N-Ti significantly increased the proliferation and differentiation of MC3T3-E1 cells compared with cells on pure Ti without any KRGE loading. After 1-4 weeks, the periimplant tissue in the edentulous mandibular of the healed rat showed a remarkable increase in new bone formation and bone mineral density. In addition, high levels of the bone morphogenesis protein-2 and bone morphogenesis protein-7, besides collagen, were expressed in the periimplant tissues. Conclusion: Our findings suggest that KRGE-induced osteogenesis and osseointegration around the miniimplant may facilitate the clinical application of dental implants.

구강악안면영역에서의 MedporTM의 임상적용 (THE USE OF MEDPORTM(POROUS HIGH-DENSITY POLYETHYLENE) IN ORAL AND MAXILLOFACIAL REGION)

  • Recently, for the reconstruction of bony defect and cosmetic improvement, many graft materials and implants have been widely used in the various surgical situations. The alloplastic materials have many advantages such as simplicity of operation, no additional need of surgery, and easy manipulation. The $Medpor^{TM}$(porous high-density polyethylene, Porex Co., USA) was initially studied in 1972 for surgical implant and introduced as an implant material for oral and maxillofacial region by Sauer and King in 1988. This material permits full ingrowth of bone into the implants, substantially increasing the implant's incorporation into the recipient site. It can be shaved during the surgery, which results in an improvement and prefabricated various size and shapes to fit into the surgical defect. The $Medpor^{TM}$ was used in 32 patients from 1995 to 1997 at the maxillofacial region. It was used for paranasal augmentation in 24 cases, for malar augmentation in 2 cases, for infraorbital augmentation in 2 cases, for mandibular angle augmentation in 2 cases, for mandibular body augmentation in 2 cases, for chin vertical augmentation in 1 case. It was mainly fixed with miniplate or screw. There were few complications except one infection and one exposure of the implant.
