Analysis of Accidents Causes in an Auto-Glass Manufacturing Company using the Comprehensive Human Error Analysis Model (통합적 휴먼에러 분석 모델을 이용한 자동차 유리공장의 사고 원인 분석)

  • To prevent similar accidents with the basis of industrial accidents already occurred in industrial plants, it would be possible only after true causes are grasped. Unfortunately, however, most accident investigation carried out with the basis of legal regulation failed to grasp them so that similar accidents have been repeated without cease. This research aimed to find out differences between results from conventional accident investigation and those from human error analysis, and to draw out effective and practical counter-plans against industrial accidents occurred repeatedly in an autoglass manufacturing company. As for analysis, about 110 accident cases that occurred for last 7 years were collected, and by adopting the Comprehensive Human Error Analysis Technique developed by the previous researchers, not direct causes but basic fundamental causes that might induce workers to human errors were sought. In consequence, the result showed that facility factors or environmental factors such as improper layout, mistakes in engineering design, and malfunction of interlock system were authentic major accident causes as opposed to managerial factors such as personal carelessness or failure to wearing personal protective equipments, and/or improper work methods.

A Management Approach of the Construction Accidents Based on Assessing the Job Stress of Korean and Foreign Construction Laborers (내외국인 건설 근로자의 직무 스트레스 평가를 통한 건설재해 관리 방안)

  • Each year, it seems inevitable that major accidents will occur on construction sites. Industrial accidents, sometimes involving foreign laborers, have been constantly increasing. Construction laborers have higher hazard rates and higher work intensity than other industries, which means that they experience more job stress, as a result of the subcontracting structure. Therefore, this study performed an influence factor analysis on job stress and its relevance to industrial accidents involving Korean and foreign construction laborers, and proposed its effectiveness with the job stress and construction accident management measures based on the results of those. A questionnaire to measure job stress was performed targeting Korean and foreign laborers, and the results were analyzed. The results of this study can be utilized as important reference materials in efforts to reduce the job stress of foreign laborers on construction sites, and can be expected to contribute to preventing construction accidents related to job stress in the future.

  • Purpose - This thesis is to develop a management plan and checklist by analyzing the prevention of accidents in advance by presenting the management plan for the major causes of railway accidents. Research design, data and methodology - In recent 5 years, we have analyzed the cases of railway accident, presented the management plan for the accident, and made a practical safety checklist focusing on the main measures according to the management plan. Results - The analysis of the cases of near-railway accidents suggests more concrete and practical safety management measures because the similar accidents are continuously occurring due to formal safety management. Conclusions - It is more valuable to apply to the adjacent construction of the railway line by creating a detailed checklist based on cases rather than the existing checklist. This study is written only as a human factor. For future real - time safety management, it is necessary to study more precisely cause analysis and safety equipment as a big data - based safety control system for more systematic safety management.

PREDICTION OF HYDROGEN CONCENTRATION IN CONTAINMENT DURING SEVERE ACCIDENTS USING FUZZY NEURAL NETWORK

  • Recently, severe accidents in nuclear power plants (NPPs) have become a global concern. The aim of this paper is to predict the hydrogen buildup within containment resulting from severe accidents. The prediction was based on NPPs of an optimized power reactor 1,000. The increase in the hydrogen concentration in severe accidents is one of the major factors that threaten the integrity of the containment. A method using a fuzzy neural network (FNN) was applied to predict the hydrogen concentration in the containment. The FNN model was developed and verified based on simulation data acquired by simulating MAAP4 code for optimized power reactor 1,000. The FNN model is expected to assist operators to prevent a hydrogen explosion in severe accident situations and manage the accident properly because they are able to predict the changes in the trend of hydrogen concentration at the beginning of real accidents by using the developed FNN model.

  • This study aims to seize the differences in drivers' attitudes and to speculate about the attitudes which may be causally related to traffic accidents based on data collected from group survey questionnaire in the four countries. The questionnaire was composed of 9 problem areas and prepared in Japanese English and Korean languages. The survey was conducted in Japan Canada U.S.A and Korea in 1987 The present study analysed the driver's attitudes toward behaviour of pedestrians traffic signal and causes of traffic accidents. The main results are :(1) The order of high percentage of those who cited the pedestrians as major cause of accidents was Korea Japan, Canada and U,S,A This markes the very high number of accidents involving pedestrians in Korea. (2) The percentage of who answered that most drivers started before the sign changed to Go sign at an intersection were higher in Japan and Korea than in U.S.A (3) Regarding the caues of accidents if the drivers were to meet with an accident the percentage of responses attributing faults to themselves for the accident was very high in Japan. Korea came next. In contrast the percentage of responses attributing faults to others is higher in Canada and U.S.A than Japan and Korea.

Development of Traffic Accident Models at Rural Signalized Intersections by Day and Night (지방부 신호교차로 주·야간 교통사고 예측모형 개발 및 비교 분석)

  • PURPOSES : The purposes of this study are to compare the day and night characteristics and to develop the models of traffic accidents. in Rural Signalized Intersections METHODS : To develop day and night traffic accident models using the Negative Binomial Model, which was constructed for 156 signalized intersections of rural areas, through field investigations and casualty data from the National Police Agency. RESULTS : Among a total of 17 variances, the daytime traffic accident estimate models identified a total of 9 influence factors of traffic accidents. In the case of nighttime traffic accident models, 11 influence factors of traffic accidents were identified. CONCLUSIONS : By comparing the two models, it was determined that the number of main roads was an independent factor for daytime accidents. For nighttime accidents, several factors were independently involved, including the number of entrances to sub-roads, whether left turn lanes existed in major roads, the distances of pedestrian crossings to main roads and sub-roads, lighting facilities, and others. It was apparent that if the same situation arises, the probability of an accident occurring at night is higher than during the day because the speed of travel through intersections in rural areas is somewhat higher at night than during the day.

  • Currently, approx. 10,532 turnouts were installed on the KR line, of which 3,644 turnouts were installed in the main line where high speed operation is performed. This shows that it is necessary to improve the performance of turnout as one of vulnerable areas for the safe operation of a train. Even though the number of railway accidents is decreasing every year due to the renewal of railway facilities, there are still many factors that cause the occurrence of an accident. In particular, an incident in turnout area does not only have a high risk of a serious accident but also affect the operation of a train on the adjacent track in most cases, and consequently a big social loss is expected due to the delay in train operation as well as the loss of life and property. The objective of this study is to examine the accidents occurred in turnout area that is one of typical vulnerable areas over the past ten years, on the basis of the results, and to take an appropriate measure by finding out the major cause of accidents in turnout area so that a systematic safety system can be established to prevent or reduce accidents in turnout area.

Analysis of Behavioral Traits in Violation related to LPG Accidents (LPG 관련 산재사고의 위반행동 특성 분석)

  • LPG-related accidents, which account for half of all gas accidents in Korea, have not shown any sign of decrease over the past decade, partially owing to the lack of effective safety improvement measures. The purpose of this study was to identify the effectiveness of improvement measures by analyzing the traits of accidents in terms of human factors, and to seek more effective accident prevention strategies. In this study, 108 accident cases were collected and analyzed in the aspect of accident characteristics such as violation type, human factors, and so on. The results showed that the work procedures of suppliers and engineers related to LPG accidents seemed to be similar in outward appearance; however, specific accident causes and unsafe behaviors were different. Particularly, type and target of violations were different, which could be visually confirmed by the Principal Component Analysis (PCA) and the Quantification Techniques (QT). Furthermore, for engineers, insufficient supervision was a major influencing factor. In conclusion, because the accident characteristics of suppliers and engineers are different, differentiated accident prevention strategies should be implemented, which was discussed in this study.

A Study on the Application of Smart Safety Technology at Construction Sites in South Korea

  • Among all industries, the construction industry still remains a traditional one with low productivity due to its labor-intensive and field-dependent production system, its supplier-oriented industrial structure, and the disruption of the information flow between participants. In addition, the construction industry in South Korea has recently been required to transform itself according to social trends such as aging, the reduction of skilled workers, and the shortening of working hours, and the disaster and death rates in the industry, which are more than twice as high as those in other industries, are making it more necessary to solve chronic safety problems. Therefore, the purpose of this study is to grasp the actual condition of safety management on construction sites in South Korea and analyze cases of K-smart technology utilization for preventing safety accidents on construction sites. The study investigated and analyzed the following. First, construction sites in South Korea were analyzed by type of safety accident, by type of construction, and by construction contract amount. Second, the current status of accidents on small-sized construction sites with a high fatal accident rate and cases of safety accidents on construction sites were investigated. The results of the study are expected to contribute to the dissemination and spread of smart safety technology for not only identifying major factors in safety accidents that occur on construction sites but also preventing workers from suffering accidents.

A Study on Systems Development for Preventing Aviation Deficiency and Accident (항공기 결함과 사고예측시스템 개발)

  • There are still occurring aviation accidents in spite of great preventing efforts all over the world. This paper contains some methods to prevent aircrafts deficiencies and accidents. First part of this paper refers to the background of those aviation deficiencies on mechanical, human and environment structures which influence directly to the air accidents and general survey on various theories of the aircraft's systems. On the way we discussed the general situations of the air tool's deficiencies which cause tragic accidents to the human lives and assets. After analysis on the situations we suggest the new systems which would forecast more detail accuracies concerned air elements for the safety flying. Then we introduce the following new systems resulting from the forecasting which can solve problems on aircraft deficiencies and complex interrelationships among air accidental factors. To simplify the complex systems, we needed to build the mechanical and organizational database for maintaining the procedures of the past troubleshooting on the major parts preventing deficiencies of those mechanical units. These suggested systems will contribute a great deal of aids, the maintenance credibility and air safety for the air operations and all customers in the world. Avoiding the past troubleshooting from just by using simple systems which can forecast main causes of the units and parts of the crafts, this system will be able to provide excellent management tools for the promoting aviational industries. The comfortable and convenient air operations are very valuable works, and the scientific method and detail maintenance will improve our daily air life by minimizing accidents. Adapting these developing systems, for the forecasting aircraft deficiencies and accidents can be integrated with the other aircraft management systems to promote more air safety in the world. This study is focused to eliminating aircraft accidents through forecasting deficiency symptom procedures by relational coordinations among all of the systems. Futhermore we need continuously detailed analysis and study for eliminating air accidents all together those who work in those fields.
