응급의료센터에서 수혈을 시행한 성인 외상환자에서 사망 예측 인자 (Predictive Factors for Mortality among Adult Trauma Victims Transfused in an Emergency Department)

  • Purpose: The most common cuase of transfusion for trauma victims in an emergency department is hypovolemic shock due to injury. After an injury to an internal organ of the chest or the abdomen, transfusion is needed to supply blood products and to compensate tissue oxygen transport and bleeding. From the 1990's, there have been some reports that transfusion is one of the major factors causing multiple-organ failure. Thus, as much as possible, tranfusion has been minimized in the clinical setting. This study aims to analyze the prognostic factors for mortality among trauma victims transfused with blood products in an emergency department. Methods: We conducted this study for the year of 2010 retrospectively. The study group included adult trauma victims tranfused with blood products in our ED. The exclusion criteria were discharge against medical advice, and missing follow-up due to transfer to another facility. During the study period, 34 adult trauma victims were enrolled. We compared the clinical variables between survivors and non-survivors. Results: the mean age of the 34 victims was 58.06 years, and males account for 58.5% of the study group. The most-frequently used form transportation was ambulance(119, 55.9%), and the most common injury mechanism was mobile vehicle accidents(67.6%). The mean revised trauma score (RTS) was 5.9, and the mean injury severity score (ISS) was 47.76. The mortality rate in the ED was 58.5%, Comparison of survivors with non-survivors showed statistical differences in injury mechanism, initial SBP, DBP, RTS, ISS, and some laboratory data such as AST, ALT, pH, PO2, HCO3, glucose (p<0.05). Regression analyses showed that mortality among adult trauma victims transfused in the ED correlated with RTS. Conclusion: When an adult trauma victim is transported to the ED and needs a tranfusion, the emergency physician carefully assess the victim by using physiologic data.

The Timing of Femur Fracture Fixation is an Important Factor for Prolonged Mechanical Ventilation

  • Purpose: Mechanical ventilation (MV) is an essential life-saving modality for severely injured patients. However, the long-term use of MV is a major risk factor for late mortality. The surgical correction of long bone fractures plays a critical role not only in improving functional outcomes, but also in reducing physiological derangements, including MV duration. This study investigated the factors affecting prolonged MV (PMV) in severely injured patients with femur fractures. Methods: We retrospectively evaluated all severely injured patients (injury severity score >15) with femur fractures who were taken to the emergency department within 12 hours of the causative accidents between January 2016 and December 2018. PMV was defined as MV lasting for ≥7 days. We analyzed the factors affecting PMV. Results: In total, 35 patients were enrolled and 21 (33.3%) were included in the PMV group. The PMV group required more red blood cell (RBC) transfusions within 7 days RBC (7dRBC) (12.8 vs. 6.8 units; p=0.03) and the time to femur fracture fixation (TFFF) was longer (7.9 vs. 2.7 days; p=0.018). The area under the curve (AUC) for TFFF was 0.740 (95% confidence interval [CI]: 0.572-0.908; p=0.018) and the AUC for 7dRBC was 0.718 (95% CI: 0.546-0.889; p=0.031). Conclusions: This study indicates that TFFF is an independent risk factor for PMV. Early fixation of femur fractures might prevent PMV and its associated complications.

휠체어 사용자의 주거환경디자인 지침에 관한 연구 (A Study on the Guidelines of Residential Space Design for Wheelchair Users)

  • 노인 인구의 급격한 증가와 교통사고, 산업재해 등으로 인해 휠체어사용자의 수가 점차 늘어나고 있어 커다란 사회문제가 되고 있다. 이에 따라 공공시설물에 대한 장애인, 노인, 임산부 등을 위한 편의시설 설치 및 이용에 관한 '편의 증진법'이 제정되어 현재는 신축되는 모든 건축물뿐만 아니라 도로, 공원 등도 가능한 한 모든 사람들이 큰 지장 없이 접근 및 이동이 가능하도록 하고 있다. 그러나 이러한 공공시설물 외에 횔체어사용자의 개인주거환 경에 대한 디자인적 측면의 연구는 거의 이루어지지 않기 때문에 횔체어사용자는 자신의 신체적 특성이 전혀 고려되지 않고 일반적인 사람의 기준에 맞추어 설계된 주거환경에서 불편한 생활을 영위할 수밖에 없는 실정이다. 따라서 휠체어사용자를 위한 주거환경 개발 시 그들의 신체적 특성을 감안하되 그들과 함께 생활하는 가족, 보호자 등이 모두 편리하게 생활 할 수 있는 주거공간개발에 필요한 디자인지침의 개발이 절실히 요구되고 있다. 이에 유니버설 디자인적 관점에서 휠체어사용자와 일반인이 모두 사용하기에 편리한 주거환경디자인 시에 적용될 수 있는 디자인지침을 각 주거공간별로 구축하는 것을 목표로 본 연구가 진행되었으며 연구목표 달성을 위해 실태조사, 설문조사, 비디오관찰조사 방법을 활용하였다.

철도신호시스템 교체에 따른 운행 시나리오 도출에 관한 연구 (Study on the Design of Operation Scenario for Replacement of a Railway Signaling System)

  • The railway signalling system plays an essential role in the headway and routing control for a safe and efficient train operation. The reliable and safe operation of the system is very important because the failure of the railway signalling system can lead to the collision, derailment, or unexpected stop of a train. So far, the conventional wayside signal mode (ATS: Automatic Train Stop) has been generally used as the railway signalling system. However, this system is highly linked to a risk of major accidents resulted from human mistakes such as missing a signal or careless control of train speed. Accordingly, the onboard signal mode (ATC: Automatic Train Control) as an alternative of ATS has been recently introduced and applied to transmit effectively the information on speed control of a train by using computers and communication equipment. In the process of replacing the obsolete signal system, it is necessary to switch over the system while providing passengers with normal services. Therefore, the integration of a railway signaling system compatible for both ATS and ATC and its interface is discussed in this study. In particular, the implementation scenario required for operation planning of the integrated system was designed, and the results as well as effects of its applicability test were also presented.

휴대용 GPS를 이용한 부선의 안전예항시스템 구축에 관한 연구 (A Study on the Safety-Towing System for Barge Using Portable GPS)

  • 피예선인 부선으로 인하여 예선 자체의 조종성능에 제한을 받고, 이러한 예부선은 바람, 파도, 조류 등과 같은 외력의 영향이 크게 작용하여 예부선 운항의 안정성 확보에 많은 어려움이 수반되고 있다. 예선의 선장, VTS 및 육상의 운항부서 등에서는 부선과 같은 피예선의 실해역에서의 침로, 속도, 위치 등을 실시간으로 추적하여 해난사고를 사전에 예방할 수 있는 시스템의 구축이 절실히 필요한 실정 이다. 본 연구에서는 비교적 저렴하고 이동이 용이한 휴대용 GPS를 이용한 부선의 안전예항시스템의 구축에 대한 기초적인 방법을 제안하고, 이를 토대로 실선에서의 테스트를 실시하고 그 결과에 대하여 논의한다. 본 연구를 통하여 안전 예항에 필요한 여러 항해 정보를 취득할 수 있었으며, 이러한 정보는 다양한 분야에서의 활용이 가능할 것으로 사료된다.

  • 해상에서 해양사고를 예방하고 항행안전성을 증대시키기 위해 연안 VTS와 항행보조시설이 적극 검토되고 있으나 막대한 비용과 예산이 든다. 따라서 연안해역에 항행 안전성을 평가하여 우선순위를 결정해야하고 항행 안전성평가는 조선자의 관점에서 평가되어야 한다. 한편, 인간의 사고에는 많은 형태의 애매함이 존재하기 때문에 이러한 애매함을 반영하기 위해 본 연구에서는 퍼지측도와 쇼케적분을 이용하였다. 본 연구의 목적은 퍼지측도와 쇼케적분을 이용하여 조선부담감의 관점에서 평가모델을 개발하고 우리나라 연안해역을 8개로 나누고 연안 VTS와 항행보조시설 설치의 우선순위를 결정하기 위한 평가를 실시하였다.

동파방지열선 화재 흔적분석과 발화원인 연구 (A Study for the Fire Analysis and Igniting Cause of Freezing Protection Heating Cables)

  • There have been a number of major fatal fire accidents in Korea recently. The number of fires in 2017 were 44,178, which is not only increasing number of fires but also increasing in casualties. Particularly, the fire at Jecheon Sports Center, which suffered many casualties, is expected to have a huge impact. The cause of the fire has not been determined yet, but heat waves on the ceiling have also been pointed out. As such, the copper heating waves, which are used as a preventive measure against damage of pipes due to freezing of pipes, etc., always have a fire hazard. To determine the possibility of a flame-resistant heated fire, a positive electric cable product was used to artificially ignite and analyze the results. In case of a short circuit, the external covering of the positive electric cable is damaged, but not short circuit unless the heating material surrounding the wire is damaged. Due to the characteristics of heating cable for preventing copper waves, the chances of insulation becoming more severe due to moisture and temperature changes are higher than normal wires. If the internal heating system is carbonized by insulating deterioration without damage to the outer coating, it is likely to cause trekking, to form a winding loop in the heating materials, and to cause short circuit in the heated materials. For the positive temperature line, if the middle is shorted, the current continues to flow to the short circuit unless the breaker disconnects. Consequently, a heated fire that does not cut off the power immediately may leave multiple marks or cuts.

자율운항선박의 개발 및 운용을 위한 육상 운항사의 인적요인 이슈 고찰 (A Review of Human Element Issues of Remote Operators on Maritime Autonomous Surface Ships)

  • 최근 보다 안전하고 효율적인 해상운송시스템을 위한 해결책으로 자율운항선박의 개발 및 운용에 대한 논의가 활발히 진행되고 있다. 자율운항기술은 해양사고 방지, 선박의 연비 증진, 비용 절감 등 긍정적인 측면과 함께 선원의 일자리 감소, 업무의 변화, 보안문제 등 부정적인 측면도 상존하고 있다. 향후 자율운항선박의 시대가 도래 하더라도 육상 센터에서의 제어가 이루어지게 되므로, 육상 운항사의 상황인식과 같이 새로운 인적요인 이슈가 존재할 것으로 예측된다. 본 논문에서는 자율운항선박의 개발 및 운용과정에 고려해야 할 주요 인적요인 이슈를 고찰하고, 육상 센터의 제어에 따라 예상되는 육상 운항사의 의사결정 및 업무수행에 영향을 미치는 요소에 대한 식별과 영향 요소의 평가를 위한 인적 신뢰성 분석 방안을 제시하였다.

기계적 모터 고장진단을 위한 머신러닝 기법 (A Machine Learning Approach for Mechanical Motor Fault Diagnosis)

  • In order to reduce damages to major railroad components, which have the potential to cause interruptions to railroad services and safety accidents and to generate unnecessary maintenance costs, the development of rolling stock maintenance technology is switching from preventive maintenance based on the inspection period to predictive maintenance technology, led by advanced countries. Furthermore, to enhance trust in accordance with the speedup of system and reduce maintenances cost simultaneously, the demand for fault diagnosis and prognostic health management technology is increasing. The objective of this paper is to propose a highly reliable learning model using various machine learning algorithms that can be applied to critical rolling stock components. This paper presents a model for railway rolling stock component fault diagnosis and conducts a mechanical failure diagnosis of motor components by applying the machine learning technique in order to ensure efficient maintenance support along with a data preprocessing plan for component fault diagnosis. This paper first defines a failure diagnosis model for rolling stock components. Function-based algorithms ANFIS and SMO were used as machine learning techniques for generating the failure diagnosis model. Two tree-based algorithms, RadomForest and CART, were also employed. In order to evaluate the performance of the algorithms to be used for diagnosing failures in motors as a critical railroad component, an experiment was carried out on 2 data sets with different classes (includes 6 classes and 3 class levels). According to the results of the experiment, the random forest algorithm, a tree-based machine learning technique, showed the best performance.

Primary Idiopathic Myelofibrosis: Clinico-Epidemiological Profile and Risk Stratification in Pakistani Patients

  • Background: Primary idiopathic myelofibrosis (PMF) is a clonal Philadelphia chromosome-negative myeloproliferative neoplasm characterized by extramedullary hematopoiesis and marrow fibrosis. It is an uncommon hematopoietic malignancy which primarily affects elderly individuals. The rational of this study was to determine its clinico-epidemiological profile along with risk stratification in Pakistani patients. Materials and Methods: In this retrospective cross sectional study, 20 patients with idiopathic myelofibrosis were enrolled from January 2011 to December 2014. Data were analyzed with SPSS version 22. Results: The mean age was $57.9{\pm}16.5years$ with 70% of patients aged above 50. The male to female ratio was 3:1. Overall only 10% of patients were asymptomatic and the remainder presented with constitutional symptoms. In symptomatic patients, major complaints were weakness (80%), weight loss (75%), abdominal discomfort (60%), night sweats (13%), pruritus (5%) and cardiovascular accidents (5%). Physical examination revealed splenomegaly as a predominant finding detected in 17 patients (85%) with the mean splenic span of $22.2{\pm}2.04cm$. The mean hemoglobin was $9.16{\pm}2.52g/dl$ with the mean MCV of $88.2{\pm}19.7fl$. The total leukocyte count of $17.6{\pm}19.2{\times}10^9/l$ and platelets count were $346.5{\pm}321.9{\times}10^9/l$. Serum lactate dehydrogenase, serum creatinine and uric acid were $731.0{\pm}154.1$, $0.82{\pm}0.22$ and $4.76{\pm}1.33$ respectively. According to risk stratification, 35% were in high risk, 40% in intermediate risk and 25% in low risk groups. Conclusions: The majority of PMF patients were male and presented with constitutional symptoms in our setting. Risk stratification revealed predominance of advanced disease in our series.
