경부에 발생한 낭포성 활액종의 증례보고 (A CASE REPORT OF CYSTIC HYGROMA IN THE NECK)

  • Cystic hygroma is a special form of lymphangioma. It most often occurs in the neck of children. We observed a 14-year-old male whose chief complaint was a abnormal swelling of the right mandibular angle area. The computerized tomography showed the well-defined nonenhancing cystic mass on right submandibular gland area and anterior triangle of the neck. The histopathologic fidings exhibited the multiloculatedfluid-filled cysts, which are lined a flattened layer of endothelial cells with foci of lymphocytes found lying adjacent to the lining. After evaluation of above findings, the mass was diagnosed as a cystic hygroma in the neck.

Reduction glossectomy of congenital macroglossia due to lymphangioma

  • Macroglossia is a rare clinical condition defined as an enlarged tongue. Macroglossia can cause structural deformities like diastema and disproportionate mandibular growth and present functional disorders such as dysarthria, dysphonia, and respiratory problems. A 7-year-old boy who had lymphangiomatous macroglossia was treated with a reduction glossectomy by anchor-shaped combination of a U-shape and modified key-hole resection. Postoperatively, the reduced tongue was contained completely within the oral cavity, but open bite remained due to prognathism. Sensory and motor nerves to the tongue appeared to be intact, and circulation was adequate. This patient will be monitored for recurrence of tongue enlargement.

  • 종격종 림프관종은 림프관에서 기원하는 드문 양성 종양이다. 대부분 출생 시 존재하며 생후 첫 2년 이내 발견되는데, 성인에서는 매우 드물게 발생하며, 대부분 증상이 없이 단순 흉부 방사선촬영에서 이상소견이 발견되어 진단된다. 저자들은 단순 흉부 방사선촬영에서 이상소견이 발견된 3명의 환자에서 수술을 통하여 조직학적으로 종격동 림프관종임을 진단하고 치료한 3명의 환자를 경험하여 이를 보고하는 바이다.

  • Lymphangioleiomyomatosis(LAM) is defined as an abnormal proliferation of smooth muscles in the lung tissue throughout lymphatics, vascular and bronchial structure. A 52-year-old postmenopausal woman was admitted to our hospital for recurrent pneumothorax. She was treated for medroxyprogesterone by LAM 1 month ago. We performed operation of pulmonary partial resection and pleurodesis. The patient is receiving continuous medroxyprogesterone and Leuplin administration, and currently, 6 months after the operation, is still showing good results.

종양성 병변으로 인한 구강점막질환

  • 구강점막에 발생하는 종양성 병변으로는 대표적으로 구강점막암(oral mucous membrane cancer), 유두종(papilloma),혈관종(hemangioma), 임파관종(lymphangioma),섬유종(fibroma)등이 있는데 이들 병변들은 구강내 협점막, 치은, 경구개, 연구개, 구강저, 혀 등에 궤양이나 증식성 병소로 나타나서 동통, 출혈, 치아동요, 종창 등의 증상을 유발시킨다. 이러한 병변들에 대한 명확한 치료와 예후를 결정하기 위해서는 무엇보다도 이들 병변들에 대한 정확한 소견에 근거한 진단이 우선되어야 할 것이다. 따라서 본 장에서는 정확한 진단과 치료를 위해 이들 병변들에 대해 소개하고자 한다.

  • A dermoid cyst is a rare congenital midline neck mass with usually develops in the submental region. It is most often seen in young adults and can become rather large than almost no symptoms. As it increases in size, dysphasia, or dyspnea can develop. The differential diagnosis of the midline lesion includes ranula, thymglossal duct cyst, cystic hygroma, and cystic lymphangioma. The treatment of choice is complete surgical removal. With a review of the literature, we report two cases of a huge dermoid cyst inducing dyspnea.

  • Non-odontogenic tumors can be classified as malignant or benign. Most oral tumors in children are benign. In the Belfast series only 7.5 percent of soft tissue tumors were malignant and Bhaskar(l963) found only 9 percent of 293 oral tumors of all kinds to be malignant. Benign tumors may be classified as epithelial and mesenchymal. The most common tumor of surface epithelium is the squamous papilloma. These are easily recognized clinically as cauliflower-like lesions. Fibrous lesions are very common in children's mouths. Many of these are not true neoplasms but are related to fibrous hyperplasia. Another common oral tumor in children is angiomatous tumors. Hemangioma occurred more frequently than lymphangioma. Cystic hygroma, a cystic subtype of lymphangioma, is a developmental tumor of lymphatic origin. It is a considered to be a relatively rare lesion. About 50 percent of cystic hygroma are present at birth, and most of the remaining 50 percent appear in the early years of life during the period of active lymphatic growth. The preferred treatment for these lesions, except for hemangioma, is complete surgical excision. With proper surgical techniques, recurrence is not expected.

  • Macroglossia is a tongue pathology of multiple etiology as systemic disease like cerebral palsy and Down syndrome etc. It can cause abnormal oral conditions including anterior open bite or dyspnea by changing occlusion and oral habits. So many handicapped children who have macroglossia need to get surgery of large tongue to improve esthetics, function, and treatment stability. The purpose of this report is to evaluate 2 patients, one has cerebral palsy and the other lymphangioma, who have experience of glossectomy with review of literature.

  • 저자는 하악 좌측 제2소구치의 치근단 육아조직 수술 도중 우연히 발견된 하치조 관에 발생된 혈관임파종을 치험하였기에 문헌적 고찰과 함께 증례를 보고하며, 혈관임파종의 예후, 치료방법에 대한 보다 더 많은 연구가 필요하리라 사료된다.

  • An oral foregut cyst is a rare congenital choristoma lined by the respiratory and/or gastrointestinal epithelium. The exact etiology has not been fully identified, but it is thought to arise from misplaced primitive foregut. This lesion develops asymptomatically but sometimes causes difficulty in swallowing and pronunciation depending on its size. Thus, the first choice of treatment is surgical excision. Surgeons associated with head and neck pathology should include the oral foregut cyst in the differential diagnosis for ranula, dermoid cyst, thyroglossal duct cyst and lymphangioma in cases of pediatric head and neck lesions.
