  • Cystic lesions of the neck are frequently encountered on clinical basis. The usual differential diagnosis includes branchial cleft, cystic hygroma, dermoid cyst, tuberculous lymphadenitis, benign detached goiterous thyroid cyst, and Cavitation epidermoid carcinoma. Most of cysts are benign and malignant cysts are rare. The most common form of cystic malignancy in the lateral neck is cystic degeneration of epidermoid carcinoma metastatic to cervical lymph nodes. Cystic neoplasia may result from malingnant changes occurring within the wall of a previously benign cyst Complete head and neck examination on the primary focus is important Especially, when palpation of thyroid is negative, thyroid carcinoma is easily overlooked. Recently, we experienced papillary carcinoma of the thyroid seen as lateral neck cyst. So we report this case with review of literatures.

Cloning and Characterization of hydroxypyruvate isomerase (EC 5.3.1.22) gene in silkworm Bombyx mori

  • The sequence of hydroxypyruvate isomerase gene was obtained in NCBI. In this study, the hydroxypyruvate isomerase gene of Bombyx.mori was identified and annotated with bioinformatics tools. The result was confirmed by RT-PCR, prokaryotic expression, mass spectrographic analysis and sub-cellular localization. The hydroxypyruvate isomerase cDNA comtains a 783bp ORF, and has 4 exons. The deduced protein has 260 amino acid residues with the predicted molecular weight of 29169.30 Da, isoelectric point of 6.10, and contains conserved PRK09997 and Hfi domains. The hydroxypyruvate isomerases of Nasonia vitripennis and Bombyx mori have a high homology. Through RTPCR analysis, we found that this transcript was present in testis, ovary, blood-lymph, fat body, midgut, silk gland and tuba Malpighii. This protein was located in cytoplasm through immunohistochemistry. We submitted the cloned gene under the accession number EU344910. The enzyme has been classified under accession number EC 5.3.1.22.

  • Fine needle aspiration cytology (FNAC) is an easy convenient non-invasive method in the diagnosis of superficial palpable masses. The cytologic findings by FNAC of reactive and neoplastic lesions in various organs including breast, lymph node, thyroid, salivary gland, etc., have been described, but, those of soft tissue lesions including proliferative fasciitis are relatively rare to find. We recently experienced a case of FNAC of proliferative fasciitis in the left back of a 72-year-old male. The FNAC smears were scant in cellularity and contained large cells with abundant basophilic cytoplasm, one to two nuclei lying at the periphery, and prominent nucleoli that resemble ganglion cells.

  • Drosophila hematopoiesis is comparable to mammalian differentiation of myeloid lineages, and therefore, has been a useful model organism in illustrating the molecular and genetic basis for hematopoiesis. Multiple novel regulators and signals have been uncovered using the tools of Drosophila genetics. A Runt domain protein, lozenge, is one of the first players recognized and closely studied in the hematopoietic lineage specification. Here, we explore the role of lozenge in determination of prohemocytes into a special class of hemocyte, namely the crystal cell, and discuss molecules and signals controlling the lozenge function and its implication in immunity and stress response. Given the highly conserved nature of Runt domain in both invertebrates and vertebrates, studies in Drosophila will enlighten our perspectives on Runx-mediated development and pathologies.

종격동 종괴로 발현된 잠재성 갑상선암 (Thyroid Carcinoma Presenting as an Anterior Mediastinal Mass)

  • We have experienced a case of occult papillary thyroid carcinoma presenting as an anterior mediastinal mass in a 40-year-old man. The CT scan revealed a huge mass behind the manubrium of the sternum but the ultrasound examination failed to detect any lesion and developmental defect in the thyroid. Excision of the mediastinal mass and total thyroidectomy were carried out. Histologically, the mediastinal mass turned out to be papillary carcinoma without any portion of the normal thyroid tissue or normal lymph node tissue and the thyroid gland showed a tiny papillary carcinoma with the diameter of 0.3cm. Although a mediastinal mass as the sole presentation of the thyroid carcinoma is very rare, we suggest that a mediastinal mass should be added to the list of possible metastatic thyroid carcinoma.

Pheochromocytoma with Brain Metastasis: A Extremely Rare Case in Worldwide

  • Pheochromocytoma (PCC) is a neuroendocrine tumor that mainly arises from the medulla of the adrenal gland. Some PCCs become malignant and metastasize to other organs. For example, it typically involves skeletal system, liver, lung, and regional lymph nodes. However, only a few cases of PCC with brain metastasis have been reported worldwide. We report a case of metastatic brain tumor from PCC in South Korea in 2016. A 52-year-old man presented with headache, dizziness and motor aphasia. He had a medical history of PCC with multi-organ metastasis, previously underwent several operations, and was treated with chemotherapy and radiotherapy. Brain MRIs showed a brain tumor on the left parietal lobe. Postoperative pathology confirmed that the metastatic brain tumor derived from malignant PCC. This is the first report PCC with brain metastasis in South Korea.

  • A giant thyroid mass can be associated with severe compressive symptoms, requiring surgery for treatment. This paper reports the case of an 82-year-old man who presented with a giant thyroid mass that was suspected of being a metastatic papillary thyroid carcinoma on fine-needle aspiration cytology. A harmonic scalpel was used to excise the giant thyroid mass. A pectoralis major myocutaneous flap was used for reconstruction. The histopathology assessment confirmed the metastases in the lymph node and papillary thyroid carcinoma in the resected gland.

  • Kimura disease (KD) is a chronic inflammatory disorder that frequently involves the subcutaneous tissue of the head and neck regions. It often manifests as regional lymphadenopathy or salivary gland enlargement in the affected area. The histologic architecture of the lymph nodes in KD patients is preserved, while the affected tissues show follicular hyperplasia and eosinophilic infiltration. No single modality has been adopted as an optimal treatment for KD. This article concentrates on the fundamental features of KD and reviews current approaches to its treatment.

두경부 암 환자의 경부 림프절 전이 분석 (Nodal Status of the Head and Neck Cancer Patients)

  • 목적 : 두경부 암 환자에서 경부 림프절 전이 가능성이 높아지는 것은 원발병소의 해부학적위치 및 특성, 원발병소의 크기 즉 병기와 조직병리학적 종류 따라 영향을 받는다. 두경부 암 환자에서 경부 림프절의 전이 양상은 다른 나라에서 분석, 연구되어 있고, 현재 두경부 암 환자 치료와 처치를 이를 기조로 하여 사용한다. 그러나, 두경부 암 환자에 대한 경부 림프절 전이에 대한 우리나라 환자의 통계는 없어 이번 통계를 시행하게 되었다. 대상 및 방법 : 1981년 11월부터 1995년 12월까지 본 병원 방사선 종양학과에 내원한 환자중초사가 가능한 997명을 대상으로 하였다. 조사는 방사선 종양학과에 내원시 면밀한 경부 촉진과향께 컴퓨터 단층 촬영영상 상에서 확인을 하였다. 환자는 크게 경부 림프절 전이가 있는 군과 전이가 없는 군으로 나누었고, 경부 림프절의 병기는 미국 암 연합회에서 추천한 방식에 의하였다. 원발병소의 위치 및 주위로의 침윤 등은 면밀한 관찰과 촉진, 컴퓨터 단층 촬영 영상, 수술소견 및 조직병리학 소견을 참조하였다. 결 과 :방사선 종양학과에 방사선 치료를 위해 내원한 997명 중 416명$(42\%)$에서 경부 림프절 전이를 뽀였고, 581명$(58\%)$에서 전이를 보이지 않았다. 경부 림프절 전이를 보인 림프절의 병기분포를 관찰한 결과 다음과 같았다. Hl : 106$(25.5\%)$, N2a 100$(24\%)$, N2b :68$(16.4\%)$, N2c : 69$(16.6\%)$, N3 :73$(15\%)$. 두경부 암 환자에서 원발병소의 빈도는 다음과 같다. 후두암 : 283$(28.5\%)$, 부비동암 : 152$(18\%)$, 구인두암: 144$(14.5\%)$, 비인강암: In$(12\%)$, 구강암 :92$(9\%)$, 하인두암:71$(7\%)$, 타액선암:$58(6\%)$, 미지원발부위암:31$(3\%)$, 피부암: $14(2\%)$. 두경부 암 환자 중 가장 경부 림프절 전이가 높은 원발병소는 비인강암$(71\%)$으로 나타났고, 다음은 하인두암$(69\%)$, 구인두암$(64\%)$, 구강암$(39\%)$ 순이었다. 경부 림프절 전이를 보였던, 416명 중에서는 구인두암 92명$(22\%)$, 비인강암 8f명$(21\%)$, 후두암 79명$(19\%)$의 순이였다. 병리조직학적으로 가장 빈도가 높을 것은 편평상피암증으로 전체 환자 중 $65.4\%$을 차지하였고, 다음은 악성 림프종이 109명으로 $11\%$였다. 결론 : 두경부 암 환자에서 경부 림프절 전이의 양상이 외국 통계와 비교 매우 유사하게 나타났으며, 원발병소의 해부학적 특성에 따라 경부 림프절 전이가 좌우되며 원발병소의 병기가 진행됨에 따라 경부 림프절 전이 빈도도 증가 되며 또한 경부 림프절의 병기도 증가되는 것을 관찰 할 수 있었다.

Tissue microarray를 이용한 사이모신 베타4(Thymosin β4)와 vascular endothelial cell growth factor (VEGF)의 정상 인간 조직 발현 양상 연구 (Analysis of Thymosin β4 and Vascular Endothelial Cell Growth Factor (VEGF) Expression in Normal Human Tissues Using Tissue Microarray)

  • 사이모신 베타 4와 VEGF의 발현을 여러 인간 조직에서 tissue microarray를 사용하여 조사하였다. 사이모신 베타 4는 간, 이자, 침샘의 관상피, 심장에서 강한 발현을 보였으며 피부, 폐, 이자, 림프절, 갑상선, 요관, 폐와 부신의 혈관 내피세포 등에서 중간 수준의 발현 양상을 보였다. VEGF의 발현 양상은 대체적으로 사이모신 베타 4와 동일하였으며 이자, 요관, 유선, 간, 식도, 신장, 폐, 부신 등의 혈관 내피세포에서 강하게 발현되었다. 이러한 결과를 통해 사이모신 베타 4는 간, 이자, 침샘의 관상피, 심장에서 중요한 역할을 담당하며 VEGF와 같은 발현 양상을 보여 혈관 신생작용에 관여함을 확인하였다.
