Effects of 3,5,3'-triiodo-L-thyronine ($T^3$) on Growth of GR-transgenic Coho Salmon, Oncorhynchus Kitsutch

  • GH-transgenic coho salmon (Oncorhynchus kitsutch) juveniles in tGH*T$_3$and tGH*PTU were fed with the diets containing 1 ug/g fish of 3,5,3'-triiodo-L-thyronine (T$_3$) and 30 ug/g fish of 6-n-propyl-2- thiouracil (PTU), respectively, to assess the effect of these drugs on the change of physiological activity, growth and survival rate in comparison with normal transgenic (tGH*C) and nontransgenic coho salmon (Wild) for 90 days. Although the daily food intakes of all transgenic (tGH)-groups were higher than Wild, the amount was reduced by exogenous PTU supply. The fred efficiencies of tGH-groups were lower than Wild, but the efficiency was reduced both by T$_3$and PTU. The survival rate of tGH-group was significantly higher than that of Wild, but there was no significant difference among tGH-groups. Although the growth of tGH-coho salmon was faster than Wild. the growth rate of transgenic salmon was increased by exogenous T$_3$, but was reduced by PTU Plasma TT$_4$levels of tGH-groups was approximately 2-fold higher relative to Wild, but there were no difference of plasma TT$_4$levels among tGH-groups. plasma TT$_3$level or tGH-coho salmon was increased by exogenous T$_3$administration, but was reduced by exogenous PTU. In addition, although plasma GH levels of all tGH-groups were higher than that of Wild, the GH level in plasma of transgenic coho salmon was increased by exogenous T$_3$and reduced by exogenous PTU. In the meantime, the transgenic fishes also displayed head, jaw and opercular abnormalities typical of the offsets of this gene construct in coho salmon, indicating that some imbalance in growth processes has been induced. However, the abnormalities of transgenic coho salmon was reduced following exogenous PTU administration.

한국산 미끈망둑 Luciogobius guttatus (망둑어과) 피부의 구조 및 조직화학적 특성 (Structure and Histochemistry of the Skin of a Flat-headed Goby, Luciogobius guttatus (Gobiidae, Pisces) from Korea)

  • 한국산 미끈망둑(Luciogobius guttatus)의 피부구조가 머리, 위턱, 아래턱, 뺨, 등, 체측, 배, 아가미뚜껑 등 모두 8개 부분에서 비교 조사되었다. 이들 모두 표피는 맨바깥층, 중간층, 기저층으로 구성되었다. 맨바깥층은 1~4층의 편평세포와 점액세포로 구성되었으며, 중간층은 1~10층의 대형 표피세포를 가지며 이러한 표피세포는 마치 거미줄 같은 형태를 보여주고 있다. 이러한 대형표피세포의 존재는 두꺼운 표피를 갖도록 하며, tonofiilament와 desmosome을 가진다. 한편 기저층은 한 층의 입방상피로 구성되어 있으며, 많은 모세혈관들이 기저층 바로 아래부분에 존재한다. 진피에는 비늘이 존재하지 않으며 거의 대부분 모세혈관을 가지는 치밀층으로 구성되어 있다.

LOADED IMPLANT와 UNLOADED IMPLANT의 조직학적 비교 연구 (A HISTOLOGIC COMPARATIVE STUDY OF LOADED AND UNLOADED TITANIUM IMPLANTS)

  • In order to see the possible effect of the functional load-bearing after osseointegration of the titanium root form implant in dog a histologic study was conducted. One side of lower jaw was surgically prepared edentulousness and titanium implants were inserted. Some implants were functionally loaded through fixed detachable prosthesis and some are isolated and unloaded. The dog was sacrificed four months later and bone sections with implants were processed for histologic evaluation and the results were as follows ; (1) The bone to implant interface after four months of load bearing presented no mobility and no marginal bone loss radiographically and histologically. (2) The interface zone between compact bone and implant revealed a direct bone to implant contact and in some areas marrow tissue contacts were examined at the light microscopic level. (3) At the ultrastructural level the interface of surrounding compact bone matrix and implant, three types of superficial layers were found ; one with moderate electron dense amorphous granular substance layer, other with high electron dense fine granular substance layer, and another type of amorphous granular substance covered with high electron dense line of minute granules. (4) The osteoblasts in the marrow tissue neighboring implants and osteocytes in compact bone showed typical normal characteristics and in the marrow tissues some of lymphocytes and mast cells were observed. (5) The abscence of abnormal tissue reactions at a cellular level indicates a high degree of biocompatibility for the experimental titanium implant and basically no difference was found between functionally loaded and unloaded implants.

Infraorbital nerve transpositioning into orbital floor: a modified technique to minimize nerve injury following zygomaticomaxillary complex fractures

  • Objectives: Transpositioning of the inferior alveolar nerve to prevent injury in lower jaw has been advocated for orthognathic, pre-prosthetic and for implant placement procedures. However, the concept of infra-orbital nerve repositioning in cases of mid-face fractures remains unexplored. The infraorbital nerve may be involved in trauma to the zygomatic complex which often results in sensory disturbance of the area innervated by it. Ten patients with infraorbital nerve entrapment were treated in similar way at our maxillofacial surgery centre. Materials and Methods: In this article we are reporting three cases of zygomatico-maxillary complex fracture in which intra-operative repositioning of infra-orbital nerve into the orbital floor was done. This was done to release the nerve from fractured segments and to reduce the postoperative neural complications, to gain better access to fracture site and ease in plate fixation. This procedure also decompresses the nerve which releases it off the soft tissue entrapment caused due to trauma and the organized clot at the fractured site. Results: There was no evidence of sensory disturbance during their three month follow-up in any of the patient. Conclusion: Infraorbital nerve transposition is very effective in preventing paresthesia in patients which fracture line involving the infraorbital nerve.

Clinical investigation of bisphosphonate-related osteonecrosis of the jaws in patients with malignant tumors

  • Objectives: This study evaluated bisphosphonate-related osteonecrosis of the jaws (BRONJ) in patients diagnosed with malignant bone tumors. Demographic findings, laboratory, and radiographic analyses were performed to characterize disease severity and progression. Materials and Methods: Patients who had been diagnosed with BRONJ (2005-2010) at the authors' hospital according to the American Association of Oral and Maxillofacial Surgeons were investigated. Twenty-one patients (12 with multiple myelomas, 7 with breast cancer, and 2 with prostate cancer) who had been treated with bisphosphonates (BPs) for malignant bone tumors were included. Radiographic evaluations with a panorama, computed tomography, whole body bone scan, and laboratory findings were evaluated for erythrocyte sedimentation rate (ESR), c-reactive proteins (CRPs), and c-terminal cross-linked telopeptides (CTXs). Results: The average age of the patients was 64.3 (range 51-80), and they were treated with BPs for an average of $35{\pm}19$ months before BRONJ was diagnosed. Types of BPs were zolendronic acid (81%, intravenous [IV]), pamidronate (4.8%, IV), zoledronic acid+pamidronate (4.8%, IV), alendronate (4.8%, per os [PO]), and ibadronate (4.75%, PO). Extraction (67%) and persistent irritation of dentures (20%) were the most common triggering factors. BRONJ in the mandible was reported in 62% of the cases, in the maxilla 24%, and both 14%. BRONJ occurred more frequently in patients with multiple myelomas (n=12, 57.1%). Most of the patients revealed an advanced BRONJ stage; Stage I (n=2, 9%), Stage II (n=13, 62%), and Stage III (n=6, 29%). Conclusion: The differences of the ESR, CRP, and CTX values between the BRONJ-recurring and non-recurring patients after the treatment were not evident. Later stage BRONJ patients showed lower CTX levels. A drug holiday after the diagnosis of BRONJ did not remarkably influence the surgical outcomes. However, the limited number of patients in the study should be considered.

편평세포암종과 병발한 치성점액종 (Simultaneous occurrence of an Odontogenic Myxoma and a Squamous Cell Carcinoma of the Mandible)

  • Squamous cell carcinoma is the most common type of oral cancer and odontogenic myxoma is relatively uncommon benign tumor of mesenchymal origin. There are, to our knowledge, no prior reports of simultaneously occurring squamous cell carcinoma and odontogenic myxoma of the jaw bones. In this case, at first, the plain films and computed tomograms revealed a large expansile multilocular radiolucent lesion on left mandible and marked expansion of cortical plate. In addition this radiograms revealed also infiltrative bony destruction of anterior and medial border of ascending ramus of left mandible and alveolar bone of left maxilla, floating teeth on left lower molar area and metastatic enlargement of left submandibular, jugular digastric and spinal accessory lymphnodes. Magnetic resonance imaging of this patient revealed infiltrative growth of tumor on alveolar bone of left maxilla, left retromolar fat pad. left masseter and left medial pterygoid muscle. Intraoral presurgical biopsy presented typical features of squamous cell carcinoma. After chemotherapy with radiation therapy during 6 months. this central lesion was diagnosed as odontogenic myxoma by the postsurgical biopsy. After 3 months, this patient presented multiple metastatic signs at lumbar spines, rib and liver. Consequently, our case is simultaneous occurrence of squamous cell carcinoma and odontogenic myxoma.

저작기능이 흉쇄유돌근의 근활성도에 미치는 영향 (Electromyographic Activities of the Sternocleidomastoid Muscle during Masticatory Function)

  • The author has synchronously recorded the average electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muacles and the masseter muscles. The marious levels of occlusal force were checked during clenching the resin plates which are made to fit each of the maxillary and the mandibular teeth. These activities were recorded in order to study the EMG activity pattern of the sternocleidomastoid muscle during the masticatory function of the jaw in 11 healthy subjects. The obtined results were as follows : 1. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles were increased as the occlusal force increased, and the sternocleidomastoid muscles have a lineal correlationship with the occlusal force. 2. The sternocleidomastoid muscles and the masseter muscles showed higher EMG activity during clenching at the ventroflexed head position rather than at the extended head position. (p<0.05) However the EMG activities of the anterior temporal muscles showed no difference between the ventroflexed position and the extended position of the head. 3. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles during clenching are similar at the habitual position and at the retruded condylar position. 4. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles on the working side during mastication of gum and almonds are significantly higher than on the balancing side except the masseter muscles during mastication of almonds. (p<0.05, P<0.01) 5. The asymmetry of gum are lower than that during mastication of almonds. (p<0.05) The asymmetry indices of the sternocleidomastoid muscles are higher than those of the anterior temporal muscles and masseter muscles.

지각과민증에 대한 미세결정형 수산화인회석 함유 치약의 임상적 효과 (The effects of microcrystalline hydroxyapatite containing toothpaste in the control of tooth hypersensitivity)

  • The purpose of this study was to evaluate the effects of toothpaste containing hydroxyapatite for patients who complained dental hypersensitivity. Before baseline of application of toothpaste with hydroxyapatite, tooth brushing instruction was done respectively and the several indices were measured at baseline, 2, 4, 8 weeks. Clinical indices were estimated, and responses to cold, compressive air, tactile stimulus were evaluated with verbal rating score. Relief effects and visual analogue scale were also evaluated. The results of this study were as follows 1. The occurrence rate of hypersensitivity in upper jaw was higher than that of lower jaws, and molar area showed more hypersensitivity than premolar and incisor area. Buccal site was hypersensitive followed by interproximal and lingual site. 2. Plaque index, gingival index and probing depth reduction were gradually improved after Tooth Brushing Instruction and using toothpaste. 3. Subjects showed most sensitive response to cold stimuli than compressive air and tactile stimuli. 4. The relief effect was increased during using tooth paste and complete relief was increased especially at 8 weeks. 5. Visual analogue scale was increased. In conclusion, it was confirmed that toothpaste containing microcrystalline hydroxyapatite have the relief effect of tooth hypersensitivity. During 8 weeks, stimulus responses were decreased and hypersensitivity relief effect was increased.

7-mm-long dental implants: retrospective clinical outcomes in medically compromised patients

  • Objectives: Dental implants shorter than 8 mm, called short dental implants (SDIs), have been considered to have a lower success rate than standard length implants. But recent studies have shown that SDIs have a comparable success rate, and implant diameter was more important for implant survival than implant length. Also, SDIs have many advantages, such as no need for sinus lifting or vertical bone grafting, which may limit use in medically compromised patients. Materials and Methods: In this study, 33 patients with 47 implants 7-mm long were examined over the last four years. All patients had special medical history and were categorized into 3 groups: systemic disorders, such as diabetes mellitus (controlled or uncontrolled), mental disability, and uncontrolled hypertension; oral cancer ablation with reconstruction, with or without radiotherapy; diverse osteomyelitis, such as osteoradionecrosis and bisphosphonate-related osteonecrosis of the jaw. Most of these patients have insufficient residual bone quality due to mandible atrophy or sinus pneumatization. Results: The implant diameters were 4.0 (n=38), 4.5 (n=8), and 5.0 mm (n=1). Among the 47 implants placed, 2 implants failed before the last followup. The survival rate of 7-mm SDIs was 95.74% from stage I surgery to the last follow-up. Survival rates did not differ according to implant diameter. The mean marginal bone loss (MBL) at 3 months, 1 and 2 years was significantly higher than at implant installation, and the MBL at 1 year was also significantly higher than at 3 months. MBL at 1 and 2 years did not differ significantly. Conclusion: Within the limitations of the present study, the results indicate that SDIs provide a reliable treatment, especially for medically compromised patients, to avoid sinus lifting or vertical bone grafting. Further, long-term follow-up is needed.

Comparison of intermaxillary fixation techniques for mandibular fractures with focus on patient experience

  • Background: Intermaxillary fixation (IMF) is a technique that allows for the reduction and stabilization of mandibular fractures. Several methods of IMF, such as self-tapping screws or arch bars, have been developed. This study aimed to validate the usefulness of IMF with a self-tapping screw compared to IMF with arch bars with focus on the patients' perspective. Methods: We retrospectively reviewed the medical records of all patients who were treated for mandibular fractures at our hospital between August 2014 and February 2021. A total of 57 patients were enrolled in this study. Thirteen patients were excluded from the analysis: three patients were lost to follow-up, and 10 patients did not undergo IMF. Finally, 44 patients were analyzed, of which 31 belonged to the arch bar group, and 13 belonged to the screw group. Patient discomfort and pain during IMF application and removal were analyzed using a patient self-assessment questionnaire. The surgeon also assessed oral hygiene, IMF stability, and occlusion. Results: We applied IMF to 34 men (77%) and 10 women (23%). The mean age of the patients was 37.3 years. The most common fracture site was the angle (30%), followed by the parasymphysis (25%), the body (23%), the condyle (11%), and the ramus (11%). Patient discomfort and oral hygiene were statistically favorable in the screw group. The IMF application time was statistically shorter in the screw group (p< 0.001). IMF stability was not statistically different between the two groups. The pain score during IMF removal was lower in the screw group (p< 0.001). Conclusion: Compared to arch bars, IMF screws provide more comfort during the IMF period, help maintain favorable oral hygiene, and have a shorter application time. From the patient's perspective, IMF screws are an excellent alternative to conventional arch bars when applicable.
