The study of the consuming time and difficulties depending on the cavity types (와동 형태에 따른 소요 시간 및 난이도에 관한 연구)

  • The dental insurance had been enforced in Korea past 20 years. The major part of the insurance was filling, and the filling was divided into two parts-the cavity preparation and cavity filling, The Cavity filling was estimated into 6 levels according to the cavity types. But the tooth cavity preparations was not evaluated by the cavity types but merely only one-the one visit filling or cavity preparation. Korean dentists tend to turn away from the amalgam to the non-insurance treatments. The purpose of this study was to relate the cavity preparations and cavity fillings to the cavity types. So I measured the real time and surveyed the relative difficulties of cavity preparations from the 31 dentists according to the cavity types, who had been trained in conservative dentistry. The conclusion of this study was below. 1. The estimations of tooth cavity preparation was divided same as the cavity filling. 2. There must be reasonable considerations about the times and difficulties in cavity preparation. 3. There was differences between the premolars and molars, upper jaw and lower one. 4. Two or more cavities in one tooth must be evaluated according to the number of cavities, not in one cavity.

The Effect of Respiration and Articulator Training Programs on Basic Ability of Speech Production in Cerebral Palsy Children (호흡 및 조음기관 훈련 프로그램이 뇌성마비아동의 말 산출 기초능력에 미치는 효과)

  • Cerebral palsy children represent abnormal vocalization pattern caused by respiration problem and paralyzed oral motor muscle that are the basics of speech production. Thus, this study examined the effect of respiration and articulator training programs on the basic ability of speech production in CP children. The subjects of this study were 4 children with 3 of spastic CP and 1 of ataxia CP. The respiration and articulator program was conducted in 30 sessions for 30 minutes each. Pre-test was administered twice before the program, ongoing test was administered every 5 session during the period of experiment, and post-test was administered twice. The program included speech production such as respiration training, lips, jaw, cheek, and tongue exercise, and velopharyngeal training, and related articulator training. The following results were obtained. First, all subject children were less than 5 seconds in maximum phonation time before the experiment and 2 were improved by more than 4$\sim$5 seconds during the experiment, but 2 had relatively low rising width. Second, while children with less than 30dB before the experiment became bigger in strength during the experiment, children with more than 35dB before the experiment showed a minor change. Subject child 4 had lower vocal strength in the post-test period. Finally, although each subject had individual difference in syllable diadochokinetic ability, the function was improved and the number of repetition in one respiration was also increased.

RADIOGRAGHIC STUDY OF MARGINAL BONE LOSS AROUND OSSEOINTEGRATED IMPLANTS AFTER FUNCTIONAL LOADING (보철물 장착후 골 유착성 임플란트 변연골 소실에 관한 방사선학적 연구)

  • Regular radiographic examination has been considered as an essential diagnostic method for the osseointegrated dental implantation. This study investigated the marginal bone loss through the measurement on the periapical radiograph around 46 endosseous dental implants. Marginal bone loss have been observed for 5 years after delivery of prostheses. The results were as follows : 1. According to the experimental periods, average marginal bone loss in total implant was 1.821mm in the first year, 1.833mm in the second, 1.892mm at third, 1.910mm at forth and 1.957mm at fifth after delivery of prostheses(P<0.05). 2. The average bone loss was 1.832mm in the maxillae and 1.819mm in the mandible in three years but there was no significant difference between the upper and lower jaw(P>0.05). 3. The average bone loss was 1.824mm in males and 1.818mm in females in five years but there was no significant difference according to the sex(P>0.05). These results indicate that the most of the alveolar bone loss occur within the first year after delivery of dental prostheses resulting in the exposure of polished neck positions, and the bone level stabilizes at first thread portion of the implant fixtures.

New Record of a Bothid, Kamoharaia megastoma (Pleuronectiformes), in Southern Jejudo Island, Korea (한국산 둥글넙치과 1미기록종, Kamoharaia megastoma)

  • A single bothid specimen (113.9 mm in standard length) was collected by bottom trawl from southern Jejudo Island, Korea. It was easily identified as Kamoharaia megastoma (Kamohara, 1936), based on extremely large mouth. The species is characterized by maxillary extending beyond eyes, three pairs of long curved canines on lower jaw, tip of vomer projecting into mouth cavity, blackish pectoral fin, and blind side without lateral line. We described it as the first record to Korean fish fauna, and proposed the new Korean name, "Keun-ip-dung-geul-neop-chi" for this species.

  • Objective : Thread embedding acupuncture has become popular as a minimally invasive treatment for facial wrinkles and laxity. However, there is little published clinical practice guidelines. This study is to developing a specific procedure of thread embedding acupuncture for facial wrinkles and laxity. Method : We reviewed and summarized 6 studies on thread embedding acupuncture for facial wrinkles and laxity. And, four practitioners who have more than four year of clinical experience and one hundred of cases were participated in developing a thread embedding acupuncture procedure for facial wrinkles and laxity. Result and conclusion : We developed a thread embedding acupuncture procedure for facial wrinkles and laxity which consists of correction of lower jaw, facial laxity, nasolabial fold and eye wrinkles.

The Comparison of the Effects of Nicardipine and Remifentanil on Fluctuation of Blood Pressure during Controlled Hypotension (유도 저혈압하 양악 교정술 시 Nicardipine과 Remifentanil의 혈역학적 비교)

  • Background: Controlled hypotension is used to reduce bleeding and improve surgical conditions during surgery. Nicardipine and remifentanil have been used to induce controlled hypotension in healthy patients. This study compared controllability of nicardipine and remifentanil on hemodynamic stability during controlled hypotension. Methods: Forty healthy patients scheduled for orthognathic two jaw surgery were randomly allocated to nicardipine (group N) and remifentanil (group R) group. After induction of anesthesia, group N (n = 20) was infused with nicardipine to induce hypotension and group R (n = 20) was infused with remifentanil. Mean arterial pressure (MAP) and heart rate (HR) were measured over 5 minute interval. Surgical field rating and blood loss were measured after surgery. Results: HR was lower in group R than group N. MAP was maintained at 50-60 mmHg during controlled hypotension but the fluctuation of blood pressure and HR were more frequent in group N than group R. There were no significant differences in the surgical field rating and blood loss between the groups. Conclusions: Nicardipine and remifentanil enabled controlled hypotension and provided good surgical conditions, but the fluctuation of hemodynamic parameters was more frequent in nicardipine group.

Squamous Cell Carcinoma in an African Pigmy Hedgehog (Atelerix Albiventris) (아프리카 피그미 고슴도치에서 발생한 편평상피세포암종)

  • Squamous cell carcinoma was diagnosed in a 4.5-year-old male African hedgehog (Atelerix albiventris). The patient was referred to Seoul National University, Veterinary Medical Teaching Hospital after 5 days history of left eye exophthalmos and corneal trauma. He had enlarged upper and lower jaw at the left part of the face and endophthalmitis of the left eye. On radiographic examinations, loss of bone density at the left zygomatic arch and sun-burst type periosteal reaction of left mandible with decreased bone density was noted. Histologically, the neoplastic mass consisted of markedly invasive, cords of nests of squamous epithelial cells. Intercellular bridges and keratin pearl formation were also noted.

Identification of Trichiurus (Pisces: Trichiuridae) Eggs and Larvae from Korea, with a Taxonomic Note

  • The hairtail (currently recognized as Trichiurus lepturus in Korea) is one of the most important commercial fish species in Korea, Japan, China, and Taiwan. Because the amount of catches has been steadily declining, we must determine the early life stages of the hairtail from the viewpoint of resource management. Furthermore, the taxonomic status of the hairtail is unclear among ichthyologists, potentially creating management difficulties. Therefore, the purpose of this study was to compare morphological and molecular information on eggs, larvae, and adults of hairtail from Korea with that of T. lepturus from the Atlantic Ocean, and to review the taxonomic status of the hairtail. A total of 510 base pairs of the mitochondrial DNA cytochrome oxidase subunit I sequences of 12 eggs, 2 larvae, and 11 adults of the hairtail from the Korean waters clearly matched those of Trichiurus japonicus adults (d = 0.000-0.014) from the East China Sea rather than those of T. lepturus (d = 0.100-0.110) from the Atlantic Ocean. Our results also showed that larvae of the Korean hairtail are different than those in the Atlantic Ocean in having no melanophores along the ventral edge of the lower jaw. Therefore, our findings suggest that the hairtail in the Korean waters may not be T. lepturus, but T. japonicus.

Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year

  • PURPOSE. The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIALS AND METHODS. Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS. All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION. Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

INDIVIDUALIZED IDEAL OCCLUSAL PLANE FOR ORTHOGNATHIC SURGERY AND ORTHODONTIC TREATMENT (PRELIMINARY STUDY) (악교정수술 및 교정치료를 위한 개별화된 이상적인 교합평면에 관한 연구)

  • Introduction: To correct abnormal occlusal plane by orthognathic surgery, we need to have clear criteria for therapeutic occlusal plane. Authors introduced the concept of individualized ideal occlusal plane(Y-plane), which is determined by the size and form of the mandible, and the ideal incisor tip considering upper and lower lip. Authors studied the following to verify if the actual occlusal plane of the patients with optimal jaw relationship corresponds with the individualized ideal occlusal plane. Patients: We reviewed 44 patients who have normal occlusion visitied in the Dept. of orthodontics, Pundang CHA hospital. Methods: We evaluated if there are agreement between individualized ideal occlusal plane(Y plane) and occlusal plane of actual patients. And we confirmed if tested group has a normal face by measuring FABA, FMA, AB-LOP. Results: There were no significant differences of FABA, FMA, AB-LOP, Mo-Y plane between male and female. FABA, FMA and AB-LOP were included in the normal value. Average distance of Mo-Y plane was $0.75{\pm}0.78mm$. Conclusion: Individualized ideal occlusal plane may be applied to orthognathic surgery.
