임신 중 일개 한방병원 한방부인과에 내원한 환자에 대한 후향적 분석 (A Retrospective Analysis on Pregnant Patients Visiting Korean Gynecology Clinic of Korean Medicine Hospital)

  • Objectives: The purpose of this study was to identify the current status of Korean medicine clinical practice for pregnant patients. Methods: The study is conducted by searching medical records of 209 pregnant patients using Korean Standard Classification of Diseases related pregnancy at the Korean Gynecology Clinic of traditional Korean medicine hospital from March 1, 2016 to February 28, 2021. We retrospectively investigated characteristics, symptoms, pregnancy outcomes of the patients and treatments for the patients. Results: The most frequent symptoms of pregnant patients were musculoskeletal symptoms (60.63%) followed by neuropsychiatric symptoms (14.55%) and genital symptoms (10.82%). The most common contributory factor of the symptoms was the motor vehicle accident (74.64%). Acupuncture (94.94%) and moxibustion (90.91%) were performed to most pregnant patients. Cupping (86.60%), herbal medicine (52.63%), Chuna therapy (30.62%), and pharmacopuncture (21.05%) were performed to the patients. Normal pregnancy maintenance and delivery were reported by 147 patients (91.30%). Premature birth was reported by 11 patients (6.83%), and miscarriage was reported by 3 patients (1.86%). Conclusions: Most patients were the patients with musculoskeletal symptoms injured by motor vehicle accidents. Various Korean medicine treatments were performed during pregnancy with safety outcomes. This study could be used as basic data to provide instructions for the development of traditional Korean medicine in the obstetric clinical fields.

신경가동술의 국내 연구 동향과 임상적 활용: 주제범위 문헌고찰 (Research trends and clinical applications of neural mobilization in Korea: A scoping review)

  • Objectives The purpose of this scoping review is to investigate the trends and gaps in existing research on neural mobilization in Korea and to suggest further directions for study. Methods A scoping review was sequentially conducted according to the five steps outlined by Arksey and O'Malley, and the PRISMA-ScR checklist. We searched five domestic databases (RISS, DBpia, KISS, ScienceON, and KMBASE) and identified relevant literature reported until April 15, 2021. The key search terms used were "Neural mobilization" and "Neurodynamic". Results Of the 1383 studies identified in the search, 50 were finally selected. Of these, 45 studies were in the field of medicine and pharmacy and physical therapy as the most common sub-field. Thirty-eight of the 50 studies (76%) were randomized controlled trials. Neural mobilization was most frequently used in the treatment of stroke (n = 13) and of various neuromusculoskeletal diseases such as cervical radiculopathy (n = 8) and low back pain (n = 5). The main targets of neural mobilization were the median nerve (n = 20) and sciatic nerve (n = 14). The most commonly used technique for neural mobilization was that developed by Butler and Jones in 1991 (n = 10). Conclusions This is the first scoping review of neural mobilization in Korea. We believe that further research on neural mobilization with other types of research design is necessary to investigate the utility of neural mobilization and to establish standard protocols. Our findings indicate that neural mobilization can be considered as an intervention for neuromusculoskeletal diseases in Korean medicine.

Comparison of Abdominal Muscle Activity Between the Abdominal Bracing Technique Emphasizing Inhalation and the General Bracing Technique and Hollowing Technique

  • Objective: This study aimed to investigate the abdominal muscle activity difference while performing the abdominal bracing technique focusing on inspiration (abdominal bracing group), the general abdominal bracing technique (general bracing group), and the abdominal hollowing technique (abdominal hallowing group) Design: A cross-sectional study design. Methods: Thirty-three healthy participants were recruited for this study. The participants were allocated to 3 different groups; Abdominal bracing group, general bracing group, and abdominal hallowing group. The surface electromyography was placed over the rectus abdominis, external oblique, and internal oblique muscles to collect the activation of abdominal muscles during the trial. Results: The muscle activity of the abdominal bracing group and general bracing groups was significantly higher in all abdominal muscles than in the abdominal hollowing group (p<0.05) Both rectus abdominis and external oblique muscles showed higher muscle activations in the abdominal bracing group over the general bracing group (p<0.05). However, the ratio of bilateral external obliques and rectus abdominis to bilateral internal obliques was highest when the hollowing technique was applied (p<0.05). Conclusions: The results of study showed the abdominal bracing technique that emphasized inhalation rather than the abdominal hollowing technique or general abdominal bracing technique increased the activity of the abdominal muscles. Therefore, this study is considered to be a data for effective training if the abdominal bracing technique that emphasizes inhalation is applied as a method to increase the activation of the abdominal muscles.

Measuring Trends in the Socioeconomic Burden of Disease in Korea, 2007-2015

  • This study estimated the direct and indirect socioeconomic costs of 238 diseases and 22 injuries from a social perspective in Korea from 2007 to 2015. The socioeconomic cost of each disease group was calculated based on the Korean Standard Disease Classification System. Direct costs were estimated using health insurance claims data provided by the National Health Insurance Service. The numbers of outpatients and inpatients with the main diagnostic codes for each disease were selected as a proxy indicator for estimating patients' medical use behavior by disease. The economic burden of disease from 2007 to 2015 showed an approximately 20% increase in total costs. From 2007 to 2015, communicable diseases (including infectious, maternal, pediatric, and nutritional diseases) accounted for 8.9-12.2% of the socioeconomic burden, while non-infectious diseases accounted for 65.7-70.7% and injuries accounted for 19.1-22.8%. The top 5 diseases in terms of the socioeconomic burden were self-harm (which took the top spot for 8 years), followed by cirrhosis of the liver, liver cancer, ischemic heart disease, and upper respiratory infections in 2007. Since 2010, the economic burden of conditions such as low back pain, falls, and acute bronchitis has been included in this ranking. This study expanded the scope of calculating the burden of disease at the national level by calculating the burden of disease in Koreans by gender and disease. These findings can be used as indicators of health equality and as useful data for establishing community-centered (or customized) health promotion policies, projects, and national health policy goals.

엉덩허리근에 대한 수동 신장이 폐 기능에 미치는 영향 (The Effects of Passive Stretching of the Iliopsoas Muscles on Pulmonary Function)

  • PURPOSE: By virtue of its location, the iliopsoas muscle has the potential to affect the function of the diaphragm. Therefore, the purpose of this study was to investigate the effects of passive stretching of the iliopsoas muscles on pulmonary function. METHODS: Twenty male university students participated in this study. Subjects with low back pain, negative results on the modified Thomas test, and chest breathing patterns not directly related to the function of the iliopsoas muscles were excluded from this study. A digital pulmonary function measuring device (Pony FX, COSMED Inc, Italy) was used to test pulmonary function. The test was performed three times: the first test before passive stretching, the second test 10 minutes after the first test, and the third after passive stretching. Passive stretching was performed 5 times for each of the left and right iliopsoas muscles. Passive stretching was carried out for 20 seconds followed by a 10-second break. The SPSSWIN (ver. 27.0) statistics program was used for data analysis, and the significance level was α =.05. RESULTS: Among slow vital capacity (SVC) parameters, tidal volume (VT), inspiratory reserve volume (IRV), inspiratory capacity (IC) and vital capacity (VC) improved significantly after passive stretching. Also, among the maximal effort vital capacity (MEVC) parameters, forced vital capacity(FVC) and forced expiratory volume in one second(FEV1) improved significantly after passive stretching. CONCLUSION: The results showed that among the various pulmonary function parameters measured, passive stretching of the iliopsoas muscles can improve the SVC and MEVC. Therefore, passive stretching of the iliopsoas muscles may be considered for use as a treatment method to improve pulmonary function.

Magnetic Resonance Imaging Assessment of Paraspinal Muscles in Dogs with Intervertebral Disc Herniation

  • A decrease in the paraspinal muscle cross-sectional area (CSA) and functional cross-sectional area (FCSA) are associated with low back pain and disc herniation in humans. This study examined whether chronicity or lateralization of disc herniation affects the CSA and FCSA of the paraspinal muscles. The CSA and FCSA of the paraspinal muscles between the 12th and 13th thoracic vertebrae were measured in 31 dogs with intervertebral disc herniation (IVDH). The muscle CSA and FCSA were evaluated by dividing the values of the body weight, spinal disc CSA, and spinal canal CSA to offset the differences in body type between subjects. In the chronic IVDH group, the ratio of the paraspinal muscle CSA divided by the body weight was significantly lower, and fat infiltration in the paraspinal muscle was significantly higher than in the acute group. The lateralization of the disc herniation was significantly related to the changes in the paraspinal muscle CSA. In the right-sided disc herniation group, right epaxial muscle CSA was significantly reduced compared to the left-sided disc herniation group. The change in the paraspinal muscle might be a helpful indicator to localize less obvious disc pathologies and target the search for the pathology responsible for disc-related symptoms in dogs.

The Effects of Hip Joint Movement on the Lumbo-pelvic Muscle Activities and Pelvic Rotation During Four-point Kneeling Arm and Leg Lift Exercise in Healthy Subjects

  • Background: The gluteus maximus (GM) muscle comprise the lumbo-pelvic complex and is an important stabilizing muscle during leg extension. In patients with low back pain (LBP) with weakness of the GM, spine leads to compensatory muscle activities such as instantaneous increase of the erector spinae (ES) muscle activity. Four-point kneeling arm and leg lift (FKALL) is most common types of lumbopelvic and GM muscles strengthening exercise. We assumed that altered hip position during FKALL may increase thoraco-lumbar stabilizer like GM activity more effectively method. Objects: The purpose of this study was investigated that effects of the three exercise postures on the right-sided GM, internal oblique (IO), external oblique (EO), and multifidus (MF) muscle activities and pelvic kinematic during FKALL. Methods: Twenty eight healthy individuals participated in this study. The exercises were performed three conditions of FKALL (pure FKALL, FKALL with 120° hip flexion of the supporting leg, FKALL with 30° hip abduction of the lifted leg). Participants performed FKALL exercises three times each condition, and motion sensor used to measure pelvic tilt and rotation angle. Results: This study demonstrated that no significant change in pelvic angle during hip movement in the FKALL (p > 0.05). However, the MF and GM muscle activities in FKALL with hip flexion and hip abduction is greater than pure FKALL position (p < 0.001). Conclusion: Our finding suggests that change the posture of the hip joint to facilitate GM muscle activation during trunk stabilization exercises such as the FKALL.

  • ■ Objectives The purpose of this case study is to report that a tremor patient after Acute Cerebral infarction was treated with 'Dodamhwalhul-tang' and then the symptoms were improved. ■ Methods This patient was 81 years old woman whowas diagnosed with acute cerebral infarction. This patient had hypertension and symptoms of tremor on left lower limb, both side weakness, both lower limb numbness, dizziness and low back pain. She was treated with herb medicines, acupuncture, moxibustion during 22 days of hospitalization. Her tremor was evaluated by the tremor frequency and period and other general conditions were measured. ■ Results After taking Dodamhwalhul-tang, symtoms of tremor and patient's general conditions were improved. ■ Conclusion The result shows that Dodamhwalhul-tang is effective for tremor with acute cerebral infarction.

Occupational Characteristics and Health Status of Vietnamese Male Migrant Workers in the Republic of Korea

  • Background: The objective of this study is to identify the working conditions and health status of Vietnamese male migrant workers in Republic of Korea, in comparison to the Korean general population. Methods: We conducted our survey through the Migrant People Center, and we received completed questionnaires from 87 male Vietnamese migrant workers. The questionnaire employed was identical to those used in the Korean Working Conditions Survey and the 2020 Korea National Health and Nutrition Examination Survey. The collected data from the Vietnamese migrant workers was then compared with the Korean reference population using indirect age-standardization. Results: Vietnamese male workers demonstrated a higher prevalence of health problems including hearing problems (age-standardized prevalence ratio (aSPR) 13.22, 95% confidence interval [CI]: 8.07-20.4), skin problems (aSPR 13.49, 95% CI: 8.07-20.4), and low back pain (aSPR 8.40, 95% CI: 6.50-10.69). Elevated exposure to workplace hazards such as chemicals (aSPR 2.36, 95% CI: 1.51-3.51), organic solvents (aSPR 2.22, 95% CI: 1.44-3.28), handling of heavy objects (aSPR 1.67, 95% CI: 1.24-2.21), and high temperatures (aSPR 1.96, 95% CI: 1.46-2.57) was observed among them. Additionally, they faced a higher risk of no personal protective equipment (aSPR 2.53, 95% CI: 1.26-4.52) and a greater prevalence of unmet medical needs (aSPR 7.14, 95% CI: 4.74-10.32). Conclusion: Our findings highlight the elevated workplace hazards, health problems, and unmet medical needs among Vietnamese male workers compared to the Korean reference population. These findings underscores the urgency for enhanced scrutiny over working conditions and protective equipment provision, coupled with efforts to improve healthcare accessibility and worker education.

분심기음을 포함한 한방치료로 호전된 섬유근육통 환자의 불면 치험 1례 (A Case Report of a Patient with Insomnia Related To Fibromyalgia Treated with Korean Medicine, including Bunshimgi-eum)

  • In this case report, we describe the effects of Korean Medicine therapies, including Bunshimgi-eum, on a patient hospitalized with insomnia due to fibromyalgia in a Korean Medicine Hospital. We treated the patient with Bunshimgi-eum for 21 of 27 hospital days. Post-treatment, we used the Insomnia Severity Index (ISI) to measure the severity of insomnia. To determine the severity of multiple pain, including neck, low back, Lt. shoulder, Lt. elbow, Rt. knee, and hand, we used the Numerical Rating Scale (NRS). For evaluation of generic health status, we used the European Quality of Life-5 Dimensions (EQ-5D) scale. After the treatment, the patient's clinical symptoms improved, according to the ISI, NRS, and EQ-5D. The results of this case study suggest that Korean Medicine therapies, including Bunshimgi-eum, may have positive effects as a treatment for insomnia related to fibromyalgia.
