눈썹거상술 후 발생한 만성 통증에 대한 수술적 치험례 1례 (A Case Report of Surgical Treatment for Relief of Intractable Pain Developed after Browlift Surgery)

  • Purpose: Nerve injury is one of the complication which can develop after brow lift. Peripheral nerve ending which is stretched from supraorbital nerve and supratrochlear nerve can be injured and symptoms such as pain, dysesthesia may appear. Usually, developed pain disappeared spontaneously and does not go on chronic way. We experienced a case that a patient complained chronic pain after brow lift which was not controlled by conservative management such as medications, local nerve block and report a successful surgical treatment of chronic pain after brow lift. Methods: A 24-year-old male who received brow lift with hairline incision at local hospital was admitted for chronic pain at the right forehead. The pain was continued for 3 months even though fixed thread was removed. Local nerve block at trigger point with mixed 1 mL 2% lidocaine and 1 mL Triamcinolone acetonide was done and oral medications, Gabapentine and carbamazepine, were also applied but there was no difference in the degree of pain. Therefore the operation was performed so that careful dissection was carried out at right supraorbital neurovascular bundle and adhered supraorbital nerve was released from surrounding tissues and covered with silastic sheet to prevent adhesion. Results: The pain was gradually relieved for a week. The patient was discharged without complications. No evidence of recurrence has been observed for 2 years. Conclusion: The pain developed after brow lift was engaged with nerve injury and sometimes remains chronically. Many kinds of conservative management to treat this complication such as medications, local nerve block have been reported and usually been used. But there are some chronic cases that conservative treatment do not work. In sum, we report 1 case of successful surgical treatment for relief of intractable pain developed after brow lift surgery.

Improvement of Congenital Muscular Torticollis with Mild Symptoms in Non-Treated Adult after Simple Surgical Myotomy of Sternocleidomastoid Muscle under Local Anesthesia

  • In adult congenital muscular torticollis (CMT) patients, physical therapy is not as effective because the development of sternocleidomastoid muscle (SCM) muscle is complete. While surgical release can address CMT in adult patients, the risk of general anesthesia and visible postoperative scar is a concern, expecially in patients with mild symptoms. In this paper, we report our experience in treating such patients with minimal-incision myotomy under local anesthesia. A review was performed for all adult patients who had undergone the simple myotomy procedure. Surgical indication was reserved for patients with mild fibrotic band in the SCM muscle with minimal lengthdiscrepancybetween the muscles. All patients had recognizable head tiltand palpation of fibrotic band on affected side of the neck. Surgical details are described in the main body of text. Three female patients had undergone the procedure. Torticollis was resolve in all patients with complete restoration of ranage of motion. There were no postoperative complications, and patient satisfaction was high. We have reported three cases of mild CMT in adult female patients, who had undergone minimal-incision myotomy under local anesthesia. Outcomes were satisafactory with no morbidity to report. With careful patient selection, this method offers an alternate treatment option for adult CMT patients with mild symptoms.

무가선 산악트램 급전을 위한 지상 집중식 급전장치 개발 (Development of Local Ground Pantograph for Power Supply to Wireless Mountain Trams)

  • 국내 산악 지역 자연공원에서는 자연 환경과 주변 조망을 보존하기 위해 전차선의 설치가 불가하다. 따라서 산악 트램은 무가선으로 운행되어야 한다. 이를 위해 본 연구에서는 차량의 추진 배터리에 전기를 공급할 수 있는 지상 집중식 급전장치를 개발하고 동작 및 기능 시험을 실시하여 성능을 검증하였다. 개발한 지상 집중식 급전장치는 검수고 또는 정거장의 한 지점에 고정 설치되어 있으며, 차량의 대차가 진입하여 정차하면 스프링 장력으로 암과 집전 슈를 올려 대차 하부에 있는 차량측 집전 바와 접촉하고, 전기를 흘려 배터리에 전력을 공급한다. 지상 집중식이므로 차량 지붕 위의 집전장치와 전차선이 필요 없게 되고, 산악트램은 무가선 상태로 운행을 할 수 있다. 정거장이나 검수고에서 움직이지 않고 정지한 상태에서 집전하므로, 이선이나 아크 발생이 없다. 집전 슈의 마모나 손상을 초래할 수 있는 가동 접촉면이 사라지게 되므로 집전장치 수명이 연장되는 장점이 있다. 시험 결과에 따르면, 절연저항은 기준치인 10㏁ 이상이었고, 전류 335A를 1시간 동안 일정하게 공급하여도 이상 발열이 없음을 확인하였다.

Effect of Molasses Addition Level to the Mixture of Calf Starter and Corn Fodder on Pellet Quality, Rumen Development and Performance of Holstein-Friesian Calves in Indonesia

  • Effect of molasses addition to complete calf starter (CCS) and the following pellet processing was evaluated with 12 Holstein-Friesian (HF) calves (7-14 d old with $42\;{\pm}\;5.5\;kg$ average BW), with 6 calves (replicate) per treatment. The CCS was formulated with 65% concentrate calf starter and 35% local corn fodder. On the CCS, molasses was added either 5% (M5) or 10% (M10), then the mixture was pelleted and fed to the calves for 7 weeks. Molasses addition, regardless of addition level, improved (p<0.05) both the durability and hardness of the pellet which contains 35% of high fibrous local corn fodder. Upon feeding to calves, the feed intake and daily gain were numerically higher with 5% molasses addition compared to 10% molasses addition. Blood VFA level was remarkably higher (p<0.01) in calves fed M10 than calves fed M5. There was no difference (p>0.05) in blood glucose level between M5 and M10. Length and number of papillae were not different (p>0.05) by the addition levels of molasses. However, there was one exception in number of papillae at caudo-dorsal sac which were higher (p<0.05) number in M5 than M10 calves. Considering the quality of CCS pellet, calf performance and rumen development, there was no additional benefit of 10% molasses addition compared to 5%. From the results of this study, 5% molasses addition could be recommended for local farmer when they utilize local corn fodder to make CCS for HF calves in Indonesia.

고도에 따른 지역별 복숭아혹진딧물 집단 변이 마커 개발 (Development of Variation Marker of Myzus persicae by Altitude)

  • 기후변화에 따른 고도별 곤충의 유전 변이를 분석하고자 배추에서 복숭아혹진딧물(Myzus persicae)을 지표해충으로 변이 분석이 가능한 마커를 개발하고자 하였다. 즉, 지역의 기후가 변하면 유전자가 변동할 것이라는 가설 하에 실내에서 누대 사육된 계통을 비교집단으로 가정하고 횡성, 평창과 강릉 지역에서 고도별 5개 지역(157 m, 296 m, 560 m, 756 m, 932 m)에서 채집된 지역 집단간의 변이를 찾고자 하였다. 생태적 변이는 모든 집단에서 찾을 수가 없었고, 에스터레이즈 동위효소 분석 및 inter-simple sequence repeat (ISSR) PCR 결과에서 실내 사육 계통과 지역 집단간의 차이는 보였으나 지역 집단간의 큰 차이는 찾을 수가 없었다. 그러나 rRNA와 mtCO I 부분 염기서열을 분석하여 5개의 지역집단 내에서 internal transcribed spacer-2 (ITS-2) 와 mtCO I에 각각 한 개씩의 단일염기다형성(SNP)를 발견하였다. 이 SNP는 유전 변동 추적이 가능한 매우 유용한 마커로 사용 될 수 있다.

비파엽약침이 고지방식이 유발 비만 생쥐의 국소부위지방에 미치는 영향 (The Efficacy of Eriobotryae Folium Pharmacopuncture on Local Fat of High Fat Diet Induced Obesity Mice)

  • Objectives : This study was designed to investigate the anti-obesity effects in local area adipose tissue using by pharmacopuncture of Eriobotryae Folium on mice fed high fat diet. Methods : Male ICR mouse were divided into three groups. Normal diet group (N), High-fat diet(HFD) group, HFD+E(pharmacopuncture of the Eriobotryae Folium) group for 8 weeks. HFD+E group was injected in the concentration of 50 mg/kg once a day for 4 weeks and every other day during the next 4 weeks. We measured body weight, food intake, absolute weight and relative weight in the liver and adipose tissue. Also, we analyzed hematological levels and histopathological changes on adipose tissue. Results : 1. We observed no difference in food intake among three groups. 2. HFD+E group significantly decreased body weight gain and absolute weight and relative weight in the liver and adipose tissue compared with HFD group. 3. Blood AST and ALT were no differences among the experiment groups. However, HFD group was significantly increased higher TG, TC, HDL and LDL than in N group and HDL/LDL ratio was remarkably decreased. On the other hand, HFD+E group was significantly decreased in obesity markers and HDL/LDL ratio was remarkably increased. 4. Levels of adiponectin in serum of HFD+E group was remarkably increased compared with those of HFD group. 5. HFD group was significantly decreased compared with N group in adipocyte number. Compared with the HFD group, HFD+E group was displayed a significant increase by adipocyte number. Conclusions : We suggest that pharmacopuncture of the Leaves of Eriobotrya japonica can play the anti-obesity effects in local area adipose tissue.

일반은행의 시스템적 경영효율성 평가 (A Systematic Evaluation on the Management Efficiency of General Bank)

  • 본 연구에서는 일반은행의 경영효율성 변화에 대한 분석을 하였다. DEA를 통하여 은행별 효율성을 규명한 후, 상대적으로 효율적인 은행과 비효율적인 은행으로 구분하였다. 비효율적인 은행에 대해서는 비효율적인 부문 및 정도를 파악하고자 하였으며, 효율성 개선방안의 제시와 BCC모형으로 은행의 규모가 경영효율성에 미치는 영향도 분석되어졌다. 또한, 시중은행과 지방은행간 효율성에 차이가 나타나는 가를 검증하였다. 분석 결과 시중은행과 지방은행 모두 2000년 이후 지속적인 경영효율성 개선을 보여주고 있으며, 시중은행의 경영효율성이 2000년을 제외하고는 지방은행보다 높은 것으로 나타났다. 시중은행과 지방은행간 자기자본비율을 제외한 검정변수에 있어서는 차이가 존재하는 것으로 나타났으며, 경영효율성 차이에서는 별다른 차이가 없음을 보여주고 있다. 규모가 경영효율성에 미치는 영향에서는 구조조정과 합병에 의한 규모의 효과가 없거나 오히려 부의 영향을 미치는 것으로 나타나고 있다.

Clinical efficacy of 0.75% ropivacaine vs. 2% lignocaine hydrochloride with adrenaline (1:80,000) in patients undergoing removal of bilateral maxillary third molars: a randomized controlled trial

  • Background: Lignocaine with adrenaline is routinely used as a local anesthetic for dental procedures. Adrenaline was added to increase the duration of anesthesia. However, epinephrine containing a local anesthetic solution is not recommended in conditions such as advanced cardiovascular diseases and hyperthyroidism. Recently, ropivacaine has gained popularity as a long-acting anesthetic with superior outcomes. The goal of this study was to assess and compare the effectiveness of 0.75% ropivacaine alone and 2% lignocaine with adrenaline (1:80,000) in the removal of bilateral maxillary wisdom teeth using the posterior superior alveolar nerve block technique. Methods: This was a single-blind, randomized, split-mouth, prospective study assessing 15 systemically sound outpatients who needed bilateral removal of maxillary third molars. We randomly allocated the sides and sequences of ropivacaine and lignocaine with adrenaline administration. We evaluated the efficacy of both anesthetics with regard to the onset of anesthesia, intensity of pain, variation in heart rate, and blood pressure. Results: The onset of anesthesia was faster with lignocaine (138 s) than with ropivacaine (168 s), with insignificant differences (p = 0.001). There was no need for additional local anesthetics in the ropivacaine group, while in the lignocaine with adrenaline group, 2 (13.3%) patients required additional anesthesia. Adequate intraoperative anesthesia was provided by ropivacaine and lignocaine solutions. No significant difference was observed in the perioperative variation in blood pressure and heart rate. Conclusion: Ropivacaine (0.75%) is a safe and an adrenaline-free local anesthetic option for posterior superior alveolar nerve block, which provides adequate intraoperative anesthesia and a stable hemodynamic profile for the removal of the maxillary third molar.

노인일자리 지원을 위한 기초자치단체 조례 분석 : 서울특별시의 자치구 조례를 중심으로 (Analysis of Basic Local Government Ordinance for Supporting Senior Employment : Focused on the Autonomous Ordinances of Seoul City)

  • 본 연구는 노인일자리 지원조례의 형식과 내용을 분석하여 해당 조례가 형식적 타당성과 실효성을 갖고 있는지를 확인하는데 목적이 있다. 이를 위해 서울특별시 내 자치구의 조례 13건을 분석대상으로 선정하여 각 조례의 내용과 구성을 분석하였다. 분석결과 조례의 제정목적과 사업내용 등 주요한 형식구성에는 큰 차이가 발견되지 않았다. 그러나 정책 적용대상 범위와 전달체계 관련 조항 유무에는 각 조례별 차이가 확인되었다. 특히 서비스에 대한 주민의 권리는 모든 조례에서 존재하지 않았으며 노인일자리 제공에 대한 자치단체장의 의무는 그 구체성이 부족하여 개선이 필요하였다. 실효성 측면에서는 전달체계 관련 조항이 없거나 규정의 강행적 성격이 부족하였으며 위원회 구성과 권리구제 관련 조항은 전반적으로 존재하지 않았다. 본 연구에서는 이 같은 분석을 기반으로 조례의 개선 보완 방향을 제안하였다.

겨울철 도시지역 대기 수용성 에어로졸 입자의 크기 분포를 결정하는 주요 인자 (Major factors determining the size distributions of atmospheric water-soluble aerosol particles at an urban site during winter)

  • Size distributions of atmospheric particulate matter (PM) and its water-soluble organic and inorganic components were measured between January and February 2021 at an urban site in Gwangju in order to identify the major factors that determine their size distributions. Their size distributions during the study period were mainly divided into two groups. In the first group, PM, NO3-, SO42-, NH4+ and water-soluble organic carbon (WSOC) exhibited bi-modal size distributions with a dominant condensation mode at a particle size of 0.32 ㎛. This group was dominated by local production of secondary water-soluble components under atmospheric stagnation and low relative humidity (RH) conditions, rather than long-range transportation of aerosol particles from China. On the other hand, in the second group, they showed tri-modal size distributions with a very pronounced droplet mode at a diameter of 1.0 ㎛. These size distributions were attributable to the local generation and accumulation of secondary aerosol particles under atmospheric conditions such as atmospheric stagnation and high RH, and an increase in the influx of atmospheric aerosol particles by long-distance transportation abroad. Contributions of droplet mode NO3-, SO42-, NH4+ and WSOC to fine particles in the second group were significantly higher than those in the first group period. However, their condensation mode contributions were about two-fold higher in the first group than in the second group. The significant difference in the size distribution of the accumulation mode of the WSOC and secondary ionic components between the two groups was due to the influx of aerosol particles with a long residence time by long-distance transport from China and local weather conditions (e.g., RH).
