This study has been done with 1,431 subjects who visited Health Promotion Centers of the hospitals in Gumi for National Health Screening Program for People at Transitional Ages from April to December 2007. Serum biochemical tests related with metabolic syndrome were performed. Among biochemical factors related with metabolic syndrome, the mean values of serum glucose, AST, ALT, triglyceride and HDL cholesterol except LDL cholesterol were significantly higher in males than in females, so a significant difference by sex was observed (P<0.001). AST, ALT, triglyceride and HDL were thought to be significantly affecting serum GGT for males. In contrast, ALT and HDL cholesterol were important factors for females (P<0.001). For both sexes, serum glucose and LDL cholesterol did not produce any meaningful effect on serum GGT. In males AST, ALT and HDL cholesterol were associated with high risk of abnormality of serum GGT and in females AST, ALT and LDL cholesterol were related with high risk of abnormality of serum GGT. Therefore, AST and ALT showed a significant effect on abnormality of serum GGT in both males and females. It was observed that males exhibited significantly high correlation between metabolic syndrome related biochemical factors and serum GGT than females, and their influence on abnormality of serum GGT was also higher in males than in females. Therefore, serum GGT tests performed for health screening are considered to be useful for managements of cardiovascular diseases and metabolic syndrome as well as liver function test.
Objective: We conducted this study to report the clinical manifestations of integrative treatment with Korean and conventional medicines in a patient with acute hepatitis A after conventional medicine therapy for chronic obstructive pulmonary disease and pneumonia. Methods: A patient was treated with herbal medicine, western medicine, and moxibustion. The liver function tests (GOT, GPT, ALP, ${\gamma}-GT$), total bilirubin, and 4-scale scoring (none, mild, moderate, and severe) of symptoms were used to assess any improvement in symptoms. Results and Conclusion: The symptoms at the time of admission were improved. The levels of GOT, GPT, ALP, ${\gamma}-GT$, and total bilirubin were decreased in laboratory tests. We obtained positive effects for the integrative treatment of acute hepatitis A.
Background Extensive research has been conducted on islet transplantation as a possible cure for diabetes. Islet transplantation in the liver via the portal vein has shown remarkable results, but numerous other recipient sites are currently being investigated. We aimed to show the effectiveness of using a muscle flap as a recipient site for islet transplantation. Methods Islet cells were harvested from 12 isogenic Lewis rats, and then diabetes was induced in another 12 isogenic Lewis rats by streptozotocin injection. In six rats, 3,000 islets were transplanted into gastrocnemius muscle flaps, and in the other six rats, the same number of islets were transplanted into the gastrocnemius muscle. The transplanted islet cell function between the two groups was compared by means of blood glucose tests, glucose tolerance tests, immunohistochemistry, and real-time reverse transcription polymerase chain reaction. Results In the muscle flap group, blood glucose levels significantly decreased after islet transplantation. Blood glucose levels were significantly different between the two groups at 3 weeks after transplantation. The muscle flap group showed nearly normoglycemic results upon the glucose tolerance test, whereas the muscle group was hyperglycemic. Immunohistochemical evaluation showed positive results against insulin and glucagon in biopsies of both groups, and the islet cell density was higher in the muscle flap group. There were no statistically significant differences between the two groups in real-time reverse transcription polymerase chain reaction results. Conclusions Our results suggest that muscle flaps are promising candidates for islet cell transplantation.

In this study, in an attempt to elucidate the relevance and the association between AUDIT level and health related indices such as obesity-related indices (BMI, waist circumference, body fat percentage), serum liver-enzyme levels (AST, ALT, GGT, total bilirubin) and serum lipid levels (TC, TG, HDL-C, LDL-C), data about including results of health checkup examination and questionnaire from 558 male examinees of 30-79 years old, who received a comprehensive health examination in the health promotion center of a university hospital during the period of January 1 to December 31, 2012, was analysed. As a results, the distribution of AUDIT rating of subjects was Zone I 58.2%, Zone II 25.3%, Zone III 7.0%, and Zone IV 9.5%. And AUDIT levels have significant relevance with age, smoking, body fat, GGT and TG. Especially, for the waist circumference, GGT and TG, the odds ratio indicating abnormality significantly increased in the higher AUDIT groups (Zone II~Zone IV) than a low-grade group (Zone I). This suggests that drinking problem with a high AUDIT level has a significant relevance with an abnormal increase of indices for obesity, serum liver-enzyme levels, serum lipid levels.

Background: Hepatitis B and C are the leading causes of liver diseases worldwide. For hematological and solid malignancy patients undergoing chemotherapy, increases in HBV DNA and HCV RNA levels can be detected which may result in reactivation and hepatitis-related morbidity and mortality. The aim of this study was to determine the seroprevalence of Hbs ag and Anti HCV positivity in patients with solid malignancies undergoing chemotherapy and consequences during follow-up. Materials and Methods: The files of 914 patients with solid malignancies whose hepatitis markers were determined serologically at diagnosis were reviewed retrospectively. All underwent adjuvant/palliative chemotherapy. For the cases with HBV and/or HCV positivity, HBV DNA and HCV RNA levels, liver function tests at diagnosis and during follow-up and the treatment modalities that were chosen were determined. Results: Of 914 cases, Hbs Ag, anti Hbs and anti HCV positivity were detected in 40 (4.4%), 336 (36.8%) and 26 (2.8%) of the cases respectively. All of the Hbs ag positive patients received prophylactic lamuvidine before the start of chemotherapy. In the Hbs ag and anti HCV positive cases, liver failure was not detected during chemotherapy and a delay in chemotherapy courses because of hepatitis was not encountered. Conclusions: Just as with hematological malignancies, screening for HBV and HCV should also be considered for patients with solid tumors undergoing chemotherapy. Prophylactic antiviral therapy for HBV reduces both the reactivation rates and HBV related mortality and morbidity. The clinical impact of HCV infection on patients undergoing chemotherapy is still not well characterized.

Purpose: Infantile hepatic hemangioendothelioma (IHHE) is the most common type of hepatic vascular tumor in infancy. We conducted this study to review our clinical experience of patients with IHHE and to suggest management strategies. Methods: We retrospectively analyzed the medical records of 23 IHHE patients (10 males, 13 females) treated at the Asan Medical Center between 1996 and 2009. Results: Median age at diagnosis was 38 days (range, 1 to 381 days). Seven patients (30%) were diagnosed with IHHE based on sonographically detected fetal liver masses, 5 (22%) were diagnosed incidentally in the absence of symptoms, 5 (22%) had congestive heart failure, 3 (13%) had skin hemangiomas, 2 (9%) had abnormal liver function tests, and 1 (4%) had hepatomegaly. All diagnoses were based on imaging results, and were confirmed in three patients by histopathology analysis. Six patients were observed without receiving any treatment, whereas 12 received corticosteroids and/or interferonalpha. One patient with congestive heart failure and a resectable unilobar tumor underwent surgical resection. Three patients with congestive heart failure and unresectable tumors were managed by hepatic artery embolization with/without medical treatment. At a median follow-up of 29 months (range, 1 to 156 months), 21 (91%) patients showed complete tumor disappearance or >50% decrease in tumor size. One patient died due to tumor-related causes. Conclusion: IHHE generally has a benign clinical course with low morbidity and mortality rates. Clinical course and treatment outcome did not differ significantly between medically treated and non-treated groups. Surgically unresectable patients with significant symptoms may be treated medically or with hepatic artery embolization.
Development of antiviral resistance to lamivudine is the most important factor for the treatment failure. It is necessary to establish proper guidelines to overcome drug resistance for children with chronic hepatitis B. Primary treatment with lamivudine should be considered if patients are in immune-clearance phase and have persistently elevated ALT levels more than twice the upper limit of normal value. Before initiating the therapy, careful consideration of the patient's status is required to exclude abnormal liver function tests due to other causes. The treatment option should be carefully decided to suppress the viral replication effectively. To obtain good compliance, clinicians should educate patients and their parents. Appropriate monitoring for virologic breakthrough and genotypic resistance is important in deciding to change the treatment plan. Sequential monotherapy should be avoided and a combination of drugs in other categories is recommended. New antiviral agents, such as entecavir and tenofovir, which have high potency and high genetic barrier, are soon expected to be available for use with children.

A study on the occurrence of hepatitis B has been done for 2,680 employees in Daehan Life Insurance Company. The positive rates of HBsAg and anti-HBs were 6.56% and 18.9% respectively. The positive rates of HBsAg and anti-HBs were higher by 2.25% and 2.06% respectively in male than in female. HBsAg and anti-HBs positives were higher at age of 30 to 50 years than other age groups. The drinker group shows higher positive of HBsAg and anti-HBs by 10.44% and 4.26% respectively than non-drinker group. Most of the results of liver function tests in HBsAg positive cases were within reference ranges. HBsAg or anti-HBs positive rate in the family members of HBsAg or anti-HBs positive employees was higher than the over-all positive rates of all subjects, by 14.55% in case of HBsAg and by 2.21% in case of Anti-HBs.
Although halothane is one of the most widely used inhalation anesthetics, it may cause postanesthetic complications such as halothane hepatitis. Halothane hepatitis has been reported intermittentely with variable incidence. However it is not easy to prove halothane as a causative agent, because there are many factors causing postoperative hepatic dysfunction. The author had a case of acute hepatitis developing after open heart surgery used halothane. 37-year-old female underwent an open heart surgery for ASD repair under halothane anesthesia On the 14th postoperative day, she developed high fever of 38 C. Liver function tests showed marked elevation of SGOT, SGPT, and bilirubin, followed by gross jaundice. HB, Ag(-) and HB, Ab(+) were reported. She died of acute respiratory, hepatic, and renal failure on the 19th postoperative day Possible causes of the hepatitis were considered halothane, blood transfusion, and drugs.
