MMR(Measles-Mumps-Rubella) 약독화 생백신인 프리오릭스주를 접종한 후 안전성과 유효성의 평가에 관한 연구 (The Safety and Immunogenicity of a Trivalent, Live, Attenuated MMR Vaccine, PriorixTM )

  • 목 적 : 본 연구는 한국인 소아를 대상으로 MMR 약독화 생백신인 프리오릭스주를 단독 접종한 후 공개, 다 기관, 비비교 시험으로 프리오릭스주의 안전성과 면역성 평가를 목적으로 수행되었다. 방 법 : 2002년 7월부터 2003년 2월까지 한일병원, 경희대학교 부속병원, 가톨릭대학교 성바오로병원, 고려대의과대학부속 안산병원 등 4개의 병원에 예방 접종을 위해 내원한 12-15개월 또는 4-6세의 건강한 남녀 소아 252명을 대상으로 하였으며, 이중에서 임상시험계획서의 모든 기준을 충족하는 피험자를 최종 분석대상군에 포함시켰다. 접종 후 이상 반응 및 중대한 이상 반응에 대하여 42일간의 추적관찰을 통하여 증례기록서의 이상 반응란에 기록하였다. 각 피험자들은 시험백신 투여 직전과 투여 후 42일 후 혈액검체를 채취하여 벨기에 GlaxoSmithKline Biologicals 실험실에서 면역분석법을 통하여 항체가를 측정하였다. 결 과 : 등록된 피험자 총 252명 중 최종적으로 12-15개월군에서 103명, 4-6세군에서 96명으로 총 199명이 분석대상으로 선정되었다. 본 임상시험 기간 동안 발생한 이상반응 중 시험약물과 관련 있는 국소 반응은 10.1% 전신 반응은 6.5%에 불과하였으며, 우려하였던 이상 반응, 예를 들면 무균성 뇌막염, 열성 경련 등은 발생하지 않았으며, 시험백신과 관련된 중대한 이상 반응은 없었다. 유효성 분석에서, 항체 음성인 피험자들의 혈청전환율은 홍역인 경우 99%, 유행성 이하선염인 경우 93%, 풍진인 경우 100%이었다. 시험백신 투여 전 항체 음성인 피험자군의 혈청전환시 항체가의 기하평균은, 홍역의 경우 12-15개월에 속하는 피험자군에서는 $383{\pm}8.6mIU/mL$, 4-6세에 속하는 피험자군에서는 $188{\pm}6.2mIU/mL$, 유행성 이하선염의 경우 12-15개월에 속하는 피험자군에서는 $95{\pm}6.3U/mL$, 4-6세에 속하는 피험자군에서는 $247{\pm}3.8U/mL$, 풍진의 경우 12-15개월에 속하는 피험자군에서는 $94.5IU/m{\pm}$, 4-6세에 속하는 피험자군에서는 $16{\pm}8.9IU/mL$을 보였다. 결 론 : 12-15개월 또는 4-6세의 건강한 소아를 대상으로 프리오릭스주를 접종하였을 때, 이와 관련된 이상 반응은 대부분의 백신 접종 후 나타날 수 있는 일반 반응에 한정된 것으로 나타나 한국인 소아에게서도 안전하게 사용될 수 있으며 또한 매우 높은 항체양전율 및 항체가의 기하평균치를 보여 우수한 면역효과를 기대할 수 있는 백신으로 사료된다.

면역기능이 정상인 영아에서 동경주 BCG 백신 접종 후 발생한 파종성 Mycobacterium bovis 감염 1예 (A Case of Disseminated Mycobacterium bovis Infection after BCG Vaccination (Tokyo strain) in an Apparently Immunocompetent Infant)

  • BCG는 M. bovis로부터 유래된 생백신으로 5-6세 미만의 소아 결핵 특히 결핵성 수막염이나 속립성 결핵과 같은 중증 결핵의 예방 효과가 인정되고 있다. BCG 접종 후 이상반응은 국소 궤양 형성, 국소 림프절염이 가장 흔하며, 드물지만 골수염, 파종성 감염과 같은 치명적인 합병증도 있다. 파종성 BCG 감염은 주로 면역저하 환자에게서 흔하게 관찰되며 면역 저하가 없는 소아에게서 발생한 파종성 BCG 감염은 국내 문헌상 보고가 없었다. 저자들은 13개월 여환이 BCG 접종 5개월 뒤 발생한 발열, 피부 결절 및 다발성 림프절 종대를 주소로 내원하여, 백 신 접종 부위 외에 2군데에서 M. bovis BCG를 확인한 후 파종성 BCG 감염으로 진단된 1례를 경험하였다. 환아는 isoniazid와 rifampin으로 치료받았으며 약 2주 간의 입원 치료 후 임상 증상이 호전되어 퇴원하였다. 3주뒤 외래 추적 관찰에서 역설적 반응으로 생각되는 림프 절 종대와 피부 결절이 진행하였으나 동일 약제를 유지하면서도 다시 임상 증상이 호전되었고 환아의 면역학적 검사상 이상 소견이 관찰되지 않았기에 이를 보고하는 바이다.

백신접종 방법에 따른 뉴캣슬병 $B_1$생독백신의 효능비교 (Comparison of Efficacy of Newcastle Disease Virus $B_1$ Vaccine by Different Administration .Methods in Commercial Layer Chickens)

  • 실용 산란계 병아리에 대해서 시중에서 유통되고 있는 B$_1$생독 뉴캣슬병백신의 효능을 접종방법과 접종횟수를 달리하여 비교하였다. 실용산란계 372수를 93수씩 4군으로 나눈 후 3군에 대해서는 각각 음수. 점안, 분무방법으로 28일령까지 3회에 걸쳐 백신을 접종하였고 나머지 1군은 백신을 접종하지 않은 대조군으로 하였다. 매회 백신접종 2준 4주 후에 강독 바이러스로 공격접종하여 방어율을 조사하고 2주 간격으로 채혈하여, 혈구응집억제(HI) 항체가를 조차하였다. 방어율은 1일령에 1회 접종하는 것보다 2회 또는 3회 접종하는 것이 높았으나 2회. 3회간에는 큰 차이가 없었다. 접종방법간에는 분무방법이 2회 이상 접종할 경우 방어율이 높았으며 다음은 점안, 음수 순이었다. 백신을 2회 또는 3회 접종하여도 HI항체가는 대조군에 비해 크게 상승하지 않았다. 전반적으로 방어율이 낮은 것은 백신의 역가가 낮기 때문으로 생각되었다. 공격접종 후 뉴캣슬병 임상증상은 84% 이상에서 원기 및 식욕저하와 기면을 보였고 27%에서 설사, 15%에서 호홉곤란, 10%에서는 사경을 보였다. 부견소견상 출혈반은 십이지장(51%), 기관(36%), 회장(13%), 맹장편도(11%), 선위(10%) 순으로 관찰되었다.

급성폐혈증형 돼지단독예에서 분리한 Erysipelothrix rhusiopathiae의 혈청형 및 병원성 (Serotype and pathogenicity of Erysipelothrix rhusiopathiae isolated from the cases of acute septicemic swine erysipelas)

  • The serotypes and pathogenicity of 47 isolates of Erysipelothrix rhusiopathiae from pigs with acute septicemic swine erysipelas were investigated. The serotypes were determiend by agar gel precipitation test using autoclave extracted antigens. All of the 47 isolates(42 strains from Youngnam and fi fotrains from Kyunggi province) were serotype lao Five field isolates of Erhusiopathiae serovar la were tested for pathogenicity in mice. All of 5 selected isolates were pathogenic for mice and $LD_{50}$ was $3.7{\times}10^3$ viable cells. Mice immunized subcutaneously with live vaccine did not die after challenge with virulent isolates of E rhusiopathiae.

  • A cold-adapted porcine reproductive and respiratory syndrome virus (CA-VR2332) was generated from the modified live virus strain VR2332. CA-VR2332 showed impaired growth when cultured at 37℃ with numerous mutations (S731F, E819D, G975E, and D1014N) in the hypervariable region of the NSP2, in which the mutation S731F might play a vital role in viral replication at 30℃. Conserved amino acid sequences of the GP5 protein suggests that CA-VR2332 is a promising candidate for producing an effective vaccine against PRRSV infection. Further studies on replication and immunogenicity in vivo are required to evaluate the properties of CA-VR2332.

Immunological Characterization of Full and Truncated Recombinant Clones of ompH(D:4) Obtained from Pasteurella multocida (D:4) in Korea

  • We cloned a gene of ompH(D:4) from pigs infected with P. multocida D:4 in Korea [16]. The gene is composed of 1,026 nucleotides coding 342 amino acids (aa) with a signal peptide of 20 aa (GenBank accession number AY603962). In this study, we analyzed the ability of the ompH(D:4) to induce protective immunity against a wild-type challenge in mice. To determine appropriate epitope(s) of the gene, one full and three different types of truncated genes of the ompH(D:4) were constructed by PCR using pET32a or pRSET B as vectors. They were named ompH(D:4)-F (1,026 bp [1-1026] encoding 342 aa), ompH(D:4)-t1 (693 bp [55-747] encoding 231 aa), ompH(D:4)-t2 (561 bp [187-747] encoding 187 aa), and ompH(D:4)-t3 (540 bp [487-1026] encoding 180 aa), respectively. The genes were successfully expressed in Escherichia coli BL21(DE3). Their gene products, polypeptides, OmpH(D:4)-F, -t1, -t2, and -t3, were purified individually using nickel-nitrilotriacetic acid (Ni-NTA) affinity column chromatography. Their $M_rs$ were determined to be 54.6, 29, 24, and 23.2 kDa, respectively, using SDS-PAGE. Antisera against the four kinds of polypeptides were generated in mice for protective immunity analyses. Some $50{\mu}g$ of the four kinds of polypeptides were individually provided intraperitoneally with mice (n=20) as immunogens. The titer of post-immunized antiserum revealed that it grew remarkably compared with pre-antiserum. The lethal dose of the wild-type pathogen was determined at $10{\mu}l$ of live P. multocida D:4 through direct intraperitoneal (IP) injection, into post-immune mice (n=5, three times). Some thirty days later, the lethal dose ($10{\mu}l$) of live pathogen was challenged into the immunized mouse groups [OmpH(D:4)-F, -t1, -t2, and -t3; n=20 each, two times] as well as positive and negative control groups. As compared within samples, the OmpH(D:4)-F-immunized groups showed lower immune ability than the OmpH(D:4)-t1, -t2, and -t3. The results show that the truncated-OmpH(D:4)-t1, -t2, and -t3 can be used for an effective vaccine candidate against swine atrophic rhinitis caused by pathogenic P. multocida (D:4) isolated in Korea.

Acute porcine reproductive and respiratory syndrome outbreaks in immunized sow herds: from occurrence to stabilization under whole herd vaccination strategy

  • Outbreaks of porcine reproductive and respiratory syndrome virus (PRRSV) in vaccinated sow herds from occurrence to stabilization were monitored and analyzed in terms of serology and reproductive performance. Three different conventional pig farms experienced severe reproductive failures with the introduction of a type 1 PRRSV. These farms had adopted mass vaccination of sows using a type 2 PRRSV modified live vaccine (MLV). Therefore, to control the type 1 PRRSV, an alternative vaccination program utilizing both type 1 and type 2 MLV was undertaken. Following whole herd vaccinations with both types of MLV, successful stabilization of PRRS outbreaks was identified based on serological data (no viremia and downward trends in ELISA antibody titers in both sows and suckling piglets) and recovery of reproductive performance. Additionally, through comparison of the reproductive parameters between outbreak and non-outbreak periods, it was identified that PRRSV significantly affected the farrowing rate and the number of suckling piglets per litter at all three pig farms. Comparison of reproductive parameters between periods when the different vaccination strategies were applied revealed that the number of piglets born in total and born dead per litter were significantly increased after the introduction of the type 1 PRRS MLV.

In vivo evaluation of preventive effect of Lactobacillus reuteri on porcine epidemic diarrhea in suckling piglets

  • Lactic acid bacteria have been reported their beneficial roles on host including reduction of infectious diarrhea problems. In this study, preventive effect of Lactobacillus (L.) reuteri HY25101 and L. johnsonii HY25103 on porcine epidemic diarrhea virus (PEDV) was investigated in suckling piglets. Two groups of one day old PEDV naïve piglets were orally administered L. reuteri HY25101 and L. johnsonii HY25103 for three days respectively before challenge with lethal dose of PEDV. In second experiment, passive immunized one day old piglets using colostrums containing PEDV specific IgA were used. The survival rates of the L. reuteri HY25101 administered group were significantly higher than that of L. johnsonii HY25103 administered group and viral shedding was rapidly diminished in L. reuteri HY25101 administered group. Interestingly piglets born from the sow immunized with attenuated PEDV vaccine were not completely protected from PEDV challenge, however coadministeration of L. reuteri HY25101 and colostrums containing PEDV specific IgA were more effectively prevent PEDV infection. These results suggested that dietary treatment using L. reuteri HY25101 could reduce diarrheal problem and mortality rate caused by PEDV in suckling pigs. In addition, L. reuteri HY25101 could be used as one of effective compensation treatment with attenuated live vaccine for PED.

Shigellosis

  • Shigellosis is a global human health problem. Four species of Shigella i.e. S. dysenteriae, S. flexneri, S. boydii and S. sonnei are able to cause the disease. These species are subdivided into serotypes on the basis of O-specific polysaccharide of the LPS. Shigella dysenteriae type 1 produces severe disease and may be associated with life-threatening complications. The symptoms of shigellosis include diarrhoea and/or dysentery with frequent mucoid bloody stools, abdominal cramps and tenesmus. Shigella spp. cause dysentery by invading the colonic mucosa. Shigella bacteria multiply within colonic epithelial cells, cause cell death and spread laterally to infect and kill adjacent epithelial cells, causing mucosal ulceration, inflammation and bleeding. Transmission usually occurs via contaminated food and water or through person-to-person contact. Laboratory diagnosis is made by culturing the stool samples using selective/differential agar media. Shigella spp. are highly fragile organism and considerable care must be exercised in collecting faecal specimens, transporting them to the laboratories and in using appropriate media for isolation. Antimicrobial agents are the mainstay of therapy of all cases of shigellosis. Due to the global emergence of drug resistance, the choice of antimicrobial agents for treating shigellosis is limited. Although single dose of norfloxacin and ciprofloxacin has been shown to be effective, they are currently less effective against S. dysenteriae type 1 infection. Newer quinolones, cephalosporin derivatives, and azithromycin are the drug of choice. However, fluoroquinolone-resistant S. dysenteriae type 1 infection have been reported. Currently, no vaccines against Shigella infection exist. Both live and subunit parenteral vaccine candidates are under development. Because immunity to Shigella is serotype-specific, the priority is to develop vaccine against S. dysenteriae type 1 and S. flexneri type 2a. Shigella species are important pathogens responsible for diarrhoeal diseases and dysentery occurring all over the world. The morbidity and mortality due to shigellosis are especially high among children in developing countries. A recent review of literature (KotIoff et al.,1999) concluded that, of the estimated 165 million cases of Shigella diarrhoea that occur annually, $99\%$ occur in developing countries, and in developing countries $69\%$ of episodes occur in children under five years of age. Moreover, of the ca.1.1 million deaths attributed to Shigella infections in developing countries, $60\%$ of deaths occur in the under-five age group. Travellers from developed to developing regions and soldiers serving under field conditions are also at an increased risk to develop shigellosis.

오리 간염 바이러스의 분리와 국내 분리주의 약독화 (Isolation of duck hepatitis virus and it's attenuation in chicken embryos)

  • Duck viral hepatitis is an acute, highly infectious viral disease of young dacklings aged from two days to three weeks. The significant lesion associated with the disease was enlarged liver including necrotic foci and numerous hemorrhagic spots. We have isolated five strains of duck hepatitis virus (DHV) from field cases showing about 20% mortality with a sign of opisthotonos. When a-day-old ducklings were intramuscularly inoculated with one of the isolates, 92% of the birds were died within 5 days. We attempted to develop an attenuated strain of duck hepatitis virus (DHV) using one of the isolates by serial chicken embryo passages. The propagation of DHV in chicken embryos was carried 140 passages. The virus titer increased gradually from the $21^{st}$ through the $50^{th}$ passage, but there was no significant increase of virus titer in subsequent passages after then. Through the serial passages, the virulence of the virus for chicken embryos was gradually increased but decreased for ducklings. The pathogenicity of the virus for ducklings was preserved up to the $21^{st}$ passage but disappeared at the $50^{th}$passage. An attenuated Korean isolate which was passaged 140 times in chicken embryos gave good protection in ducklings against both challenge infection to a Korean virulent strain and to a DHV-DRL strain, a type 1 reference strain of DHV, which indicated that the Korean isolates could be classified as DHV type 1. And the above results suggest that an attenuated Korean isolate can be used for developing a live DHV vaccine.
