Pathophysiology of Articular Cartilage Injury (관절 연골 손상의 병태 생리)

  • Injury of articular cartilage can be classified into acute injury and chronic degenerative osteoarthritis Acute mechanical trauma on articular cartilage causes injuries that are divided into three distinct types based on the depth of injury: microdamage, chondral fracture, osteochondral fracture and each type has different potential of healing response and long-term prognosis. Articular cartilage undergoes degradation in response to a number of stimuli and eventually degenerative osteoarthritic changes will progress. The extent of initial injury to the articular cartilage is the most important factor affecting the long-term outcome of the healing response and other variables such as the size of lesion, site, age, activity level, obesity, limb alignment are also important factors. In this review, the pathophysiology that occurs within articular cartilage after different injuries and the effect of nonsurgical treatment mainly in physicochemical aspect and biological aspect will be discussed.

Early Stage Legg-Calve-Perthes Disease in a Dog: Clinical, Surgical, Radiological, Computed Tomography and Histological Findings (초기 허혈성 대퇴골두 괴사증을 보이는 개에서 임상학적, 수술적, 방사선학적, 컴퓨터단층촬영, 조직학적 소견)

  • A 7-month-old Pomeranian presented with non-weight bearing lameness on the right pelvic limb. Physical examination revealed pain on extension of the right hip joint, and serum chemistry results showed an increased concentration of creatine kinase. Radiographic findings at admission included a weak radiolucent line on the right femoral head and widening of the right hip joint space, and mild displacement of the right capital femoral epiphysis was additionally identified on day 14. Computed tomography (CT) showed a decreased Hounsfield Unit measurement and a fracture line on the right femoral head. Early stage Legg-Calve-Perthes disease (LCPD) was diagnosed and femoral head and neck ostectomy was performed on the right femur. A fracture fissure and osteophytes at the epiphyseal plate of the femoral head were identified surgically. Necrosis in the femoral metaphysis and epiphysis was observed histologically. This case report describes the radiography, CT images, and surgical and histological findings in a dog with early stage LCPD.

Effect of Intravenous Intraoperative Esmolol on Pain Management Following Lower Limb Orthopedic Surgery

  • Background: Lack of proper control of acute postoperative pain often leads to lingering or chronic pain. Several studies have emphasized the role of beta-blockers in reducing postoperative pain. Esmolol is a selective short-acting beta-blocker that produces few side effects. The purpose of this study was to examine the effect of intravenous intraoperative esmolol on postoperative pain reduction following orthopedic leg fracture surgery. Methods: In a clinical trial, 82 patients between 20-65 years of age with tibia fractures and American Society of Anesthesiologists (ASA) physical status I & II who underwent surgery were divided into two groups. Group A received esmolol and group B received normal saline. Postoperative pain was measured at three time points: entering the recovery unit, and at 3 h and 6 h following surgery, using the Visual Analogue Scale (VAS). A P value of < 0.05 was considered significant. Results: Mean VAS scores at all three time points were significantly different between the two test groups (P = 0.02, P = 0.0001, and P = 0.0001, respectively). The consumption of pethidine was lower in group A than in group B (P = 0.004) and the duration of its effect was significantly longer in time (P = 0.026). Conclusions: Intravenous intraoperative esmolol is effective in the reduction of postoperative pain following leg fracture surgery. It reduced opioid consumption following surgery and delayed patient requests for analgesics.

  • Objective: The purpose of this study is finding tendencies admission patients to orient medical hospital due to Traffic Accident. Method: We studied 34 cases who were admitted in Dong-eui University Oriental Hospital from 1st, December, 2003 to the 30st, May, 2005. And we came to some conclusion about clinical tendencies as follows. Results and Conclusions: 1. Women had occupied more than men, And 10 persons in 30th were the most distribution of age. 2. Most patients used western medical hospital as primary hospital and after several days they started to search for orient medical treatments. 3. In the diagnosis through radiological test, cervical spine sprain was most(19 cases, 55.6%). The others were lumbar spine sprain(14 cases, 41.2%), fracture of other bone{4 cases, 11.8%), fracture of spine(3 cases, 8.8%), Intercranial hemorrhage(3 cases, 8.8%). 4. In chief complaint, low back pain was most(19 cases, 55.9%), The others were neck stiffness(17 cases, 50.0%), headache(13 cases, 38.2%), lower limb pain(9 cases, 26.5%). 5. Simple Disease had higher than complex disese at effective rate. 6. Both $43{\sim}60$ days admission and $15{\sim}21$ days admission were the best effective. $8{\sim}14$ days admission was next effective. 7. Early visit made more effective.

  • Complex Regional Pain Syndrome(CRPS) is a chronic neruopathic pain syndrome, most frequently encountered after trauma, nerve injury, fracture, cerebral vascular accident(CVA), spinal injury, and surgery to a limb, and affected by diverse factors such as disease process, intervention method, and other biopsychosocial components. Occupational therapy as a multidisciplinary treatment approach may be potentially useful when simultaneously targeting such biological, psychological, and social components. Biological treatment can focus on increasing functional use of extremity for promoting independence. Psychological intervention can include relaxation/biofeedback training and cognitive-behavioral treatments, and social approach can focus on recreational therapy and vocational rehabilitation. Occupational therapy will encourage functional restoration, pain management, and regaining of psychological components with collaboration between different disciplines.

Radiographic Features of Traumatically Caused Retroflexion of Urinary Bladder in Two Dogs

  • The urinary bladder retroflexion, flipped over backwards into the pelvic canal, by trauma has been rarely reported in dogs. This paper describes clinical and radiological features of urinary bladder retroflexion in two dogs with a history of traffic accident. The main clinical signs were hind limb lameness, severe pain and dysuria. Radiography and ultrasonography were performed to evaluate the patient's damages. No remarkable findings were observed except pelvic fracture on survey radiograph. On cystourethrography, caudal displacement and retroflexion of the bladders were identified. Cystopexy was performed and the bladders were repositioned. After surgery one dog with severe azotemia and bilateral hydroureteronephrosis was deteriorated noticeably and euthanized by request of the owner, while the other dog was recovered no recurrence of the problem by 12 months post-surgery. It should be considered that contrast study and serial assessment in urinary tract be important to demonstrate the evidence of bladder retroflexion and evaluate the prognosis in dogs with severe trauma.

Percutaneous screw fixation and external stabilization as definitive surgical intervention for a pelvic ring injury combined with an acetabular fracture in the acute phase of polytrauma in Korea: a case report

  • Unstable pelvic ring injuries are potentially life-threatening and associated with high mortality and complication rates in polytrauma patients. The most common cause of death in patients with pelvic ring injuries is massive bleeding. With resuscitation, external fixation can be performed as a temporary stabilization procedure for hemostasis in unstable pelvic fractures. Internal fixation following temporary external fixation of the pelvic ring yields superior and more reliable stabilization. However, a time-consuming extended approach to open reduction and internal fixation of the pelvic ring is frequently precluded by an unacceptable physiologic condition and/or concomitant injuries in patients with multiple injuries. Conservative treatment may lead to pelvic ring deformity, which is associated with various functional disabilities such as limb length discrepancy, gait disturbance, and sitting intolerance. Therefore, if the patient is not expected to be suitable for additional surgery due to a poor expected physiologic condition, definitive external fixation in combination with various percutaneous screw fixations to restore the pelvic ring should be considered in the acute phase. Herein, we report a case of unstable pelvic ring injury successfully treated with definitive external fixation and percutaneous screw fixation in the acute phase in a severely injured polytrauma patient.

Diagnosis of Coxofemoral Joint Luxation in a Whooper Swan (Cygnus Cygnus) Using Computed Tomography and Radiography

  • A wild Whooper swan (Cygnus Cygnus) with limping due to an injured left pelvic limb in an accident was rescued on the seashore and transferred to the Jeju Wildlife Rescue Center on November 23rd, 2020. On physical examination, its body condition score was 1 out of 5 due to starvation and dehydration. The left coxofemoral joint was also examined by careful palpating and estimating the damage. Moderated soft tissue swelling and crepitus surrounding the hip joint were confirmed. Radiography and computed tomography (CT) were used together for an accurate diagnosis of the joint. By radiographs readings, it was difficult to accurately confirm the condition of the proximal femur due to superimposition of the synsacrum and internal organs. However, signs such as avulsion fracture of the femoral head and a few fragments around the joint were revealed by CT imaging. Besides, through three-dimensional (3D) image analysis of CT, the dislocated area and condition of the left hip joint could be accurately and easily confirmed. The diagnostic process showing in this paper could be used as a good reference for diagnosing coxofemoral joint luxation in wild swan.

A Case Study of Geometrical Fracture Model for Groundwater Well Placement, Eastern Munsan, Gyeonggido, Korea (지하수개발을 위한 단열모델 연구사례(경기도 문산 동쪽지역))

  • This study is the case of groundwater development based on the geometrical fracture model of target area established only through geological fracture mapping technique. A fracture mapping of $9km^2$, eastern Munsan, has been conducted to determine geological and hydrological factors for new water well placement in the Gyeonggi gneiss complex. Geophysical exploration was not applicable because of small restricted area and dense underground utilities at the site. Form line mapping on the basis of foliation orientation and rock type revealed a synform of NS fold axis bearing to the south. An EW geological cross-section passed through the site area shows a F2 synform as a double-wall ice cream spoon shape. Three regional faults of $N20^{\circ}E,\;N30^{\circ}W$, and NS have been dragged into the site to help understand extensional fault paths. The $N20^{\circ}E$ fault with dextral sense is geometrically interpreted as a western fault of two flexural conjugate type-P shear faults in the F2 synformal fold. The NE cross-section reveals that a possible groundwater belt in the western limb of super-posed fold area is formed as a trigonal prism within 100 m depth of the intersectional space between the $N20^{\circ}E$ fault plane and the weakly sheared plane of transposed foliation. Another possible fault for water resource strikes $N40^{\circ}E$. Recommended sites for new water well placement are along the $N20^{\circ}E\;and\;N40^{\circ}E$ faults. As a result of fracture mapping, 145 ton/day of water can be produced at one well along the $N20^{\circ}E$ fault line. Exploration of groundwater in the area is succeeded only using with geological fracture mapping and interpretation of geological cross-section, without any geophysical survey. Intersection of fault generated with the F2 synformal fold and foliation supply space of groundwater reserver.

Geochemical Exploration Technics in the Pungchon Limestone Area (풍촌 석회암지대 탐사에 적용될 새 지화학탐사법 연구)

  • Most of significant ore deposits in South Korea such as the Sangdong W - Mo, the Yeonhwa Pb-Zn and the Geodo Cu-Fe skarn ore deposits occur at the southern limb of the Hambaeg syncline in the Taebaeg Basin. The mineralization took place in the interbedded limestone of the Myobong Formation and the Pungchon limestone of the Great Limestone Group of the Cambrian age, generally striking E-W and dipping 25-30 degrees north. There are no outcrops of the skarn-type orebody at the northern limb of the syncline. In order to find a clue of a possible hidden orebody localized at the limestones in the northern limb, a lithogeochemical exploration by using carbon isotope and some elements such as Si, Ca, Fe and Al at the Sangdong Mine area has been attempted as for a modelling study. For this study, 45 samples from the Pungchon limestone which do not show any megascopic indication of mineralization have been taken in both the mineralized zone and the unminerallized zone at the Sangdong Mine area. Analytical data show that there are big differences in the contents of CaO and $Al_2O_3$ between the Pungchon limestone of the mineralized zone and that of the unmineralized zone. Carbon isotope data exhibit that ${\delta}^{13}C$ values of the Pungchon limestone in the mineralized zone are highter than those in the unmineralized zone. The difference in the analytical values of CaO, $Al_2O_3$ and the carbon isotope between the mineralized and the unmineralized zones is as follows ; Unminerallized zone Mineralized zone CaO 51.3% 43.5% $Al_2O_3$ 0.6% 2.4% ${\delta}^{13}C$ -0.39 permil -0.56 permil $Fe_2O_3$ 0.9% 1.4% $SiO_2$ 3.0% 2.4% The decrease in the Si content of the Pungchon limestone in the mineralized zone is contrary to the result of the previous study (Moon, 1987). On the basis of identification of the increase in the Al content of the limestone in the mineralized zone, it could be deduced that the decrease in the Si content of the Pungchon limestone might be due to the result of increase in the alteration products mainly occurred along fracture-system such as joint cracks or minor faults and that the phenomena shown by the Si and Al content in the mineralized zone might be derived from the thermal effect of granite extended mineralizing activity to the overlied limestone on the surface. Higher mean values of Fe and Al as well as lower mean values of carbon content and the ${\delta}^{13}C$ than mean values of those in the Pungchon limestone at the northern limb of the Hambaeg Syncline may be applicable in exploration for blind orebodies.
