Giant cell tumor of temporomandibular joint masquerading as temporomandibular joint pain dysfunction syndrome: a rare case report

  • Giant cell tumor (GCT) of the craniofacial bones has been reported but they are not common. This tumor occurs more often in women than in men and predominantly affects patients around the third to fifth decade of life. GCTs are generally benign but can be locally aggressive as well. We report a case of GCT involving the temporomandibular joint (TMJ), which was initially thought to be temporomandibular disorder (TMD). A 22-year-old female presented with swelling and pain over the right temporal region for 18 months associated with jaw locking and clicking sounds. On examination, her jaw deviated to the right during opening and there was a $2{\times}2$ cm swelling over the right temporal region. Despite routine treatment for TMD, the swelling increased in size. Computed tomography and magnetic resonance imaging of the brain and TMJ revealed an erosive tumor of the temporal bone involving the TMJ which was displacing the temporal lobe. Surgical excision was done and the tumor removed completely. Histopathological examination was consistent with a GCT. No clinical or radiological recurrence was detected 10 months post-surgery.

  • Today's environment of enterprise is changing. They have to face customer' demands with the right product, the right service and supply them at the right time. And also cut down logistics and inventory cost and bring up the profit as much as they can. This means the change of putting enterprise first in importance to putting customer first importance. therefore to correspond to customer's demand, shorting lead time is becoming a essential condition. The answer to this changes of environment is supply chain management. In the Supply chain, The ATP function doesn't only give customers to conformation of delivery. It can be used by the core function with ATP rule that can reconcile supplies and demands on the supply chain. Therefore We can be acquire the conformation about on the due date of supplier by using the ATP function of management about real and concurrent access on the supply chain, also decide the affect about product availability due to forecasting or customer's orders through the ATP. In this paper, It consolidates the necessity on a CTP and analyzes data which is concerned of ATP. Under the these environments, defines the ATP rule that can improve the customer value and data flow related the LTV(Life Time Value) and builds on a algorithm.

  • The growing number of threats to society through the uncontrolled distribution of information is forcing library communities in many countries to reconsider their views on free access to collections. Based on the content of numerous documents of international importance, it can be concluded that in any democratic country access to information is one of the most important human rights, along with the right to life, liberty, and security of person. However, the state has the right to restrict citizens' access to information within the framework of existing legislation. Constantly, restrictions on access to information are established in order to protect the ethical foundations of the constitutional order, morality, health, rights, and legitimate interests of others, to ensure the country's defense and state security. It goes without saying that each country has the right to independently decide where the boundaries lie between permitted and prohibited information, including printed information, contained in library collections. This article describes three levels of access restriction: foreign, state, and regional. The authors have analyzed the legal and regulatory documents that govern libraries, as well as the reasons and methods of limitation. A comparative analysis of the restriction of access to information in the countries of Europe and Asia is presented.

Plantar Pressure in Skilled and Unskilled Players during Baseball Batting (야구 타격시 숙련자와 미숙련자의 족저압력 분석)

  • This study examined 24 right-handed amateur baseball players. Twelve who had played baseball for more than 6 years were grouped as skilled players, while 12 who had played for 1-3 years were the unskilled player group. The swing motion was divided into four event phases: stance, backswing, impact, and follow-through. The mean and maximum plantar pressure, center of pressure, and ground reaction force were measured during each event phase. The mean and standard deviations for each variables were calculated and differences were validated with the independent sample t-test. A p-value <0.05 was considered statistically significant. The results were as follows. 1)The ideal stance is a stable, balanced position with more than 65% of weight on the right foot. There was significant difference in mean left plantar pressure, while the maximal plantar pressure and mean right plantar pressure did not differ significant. 2)The effective backswing of a skilled player is comprised a rightward shift in weight to build maximum energy. More than 90% of the weight was on the right foot. There was a significant difference in the mean left plantar pressure, while the maximal plantar pressure and mean right plantar pressure did not differ significantly. 3) For an effective impact, a rapid shift in weight to the left foot is essential, so that a power hit is obtained. Significant difference in the mean and maximum plantar pressures of both feet were observed. 4)Follow-through requires wight balance, more on the right than the left, without leaning leftward. There was no significant difference in the mean or maximum plantar pressure. 5)The center of plantar pressure should move from the center of the foot to the toe. 6)The analyses of the ground reaction force suggest that a good swing involves a gradual shift in weight to the right side and a rapid leftward shift at impact. Good balance, with the center of gravity on the right side at follow-through, is also required.

  • In this study, the following findings were obtained: First, life-long educational system should be reinforced that can train and educate people to fit their situation and provide the necessary manpower in a just-in-time manner by getting away from the school-centered education and rapidly introducing the knowledge required in both the world market and the domestic market. This can be said to be the global trend in the ubiquitous age. Second, government should make efforts to build up the life-long educational system that can make the persons trained and educated in schools the manpower required by the state and society. Third, Life-long learning policy starts with providing for the system of lifting all kinds of limits and obstacles so that anyone needing learning can learn and his learning may not discriminated from schooling. For this policy or system to be effectively promoted, government should reinforce administrative and financial support system for investment in and research on the ubiquitous department. Fourth, It is quiet right that the very effort we are going give the super to the ubiquitous education is a shortcut to solving rapidly lots of problems heaped on our present life-long educational system.

  • Rapid progress in modern medical technology has made it possible to sustain life and/or delay death using 'heroic' treatments. The availability of life-sustaining treatment brings several issues in end-of-life care such as 'dying with dignity' and an radical increase in health care costs. The use of Advance Directives(AD) have been widely heralded by health care providers, gerontologists, and advocacy groups as means of protecting patients' right to accept or refuse life-sustaining treatment in end-of-life care. The use of AD can not only improve patients' autonomy and quality of life but also bring efficiency in distributing health care resources. The proportion of older persons in Korean population has been increasing. Those 65 years of age or over were about 7 percent of the population. Death and dying is not limited to older persons, but it is more prevalent among them. In conjunction with an aging population and the increasing prevalence of death, the issues of death and dying will become crucial in near future in terms of 'dying with dignity', 'autonomy', and 'self-control'. This paper attempts to explode and establish the concept of advance directives (AD) based on literature review. Data sources are computer searches with the MEDLINE database. Due to the lack of prior study on AD for a Korean cases, studies abroad are reviewed. This paper suggests the need for future study on the possibility of the use of AD in Korea.

Medical Imaging of Pyothorax in a Dog (개에서 농흉증의 영상의학적 접근)

  • A 3-years-old female Pointer was evaluated for anorexia, dyspnea and exercise intolerance for 10 days. Auscultation revealed muffling of heart sound. There were leukocytosis, anemia, hypoalbuminaemia and hyperglobulinaemia on the blood profiles. Radiography showed severe pleural effusion. Thorax ultrasonographs described traped pleural effusion, pericardial effusion and collapsed right lung lobe(s). On CT images, thoracic neoplasia and lung torsion were ruled-out. Thoracocentesis with thoracoscopy and bacterial culture revealed pyothorax and pleuritis. The diagnosis of pyothorax in dogs and cats is based on clinical signs, radiography, thoracocentesis and cytology and culture of the exudate. In this case, various diagnostic medical imaging techniques (radiography, ultrasonography, CT and thoracoscopy) were used for diagnosis of pyothorax.

A Concept Analysis of the Quality of Life for the Children (아동의 삶의 질에 대한 개념 분석)

  • Purpose: The purpose of this study was to attempt an accurate understanding of the quality of life for the children and provide basic materials for application to nursing research, intervention and further theoretical development. Method: Data obtained from such research methods as the lexical and literature review were analysed according to Walker and Avant's(1995) process of concept analysis. Results: As a result of analysis, it was found that the attributes of the concept of the quality of life for the children included value/acceptance, family relationship, safety, good relationship with friends, right of children, school life, parent relationship, leisure, economical stability, not child abuse, and health. There was the coexistence between antecedent element and the outcomes of subjective judgement - health, family, economic strength, education, friends, welfare, social-cultural environment which indicates the coexistence of positive and negative emotions. Conclusion: nursing intervention should be provided for children by concentrating focus on the coordination of internal and external situations in order to prompt them to express their life with positive emotions. Experiential reference was used to suit the purpose of this study based on the use of objective and subjective indicators, and the development of the instrument considering the children were proposed because of the absence of the unified measurement instrument.

Ethical Issue of Physician-Assisted Suicide and Euthanasia

  • With the implementation of Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life, interests of the general public on self-determination right and dignified death of patients have increased markedly in Korea. However, "self-determination" on medical care is misunderstood as decision not to sustain life, and "dignified death" as terminating life before suffering from disease in terminal stage. This belief leads that physician-assisted suicide should be accommodated is being proliferated widely in the society even without accepting euthanasia. Artificially terminating the life of a human is an unethical act even though there is any rational or motivation by the person requesting euthanasia, and there is agreement thereof has been reached while there are overseas countries that allow euthanasia. Given the fact that the essence of medical care is to enable the human to live their lives in greater comfort by enhancing their health throughout their lives, physician-assisted suicide should be deemed as one of the means of euthanasia, not as a means of dignified death. Accordingly, institutional organization and improvement of the quality of hospice palliative care to assist the patients suffering from terminal stage or intractable diseases in putting their lives in order and to more comfortably accept the end of life physically, mentally, socially, psychologically and spiritually need to be implemented first to ensure their dignified death.
