The characteristic features of traumatic anterior shoulder instability due to an event of minor trauma

  • There are some patients who have traumatic anterior shoulder instability due to minor injuries like overhead activities. The purpose of this study was to clarify characteristic features of traumatic anterior shoulder instability due to minor injuries. According to the mechanism of injury in an initial dislocation, 83 shoulders that underwent the stabilizing surgery for traumatic anterior shoulder instability were divided into two groups. Traumatic group included patients who suffered from a fall or a direct injury. Minor injury group included patients who suffered from the other injury like overhead activity. General joint laxity, range of motion and laxity under anesthesia, and intraarticular findings were compared between two groups. The morphology of superior and middle glenohumeral ligaments, Bankart lesion, Hill-Sachs lesion, and partial articular surface tendon avulsion lesion were observed in arthroscopy. Minor injury group consisted of 19 shoulders with 8 males, 11 females and the mean age of 22.5 years. Traumatic group consisted of 64 shoulders with 52 males, 7 females and the mean age of 24.3 years. Female in minor injury group was significantly more than that in traumatic group. There was no difference in general joint laxity and intraarticular findings between two groups. Range of external rotation in injured side in minor injury group was significantly more than that in traumatic group. Inferior laxity in both sides in minor injury group was more than that in traumatic group. In conclusion, the traumatic anterior shoulder instability due to minor injuries might incline to occur the shoulder in female and with inferior laxity of shoulder.

Dominance and Performance of Real-time Scheduling Algorithms on Multiprocessors (다중처리기 상의 실시간 스케줄링 알고리즘의 우월 관계 및 성능)

  • Multiprocessor architecture becomes increasingly common on real-time systems as computer hardware technology rapidly progresses and the workload of real-time systems increases. However, efficient solutions for many real-time multiprocessor scheduling problems are not known. Hence many researchers apply uniprocessor scheduling algorithms to multiprocessor scheduling or devise new algorithms based on these algorithms. Such algorithms are EDF (Earliest Deadline First), LLF (Least Laxity First), EDF-US[m/(2m-1)], and EDZL (Earliest Deadline Zero Laxity), and comparative studies on them are necessary. In this paper, we show the dominance relation of these algorithms with respect to schedulability, and we prove EDZL strictly dominates EDF. The simulation results show that EDZL has high processor utilization and it causes a small number of preemptions.

  • 견관절의 ABER position에서 posterosuperior rotator cuff 가 posterior glenoid에 contact 하여 internal impingement가 일어나는 현상은 운동선수가 아닌 일반인에게도 발생하는 physiologic phenomenon이며 과도한 반복 동작으로 인하여 증상이 발현한다. Internal impingement는 증상이 모호할 뿐만 아니라 견관절 내 여러 병소가 함께 존재하기 때문에 각각의 pathology에 대한 자세하고 정확한 evaluation이 필수적이다. 아직 controversy가 있지만 internal impingement의 pathologic factors로서는 anterior capsular laxity (true anterior laxity), posteroinferior capsular tightness (pseudo anterior laxity)와 이로 인한 glenohumeral internal rotation deficit (GIRD), 그리고 SICK scapula syndrome (scapular dyskinesis) 등으로 요약할 수 있다. 진단은 주로 병력 청취와 신체 검사로 내려지며 ultrasonogram, MRA등으로 확진한다. Sleeper stretching exercise 등과 같은 보존적 치료에 반응이 좋지만 6개월간의 재활 치료에도 호전이 없다면 증상과 직접 관련된 주 병소(appropriate pathology)를 찾아 이에 대한 적절한 수술이 이루어져야 한다.

Minimum Preemption Scheduling Algorithm based on Least-Laxity-First for Real-Time Systems (실시간 시스템을 위한 최소 여유시간 우선 기반의 최소 선점을 갖는 스케쥴링 알고리즘)

  • 최소 여유시간 우선(Least-Laxity-First)스케쥴링 알고리즘은 여유시간이 작은 태스트가 높은 우선순위를 갖는 동적 우선순위 스케줄링 방법으로서 단일프로세서 시스템에서 최적임이 증명되었다. 그러나, 이 알고리즘은 최소 여유시간을 가진 태스크가 여러 개 존재하여 여유시간 충돌이 발생한 경우 이 태스크들 간에 빈번한 문맥교환이 발생하게 되는 문제점이 있어 실용적이지 못하다. 본 논문에서 제한하는 최소 여유시간 우선 기반의 최소선점을 갖는 스케줄링 알고리즘(Least-Laxity-First with Minimum Preemption 또는 LLF/MP)은 여유시간이 충돌했을 때에 문맥교환을 최소화함으로써 최소 여유시간 우선 스케줄링 알고리즘의 단점을 해결하였다. LLF/MP 스케줄링 알고리듬은 불필요한 문맥교환을 줄임으로써 시스템 오버헤드로 인한 시스템의 성능 저하를 방지할 수 있고 보다 많은 시스템 자원을 예측 불가능한 비주기적 태스크에게 할당할수 있다.

End-to-End Laxity-based Priority Assignment for Distributed Real-Time Systems (분산 실시간 시스템을 위한 양극단 여유도 기반의 우선순위 할당 방법)

  • Researches about scheduling distributed real-time systems have some weak points, not scheduling both sporadic and periodic tasks and messages or being unable to guaranteeing the end-to-end constraints due to omitting precedence relations between sporadic tasks. This paper describes the application model of sporadic tasks with precedence constraints in a distributed real-time system. It is shown that existing scheduling methods such as Rate Monotonic scheduling are not proper to be applied to the system having sporadic tasks with precedence constraints. So this paper proposes an end-to-end laxity-based priority assignment algorithm which considers the practical laxity of a task and allocates a proper priority to a task.

Current Perspectives in Vaginal Laxity Measurement: A Scoping Review

  • This scoping review aimed to identify and categorize the available measurement options for vaginal laxity (VL), their indications of use, and whether these measurements can sufficiently provide objective clinical judgment for cases indicated for vaginal rejuvenation with many treatment options nowadays. Systematic searches were conducted on five electronic databases, manually searching articles' bibliographies and predetermined key journals with no date or study design limitations. We included all studies involving VL in their inclusion criteria, treatment indications, and outcome parameters. We used the Arksey and O'Malley frameworks as the guideline in writing this scoping review. Of the 9,464 articles identified, 66 articles and 11,258 subjects were included in the final analysis. The majority of studies were conducted in obstetrics and gynecology (73%), followed by plastic surgery (10%), medical rehabilitation (4.5%), dermatology (4.5%), and others (8%). Most studies originated from the North American region (30%). The following measurement tools were used: (1) interviews, (2) questionnaires, (3) physical/digital examinations, (4) perineometers, and (5) others. Our results suggested that subjective perception of laxity confirmed by directed interview or questionnaire is sufficient to confirm VL. Additional evaluation of pelvic floor muscle through digital examination or perineometer or other preferred tools and evaluation of sexual function through validated questionnaire (Female Sexual Function Index, Female Sexual Distress Scale-Revised, etc.) should follow to ensure holistic care to patients. Future research on the psychometric properties (reliability and validity) of commonly used measurements and the correlation in between subjective and objective measurements should be initiated before their clinical applications.

  • Researches about scheduling of the distributed real-time systems have been proposed. However, they have some weak points, not scheduling both sporadic and periodic tasks and messages or being unable to guaranteeing the end-to-end constraints due to omitting precedence relations between sporadic tasks. So this paper proposes a new scheduling method for distributed real-time systems consisting of sporadic and periodic tasks with precedence relations and sporadic and periodic messages, guaranteeing end-to-end constraints. The proposed method is based on a binary search-based period assignment algorithm, an end-to-end laxity-based priority assignment algorithm, and three kinds of schedulability analysis, node, network, and end-to-end schedulability analysis. In addition, this paper describes the application model of sporadic tasks with precedence constraints in a distributed real-time system, shows that existing scheduling methods such as Rate Monotonic (RM) scheduling are not proper to be applied to the system having sporadic tasks with precedence constraints, and proposes an end-to-end laxity-based priority assignment algorithm.

Evaluation for canine hip dysplasia in Golden and Labrador retrievers using PennHIP method in Korea

  • Hips of the Golden and Labrador retrievers in Korea were evaluated with the University of Pennsylvania Hip Improvement Program (PennHIP) and the severity of joint laxity and degenerative joint disease (DJD) were compared to the PennHIP database. The distraction index (DI) of domestic Golden and Labrador retrievers was significantly higher than the DI of the PennHIP database. In the two breeds, the prevalence of DJD increased according to the DI. However, the severity of DJD did not show a positive correlation with the DI. Overweight dogs and dogs kept indoors showed more severe DJD and more prevalence of clinical signs. This report is thought to be the first case presentation of a large population of Golden and Labrador retrievers in Korea and the findings are representing the overall level of canine hip dysplasia (CHD) of domestic Retrievers, as the PennHIP method was not available in Korea until 2001. We can respect that the stock of retrievers can have a tighter hip joint through control of CHD using an accurate diagnostic method keyed to a phenotype especially concern for joint laxity using PennHIP method and an organized screening program. The clinical manifestation of dysplastic dog can be reduced through control of bodyweight and the environment with regular monitoring the hips with concern for joint laxity using PennHIP method.

  • This paper presents a scheme to enhance fixed priority scheduling algorithms on multiprocessors. This scheme gives the highest priority to jobs with zero laxity and schedules them Prior to other jobs. A fixed priority algorithm employing this scheme strictly dominates the original one; it can schedule all task sets schedulable by the fixed priority algorithm and some task sets not schedulable by the fixed priority algorithm. Simulation results show that the proposed scheme improves fixed priority algorithms in terms of the number of schedulable task sets and schedulable utilization bound.
