日本における 藥用植物の生産, 消費の 現況 (The Present Condition of Production and Consumption of Medicinal Plants in Japan)

  • The medicinal plants used traditional medicines are classified into two groups. The first group is formulary medicine which have been recorded on authorized books, for example, kampo drug in Japan. The second group is the fork medicines. These medicinal plants production is 165 billion yen(about 1.5 billion dollar) and which rate to to total pharmaceutical production is about 3% in 1990. The number of important medicinal plants treated in the market is 124 herbs from the total 364 herbs for kampo durgs(210 preparations kampo durgs) and other propritetary drugs (agents for common cold, laxatives, gastrointestional and so on). The production of the Ginseng and Bupleurum Root are increased about ten times in last twelve years(1976 or 1988). The 80% of these medicinal plants are imported from China, Korea and Southern Asian countries. Then the Pharmaceutical Affairs Bureau of Ministry of Health and Welfare published "The Guideline of Medicinal Plant for Cultivation" for a quality control and a stabilization supply of 80 herbs. The first part(1992) of the guideline reported 5 herbs; Coptis Rhizome, Rehmannia Root, Bupleur, Root, Japanese Angelica Root and Rhubarb, The next part will be published following 5herbs; Cnidium Rhizome, Safflower, Coix seed and Fruiet, Platicodon Root and Zedoary.

여대생의 섭식문제 관련요인 (Factors Influencing Eating Problems among Korean University Women)

  • Purpose: The purpose of the study was to investigate factors that influence university women with eating problems. Methods: Study participants were 307 women from two universities. Self-report questionnaires which included items on eating problems, satisfaction with body shape, self-esteem, body mass index, and weight control methods were used. Data were analyzed using independent t-tests, ${\chi}^2$ tests, and a multiple binary logistic regression. Results: About 20% of the undergraduate women were experiencing eating problems. The problematic eating group showed more dissatisfaction with body shape than the normal eating group, and used more diets, fasting, diet-products, and uretics/laxatives to control body weight. Significant predictors for the problematic eating group were diets, diet-products, BMI, self-esteem and dissatisfaction with body shape. The strongest predictors were diets; risk for women university students who had been on a diet was 15 times higher than their counterparts. Conclusion: It is pertinent for health professionals to start intervention programs to educate university women with eating problems. The contents should include information on healthy weight control methods, improving satisfaction with body shape and self esteem, as well as creating social atmosphere about healthy body shape for women.

생활습관 중재가 요양병원 입원 노인의 변비에 미치는 효과 (The Effects of Life Style Modification on Constipation of the Older Adults at a Geriatric Hospital)

  • Purpose: This study aimed to evaluate the effects of lifestyle modifications on the constipation relief, the time required to see the effects, and the effective lifestyles for constipation in older adults admitted to a geriatric hospital. Methods: Nonequivalent control group pre-post test design was used. The subjects consisted of 25 elderly in the control group and 23 in the experimental group. Constipation was measured with symptoms of constipation, number of bowel movement and use of laxatives. Life style modification consisted of drinking water before breakfast, having a breakfast, scheduled toileting after breakfast and walking everyday for 3 weeks. Results: Symptoms of constipation (t=2.23, p=.030) and number of bowel movement per week (t=-2.55, p=.014) were significantly different between two groups after 3 weeks. Drinking water, scheduled toileting and walking might contribute to the results. Conclusion: Lifestyle modification was effective on constipation in older adults admitted to a geriatric hospital. However, the effects were produced after 3 weeks. Nevertheless, it is recommended that nurses continuously encourage older adults for drinking water, regular trying for bowel movement and walking to relieve constipation, since the lifestyle modification does not need any special education and skill.

자의적 변비 분류에 의한 정상군과 변비군 여대생의 식사관련 요인 비교 (Comparison of the Dietary Factors between Normal and Constipation Groups by Self-reported Constipation in Female College Students)

  • The purpose of this study was to obtain baseline data on dietary management of women's constipation. We estimated the prevalence of self reported constipation, bowel habits and foods & nutrients intake. The subjects were 169 female college students (normal = 92, constipation group = 77) aged 19 to 23 years. We conducted anthropometric measurements, an evacuation habits survey, and a dietary intake assessment for three days by a 24 hour recall method. The prevalence of self reported constipation was 46% (n = 77). Results showed that bowel habits (the difficulty of evacuation, the duration of evacuation and the feeling after evacuation) were significantly different between the two groups. Over 65% of constipation group used laxatives for constipation relief. Frequency of stress and deep sleeping were related with constipation. In daily food consumption and nutrients intake, there was not a significant difference between the groups. However in relation to bowel habits which factors influence constipation, the study showed that the intake of water, potatoes, kimchi, and fruits correlated with evacuation facility. Further, there were no findings in the difference of foods consumption and nutrients intake between the two groups. But some life style changes and food intakes (potato and kimchi) may be useful to improve constipation symptoms in young women.

  • Background and Purpose : To investigate the wild plants to be used for survival on tactical activity of Special Forces soldiers at deep enemy's territory Material and Methods : To investigate the wild plants, I selected various native plants in the forests and fields that can be used for medical cure and eatables. And I selected some poisonous plants and some plants containing water or salts, after due consideration of tactical activity and use in the battle-fields. Results and Conclusion : There are many wild plants to be used for multiple purpose in our country. We can get from these plants emergency medicines, foods, even water and salts. I chose 6 species of digestives, 11 species of analgesics, 9 species of antiphlogistics, 7 species of hemostatics, 5 species of antidiarheal, 5 species of diuretics, 2 species of laxatives, 7 species of antipyretics, 1 species of vermicide and 45 species of eatable plants and 12 species of poisonous plants. and I summarized general name, scientific name, component, picking season, used portion, taking method of these plants. Building up accurate knowledge on the wild plants could contributed to raise survival rate of Special Forces soldiers in the battle-fields.

소화기계 무증상환자에 대한 소화기계 약제 투약현황 (Use of Gastrointestinal Drugs in Patients without Digestive Symptoms)

  • Gastrointestinal (GI) medications have been administered to many patients without any gastrointestinal diseases. The objectives of this study were to evaluate use of GI drugs and assess related factors. Medical records of 600 outpatients were reviewed from January 1997 to December 1997 at A Hospital, Kyunggi-do, Korea. Fifty patients every month among all outpatients were randomly selected up to total 600 patients. Surgical patients, visitors for regular health examination and inpatients were excluded. GI symptoms included nausea, vomiting, diarrhea, dyspepsia, constipation, heartburn, dysphagia and abdominal pain. The prescribed gastrointestinal drugs were antacids. $H_2$-antagonist, sucralfate, cisapride, omeprazole, laxatives, digestive enzymes and antidiarrheal agents. Patients without GI symptoms were 348 out of 600 outpatients who were screened. Two hundred and eighty two of 348 patients $(81\%)$ were given GI drugs though they did not have any GI symptoms. There were no differences in regard to sex and age of patients. Most of medical departments prescribed gastrointestinal drugs for these patients. The most frequently prescribed drugs were in order of digestive enzyme, antacids and $H_2$-antagonists. In view of economic aspects, patients paid 12.28 percents of total cost per prescription for unnecessary medicines. The medical practice of prescribing GI drugs should be assessed to define appropriate subgroups to have benefits with prophylactic administration and to reduce adverse effects caused by drug interactions. Pharmacists would have a significant role to promote rational drug therapy.

Polyethylene Glycol Plus Electrolytes with Stimulant Laxative in Paediatric Faecal Disimpaction: A Randomised Controlled Study

  • Functional constipation is common in the paediatric population all over the world. Effective disimpaction to evacuate the impacted faecal matter forms an essential initial step in the management of constipation. Though different regimens of polyethylene glycol 3350 with electrolytes (PEG+E) are accepted as the prime medication for disimpaction, response is not always satisfactory. A randomised prospective study was undertaken, in a tertiary paediatric Gastroenterology centre to find out the outcome of a 2-day disimpaction when a stimulant laxative sodium picosulphate was added to PEG3350+E (PEG+E+PS group) and comparing it with the outcome using PEG3350+E (PEG+E group) alone. Hundred and one children were randomised into two groups to receive PEG+E+PS and PEG+E. Results revealed that PEG+E+PS group proved significantly superior to PEG+E group in most of the efficacy-parameters in terms of disimpaction as well as long-term management of constipation. Though stimulant laxatives are being used for disimpaction, comparative data are lacking. This was the 1st such comparative study looking at the efficacy of these two processes of disimpaction along with long term effect on treatment.

Prevalence of Defecation Disorders and their Symptoms is Comparable in Children and Young Adults: Cross-Sectional Study

  • Purpose: We aimed to compare the prevalence rates and associated symptoms of constipation and fecal incontinence in children and young adults and evaluate how these patient groups cope with these disorders. Methods: A cross-sectional study was performed in which 212 children (8-17 years) and 149 young adults (18-29 years) from the general Dutch population completed a questionnaire about defecation disorders. Results: Constipation occurred in 15.6% of children and 22.8% of young adults (p=0.55), while the prevalence of fecal incontinence was comparable between groups (7%, p=0.91). The symptoms associated with constipation occurred as often in children as in young adults, while most fecal incontinence symptoms occurred more often in young adults. Approximately 43% of children had constipation for more than 5 years, while 26% of young adults experienced constipation since childhood. Only 27% of constipated children and 21% of constipated young adults received treatment (mostly laxatives). For fecal incontinence, 13% of children and 36% of young adults received treatment (mostly antidiarrheal medications or incontinence pads). Conclusion: In contrast to the general belief, the prevalence of defecation disorders and associated symptoms seem to be comparable in children and young adults. Only a few people with defecation disorders receive adequate treatment.

어린이 탈직장의 경화요법 (Rectal Prolapse in Children)

  • 어린이 탈직장 11례 중 7례에서 5% 페놀을 포함한 글리세린액 직장점막하주사법을 이용한 경화요법을 시행하고 그 시술방법과 치료결과를 보고하였다. 이 방법은 간단하며, 단 1회 시술 후 6례에서 완치되어 치료율도 높았다. 시술 후 합병증이 없었다는 점과 실험적 조직소견으로 보아 본 치료법이 안전한 방법이며, 경화제에 의한 점막하층의 섬유화유발이 치료기전일 가능성의 증거도 제시되었다. 외래 혹은 입원상태에서의 국소요법 혹은 전신마취하 시술 등 상황에 따라 시술법도 자유롭게 선택할 수 있으나 시술 전 장청소과정과 시술 중 좌인지감각에 의한 주의깊은 점막하층 접근은 합병증을 피하기 위하여 반드시 지켜져야할 중요한 점으로 강조되어야 하겠다. 어린이의 탈직장에 대한 치료는 성인환자들과는 반드시 구분되어야 하며, 경화요법은 안전하고 편리하며 효과적인 우선적 치료법이라고 판단된다.

고등학생의 변비 유무에 따른 생활습관의 차이 (Life Style Difference by Constipation among High School Students)

  • Objectives: The study investigated constipation in male and female high school students and in order to provide data base for development of education and intervention programs aimed at preventing and managing constipation in high school students. Methods: The study used a technique of convenience sampling to mobilize the entire students in an academic high school in D region. Of the combined 360 copies of questionnaire distributed, 340 copies were collected and used for the final analysis. The gathered data were analyzed using frequency, percentage, and ${\chi}^2-test$ in SPSS 21. Results: The ratio of female students with constipation is significantly higher than that of male students with the condition(${\chi}^2=5.73$, p=0.017), and there were significant differences in exercise habits(${\chi}^2=26.75$, p<0.001), eating habits(${\chi}^2=43.19$, p<0.001), and bowel habits(${\chi}^2=22.96$, p<0.001) between female students and male students. Also there was significant difference in water intake between the normal group and the constipation group(${\chi}^2=10.01$, p=0.040). Significant differences between the two groups were also discovered in the level of perception of constipation(${\chi}^2=98.76$, p<0.001), bowel movement time(${\chi}^2=15.74$, p=0.008), and use of laxatives(${\chi}^2=17.68$, p<0.001) and enema(${\chi}^2=16.97$, p<0.001) for constipation relief. Conclusions: To prevent and manage constipation in high school students, education about healthy dietary, and bowel habits is required. Also there is an urgent need to improve the school toilet environment in order to respect the rights of students to void or defecate when necessary, a process which will require involvement of students, teachers, and other school staff.
