Treatment Results of Early Laryngeal Carcinoma (조기후두암의 치료 성적)

  • On 147 patients with early laryngeal cancer undertaken surgery or radiation therapy at Seoul National University Hospital from January 1987 through December 1994, retrospective analysis with reviewing the medical record was performed. The number of recurred cases was twenty six. The recurrence rate was higher in the cases with radiation therapy than in those with surgical therapy. And the recurrence rate was remarkable in the cases with T2 in supraglottic cancer and in those with T1 in glottic cancer. Of the patients undertaken surgery, recurrence rate was higher in the patients with laser operation than in those with other procedures. It was, however, lower in the patients with laryngofissure with cordectomy than in those with other surgical techniques. The overall three year disease free survival rate was 72 % in early laryngeal carcinoma.

  • Laryngeal framework surgery (LFS) is a unique phonosurgical concept that enables us to influence the laryngeal biomechanics by changing the shape/position of the laryngeal cartilages. LFS procedures can be favorably combined with one another but also with other phonosurgical methods, and they are usually reversible and correctable. Type I thyroplasty and arytenoid adduction are still useful in spite of the recent popularity of injection laryngoplasty. Basic surgical principles have seldom been changed since Isshiki's development, but a number of modifications have been tried and are still going on. These delicate surgeries require exhaustive training, but the reward is great to both the surgeon and the patient.

  • Vocal cord polyps are generally removed using a direct laryngoscope and surgical microscope. In some case of laryngeal microsurgery, laryngeal exposure with surgical microscope is difficult because of severalfactors. We experienced a case of vocal polyp with difficult laryngeal exposure in a 56 year old male patient. The vocal cord polyp was removed successfully through endoscopic approach.

  • Laryngeal diseases cause significant changes in the quality of speech production. Automatic detection of laryngeal diseases by voice is attractive because of its nonintrusive nature. In this paper, we apply speech recognition techniques to detection of laryngeal cancer, and investigate which feature parameters and classification methods are appropriate for this purpose. Linear Predictive Cepstral Coefficients (LPCC) and Mel-Frequency Cepstral Coefficients (MFCC) are examined as feature parameters, and parameters reflecting the periodicity of speech and its perturbation are also considered. As for classifier, multilayer perceptron neural networks and Gaussian Mixture Models (GMM) are employed. According to our experiments, higher order LPCC with the periodic information parameters yields the best performance.

  • A laryngeal mask airway (LMA) has many advantages in the management of airway emergencies or the treatment of patients in whom intubation is difficult, but the use of LMA during positive ventilation may seem inappropriate to protect the airway because of the risk of pulmonary aspiration of regurgitated gastric contents. We experienced aspiration pneumonia after general anesthesia using LMA in patient who suffered from panperitonitis. It is considered that the main reason for aspiration pneumonia was a leakage of gastric content through the space surrounding the nasogastric tube while under high airway inflation pressure. We concluded that when LMA or the Proseal laryngeal mask airway (PLMA) is chosen for the use in difficult intubations, careful patient-suitability selection and the correct knowledge of LMA and PLMA are needed to protect the airway against aspiration.

  • Excising part or all part of a larynx as a cancer operation results in changes that transgress anatomic, physiologic, psychologic, and social priciples. The quality of life of a patient's life after any given cancer surgery usually is regarded as a second-priority consideration after oncologic safety. With laryngeal surgery, excision of malignant disease typically results in change that significantly influence an individual for the duration of his or her life. Nonetheless, with appropriate rehabilitation the surgical side effects can be minimized to allow for an excellent quality of life. Successful conservation surgery for laryngeal cancer requires careful interdependent selection for patients, lesions and procedure. The technical goal is to minimize trauma to uninvolved tissue and to wisely utilized local tissues or tree flap for reconstruction, while insuring for oncologically sound procedure. Rehabilitation should aim to produce a glottal sound source if possible, however voice therapy to promote false vocal fold vibration and arytenoid to epiglottis source of vibration can produce very satisfactory phonatory results.

  • Laryngeal saccular cyst may develop when obstruction of the orifice of the saccule occurs with resultant dilatation of saccule. It is classified into anterior saccular cyst and lateral saccular cyst. When saccular cyst occurs in infancy, it usually may cause respiratory distress, inspiratory stridor, cyanosis and dysphagia. In adults, it may cause hoarseness, dyspnea, dysphagia, pain and neck mass. The laryngeal saccular cyst may be treated by its removal with endoscopic or external approach, depending on the classification of saccular cysts, its size and individual patient factor. Recently, we have experienced a case of laryngeal saccular cyst so we report our clinical experience with brief review of literature.

  • We describe a case of laryngeal venous malformation in 43 year-old patient, discovered incidentally. Laryngeal venous malformation is a comparatively rare condition in adults. It presents as a dark bluish mass that may cause bleeding, hoarseness or stridor, but he complained only mild throat discomfort. We found dark-bluish tumor on the right arytenoid area, and treated the lesion by Ethanol sclerotherapy. All lesions disappeared after one month without any complication. Sclerotherapy with Ethanol can be an easy and effective treatment for laryngeal venous malformations, so we present the case with a review of the related literatures.

  • Background and Objectives: Heat shock protein(HSP) 27 is a member of the small HSP family that plays a part in the epithelial cell growth and differentiation, wound healing. apoptosis and cell protection against inflammatory cytotoxic mediators. The expression of HSP27 was investigated in normal laryngeal tissue and squamous cell carcinoma of the larynx. Materials and Methods : We studied expression of HSP27 by Western blot on 20 patients of laryngeal squamous cell carcinoma. Results: HSP27 expressed in all normal and cancer tissues. In 9 cases(45%), expression of HSP27 more prominent in cancer tissue. Statistically, there were no significant difference in the expression of HSP27 in normal and cancer tissue, clinical stage and tumor differentiation. Conclusion 45% of cancer tissues was more prominent than normal tissue. But further studies on expression of HSP27 in laryngeal cancer and relationship with clinical parameter should be done.
