Case of Bilateral Primary Carcinoma of the Fallopian Tube (양측성 원발성 난관암 1예)

  • Primary carcinoma of the fallopian tube is exceedingly rare, accounting for 0.3 percent of female genital tract cancer. It is usually unilateral, but 20 percent of cases are bilateral. It occurs most commonly in women between 40 and 60 years old. Approximately 50 percent of the patients are nulliparous. Because of variable presenting symptoms and rarely incidence, preoperative diagnosis of fallopian tube carcinoma is seldom made. Evaluation and treatment are also essentially the same at that of ovarian carcinoma. We experienced a case of bilateral primary fallopian tube carcinoma in women presented with vaginal bleeding and pelvic mass, postoperatively pathologic examination of resected specimen after laparatomy and reported with brief review of literature.

  • From january 1970 through december 1990, 130 cases of patients with chest penetrating injury were admitted to department of thoracic and cardiovascular surgery in Chosun University Hospital. We analyzed above patients and obtained results were as follows: 1. The ratio of male to female was 7.1:1 in male predominance, and the majority[69.6%] was distributed from 2nd to 3rd decade. 2. The most common cause of chest penetrating injuries was stab wound. 3. 110 cases[84.5%] were arrived to our emergency room within six hours after trauma. 4. The most common injuring mode was hemo, pneumothorax. 5. The frequently injured site of the penetrating chest trauma was left side of the chest [64.65%]. 6. The common associated injuries of penetrating chest injuries were extremities injuries, abdominal injuries, head & facial injuries. 7. The common method of surgical treatment were closed thoracostomy[78 cases], open thoracotomy[20 cases], laparatomy[12 cases]. 8. The overall motality was 3.07%[4/130], and the causes were hypovolemic shock, sepsis and asphyxia.

  • Appendicitis is a common disease in children. But left lower abdominal pain in acute appendicitis is a rare clinical feature. A 6 year-old-girl complained of left sided abdominal pain for 2 days. Past medical history was not contributory. Abdominal tenderness and guarding in left lower quadrant were noticed. Abdominal sonography and abdominal computed tomography scan demonstrated reversed position between superior mesenteric artery and vein, and a mass in the left lower quadrant abdomen suggesting appendicitis. Acute appendicitis in left lower quadrant, associated with intestinal malrotation, was found at laparatomy.

  • Pericardial rupture due to blunt trauma is very rare, but can lead to serious complications. It occurs mainly on the left, is found incidentally during surgery, and is seldom discovered radiologically unless accompanied by cardiac herniation. The following case describes a 53-year-old traffic-accident victim who received emergency pericardial repair and bleeding control via an exploratory thoracotomy and an exploratory laparatomy. The patient was discharged without any complication and remained healthy at six month after injury.

  • Trichobezoars are commonly found within the stomach, but rarely induce small bowel obstruction. We report an 8-year-old girl who presented with bilious vomiting and colicky abdominal pain. She had a history of depression and trichophagia. A diagnosis of small bowel obstruction was made on computed tomography and a trichobezoar in the ileum was evacuated by exploratory laparatomy.

  • Eleven cases of traumatic diaphragmatic hernia treated at the department of thoracic and cardiovascular surgery, for the period of seven years up to February 1975, from the base of this report. 1. Peak incidence was found in the age group of one to four years and thirty to forty years, four cases in each group respectively, occupying 63.6% of all cases. 2. In all the lesions the left diaphragm was affected and the herniated organs were in order of frequency the small bowel, the large intestine, the stomach, and the spleen. 3. The methods of the adapted treatment were laparatomy in five, thoracoabdominal approach in four, and thoracotomy in two cases. 4. The mortality rate was low, only one out of eleven died It seems that the immediate appropriate treatment can bring forth the excellent outcome.

Primary Omental Torsion (대망염전)

  • One case of primary omental torsion in a 10 year-old, 43 kg boy is presented. He presented with a history of acute, continuous pain in the upper abdomen, aggravated by changes of position. Physical examination of his abdomen showed board-like rigidity. tenderness and rebound tenderness over the entire abdomen. The preoperative diagnosis was perforated peptic ulcer. Exploratory laparatomy revealed torsion of the greater omentum. The torsed omentum was excised and the outcome was good. Omental torsion is a rare surgical condition and is difficult to diagnose prior to operation. Therefore, in case of negative exploration for acute abdomen, the omentum should be included among the organs examined. In addition, careful search for an inflammatory focus should be made, because secondary omental torsion with intraabdominal sepsis is much more common than primary torsion.

  • This study was carried out to compare the efficacy of sodium carboxymethyl cellulose (SCMC) with the combination of low molecular weight heparin (LMWH) and SCMC for the prevention of postoperative adhesions in the dog. After laparotomy, abrasions were made on the surface of ileal serosa in 17 dogs. The animals were randomly divided into the control group (N = 5) which received no treatment, the SCMC-treated group (N = 6), and the SCMC & LMWH-treated group (N = 6). Hematologic values were monitored before laparotomy and 1, 4, 7, 10 and 14 days after laparotomy. The location and score of adhesion were assessed at second laparotomy 3 weeks later, Although the mean adhesion score both in the SCMC-treated group (7.17${\pm}$1.17) and in the SCMC & LMWH-treated group (4.50${\pm}$1.38) was found to be significantly lower than that in the control group (9.40${\pm}$0.89) (p<0.01 and p<0.01, respectively), more favorable adhesion prevention was achieved in the SCMC & LMWH-treated group in comparison with the SCMC-treated group without any hemor-rhagic complications (p<0.01). This study could conclude that SCMC & LMWH are highly effective in prevention against postoperative adhesion in the dog.

Effect of Rabbit Peritoneal Fluid(PF) on in vitro Maturation of Porcine Follicular Oocytes (가토복수가돼지 난포란의 체외성숙에 미치는 효과)

  • This study was undertaken to evaluate the effect of rabbit peritioneal fluid(rPF) on in vitro maturtion of porcine follicular oocytes. From does 20h after hCG injection, rPF was aspirated aseptically at laparatomy, and then centrifuged, filtrated, and preincubated immediately for 12h. Porcine follicular oocytes isolated from ovaries of slaughtered animals were incubated in TCM-HEPES+10% FCS, TCM-HEPES+rPF(v/v, 50/50), or rPE only and examined the nuclear maturation after aceto-orcein or hochest staining. After identifying the optimal incubation time, this experiment was repeated for 5 times. Under the TCM-HEPES containing hormones and serum codition, the time range of porcine follicular oocyte maturation was 38 to 44 hours and the optimal time of maturation of follicular oocyte in vitro was 42 hour cultivation, respectively. The maturatin rates(89.4% and 92.7%) of porcine follicular oocytes cultured in the media with 50% rPF or only rPF were signifciantly higher thanthat (84.6%) of oocytes cultured with TCM-HEPES, respectively. These results suggest that the unknown components(s) of rPF promoted in vitro maturation of porcine follicular oocytes.

  • To elucidate the effect of the acupoints of the limbo for canine electroacupunhur anesthesial total 140 heads of dogs were used. The animals were divided into 3 experimental groups(Gong-sun, San-yin-jiao and Qiang-feng and contro1 group(non-acupoint). The induction time, clinical findings and the results of operation using laparatomy were investigated under the electroacupunture anesthesia and compared with those of control. The induction time of electroacupunture anesthesia was very rapids about 1 minute in experimental groups, while it is unable to succeed in anesthesia of control group. As far clinical Bindings during electroacupunture anesthesia, the experimental animals had vivid consciousness and stability without tumult and barking. The success rate of operation was high with decreasing order of Gong-sun group(50/50, 100%) > San-yin·jiao group(26/30, 86.7%) 1 Qiag-feng group(18/ 30, 60%) in experimental groups. Considering above findings collectively, the combination of Gong-sun and San-yang-luo is thought to be best method among the acupoints of the limbs for canine electroacupunture anesthesia.
