Dorsal Extradural Lumbar Disc Herniation Causing Cauda Equina Syndrome : A Case Report and Review of Literature

  • A 73-year-old male presented with a rare dorsally sequestrated lumbar disc herniation manifesting as severe radiating pain in both leg, progressively worsening weakness in both lower extremities, and urinary incontinence, suggesting cauda equina syndrome. Magnetic resonance imaging suggested the sequestrated disc fragment located in the extradural space at the L4-L5 level had surrounded and compressed the dural sac from the lateral to dorsal sides. A bilateral decompressive laminectomy was performed under an operating microscope. A large extruded disc was found to have migrated from the ventral aspect, around the thecal sac, and into the dorsal aspect, which compressed the sac to the right. After removal of the disc fragment, his sciatica was relieved and the patient felt strength of lower extremity improved.

  • Neurosarcoma is relatively rare, but is seen frequently in association with stigmata of neurofibromatosis ie., Cafe au lait pigmentation and cutaneous neurofibromatosis. Both advanced age and association with neurofibromatosis increase the likelihood of malignant degeneration to neurosarcoma. It also may be dumb bell-shaped with intraspinal extension. Thoracic meningocele is rare and represents a protrusion of the dural sac through an abnormally large or malformed intervertebral foramen. The patient is usually asymptomatic, with evidence of paravertebral mass on the chest rentgenogram. In one operation we removed a "Dumb-bell" neurosarcoma and intrathoracic meningocele in a 52 year old woman using a cervical laminectomy with a trap-door incision. She was discharged at 17 days after the operation with a sequelae of excision of nerve roots C8 and T1. Postoperative radiation therapy was performed during the follow-up period.up period.

황색인대골화에 의한 흉부 늑간신경통 1예 (A Case of Thoracic Intercostal Neuralgia due to Ossification of the Ligamentum Flavum)

  • Ossification of the ligamentum flavum(OLF) is not infrequent in the cervical and lumbar regions but is very rare in the thoracic spine. We reported a 62-year-old women with left thoracic intercostal neuralgia due to an OLF. Physical and neurological examination were normal. MRI showed an OLF with compression of the thecal sac in the posterolateral aspect of the T9-T10 level. Laminectomy and removal of the ligament resulted in marked clinical improvement. OLF is known to cause thoracic radiculomyelopathy, but presentation with intercostal neuralgia only is very rare.

Solitary Xanthogranuloma of the Upper Cervical Spine in a Male Adult

  • We present the rare case of solitary xanthogranuloma in the upper cervical column mimicking a Brown-Sequard syndrome. A 29-year-old man complained with right hemiparesis and left hypoesthesia after a car accident. Computed tomography and magnetic resonance images revealed a lobulated homogenously well-enhancing mass in between posterior arch of the atlas (C1) and spinous process of the axis (C2) resulting in a marked spinal canal narrowing with cortical erosions. The patient was managed by complete resection of the tumor with partial laminectomy with lower half of C1 posterior arch and upper half of C2 spinous process. The authors advise complete removal of the xanthogranuloma and consideration as a differential diagnosis of lesions among upper cervical lesions.

Posterior Epidural Migration of Thoracic Disc Fragment

  • Migration of a disc fragment to the posterior epidural space is rare, especially in the thoracic spine. Only four such cases of posterior epidural migration of thoracic disc fragments have been reported. The authors report a case of 66-year-old man who presented with back pain and right leg weakness due to posterior epidural migration of thoracic disc fragment. The patient was successfully treated by laminectomy and partial facetectomy with disc removal.

Acute Hydrocephalus Following Cervical Spinal Cord Injury

  • We present a case of acute hydrocephalus secondary to cervical spinal cord injury in a patient with diffuse ossification of the posterior longitudinal ligament (OPLL). A 75-year-old male patient visited the emergency department with tetraparesis and spinal shock. Imaging studies showed cervical spinal cord injury with hemorrhage and diffuse OPLL from C1 to C4. We performed decompressive laminectomy and occipitocervical fusion. Two days after surgery, his mental status had deteriorated to drowsiness with dilatation of the right pupil. Findings on brain computed tomography revealed acute hydrocephalus and subarachnoid hemorrhage in the cerebellomedullary cistern, therefore, extraventricular drainage was performed immediately. Acute hydrocephalus as a complication of cervical spine trauma is rare, however, it should be considered if the patient shows deterioration of neurologic symptoms.

  • Steroid and local anesthetics were injected into the epidural space through the posterior sacral foramen or lumbar approach in 4 patients for the purposes of relieving severe low back pain and radiculopathy. These patients were referred from other sections dealing with low back pain and radiculopathy, because one could not tolerate the surgical stress due to a heart problem, another two did not want the operation and the last had post-laminectomy status. All patients were satisfied with the results subjectively.

  • 하부 요추 추간판 탈출증시에 척추 후궁 절제술 및 추간판 제거술은 흔히 실행되며, 그로 인한 혈관 손상 합병증은 드물게 생기고 있다. 혈관 손상으로 인해서 수술 환자의 혈압이 떨어져도 외부로 출혈이 없으면 무시되기도 하지만, 만약 수술후 환자의 혈압이 심하게 떨어지거나, 저혈압 상태가 오래 지속된다면, 먼저 혈관 손상을 의심해야 하며, 즉시 개복술을 시행해야 된다. 부산 백병원 흉부외과 의국에서는 2례에 대한 경험을 했기에 보고한다.

A Case of Spinal Intradural Extramedullary Cavernous Angioma

  • Cavernous angiomas represent 5 to 12% of spinal vascular malformations and usually are located at the vertebral body level with possible extension into the extradural space. The intradural intramedullary cavernous angioma occurs in about 3% of cases, whereas extramedullary localization is extremely rare. We report a case of intradural extramedullary cavernous angioma in which the patient presented with low back pain and both leg pain. The magnetic resonance imaging study showed intraspinal mass lesion at L1-2. It was removed totally through laminectomy of L1-2 and confirmed as cavernous angioma. The postoperative course was uneventful without any neurologic deficit. We report this unusual spinal malformation.

Giant Cell Tumor of Upper Thoracic Spine

  • Giant cell tumor (GCT) of the spine is a rare benign tumor, but can be aggressive and can exhibit a high local recurrence rate. Furthermore, GCT of the upper thoracic spine may pose diagnostic and management difficulties. Here, we report a rare case of GCT of the upper thoracic spine with soft tissue extension to the spinal canal. The patient was managed by decompressive laminectomy and posterolateral fusion followed by an injection of polymethylmethacrylate into the vertebral lesion. The patient recovered clinically and showed radiological improvement after surgical treatment without tumor recurrence at his last follow-up of postoperative 7 years. We present this unusual case of GCT and include a review of the literature.
