만성 재가 기동장애자의 가정병실 모델 개발을 위한 현장 연구 (A Ethnographic Field Study for a Model Development of the Chronic Bed-ridden Patient s Home-ward)

  • This study is designed to facilitate the creation of home environment conducive to the family taking care of chronic bed-ridden patients with more effective method. The need for this study has emerged against the background of marked changes in the structure of ailments and causes of death, resulting in the number and plights of chronic bed-ridden patients as well as of a rapid increase in demand for medical care and resulting premature discharge. Keeping these in mind, this study focused on home-wards where the majority of chronic bed-ridden patients are being cared for. Despite. their overriding importance, home-words are less than efficient in caring (or chronic bed-ridden patients. These circumstances require the designing of home-wards that can offer greater comfort to patients and at the same time make things easier for caregivers, on the basis of an overall analysis of patients' life and home - ward situation. According1y this study adopted a Participant Observation Method derived cultural anthropology, Toward this end, 3 patients were chosen as subjects of this study for intensive interviewing and participant observation. In the process of this field re-search efforts were made to collect emprical data, that is, to faithfully record the words of the subjects and their caregivers for analysis and interpretation. The findings of these analyses are as follows. Firstly, the chronic bed-ridden patients are mostly being taken care by close family members. Secondly, a room for the exclusive use of the patient, floor, kitchen, bathroom and multipurpose space were found to be necessary for proper caring of the patient. These spaces were respectively used with a view to 1) accomodating the patient as well as caregivers' activities, 2) keeping general and medical supplies and other appliances for patient's care and drying the patient's washing, 3) preparing and keeping the patient's foods and beverages, 4) keeping the supplies necessary for cleaning the patient's body and treating the patient's eliminations, 5) washing the patient's clothes, underwears and bedclothes. The patient's room in turn is subdivided into six portions in terms of uses : specifically the places for accomodating 1) the patient, 2) medical supplies, 3) medicines, 4) linens St clothes, 5) bedclothes and, 6) diapers. Thirdly, the activities of the caregiver are subdivided into seven key areas : hygiene, exercise, diet, elimination, therapeutic nursing, prevention of sore, and other activities. Each area is further classified into several different activities of caring. These activities we mainly carried out in the patient's room. Fourthly, the supplies for caring the chronic bed-ridden patient is divided into two large domains : medical and general supplies. Finally, three main problems areas were found in this study on the part of caregivers, that is, sore prevention, hygiene problem related frequent urination / defecation, the caregiver's physical, psych ological and emotional burden. In consideration of the aforesaid problem areas, a model home-ward was developed in this study. The newly-developed model has been found to have the following six advantages. Firstly, the time and effort required for maintaining the patient's hygiene are reduced, thus relievins the caregiver's physical and psychological bur-den. Secondly, the patient's hygiene can be maintained in satisfactory conditions, because the patient's eliminations are more easily removed. Thirdly, skin irritations caused by the patient's eliminations were remarkably reduced and so were the patient's sores due to moisture and bacteria. Fourthly, the home-ward have a tilt-table ef-fect thanks to the inclining room floor. This improves the patient's cardiovascular function as well as constantly changes pressed skin areas and thus prevents sores. Fifthly, improved shelf arrangements help make the best use of patient's supplies. Sixthly, the trouble of continuously changing clothes, underwears, diapers & bedclothes is remarkably reduced simply by covering the patient with cotton sheets when laid in bed. This is espected to cut down expenses by reducing the comsumptions of diapers and other disposable supplies.

일반음식점 신규영업주의 위생관리지식 및 위생행정에 대한 태도 (Knowledge and Attitude toward Restaurant-Related Sanitation of New Restaurateurs)

  • The purpose of this study was to investigate the sanitation affairs of general restaurants. The questionnaire survey on the attitude and knowledge toward sanitation, the attitude for sanitary administration and the sanitary education was conducted against new 600 restaurateurs who were educated from June 20 to July 11, 1996, at the administration hall's division of Kyungsangbook-do in charge of food industry that offered regular sanitary education to new restaurateurs annually. And the visit survey on sanitary practice was also conducted over 93 restaurateurs who obtained the commercial license for food service business. The findings from the survey were as follows; In regard to food sanitation, some 87.1 to 88.3% got the right knowledge about the reason and precaution of food poisoning, food's frozen or cold-storage, and the disposal of products after expiration of validity term. But it was about 20.8% to 50.0% who knew right about major precaution, storage temperature in refrigerator, fermented milk product's storage temperature and validity term. There was therefore a necessity for education in food sanitation. 38.2% of the subjects placed an emphasis on sanitary storage of foodstuffs as the most important thing in sanitary management. 33.8% emphasized cooking sanitation. The environmental sanitation was counted as the most important thing by 19.2%, and personal sanitation of worker was counted by 8.8%. There was differences in what they thought the most important thing was, according to the respondent's educational level and cooker. 86.6% replied it necessary to improve the sanitary level. The respondents who were younger or had better educational level emphasized more the need for it. Concerning health examination, 90.2% replied it necessary. 81.4% answered the reason was because there was a potentiality Quests might be infected with contagious disease. 78.5% pointed the need for sanitary education, but respondents with higher educational level less emphasized its needs. As the reason for poor sanitation, restaurateur's poor awareness about it was most frequently pointed out, by 46.9%. Cooking sanitation was most frequently counted, by 38.5%, as the first thing to be improved. As the most critical point in sanitary education, 34.5% indicated food's sanitary Quality control 30.9% mentioned sanitary treatment of kitchen facilities and peripheral environment, and 27.1% emphasized the summary of the general food sanitation. 77.7% answered to correct immediately in case of violating the Food Hygiene Law, and 12.0% replied to correct in the same case if they would get the order from public official or administrative action would be taken. Respondents with higher educational level answered more to correct immediately. What they wanted the government office to do toward sanitary improvement was a fund aid an facilities and management which was pointed out by 38.9%, a periodical sanitary education by 26.3% and a on-the-spot guidance of sanitary officials by 22.3%. In view of the food service business's sanitary practice, the rate of wearing a sanitary clothes was 32.9% in city and 35.0% in county. The rate of hand-washing without soap or non-washing at cooking was 73.9%, 85%, respectively. The rate of personnel sanitation was 34.2% in city and 50.0% in county. These things indicated the sanitation was not well practiced. To improve the poor sanitary conditions of the food service businesses, it is recommended to offer institutional backing and financial aid from administrative office, and encourage restaurateurs to take pride in their job. and conduct the sanitary education effectively by sanitary education institution.

도심주거지에 나타나는 일상문화로서의 도시정원가꾸기에 대한 고찰 - 용산구 용산동2가 해방촌을 중심으로 - (Examination of Urban Gardening as an Everydayness in Urban Residential Area, Haebangchon)

  • 본 연구는 도시화 과정에서 간과되었던 우리의 일상문화로서의 도시정원가꾸기(Urban Gardening)를 재발견하는 과정으로, 자생적 도심주거지 해방촌에서 나타나는 도시정원가꾸기의 이해와 장소적 특성 고찰을 통해 현대적 의미의 도시정원이 단독주택지 주민들의 삶과 연계하여 물리적 환경 개선과 지역 주민들간 관계 형성을 견고하게 하는 매개로서 기능하고 있음을 파악하였다. 연구대상지인 해방촌(解放村)은 자생적으로 형성된 단독주택지지역으로, 1940년대 후반 도시빈민의 임시 정착지로 형성되었으나 '60년대 주거지 양성화와 '70년대~'90년대 자력 재개발사업에 의한 현지 개량을 거치면서 서울 도심의 대표적인 서민 주거지로 자리 잡았다. 특히 이 지역은 용산 미군부대와 남산으로 둘러싸여 섬처럼 존재하는 지리적 여건을 지니며 주거지 개발의 붐에서 비켜 있었으므로 다른 도심주거지에 비해 물리적 환경변화가 적고 주민들 역시 오랜 시간동안 지역에서 거주해온 특징을 발견할 수 있다. 이러한 맥락을 바탕으로 해방촌을 크게 3개의 권역으로 나누고, 도시 조직과 주거형태 변화에 따라 다른 양상으로 전개된 도시정원가꾸기의 특징과 일상문화를 면밀히 관찰하였다. 서울민의 대중적 삶이 적층되어 있는 해방촌의 도시정원가꾸기는 근 현대 주거사의 변천과 생활상 변화에 따라 다양한 공간과 속성으로 변용, 진화해 온 것으로 해석할 수 있다. 이들 장소는 고밀화 된 주거지에서 발현된 자연-정원에 대한 주민들에 욕망의 표현이며, 가꾸기를 통해 드러나는 참여의 장, 실천적 행위의 장소이다. 지역 주민들의 삶의 양식이 누적된 독특한 일상생활문화로서 지역 고유의 경관을 형성하고 있다. 주거와 마당-정원의 원형적 관계가 남아있는 단독주택지에서 도시정원은 반사적 공간의 공공적 공유가 발생하는 특성이 나타나며, 중층적 공간 구조 사이에서 창조적인 공간 사용을 발견한다. 협소한 대지의 효율적 공간 활용으로 나타나는 도시정원은 지역 주민들의 교류를 촉발하는 역할로서 마이크로 규모의 녹지 공간일 뿐만 아니라 복잡한 구조로 만들어진 정원의 도시 이미지를 형성한다. 도시정원에 대한 관심은 도시에서의 먹거리 생산이라는 역할을 넘어 건강과 취미, 교육, 공동체 회복 등 도시민들의 삶의 질 향상이라는 측면과 닿아 있다. 일상문화로서의 도시정원가꾸기는 사소한 다수가 발휘하는 고유한 역량과 독특한 특성에 대한 재발견으로 일상생활문화로서의 공간 활용과 장소에 내재된 가능성, 가치의 중요성을 다시금 상기한다.

황사 발생시 아파트 실내에서 미세먼지 크기별 농도 특성 (Characteristics of Indoor Particulate Matter Concentrations by Size at an Apartment House During Dusty-Day)

  • 본 연구에서는 국내에서 가장 일반적인 유형의 주택으로서 3인 가구가 거주하는 공급면적 $117.69m^2$의 일반형 아파트를 측정 주택으로 선정하여 실증 측정을 진행하였다. 황사 발생이 포함된 3일 측정기간 동안 미세먼지 변화 특성을 관찰하여 분석하였다. 관측된 미세먼지($PM_{10}$)를 상대적으로 큰 먼지($PM_{10-2.5}$)와 초미세먼지($PM_{2.5}$)로 나누어 대기와 실내의 먼지의 농도변화를 분석하였다. 연구 결과로 재실자의 활동이 없는 외출이나 취침 기간에는 주택의 침기 특성을 확인할 수 있었다. 미세먼지 중 상대적으로 큰 $PM_{10-2.5}$ 크기 입자는 외기의 풍속이 5 m/s까지 증가하여 침기량이 증가해도 대기 농도 대비 실내 농도비가 7% 이하인 반면, $PM_{2.5}$의 경우 외기 속도가 5 m/s까지 증가하면 50% 이상으로 증가하였다. 황사가 심한 대기 상태에도 본 연구의 측정 주택의 경우 실내 $PM_{10-2.5}$는 시평균$15{\mu}g/m^3$로 대기 농도의 5% 수준으로 일상 조건과 비슷했다. 재실자의 활동이 있는 경우에는 미세먼지 중 초미세먼지를 제외한 상대적으로 큰 먼지는 자연환기와 실내 사람의 움직임과 같은 행동 영향이 지배적이었다. 실내 $PM_{10-2.5}$ 농도는 대기 농도가 낮은 경우에는 대기 농도의 약 50%를 차지하는 경우도 있었다. 재실자의 활동 중, 초미세먼지의 발생이 클 것으로 예상되는 굽기나 튀기기가 포함된 조리나 실내 흡연이 없었기 때문에, 재실자가 없는 경우와 마찬가지로 초미세먼지는 실내 농도가 실외 농도의 추이에 따라 함께 변동하는 양상을 보였다. 외부공기가 침기에 의해 창틈, 문틈으로 유입되는 과정에서 초미세먼지는 약 30~40% 정도가 제거되고 실내로 많은 비율이 유입되기 때문에 실외 초미세먼지의 실내에 대한 영향이 컸다. 본 연구의 분석 데이터는 측정이 이루어진 대표 주택에 해당하는 것으로, 새로 지어진 기밀 성능이 높은 신축 아파트나 침기가 상대적으로 클 것으로 예상되는 오래된 단독주택에서는 다른 특성이 나타날 것으로 예상되므로, 이에 대한 추가적인 연구가 필요하다고 생각된다.
