Symptom Clusters and Quality of Life in Patients on Hemodialysis (혈액투석 환자의 증상 클러스터와 삶의 질)

  • Purpose: The purpose of the study was to investigate symptom clusters experienced by patients on hemodialysis and to identify relations between symptom clusters and quality of life. Methods: Data were collected from 127 patients at 10 local hemodialysis clinics. Symptoms were measured using 10-item physical symptom checklist as well as the Hospital Anxiety depression Scale. Quality of life was measured with the Satisfaction with Life Scale. Data were analyzed using factor analysis, Pearson correlation, and stepwise multiple regression. Results: The most frequently reported symptoms included fatigue, itching, depression, numbness/tingling, and insomnia. Four distinct symptom clusters were identified: cluster 1 was comprised of dry mouth, headache, nausea (gastrointestinal); cluster 2 of decreased appetite, insomnia (basic need); cluster 3 of itching, numbness/tingling (sensory-comfort); and cluster 4 of fatigue, depression (mood-vitality). Among the clusters, the 'basic need' cluster and 'mood-vitality' cluster had a significant negative association with quality of life. The 'mood-vitality' cluster, explained 17.4% of the variance in quality of life. Conclusion: The results of the study indicate that comprehensive symptom assessment provides better symptom management for patients on hemodialysis. Further studies are needed to verify symptom clusters identified in this study.

A study on the Preference of Material quality and the Demand Performance of Clothing for Underwear Materials (내의 소재에 대한 재질선호 및 요구 성능에 대한 연구)

  • This study has been made to examine the basic information, the preferences of material quality, and the demand performance of clothing for underwear materials between atopic patients and non-patients. The measurement tool was a questionnaire. For statistical analysis of data, crosstabs, ${\chi}^2$-test, t-test and ANOVA through SPSS for Windows(version 14.0) were used. The results obtained are as follows. The degree of the basic knowledge about clothing materials indicated that women was higher than men. The material decision method and the most considering part in case of selecting underwear products showed difference between men and women. The preference factors for underwear material quality were drawn with the five factors of sense of weight/pliability, lustering/see-through, tactility, and sense of cold and warmth. The preferred underwear material showed difference according to gender, existence and nonexistence of atopic determatitis, and degree of strength of skin itching caused by clothing materials. The demand performances of clothing in case of selecting underwear were drawn with the four factors of hygiene/practicality, skin protection, quality of materials, and aesthetic attribute.   • Purpose: The purpose of this study was to report the clinical application of traditional korean medicine(TKM) treatment to hyeolheopungyeol type of dermatoses gravidarum. Methods: We experienced four patients who had been suffering from itching, plaque, erythema during pregnancy. They had already been treated with antihistamine or ointment, but they were not improved. We diagnosed their case as hyeolheopungyeol type of dermatoses gravidarum, and treated with sopungsan-gaml. Results: After TKM treatment, itching, plaque and erythema were alleviated or resolved. And they gave birth to a healthy baby. Conclusion: The results indicate that TKM, sopungsan-gami can be effective for the management of hyeolheopungyeol type of dermatoses gravidarum.

A Clinical Report of Seborrheic Dermatitis (지루성 피부염의 한방 치험 1례)

  • Objective : The purpose of this report is to know the effect of Sopung-san and Cheonggisamseup-tang(消風散 合 淸肌滲濕湯) on seborrheic dermatitis. Methods : We experienced one case of seborrheic dermatitis patient with Sopung-san and Cheonggisamseup-tang. To evaluate the effectiveness of this treatment, we used the Visual Analogue Scale(VAS) about symptoms of flare, itching, and observe scales. Results : After the treatment, the grade of flare and itching VAS was decreased, and clinical symptoms were improved including scales. Conclusion : The result indicate that Sopung-san and Cheonggisamseup-tang is helpful in the treatment of seborrheic dermatitis. And an author consider that additional clinical studies are needed to verify the effectiveness of herbal medicine with objective assessment tools.

  • Objectives Seborrheic dermatitis is a chronic superficial eczematous dermatitis characterized by the redness and dry or greasy squama. This case study was designed to assess the effect of Shi Wei Ba Du-San gamibang (Jeongan-tang) for seborrheic dermatitis by retrospective methods. Methods From June 1, 2016 to January 31, 2020, 5 patients with seborrheic dermatitis were treated with Jeongan-tang for 3 months at Oriental Medicine Clinic. We analyzed information and body conditions of 5 patients. Redness, squama, and skin discomfort such as itching were assessed in 5 patients by using VAS (Visual Analogue Scale) score. We evaluated the symptom change by photographs and VAS score before and after the treatment. Results At first, the frequency of flushing and redness was decreased. Secondly, the scaly skin was change to normal state and itching sensation was lessened. After the treatment, the total VAS score was decreased. Conclusions Jeongan-tang can be effective in seborrheic dermatitis, and it also can be helpful to regenerate the skin condition as well.

  • Objectives : The purpose of this study to report the effect of Socheongryong-tang on solar urticaria. Methods : We treated a 26-years-old woman patient with Solar urticaria on the whole body with Korean medicine. After Korean medical treatment for 7 days, we have confirmed the improvement and disappearance of rashes, wheals, and itching. We recorded pictures of changes on symptoms. Results : After the treatment the grade of VAS was decreased. And rashes, wheals, itching were disappeared. Conclusions : Socheongryong-tang showed improvement in symptoms with solar urticaria. Thus, It is concluded that Socheongryong-tang is effective on the treatment of skin diseases such as solar urticaria.

  • Objectives: The purpose of this study is to report the clinical course of a patient with mixed hemorrhoids treated with Korean medicine alone. Methods: A patient visited for an mixed hemorrhoids was treated with Korean herbal medicine. The effects of the treatment were evaluated via photography of hemorrhoids and symptoms of pain, bleeding, prolapse, itching, discharge and Total Symptom Score(TSS). Results: After treatment, in photography before and after, the size of hemorrhoids was reduced and lost. Scores of Pain (4→0), bleeding (3→0), prolapse (3→2), itching (0→0), discharge (1→0) and TSS (11.5->3) were observed. Conclusion: In light of the reduced size of hemorrhoids and scores of symptoms, herbal medicine might be a possible treatment method on mixed hemorrhoids.

  • A cutaneous adverse drug reaction (CADR) refers to an unexpected skin and mucosal reaction caused by drug administration. In the present case, a 65-year-old male presented with generalized itching and a maculopapular rash after taking Western medication, including anticonvulsants and a nonsteroidal anti-inflammatory drug (NSAID). He was treated with Gamiseungmagalgeun-tang, a traditional Korean herbal medicine. After treatment, the patient's symptoms improved, without recurrence. Based on this experience, traditional Korean herbal medicine, including Gamiseungmagalgeun-tang, may be beneficial for improving symptoms of CADRs.

  • Objectives : The purpose of this study is to report the effect of Bangpoongtongseong-san Gagam and Yukmijihwang-tang Gagam on whole-body xerotic eczema. Methods : We treated a 63-year-old male patient who has whole-body xerotic eczema by Korean medicine. He was treated from August 23rd, 2023 to November 22th, 2023. The treatment applied was acupuncture, pharmacopuncture and herbal medicine including 'Bangpoongtongseong-san Gagam' and 'Yukmijihwang-tang Gagam'. The evaluation was conducted by photo comparison, Visual Analogue Scale(VAS) and Dermatology Life Quality Index(DLQI). Results : After three months of treatment, the symptoms were disappeared. The itching and heat sensation decreased, and redness also disappeared. VAS of discomfort was decreased from 10 to 0. DLQI was also decreased. Conclusions : Bangpoongtongseong-san Gagam and Yukmijihwang-tang Gagam helped to relieve the itching and heat of whole-body xerotic eczema.

The comparison of Effectiveness between Bee Venom and Sweet Bee Venom Therapy on Chronic Lower Back Pain. (만성요통환자에 대한 Bee Venom과 Sweet Bee Venom의 치료효능 비교 연구)

  • Objective The aim of this study is to investigate if Sweet Bee Venom(SBV) Therapy has the equal effect in comparison with Bee Venom (BV) Therapy on Chronic Lower Back Pain. Methods Clinical studies were conducted to 39 patients who were treated Chronic Lower Back Pain in Dept. of Acupuncture and Moxibustion, Dongeui University from March 1 to June 30, 2008. Subjects were randomly devided into 2 groups : BV treated group(Group A, n=19), SBV treated group(Group B, n=20) In BV treated group(Group A), we treated patients with dry needle acupuncture and BV Therapy. In SBV treated group(Group B), we treated patients with dry needle acupuncture and SBV Therapy. All process of treatment were performed by double blinding method. 1. To estimate the efficacy of venom in controlling pain, we have checked Visual Analog Scale(VAS). 2. For evaluating functional changes of patients, we have checked Oswestry Lower Back Disability Questionnaire(ODI). 3. To estimate Itching which is the most prominent symptom of allergic reaction, we have checked Visual Analog Scale(VAS). Results 1. In controlling pain, the results of BV treated group(Group A) is more effective than that of SBV treated group(Group B). 2. In promoting function, the results of BV treated group (Group A) is more effective than that of SBV treated group(Group B). 3. In controlling itching, the results of SBV treated group(Group B) is more effective than that of BV treated group(Group A). Conclusions According to the study, SBV Therapy shows more effective result than BV Therapy in controlling itching. But BV Therapy is more effective than SBV Therapy in controlling pain and promoting function.
