대장정격증(大腸正格證)과 장누수증후군(Leaky Gut Syndrome)과의 관계 고찰(考察) (Study on the Relationship between Dae-Jang-Jung-Gyeok(大腸正格) and Leaky Gut Syndrome)

  • Objective : If the penetration ratio of the intestinal mucosa is increased, the toxic or unhealthy materials - which should not be absorbed into our bodies - will be come into our bodies. They cause a sort of anti-toxic response or confusion of the immune system, and ultimately bring various types of diseases. This syndrome is related "Poison in Excrement" or "Internal Damage(內傷發癍)" and Leaky Gut Syndrome, so I will study on the relationship between them. Method : Study on the relationship between Dae-Jang-Jung-gyeok and Leaky Gut Syndrome. Result : Korean doctors in ancient times called this syndrome "Poison in Excrement" or "Internal Damage(內傷發癍)" and treated it with "Dae-Jang-Jung-Gyeok(大腸正格)". Leaky Gut Syndrome is a sort of clinical lesion, which allows foreign and harmful toxins in and results in a disorder of the immune system due to the leaking intestinal mucosa. Conclusion : Based upon the analyses, Dae-Jang-Jung-Gyeok and Leaky Gut Syndrome are closely related. Therefore diseases caused by the Leaky Gut Syndrome might be expected to be cured by Dae-Jang-Jung-Gyeok. Particularly the use of treatments for self-immune diseases and allergic diseases such as atopyic dermatitis and articular rheumatism should be expected to reveal a new path of treatment for other disorders such as Leaky Gut Syndrome.

비침습적 초음파 조직 탄성도 측정 시스템 개발 (Development of a Non-invasive Ultrasonic Measurement System for tissue elasticity)

  • Diseases caused by indurate tissues of the internal organs are liver cirrhosis and abdominal sclerosis. The cause of chronic gastro-intestinal disease is a digestive system disorder and a defecation disorder. They impede peristaltic movement and digestive system with the symptom that indurate tissues. The purpose of the present study was to determine the disease grade quantitatively by measuring an indurated standard of tissues and organs. For the measurement of elasticity, we designed the system that measure the displacement of the substance and approved pressure using ultrasound transducer. For verification of developed system, we compared elasticity as results of experiment between the developed system and public elasticity measurement machine at individual plastic phantoms made by plastic hardener and softener. Elasticity of the plastic phantoms is averagely 0.007MPa lower measured by developed system than Micro-indenter, and less than 10% errors. Comparing with economical value and accuracy between developed system and Micro-indenter, the system is significant of measurement for tissue elasticity. Thus, it is possible to measure a elasticity at human tissue and organ. A chronic gastro-disease as well as grade could be decided objective validity using this system.

담도폐쇄증 진단에서 DISIDA 간담도주사의 진단적 의의 (Diagnostic Utility of Tc-99m DISIDA Hepatobiliary Scintigraphy in the Diagnosis of Biliary Atresia)

  • 목 적: 신생아 담즙정체증의 흔한 원인중 하나인 담도폐쇄증의 진단에 많이 이용되고 있는 Tc-99m DISIDA간담도 주사의 진단적 의의에 대하여 알아보고자 하였다. 방 법: 1995년 1월부터 1999년 8월까지 4년 8개월 동안 서울 중앙병원 소아과와 소아외과에서 신생아담즙정체증으로 Tc-99m DISIDA간담도 주사를 시행한 60명의 환자를 대상으로 그 결과를 분석하였다. 결 과: 전체 60명의 환자 중에서 담도폐쇄증으로 14례(23%), 신생아 간염으로 33례(55%), 간내 담도형성부전증으로 9례(15%), 경정맥 고영양법으로 인한 황달로 4례(7%)가 진단되었다. Tc-99m DISIDA 간담도주사의 민감도는 100%를 보였고 특이도는 80%였다. 결 론: Tc-99m DISIDA 간담도주사에서 장관내 방사능이 관찰되면 담도폐쇄증을 진단에서 제외할 수 있지만 관찰되지 않을 경우에는 경피간침생검이나 시험적 개복술 등의 적극적인 진단방법을 고려해야 할 것으로 생각된다.

  • 태변장폐색증은 낭성섬유증에서 가장 초기에 나타나는 임상 증상으로 태변이 장관내 축적되어 회장말단의 폐색을 일으키고 복부팽만, 담즙성 구토, 태변 배출 지연이 동반된다. 낭성섬유증은 7번 염색체 장완에 위치한 막전도조절유전자의 돌연변이로 야기되며 한국인에서는 드문 질환이다. 지금까지 대한민국에서 보고된 호흡기 질환에 동반된 낭성섬유증과 달리, 저자들은 태변장폐색증에 동반된 낭성섬유증이 유전자 검사로 진단된 1례를 경험하였기에 보고하는 바이다.

The Beneficial Effect of Adenophorae Radix on DSS-induced Colitis in Mice

  • Ulcerative colitis (UC) is an inflammatory bowel disease, which is a chronic gastrointestinal disorder. Adenophorae Radix (AR) has been used as a traditional medicine for various diseases including strengthening cardiac function, allaying a fever, and easing pain and cough. However, the regulatory effects of AR in intestinal inflammation are not yet understood. This study attempted to determine the effect of AR in dextran sulfate sodium (DSS) - induced colitis in mice. The colitis mice were induced by drinking water containing 5% DSS for 7 days. The results showed that mice treated with DSS showed remarkable clinical signs, including weight loss, and reduced colon length. Administration of AR attenuated weight loss, colon shortening and inhibited the levels of interleukin (IL) - 6 in DSS - treated colon tissues. These results provide experimental evidence that AR might be a useful therapeutic medicine for patients with UC.

과민성(過敏性) 장증후군(腸症候群)(설사형(泄瀉型)) 환자에 투여한 안신보비탕(安神補脾湯)의 임상적(臨床的) 검토(檢討) (Clinical Study on the Ansinbobitang for the Treatment of Irritable Bowel Syndrome)

  • IBS(Irritable bowel syndrome) means disorder of intestinal motility. To increase the cure rate of IBS, we have observed and analysed 26 persons who have administered Ansinbobitang(安神補脾湯), Ansinbobitang(安神補脾湯) consists of herb medicines which have tranquilizational(安神), reinforcing the spleen(補脾), and tonifying the kidney's Yang(補腎陽) effects. From this study we obtained as follows. 1. 16 cases of IBS patients were male and 10 cases were female. Male of female ratio was 1 : 1.6, and the peak age in incidence of IBS was in End and 3rd decade. 2. Duration of IBS attack, the most frequence is $0{\sim}1$ year and $3{\sim}5$ years. 3. Seasonal distribution of IBS attack reveals the peak occurrence in autumn. 4. Duration of treatments, the most frequence is $2{\sim}3$ weeks. 5. Ansinbobitang(安神補脾湯) shows 88.5% of effective rate.

Development of Modified Phenylalanine Ammonia-lyase for the Treatment of Phenylketonuria

  • Phenylketonuria (PKU) is an inherited metabolic disorder caused by mutations in the phenylalanine catabolic enzyme, phenylalanine hydroxylase (PAH). The use of phenylalanine ammonia-lase (PAL) by oral and parenteral routes as a therapeutic drug for PKU has been severely limited due to inactivation by intestinal proteolysis and immune reactions. PEGylation was applied to PAL to reduce the degrees of antigenicity and proteolytic inactivation. Kinetic experiments with native PAL and pegylated PALs were performed, and pH stability, temperature stability, and protease susceptibility were evaluated. Enzyme linked immunosorbent assay (ELISA) was carried out to measure the immune complex between pegylated PALs and antiserum that had been extracted from a PAL-immunized mouse. Pegylated PAL, especially branched pegylated PAL (10 kDa, 1:32), was more active for phenylalanine and more stable in pancreatic proteases than native PAL. Native PAL was optimal at pH 8.5, corresponding to the average pH range of the small intestine; the same finding was noted for pegylated PALs. All linear and branched pegylated PALs had low reactivity with mouse antiserum, especially the 1:16 formulation with linear 5-kDa PEG and the 1:32 formulation with branched 10-kDa PEG. Therefore, we suggest the 1:32 formulation with branched 10-kDa PEG as the most promising formulation for enzyme replacement therapy.

The Ameliorative Effect of Rubi Fructus on DSS-induced Colitis in Mice

  • Ulcerative colitis (UC) is an inflammatory bowel disease and a chronic gastrointestinal disorder. Rubi Fructus (RF), the fruit of Rubus coreanus Miquel, is known to exert several pharmacological effects including anti-oxidative, anti-obesity and anti-inflammatory properties. However, the improving effect and mechanism of RF on intestinal inflammation is not been fully understood. The purpose of this study was to investigate the regulatory effect of RF on dextran sulfate sodium (DSS)-induced colitis in mice. We evaluated the effects of RF on DSS-induced clinical signs by analyzing weight loss and colon length. The inhibitory effects of RF on inflammatory mediators such as prostaglandin E2 (PGE2), cyclooxygenase (COX)-2, as well as the activation of nuclear factor-κB (NF-κB), were determined in colitis tissue. Our data indicated that mice treated with DSS showed clinical symptoms of colitis, including weight loss, colon length decrease and diarrhea. However, we observed that RF treatment significantly improved these clinical symptoms of weight loss, colon length decrease and diarrhea induced by DSS. RF inhibited the enhanced levels of COX-2 and PGE2 caused by DSS. We also showed that the anti-inflammatory mechanism of RF by suppressing the activation of NF-kB in DSS-treated colon tissues. Collectively, the findings of this study indicate the prospect of developing new drugs from RF for UC treatment.

The Beneficial Effect of Platycodon grandiflorum on DSS-induced Colitis through Regulation of HIF-1α in Mice

  • Ulcerative colitis (UC) is a typical inflammatory colon disorder. Platycodon grandiflorum (PG) is known to exert various beneficial effects including anti-oxidative and anti-bacterial properties and improvements in liver function. However, the improving effect and mechanism of PG on intestinal inflammation are not fully understood. The present research was designed to investigate the effect of PG on the clinical signs of DSS-induced colitis in mice. The ameliorative effects of PG on inflammatory cytokine expression and the activation of hypoxia-inducible-factor (HIF)-1α in DSS-treated colon tissue were also determined. Our results showed that mice treated with DSS displayed the main clinical symptoms of colitis, including weight loss, bloody stools, decrease in colon length and diarrhea and PG treatment significantly improved the clinical features induced by DSS in mice. PG inhibited the increase in the levels of inflammatory cytokines caused by DSS in colon tissues. We also showed that the anti-inflammatory mechanism of PG involved suppressing the activation of HIF-1α in DSS-treated colon tissues. Collectively, the findings of this study indicate the prospect of developing new drugs from PG for UC treatment.

과민성대장증후군의 형상의학적 고찰 -동의보감(東醫寶鑑)을 중심으로- (Hypersensitive Large Intestine Syndrome in Hyungsang Medicine)

  • The writer reports the conclusions gained from study about the cause of the hypersensitive large intestine syndrome with Dongeuibogam as the central figure through researching the disharmony among Body Essence, Vital Energy, Mentality, and Blood, mutual action of five viscera and six bowels, and external shapes. The hypersensitive large intestine syndrome is generally chronic and recurred in many cases, so it is more efficacious than symptomatic to treat according to find the contradictions of individual shapes. The shapes and cases suffering frequently the hypersensitive large intestine syndrome are Gi-kwa and Sin-kwa, having a long nose, having a bruised spot on Triple warmer, man with inclined mouth, Taeeum type, man with congested fluids, man with colic symptoms. The hypersensitive large intestine syndrome in Oriental medicine is recognized of diarrhea, constipation, abdominal pain, abdominal distention and fullness caused by seven emotions. In Dongeuibogam it can be found out the similarity in depressive symptoms due to disorder of Gi, stagnation of Gi, dysphasia due to disorder of Gi, diarrhea due to disorder of Gi, fullness of due to Gi, diarrhea due to phlegm-retention, retention of undigested food, immoderate drinking, hypo-function of the spleen, or deficiency, abdominal pain from colic symptom, and difficulty in defecation and urination, internal injury, diarrhea due to weakness and fatigue. If the Jung, Gi, Sin, and Hyul composed the human body is broken harmony, the function of large intestinal transmission would be fallen, so similar symptoms like the hypersensitive large intestine syndrome are gotten. Especially Gi-kwa suffers diarrhea, constipation abdominal pain, and abdominal distention and fullness due to depressive symptoms from disorder of Seven emotions or Seven Gi. And Sin-kwa suffers from the hypersensitive large intestine syndrome due to emotional restlessness having an influence on rhythmic movement of abdomen. Examining between five viscera and six bowels and the hypersensitive large intestine syndrome, Liver cannot disperse well having influence on mutual relation of Liver-Large intestine, Heart reduces the function of defecation and urination not to control the seven emotions, Lung having exterior and interior relation with intestine has an influence on primordial energy and let the main symptoms occur, Spleen circulating the body fluid let the main symptoms occur due to malfunction of circulation, Kidney locating in lower part of the body has deep connection with intestine, so let the disorder. Urinary bladder is connected with intestine in moisture metabolism, Stomach is connected in receive and transmission, Small intestine is connected in absorption and excretion, from small intestine pain disturbing the abdominal movement, Samcho managing the catharsis of lower heater if declined its function causes the hypersensitive large intestine syndrome. The colic symptoms of Front private parts which disorder in lower abdomen give rise to abdominal pains, difficulty in defecation and urination due to Cold are similar to the hypersensitive large intestine syndrome. The treatments of applying the shapes of colic syndrome advocated by Master Park can be efficacious cure in clinic. Researching after the clinical cases of Master Park advocating Hyungsang medicine, we came to know that plenty of prescriptions of internal injury are applied and take good effects.
