수술실에서 중심정맥관 삽입 시 번들적용이 중심정맥관 관련 혈류감염에 미치는 영향 (Impact of Central Line Insertion Bundle on the Adherence of Bundle and Central Line-Associated Bloodstream Infections in the Operating Room)

  • Background: This study purposed to examine how the incidence of the central line-associated bloodstream infection (CLABSI) in a operating room (OR) is affected by bundle application on central line insertion (CLI) practice. Methods: The study design was a pretest-posttest experimental trial. The subjects were 83 patients before the bundle application on CLI and 70 patients after. Results: The compliance(%) of bundle on CLI of among those who observed all of the five items increased from 7.2% before the intervention to 72.9% after. By items, compliance with the maximal barrier precaution was 100% for the use of a mask and cap before and after the intervention, but increased from 73.5% before the intervention to 88.6% after for the hand hygiene, from 73.5% to 88.6% for the use of a sterile gown, and from 9.6% to 75.7% for the use of a sterile large drape covering the whole body. CLABSI did not happen on CLI either before or after the application of the bundle intervention. Conclusion: Bundle application increased compliance with the use of a sterile gown and the use of a sterile large drape. However, its effect in the prevention of CLABSI was not clear probably due to the short period of intervention in a single hospital.

  • 본 연구는 관상동맥중재술을 받은 남성의 건강행위 이행에 미치는 영향을 경과기간을 중심으로 파악하기 위한 서술적 상관관계 연구이다. 연구 대상자는 일개 대학병원에서 관상동맥중재술을 받은 남성 249명이며, 2017년 9월 1일부터 2018년 4월 30일까지 자료수집이 이루어졌다. 자료는 Stepwise multiple regression을 사용하여 분석하였다. 연구결과 건강행위 이행에 영향을 미치는 요인은 자기조절 중 자율적 동기(β=.35, p<.001), 교육요구도 중 위험요소(β=-.26, p<.001), 질병의 경과기간(β=.19, p<.001), 결혼 여부(β=.15, p=.001), 최종 학력(β=-.12, p=.035)이며, 총 설명력은 29.1%였다. 관상동맥 중재술을 받은 남성 환자의 건강행위 이행을 높이기 위해서는 중증도가 높고 경과시간이 1년 미만인 대상자의 자율적 동기를 강화할 수 있는 프로그램 개발이 필요하다.

혈액투석 환자를 위한 통합적 관리 프로그램의 개발 및 효과 (Development and Evaluation of Integrated Management Program for Hemodialysis Patients)

  • Purpose: This study aimed to develop and evaluate an integrated management program to enhance self-efficacy, compliance with sick-role behaviors, symptom management, and biomarker indication in hemodialysis patients. Methods: The integrated management program was developed through a systematic review of literature, analysis of relevant online data, and expert validation. It comprised 480 min of video-based education delivered eight times over four weeks, supplemented by weekly phone consultations and text message support from a nurse. To evaluate the program's effectiveness, it was administered to 44 patients with hemodialysis in a single group in a pre-post test experimental study. Changes in self-efficacy, sick-role behavior compliance, dialysis symptom index, and biomarkers were assessed. Results: The program yielded statistically significant improvements in self-efficacy (t=-7.13, p<.001), sick-role behavioral compliance (t=-7.35, p<.001), dialysis symptom index (t=4.32, p<.001), and blood urea nitrogen levels (t=2.55, p=.014) among the participants. Conclusion: The integrated management program is an effective intervention for improving hemodialysis patients' self-efficacy, compliance with sick-role behaviors, and experience of symptoms. Additionally, it is considered an intervention with high clinical applicability and efficiency through video reproducibility.

일 지역 노인의 운동이행 영향요인 분석 (Factors Influencing Exercise Compliance among Older Adults)

  • Purpose: The purpose of this study was to examine the relationships among perceived health status, exercise self-efficacy, social support, and exercise compliance and factors influencing exercise compliance in older adults in an area. Methods: The sample consisted of 154 older adults who attended a senior welfare center in D metropolitan city. Data were collected from the 25th to the 31th of January in 2012. Results: The mean score for perceived health status was 2.94, 911.69 for exercise self-efficacy, 46.99 for social support, and 6.83 for exercise compliance. The highest score on social support domains was emotional support, followed by self-esteem, material, and informational support. There were significant correlations between perceived health status and exercise self-efficacy, between perceived health status and exercise compliance, between exercise self-efficacy and social support, between exercise self-efficacy and exercise compliance, between emotional support and exercise compliance. Findings of multiple regression indicated that only exercise self-efficacy significantly explained exercise compliance. Conclusion: Health care providers may need to develop various intervention program to promote exercise self-efficacy in order to influence on exercise compliance and adherence among older adults.

신장이식환자의 치료지시 이행정도 (A Study on the Compliance of Kidney Transplantation Recipients)

  • Purpose: The purpose of this study was to identify compliance of kidney transplantation(=KT) recipients and the factors influencing on compliance of KT recipients. Methods: One hundred patients who visited out-patients department after KT at S hospital transplant center in Seoul were enrolled in this study. Structured questionnaires of compliance, self-efficacy and family support were used to collect data. The data was collected from April 21 to May 21 in 2011. The data were analyzed by descriptive statistics, independent t-test, ANOVA and Pearson correlation coefficient. Statistical significance was accepted to the level of p<.05. Results: The mean score of compliance was 4.37(range from 1 to 5), and that of medication compliance was the highest and that of self care compliance was the lowest. There were no differences of compliance according to characteristics of the subjects. Compliance of the subjects had positive correlation with self-efficacy(r=0.23, p<0.05) and family support(r=0.33, p<0.05). Conclusion: Based on these results, it can be concluded that the nursing intervention program to improve self care using self-efficacy and family support need to be developed.

관상동맥질환자의 D유형 성격과 치료지시이행의 관계에 미치는 극복력의 매개효과 (The Mediating Effect of the Resilience on the Relationship between Type D Personality and Compliance in Patients with Coronary Artery Disease)

  • Purpose: This study aimed to identify the mediating effect of the resilience on the relationship between type D personality and compliance of Coronary Artery Disease (CAD) patients. Methods: The subjects were 236 CAD patients who visited the cardiology outpatient clinics of D University Medical Center in B City in Korea. The type D personality, resilience, and compliance of subjects were measured using structured questionnaires from July to August, 2014. The mediating effect was analyzed by a multiple hierarchical regression. Results: The subjects with type D personality accounted for 30.9% of all study subjects. The resilience and compliance of type D subjects were significantly lower than those of non type D subjects. There was a significant correlation between resilience and compliance. The direct effect of type D personality on compliance was not significant (${\beta}=-.07$, p=.243), while the indirect effect of type D personality on compliance via resilience was significant (${\beta}=.23$, p<.001). Conclusion: On the basis of above results, it can be concluded that the resilience had completely mediating effect on the relation between type D personality and compliance of CAD patients. Further studies need to be done to develop the intervention enhancing resilience of CAD patients with Type D personality.

혈액투석 환자의 역할행위이행 영향요인 (Factors Influencing Sick Role Behavior Compliance in Patients on Hemodialysis)

  • Purpose: This study was a descriptive survey research to identify the factors that influence sick role behavior compliance in patients on hemodialysis. Methods: Structured surveys were used to collect data from 170 patients who are on hemodialysis three times a week through outpatient care at a university hospital located in G City using tools measuring resilience, family support, and sick role behavior compliance. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient, and Stepwise multiple linear regression. Results: The results showed that factors that influenced sick role behavior compliance in the subjects were in the order of family support (${\beta}=.27$, p<.001), age (${\beta}=.27$, p<.001), and resilience (${\beta}=.23$, p=.003). Resilience, family support, and sick role behavior compliance were positively correlated. Factors influencing hemodialysis patients' sick role behavior compliance included family support, age, and resilience. These variables explained 30.2% of the variance in sick role behavior compliance. Conclusion: Based on these results, care intervention and the development of a training program that involves family in the treatment plan and process to support and encourage patients are needed to increase the sick role behavior compliance in patients on hemodialysis.

학령전기 아동의 그림동화책 제공이 편도선 수술 후 통증 및 치료이행에 미치는 효과 (The Effects of Illustrated Book Information on Post Tonsillectomy Pain and Compliance of Care in Preschoolers)

  • Purpose: The purpose of this study was to evaluate the effect of illustrated children's book in providing information about tonsillectomy to improve post tonsillectomy pain control and compliance to treatment for preschoolers. Methods: The design of the study was a quasi-experimental, post-test only design. Subjects were preschoolers aged between 3 and 7 years old who were admitted to a hospital for tonsillectomy. A group of 25 children were assigned to an experimental group, and another 25 were assigned to a control group. The data were analysed using SPSS 12.0 program. The homogeneity between two groups was examined using $x^2-test$. The hypotheses testing was conducted using t-test. Results: The experimental group had lower pain score than control group at 8 hour after tonsillectomy (p<.05). Pain score was also lower in the experimental group after 24 hour after the surgery (p<.05). The experimental group had higher pain questionnaire score than control group at 8 hours after surgery, however the difference was not significant. The experimental group showed better compliance to treatment than the control group (p<.05). Conclusion: Providing illustrated children's book for the purpose of providing information about tonsillectomy was effective intervention for the control of post tonsillectomy pain and improvement of compliance to treatment for preschoolers. Illustrated children's book could be applied as a nursing intervention to relieve post tonsillectomy pain and improve compliance to treatment for preschoolers.

관상동맥중재술 시행 환자의 질병관련지식, 환자역할행위이행 및 교육 요구도에 대한 시기별 비교 연구 (A study on the Difference of Disease Related Knowledge Level, Compliance of Health Behavior, and Educational Needs according to Time in Percutaneous Coronary Intervention Patients)

  • Purpose: The purpose of this study was to determine the differences in the level of disease related knowledge, compliance of health behavior, and educational needs in relation to time (at discharge and 6 months after discharge) among patients underwent percutaneous coronary intervention (PCI). Methods: Data were collected from January 1, 2006 to September 30, 2006 and a total of 60 patients participated in the study. The survey was conducted in patients underwent PCI at the time of discharge right after discharge education was provided and at a follow up visit which was 6 months after discharge. Results: The level of disease related knowledge (p<.001), the compliance of health behavior (p<.001), educational need (p=.496), the sub-item of sexual life (p<.001), follow up (p<.001), diet (p=.021), stress (p<.001) in compliance of health behavior, and the sub-item of specific character of disease in educational needs (p=.015) were significantly different between discharge and 6 months after discharge. Conclusion: The results of this study suggested that further education should be provided to the patients underwent PCI regarding medication, smoking cessation, daily life and exercise at a time of 6 months after discharge in order to increase patient compliance of health behavior.

  • Purpose: The purpose of this study was to investigate effects of providing written information for coronary artery disease on health behavior compliance related self-efficacy, knowledge of disease, anxiety, and educational satisfaction depending on patients' health literacy. Methods: The participants in this study were 40 patients who underwent coronary angiography or coronary intervention, and depending on the level of health literacy 30 patients were high group and 10 patients were low group. Each group was evaluated on health behavior compliance related self-efficacy, knowledge of disease, anxiety, and educational satisfaction with providing written information. Results: By providing written information in the group with high health literacy, there was a significant difference in health behavior compliance related self-efficacy, knowledge of disease, but there was no significant difference in anxiety variable. On the other hand, there was no significant difference in health behavior compliance related self-efficacy, knowledge of disease, and anxiety by providing written information in the group with low health literacy. Also, there was no significant difference in the educational satisfaction between high and low group of health literacy after providing written information. Conclusion: It is necessary to develop educational materials that can be applied to clinical nursing field with considering health literacy of patients with coronary artery disease. In addition to providing written information, it is also necessary to develop other educational intervention programs such as video and personalized counseling that may be helpful for coronary patients with low health literacy and investigate their effectiveness.
