한의원에서 도침치료로 영상의학적 소견을 호전시킨 요추 추간판 탈출증 증례 1례 (Case Report: Changes in Magnetic Resonance Imaging in Lumbar Disc Herniation Treated with Korean Medicine)

  • Objectives: The purpose of this case study was to report the effectiveness of Korean medicine treatment on severe lumbar disc herniation patient diagnosed by magnetic resonance imaging (MRI). Methods: The patient was treated with complex Korean medicine treatment, including acupuncture, pharmacopuncture, acupotomy, and Chu-na treatment. The treatment effect was evaluated by a Numerical Rating Scale (NRS), and MRI. Results: After the treatment, the NRS score of pain intensity was reduced from severe to mild degree. The volume of extruded disc in MRI images was clearly reduced. Conclusion: Korean medicine treatment might be effective in patients with lumbar disc herniation.

MRI를 이용한 양측성 약관절 내장증의 임상적 평가 (CLINICAL EVALUATION OF BILATERAL INTERNAL DERANGEMENT OF TEMPOROMANDIBULAR JOINT BY MAGNETIC RESONANCE IMAGING)

  • This is a retrospective study of 31 patients, who were diagnosed as bilateral TMJ internal derangement in consequence of magnetic resonance imaging (MRI) of the temporomandibular joint (TMJ) at Department of Dentistry, Ajou University Hospital, and who were compared clinical findings with radiological findings. The results obtained were as follows. 1. The average of the patients of bilateral TMJ internal derangement was 27.3 years with a range from 12 to 74 years, and 93.6% of the patients were yonger than 40 years of age. The male-female ratio was 1 : 6.8, and the females of $20{\sim}29$ years were most common in 35.5%. 2. In the patients of bilateral TMJ internal derangement, the patients who complained clinical symptoms bilaterally were 38.7% and the patients who complained clinical symptoms untilaterally were 61.3%. 3. The most common disc position of bilateral TMJ internal derangement was anterior disc displacement without reduction on both sides in 38.7%. The most common disc configuration of bilateral TMJ internal derangement was biconcave on both sides and amorphous on both sides in closed mouth, and was amorphous on both sides in open mouth. 4. In the patients of bilateral TMJ internal derangement, the joints which really showed clinical symptoms were most common in anterior displacement without reduction of 48.9%.

Internal derangement as a predictor of provoked pain on mouth opening: A magnetic resonance imaging study

  • Purpose: This study investigated the relationship between pain and internal derangement in temporomandibular disorder(TMD) patients using magnetic resonance imaging (MRI). Materials and Methods: This study analyzed 356 TMD patients (712 temporomandibular joints [TMJs]). The inclusion criteria were the presence of spontaneous or provoked pain on one or both TMJs and having undergone MRI. The patients with provoked pain were divided into 3 groups: pain on palpation, pain on mouth opening, and pain on mastication. MRI was performed using a 1.5-T scanner. T1- and T2-weighted parasagittal and paracoronal images were obtained. According to the findings on the T1-weighted images, another 3 groups were created based on internal derangement: normal, disc displacement with reduction, and disc displacement without reduction. The MRI findings were independently interpreted by 2 experienced oral and maxillofacial radiologists at 2 different times. Statistical analysis was performed by the chi-square test using SPSS (version 12.0; SPSS Inc., Chicago, IL, USA). Results: Provoked pain on mouth opening was found to be correlated with internal derangement in TMD patients (P<.05). However, spontaneous pain and provoked pain on palpation or mastication were not associated with internal derangement(P>.05). Conclusion: These results suggest that internal derangement was a significant predictor of provoked pain on mouth opening.

Comparing Initial Magnetic Resonance Imaging Findings to Differentiate between Krabbe Disease and Metachromatic Leukodystrophy in Children

  • Purpose: To identify characteristic magnetic resonance imaging (MRI) features to differentiate between Krabbe disease and metachromatic leukodystrophy (MLD) in young children. Materials and Methods: We collected all confirmed cases of Krabbe disease and MLD between October 2004 and September 2020 at Seoul National University Children's Hospital. Patients with initial MRI available were included. Their initial MRIs were retrospectively reviewed for the following: 1) presence of white matter signal abnormality involving the periventricular and deep white matter, subcortical white matter, internal capsule, brainstem, and cerebellum; 2) presence of volume decrease and signal alteration in the corpus callosum and thalamus; 3) presence of the tigroid sign; 4) presence of optic nerve hypertrophy; and 5) presence of enhancement or diffusion restriction. Results: Eleven children with Krabbe disease and 12 children with MLD were included in this study. There was no significant difference in age or symptoms at onset. Periventricular and deep white matter signal alterations sparing the subcortical white matter were present in almost all patients of the two groups. More patients with Krabbe disease had T2 hyperintensities in the internal capsule and brainstem than patients with MLDs. In contrast, more patients with MLD had T2 hyperintensities in the splenium and genu of the corpus callosum. No patient with Krabbe disease showed T2 hyperintensity in the corpus callosal genu. A decrease in volume in the corpus callosum and thalamus was more frequently observed in patients with Krabbe disease than in those with MLD. Other MRI findings including the tigroid sign and optic nerve hypertrophy were not significantly different between the two groups. Conclusion: Signal abnormalities in the internal capsule and brainstem, decreased thalamic volume, decreased splenial volume accompanied by signal changes, and absence of signal changes in the callosal genu portion were MRI findings suggestive of Krabbe disease rather than MLD based on initial MRI. Other MRI findings such as the tigroid sign could not help differentiate between these two diseases.

  • Foot-and-mouth disease virus (FMDV) belongs to the aphthovirus genus within the picornavirus which has a single copy of a positive sense RNA. The translation initiation process of FMDV occurs by a cap-independent mechanism directed by a highly structured element (∼435 nt) termed an internal ribosome entry site (IRES). We have designed and prepared FMDV 4-2 RNA (28nt) by in vitro transcription. The 2D NMR data revealed that FMDV 4-2 IRES domain RNA has a flexible loop and bulge conformation. In further study, we need to make an isotope labeled RNA sample and conduct 3D NMR experiments to completely determine the 3D structure. This study may establish a new drug design strategy to treat foot-and mouth disease.

불규칙 지반 가진력을 받는 탄성진자계의 비선형진동응답 (Nonlinear Vibration Responses of a Spring-Pendulum System under Random Base Excitation)

  • An investigation into the response statistics of a spring-pendulum system whose base oscillates randomly along vertical and horizontal line is made. The spring-pendulum system with internal resonance examined is known to be a good model for a variety of engineering systems, including ship motions with nonlinear coupling between pitching and rolling motions. The Fokker-Planck equation is used to generate a general first-order differential equations for the random responses of the system are reduced to a system of autonomous ordinary differential equations. In view of equilibrium solutions of this system and their stability, the response statistics is examined. It is seen that increase in horizontal excitation level leads to a decreased width of the internal resonance region.

  • A characteristic imaging finding in cases of methanol intoxication is putaminal necrosis, but its presence is usually not suspected due to its rarity. Methanol intoxication generally produces serious neurological symptoms that include visual disturbances and diminished consciousness, characteristically with metabolic acidosis. We reported the case of a 59-year-old man who was admitted to the hospital with diminished consciousness. Acute methanol intoxication was determined as the cause. Laboratory tests revealed high anion gap metabolic acidosis. Diffusion-weighted MRI indicated diffuse symmetric diffusion restriction lesions in the subcortical white matter of both cerebral hemispheres.

  • A high ignition voltage is required for the metal halide (MHD) lamp. In this paper, the ballast ignited by internal LC resonance of buck converter with minimum size is proposed. For minimizing the ignition current for device safety, the characteristic impedance is maximized. But this results In a large steady state ripple, which may cause the accoustic resonance. The steady state ripple cancellation network using the coupled inductor is proposed.

Purity assessment using quantitative NMR: establishment of SI traceability in organic analysis

  • Quantitative nuclear magnetic resonance (qNMR) has been gaining attention as a purity assessment method. In particular, qNMR is recognized as the primary method to realize the Internal System of Units (SI) in organic analysis. The capability of quantitative analysis is recognized as the beginning of NMR development. NMR signals are proportional to the number of nuclei and qNMR has been used in various fields, such as metabolomics and food and pharmaceutical analysis. However, careful sample preparation and thorough optimization of measurement parameters are required to obtain accurate and reliable results. In this review, quantitative methods used in qNMR are discussed, and the important factors to be considered also introduced. The recent development of qNMR techniques including combination with chromatography and, multidimensional NMR are also presented.
