  • Hyponatremia which is due to excessive sodium loss in the urine and decrease in extracellular fluid volume following an acute or chronic central nervous system injury, has been conjunctively described as cerebral salt wasting syndrome (CSWS). This syndrome is often confused with dilutional hyponatremia due to inappropriate secretion of antidiuretic hormone. Accurate diagnosis and management are mandatory for improvement of the course of the disease. This report describes a case of a 31-year-old male patient with CSWS associated with tuberculous meningitis. The patient exhibited hyponatremia, polyuria, excessive natriuresis, volume depletion, and hypotension. He was diagnosed to manifest CSWS and was treated by administration of fluids, salt, and fludrocortisone. After the respective treatments, symptoms of polyuria and hypotension were gradually resolved and hyponatremia was corrected.

  • The purpose of this study is to present a case of gaze palsy due to thalamic infarction improved by acupuncture and herb medicine. Monocular gaze palsies which result from a supranuclear cerebral lesion are rare clinical manifestations. The pre-frontal cortico-oculomotor pathways travel across the thalamus and mesodienphalic junction terminating directly in oculomotor complexes. The acute disinhibition of these neurons by a posterior thalamic lesion results in a sustained but transient discharge of the medial rectus and tonic activation. Results of this study suggest a role for conservative therapy with herb medicine and acupuncture to treat gaze palsy due to thalamic infarction. Further research into oriental medical treatment for such disorders will be forthcoming.

  • Here we report the first fatality caused by H1N1 influenza virus infection with acute respiratory distress syndrome in Korea. A 55-year-old man presented at our emergency department with dyspnea, fever, diffuse myalgia and malaise. Bilateral lung air-space consolidation was detected on his initial chest radiograph combined with severe hypoxemia. He was supported by mechanical ventilation and treated with antibiotics. A nasopharyngeal aspirate was positive for influenza A rapid antigen and oseltamivir was started on day 3 of admission. The nasal swab sample was positive for influenza H1N1 virus by real-time reverse-transcriptase polymerase chain reaction. Despite aggressive treatment, he had refractory hypoxemia and uncontrolled septic shock. On day 5 of admission he went into cardiac arrest and expired.

  • Introduction: The aim of this study was to report the effect of Korean medicine treatments on a stroke patient with homonymous hemianopsia. Case Presentation: A 50-year-old male with a posterior cerebral artery infarction had continuously deteriorating symptoms of right homonymous hemianopsia, fatigue, dizziness, and light sensitivity. Treatment with Korean medicine therapy, including acupuncture and herbal medicine significantly improved the patient's symptoms. The result of a visual field test (VFI) was also improved after treatment, and no side effects were observed during treatment. Conclusion: Korean medicine treatment can be considered an effective treatment for homonymous hemianopsia in stroke patients.

  • Background: Zoster vaccination is recommended for people with a history of herpes zoster (HZ), but the most effective timing of vaccine administration after zoster illness is unresolved. This prospective observational study compared the immunogenicity and safety of administering HZ vaccine at 6-12 months and 1-5 years after zoster illness. Materials and Methods: Blood samples were collected before the administration of live zoster vaccine and 6 weeks after vaccination. Varicella-zoster virus (VZV) IgG concentrations and T-cell responses were assessed by glycoprotein enzyme-linked immunosorbent assay and interferon-${\gamma}$ enzyme-linked immunospot assay (ELISPOT), respectively. Results: The baseline geometric mean value (GMV) of VZV IgG was higher in the 6-12 months group than in the 1-5 years group (245.5 IU/mL vs. 125.9 IU/mL; P = 0.021). However, the GMV increased significantly in both groups (P = 0.002 in the 6-12 months group; P <0.001 in the 1-5 years group). The results of the ELISPOT assay were not significant for differences of the GMV between baseline and 6-week post-vaccination groups, while the GMV increased significantly in both groups (P = 0.001 in the 6-12 months group; P <0.001 in the 1-5 years group). Conclusion: The immunogenicity of zoster vaccine may be similar whether administered 6-12 months, or >1 year after zoster illness. Trial Registration: ClinicalTrials.gov Identifier: NCT02704572

  • Introduction: The aim of this study was to report the effect of Korean medicine treatments on an elderly patient with acute hematochezia and anemia Case presentation: An 81-year-old female with chronic multifocal middle cerebral artery infarction had symptoms of acute hematochezia and anemia. Method: The patient was treated with Korean medicine therapy. Results: Treatment with Korean medicine, including herbal medicine, significantly improved the patient's symptoms. After treatment, her hematochezia was improved and the results of a complete blood count (CBC) also improved. No side effects were observed during treatment. Conclusion: Korean medicine treatment can be considered an effective treatment for acute hematochezia and anemia in an elderly patient.

  • Objectives: This study aimed to describe the effects of traditional Korean medical treatment on a patient with asthma. Methods: The patient (female, 88) was treated with herbal medicine (Sojaganggi-tang), acupuncture, and moxibustion. The effects of these treatments were evaluated using the Modified Borg Scale (MBS) and Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA). Results: Following treatment, the MBS score decreased from 7 to 4, and the QLQAKA score increased from 45 to 57. Conclusion: The results suggest that traditional Korean medicine can effectively treat patients with asthma.

  • Intestinal epithelial cells can produce cytokines and chemokines that play an important role in the mucosal immune response. Regulation of this secretion is important to prevent inflammatory tissue damage. Lonicera japonica have been shown to inhibit inflammation. We tested the effect of luteolin, a major ingredient of Lonicera japonica, on TNF-${\alpha}$-stimulated IL-8 secretion from lntestinal epithelial cells. (omitted)
