One Case Report of Internal Derangement of Knee with Patella Tendon and Posterior Cruciate Ligament Injury - with a Focus on Burning Acupuncture Therapy and Exercise Therapy - (슬개건 및 후방십자인대 손상을 동반한 슬내장증 환자에 대한 치험 1례 - 가열식 화침 치료 및 운동 치료를 중심으로 -)

  • Objectives: The purpose of this study is to evaluate the effect of the burning acupuncture therapy and exercise therapy on the internal derangement of knee with patella tendon and posterior cruciate ligament injury. Methods: Burning acupuncture therapy was administered three times and exercise therapy was administered during 4 weeks. The improvement of clinical symptom was evaluated by VAS(visual analogue scale) and KOOS(knee injury and osteoarthritis outcome score). Results: After treatment, the patient's VAS score was reduced to 0 pant. The KOOS score of pain, symptom, ADL, sport/rec, QOL(quality of life) improved from 28 to 75, from 36 to 71, from 22 to 65, from 0 to 15, from 25 to 38 respectively. Conclusions: Burning acupuncture therapy and exercise therapy fer internal derangement of knee with patella tendon and posterior cruciate ligament injury was effective. Although this case presented valuable result, further research is encouraged to confirm the effectiveness of this treatment with large number of patient.

Coexistence of Osteochondroma around the Knee and Internal Derangement of Knee (슬관절 주위 골연골종과 슬내장의 동반 발생)

  • Purpose: To find out the pathologic symptoms, and the incidence and clinical significance of the coexistence of internal derangement of knee(IDOK) in osteochondroma around the knee. Materials and Methods: We retrospectively reviewed forty-five patients under 20 years of age treated with the excision of the osteochondroma around the knee between 1995 and 2004. We analyzed age, gender, past history, family history, solitary or multiple osteochondroma, presenting pathologic symptoms, and causes of IDOK. Results: IDOK was confirmed in nine(20%) among the 45 cases. There were four cases of multiple osteochondromatosis, and IDOK was coexisted in one case among them. The most common presenting pathologic symptoms were painless mass of 38 cases, however 9 cases among them had joint pain for IDOK. There were 7 cases of meniscal tears and 2 of pathologic plica. Discoid meniscus was found in 4 cases among the 7 cases of meniscal tears. Conclusion: Coexistence of osteochondroma around knee and IDOK in this study probably represents a coincidence rather than a real association. However the incidence might be not low, special study and close observation should be done.

  • Objective: The objective of this study is to evaluate various research that have examined dynamic knee valgus and to pinpoint a straightforward, clinically practical 2D assessment method for dynamic knee valgus that is user-friendly. Design: A literature review Methods: This literature review was conducted in Pubmed, MEDLINE® and Google Scholar with the following key words: Knee valgus angle, Knee valgus evaluation, Knee valgus assessment, Dynamic knee valgus. After removing duplicate studies, 53 articles were initially chosen using this method, with 17 studies ultimately meeting the selection criteria. Results: Based on the comprehensive review of various studies, the Single Leg Squat (SLS) was identified as the most popular test method, followed by the Single Leg Landing (SLL) as the next most common test method. The Frontal Plane Projection Angle (FPPA) method was the most representative method for measuring dynamic knee valgus (DKV) during these tests. SLS was found in a total of 10 studies, while SLL was found in 7 studies. Conclusions: The most commonly proposed test for assessing DKV is measuring the SLS using the FPPA method. However, when applied to individuals without knee pathology, the discriminative power of this method may be limited. This suggests the need for further research to explore alternative methods for assessing DKV in this population.

  • Synovial hemangioma of the knee joint has been reported as a rare cause of pain, limitation of motion and hemarthrosis, often seen as an internal derangement of the knee. A 39-year-old woman presented with spontaneous hemarthrosis of the left knee joint and physical exam revealed a small painful mass adjacent to the medial side of the patella. Magnetic resonance imaging revealed an intra-articular tumor. Arthroscopic excision was performed and the histology was characteristic of hemangioma. Through this case, we discuss the diagnosis and treatment options for the synovial hemangioma of the knee.

  • Discoid meniscus is an uncommon cause of internal derangement of the knee joint. The discoid medial meniscus is much less common than the discoid lateral meniscus. A 15-year-old male student had a incomplete discoid medial meniscus of right knee combined with a horizontal tear. This was confirmed by magnetic resonance imaging(MRI) and arthroscopic procedure. Partial meniscectomy was performed arthroscopically. An unevenful recovery followed. We report a case of discoid medial meniscus with brief review of literature.

  • We present a case of double medial plica that developed on right knee joint. There has been no documented case of double medial plica of the knee joint. In a general way, double medial plica syndrome is very difficult to diagnose because it does rarely develop and symptoms are non-specific or not present. It is difficult to distinguish between pain originating from the medial plica and from other internal derangement of the knee. This patient had symptoms including aggravating right knee pain with sitting position or knee flexion for 3 months. We performed MRI and arthroscopy for more accurate diagnosis. MRI T1, T2 images showed typical double medial plica and we had performed arthroscopic excision of symptomatic medial plica in right knee joint. Arthroscopic resection provided satisfactory relief of symptom.lasting and satisfactory relieve of symptom.

  • Purpose: In patients with chronic knee pain, the diagnostic performance of $^{99m}Tc-MDP$ knee SPECT for internal derangement of knee is deteriorated due to degenerative changes. In this study, we tried to establish diagnostic criteria to differentiate medial meniscal injury (MMI) from degenerative change (DC) when the uptake is increased in medial compartment. Materials and Methods: A total of 49 knee SPECT of the patients with chronic (more than 3 months) knee pain, which showed increased $^{99m}Tc-MDP$ uptake in the medial compartment, were included in this study. The diagnosis was confirmed by arthroscopy. On knee SPECT, 3 diagnosic criteria for MMI were investigated. In Criterion I, MMI was diagnosed when crescentic uptake was observed in the medial tibial plateau. In Criterion II, crescentic uptake was further classified into anterior, mid, posterior, and diffuse patterns, according to the location of maximal uptake; and only crescentic mid, posterior, and diffuse patterns were diagnosed as MMI. In Criterion III, MMI was diagnosed when medial tibial plateau showed higher activity then medial femoral condyle. The diagnostic performance of the 3 criteria was compared. Results: The sensitivity and specificity were 93% and 14% in Criterion I, 89% and 38% in Criterion II, and 75% and 67% in Criterion III, respectively. Criterion III had significantly improved diagnostic performance, especially, specificity. Conclusion: In this study, we established a practical diagnostic criterion to differentiate MMI from DC on knee SPECT. The result is helpful to improve the diagnostic value of knee SPECT as a screening test for chronic knee pain.

  • Purpose : The purpose of the study was to evaluate the sensitivity and specificity of proton-weighted fast spin-echo MR imaging in diagnosing the meniscal tear of knee as an reasonable substitute for conventional spin-echo imaging. Materials and Methods : 102 consecutive patients, proved by surgery, proved by surgery, were participated in this study. All of them were suspected internal derangement of knee, examined by fast spin-echo MR imaging including sagittal and coronal images on a 1.5T MR imager and underwent arthroscopic or open surgery of knee joint within 2 months. These images were reviewed retrospectively by three radiologists. The sensitivity and specificity of meniscal tear were calculated. Results : The sensitivity and specificity of meniscal tear using proton-weighted fast spin-echo MR imaging were 94%, 93% inmedialmeniscus and 92%, 88% in lateral meniscus. Conclusion : The sensitivity and specificity of meniscal tear using proton weighted fast-spin echo MR images were as high as those using conventional spin-echo images. The proton-weighted fast-spin echo MR imaging can be an alternative to conventional spin-echo MR imaging in diagnosing meniscal tear of the knee.

A Rare Case of Solitary Osteochondroma at the Temporomandibular Joint: A Case Report

  • Osteochondroma is a bone tumor with cartilaginous growth potential that generally appears near the growth plate of long bones in areas such as hip, knee, and shoulder joints, related to the nature of endochondral ossification and it is known a common benign bone tumor. However, it has been very rare in craniofacial region possibly because craniofacial bone is largely formed by intramembranous ossification. Moreover, reports on the solitary type of osteochondroma in mandibular condyle has been extremely rare. Osteochondroma in mandibular condylar may show various symptoms similar to general temporomandibular joint disorders (TMDs), such as pain in the condylar area during mouth opening, internal derangement, facial asymmetry or posterior open bite. Therefore, it can be disregarded for a long time period without any adequate treatment. Surgical excision has been the treatment option for the solitary osteochondroma with very low recurrence rate reportedly. In this case report, a rare case of solitary osteochondroma developed in unilateral mandibular condyle is presented with emphasis on differential diagnosis with general TMDs.

  • The medial meniscal root tear is relatively common internal derangement of knee that is occurred in middle age without trauma. We experienced a case of traumatic medial meniscal root tear in 11 years old child and repaired it with a metallic suture anchor. Thus we report this case with a review of current literature.
