총의치의 납의치 상에서 치은 형성 시행 후에 나타나는 시간에 따른 인공 치아 이 동에 대한 평가 (Complete denture artificial teeth arrangement deformation in wax denture after festooning: deformation over time)

  • 목적: 치은 형성 후 인공치아의 이동에 대하여 평가를 시행하였다. 연구 재료 및 방법: 10벌의 동일한 치아배열과 치은의 외형을 가진 납의치를 이용하여 실험하였다. 이 납의치 상에서 치은 형성을 시행하였고, 10분 간격으로 120분 간 모델 스캔을 시행하여 치아 간 거리를 측정하였다. SPSS Ver. 22. 0를 이용하여 통계학적 분석을 시행하였다. 결과: 치은 형성 후 평균적으로 치아 사이의 거리가 0.0999 mm에서 0.1787 mm로 나타났다. 인공치아 이동의 변화율에 대해서는 40 - 50분과 50 - 60분 사이와 50 - 60분과 60 - 70분 사이에서 통계학적으로 유의한 차이가 관찰되었고, 그 이후에는 변화율의 유의한 차이가 없었다. 결론: 120분 간 측정한 결과 인공치아의 이동이 관찰되었고, 60 - 70분 이후에는 시간에 따른 평균 이동량의 변화가 관찰되지 않았다. 이를 종합하면, 치은 형성 후 60분 이상 경과된 시점에서 교합에 대한 재평가를 시행한 후에 최종 의치를 제작하는 것이 필요하다.

An analysis on the factors responsible for relative position of interproximal papilla in healthy subjects

  • Purpose: This study examined the factors that can be associated with the appearance of the interproximal papilla. Methods: One hundred and forty-seven healthy interproximal papillae between the maxillary central incisors were examined. For each subject, a digital photograph and periapical radiograph of the interdental embrasure were taken using a 1-mm grid metal piece. The following parameters were recorded: the amount of recession of the interproximal papilla, contact point-bone crest distance, contact point-cemento-enamel junction (CEJ) distance, CEJ-bone crest distance, inter-radicular distance, tooth shape, embrasure space size, interproximal contact area, gingival biotype, papilla height, and papilla tip form. Results: The amount of recession of the interproximal papilla was associated with the following: 1) increase in contact point-bone crest, contact point-CEJ, and CEJ-bone crest distance; 2) increase in the inter-radicular distance; 3) triangular tooth shape; 4) decrease in the interproximal contact area length; 5) increase in the embrasure space size; and 6) flat papilla tip form. On the other hand, the amount of gingival recession was not associated with the gingival biotype or papilla height. In the triangular tooth shape, the contact point-bone crest distance and inter-radicular distance were longer, the interproximal contact area length was shorter, and the embrasure space size was larger. The papilla tip form became flatter with increasing inter-radicular distance and CEJ-bone crest distance. Conclusions: The relative position of the interproximal papilla in healthy subjects was associated with the multiple factors and each factor was related to the others. A triangular tooth shape carries a higher risk of recession of the interproximal papilla because the proximal contact point is positioned more incisally and the bone crest is positioned more apically. This results in an increase in recession of the interproximal papilla and flat papilla tip form.

한국인의 상악중절치 위치 및 길이에 관한 연구 (A Study on the Position and Length of the Maxillary Central Incisor in Koreans)

  • The arrangement of anterior artificial teeth is an important factor on the esthetics in the construction of complete dentures and prostheses. During construction of the complete denture the position of maxillary central incisor plays an important role in the arrangements of artificial teeth. The proper position of maxillary central incisor provides natural appearance of anterior teeth and the anterior reference point of occlusal plane. Many methods have been utilized to eatablish the position of the maxillary central incisor in complete denture prosthodontics. However, there has not been provided for the reference for Korean yet. This study was aimed to extablish a guide for the position of the maxillary central inciosr in Koreans. The horizontal and vertical distance between the maxillary central incisor and incisive papilla in Koreans were measured. 1. The mean value of horizontal distance from the maxillary central incisor to the incisive papilla had significant sexual difference, The average distance was 12.648mm in male and 11.385mm in female. 2. The horizontal distance of the incisive papilla had sexual difference, the average distance was 6.182mm in male and 5.622mm in female. 3. The vertical distance from the maxillary central incisor to the incisive papilla had not sexual difference, the average distance was 7.21mm. 4. The crown length of the maxillary central inciosrs from the distal interdental papilla had not sexual difference, the right central incisor was 6.40mm(mean) and the left central inciosr was 6.43mm(mean). 5. The clinical crown length of the maxillary central incisors had not sexual difference, the right central incisor 10.35mm(mean) and the left central incisor was 10.43mm(mean). 6. The shapes of the incisive papilla were pear(68.4%), oval(16%), triangular(6%), irregular(5%), rectangular(4%), and inverted pear(1%) shape.

Morphometric analysis of maxillary alveolar regions for immediate implantation

  • PURPOSE. The purpose of this study was to provide an actual guideline in determining the shape, diameter, and position of the implant in immediate implantation by the measurement of the thickness of facial and palatal plate, the thickness of cortical bone on the facial and palatal plate, the diameter of the root, and the distance between the roots in the cadavers. MATERIALS AND METHODS. The horizontal sections of 20 maxillae were measured and analyzed to obtain the average values. Resin blocks were produced and cut serially at 1 mm intervals from the cervical line to the root apex. Images of each section were obtained and the following measurements were performed: The thickness of the facial and palatal residual bone at each root surface, the thickness of the facial and palatal cortical bone at the interdental region, the diameter of all roots of each section on the faciopalatal and mesiodistal diameter, and the interroot distance. Three specimens with measurements close to the average values were chosen and 3-dimensional images were reconstructed. RESULTS. The thickness of the facial and palatal cortical bone at the interdental region in the maxilla, the buccal cortical bone was thicker in the posterior region compared to the anterior region. The interroot distance of the alveolar bone thickness between the roots increased from anterior to posterior region and from coronal to apical in the maxilla. CONCLUSION. In this study, the limited results of the morphometric analysis of the alveolar ridge using the sections of maxilla in the cadavers may offer the useful information when planning and selecting optimal implant for immediate implantation in the maxilla.

정중구개부의 미니 임플랜트 식립과 관련된 절치관의 해부학적 연구 (Anatomic study of the incisive canal in relation to midpalatal placement of mini-implant)

  • 정중구개봉합부에 교정용 미니 임플랜트를 식립하고자 할 때 절치관의 해부학적 위치를 파악하여 이를 피하여 식립하여야 한다. 본 연구에서는 절치관의 해부학적 위치를 절치유두나 치관의 교두첨 등의 임상적 지표를 이용하여 예측할 수 있는 해부학적 자료를 마련하고자 하였다. 성인 남녀 25명의 진단 모형과 전산화 단층촬영(CT) 자료를 통해 절치관의 해부학적 형태, 절치유두와 절치관의 전후방적인 위치의 관련성 및 치관 교두첨의 전후방 위치에 따른 절치관 후방 정중구개봉합부의 골두께를 계측하였다. 계측 결과 중절치 사이 설측 치간유두에서 절치공 후연까지 구개면상의 거리는 치간유두-절치유두 후연 길이의 약 1.7배에 해당하였으며 절치유두 후연에서 절치공 후연까지의 구개면상의 거리는 평균 6.15 ${\pm}$ 1.75 mm였다. 절치공 후연의 전후방적 위치는 교두첨을 기준으로 평가 시 제1소구치의 설측교두보다 약간 전방에 해당하였으며, 이 부위 직후방에서는 골표면에 수직으로 식립하는 것보다 교합평면에 수직으로 식립할 때 골두께가 더 두꺼웠다. 또한 절치관의 폭경이 구개에서 4.03 ${\pm}$ 0.64 mm인 것을 고려할 때 절치관의 측방에 식립코자 하는 경우 정중에서 3 mm 이상 측방에 식립하는 것이 바람직하다. 교정용 미니 임플랜트를 정중구개부에 식립할 때 본 연구 결과를 절치공의 위치를 짐작하는 데 활용할 수 있을 것이다.

Evaluation of alveolar crest bone loss via premolar bitewing radiographs: presentation of a new method

  • Purpose: This study aimed to assess the interdental bone level in premolar bitewing radiographs while retracting the cheeks. Methods: Seventy-two horizontal bone defects were created on dried mandibles and maxillae. The distance from the bone level to the cement-enamel junction of premolars was detected by a modified digital caliper (considered the gold standard). The reliability of all radiographs was assessed by intraclass correlation coefficient (ICC), and the validity was compared to the gold standard using the analysis of variance test. P-values less than 0.05 were considered statistically significant. Results: This study showed that the reliability of radiographs without a cheek simulator and with 0.16 second exposure time was significantly higher than that of the two other groups (ICC=0.96 compared to 0.93 and 0.88, respectively). The results from the radiographs without a cheek simulator and with 0.16 second exposure time were more similar to the gold standard measures than those of the two other groups, although the difference was not statistically significant. Conclusions: Retracting the buccal soft tissue plays an important role in increasing the accuracy of radiographs in detecting the interdental alveolar bone level and produces more accurate results than increasing the exposure time, although it does not have a significant role in reliability of results.

Development of Novel Dental Chews for Dogs Considering Breed Variations in Anatomical Features of Dentition

  • The purpose of this study was to find anatomical variation of each small breed dog's dentition for designing more effective dental chews. Small breed dogs were volunteered for dental impressions with dental stone and alginate under tiletamine-zolazepam, tramadol, and medetomidine intravenous anesthesia. Twenty-two criteria were measured to compare dental impressions. Twenty-five dogs (9 Malteses, 8 Miniature Poodles, and 8 Shih-Tzus) were recruited. Statistically, Shih-Tzus had smaller teeth and shorter interdental spaces than those of Maltese and Miniature Poodles. Grossly, the distance between upper teeth and lower teeth was wider in Shih-Tzus. Shih-Tzu had different dental anatomy in comparative to Miniature Poodle and Maltese. It was recommended to design the dental chew considering these differences.

Cortical Bone Thickness for Mini-implant Placement in Korean

  • Recently, mini-implant is popular in the orthodontic treatment due to its simplicity and convenient surgical procedure. The objective of this study is to provide the anatomical guideline for mini-implant placement by analysing the cortical bone thickness in Korean. Hemi-sections of sixteen maxillae and twenty-two mandibles with normal teeth were used. Interdental areas between the 1st premolar and the 2nd premolar (Group 1), the 2nd premolar and the 1st molar (Gruop 2), and the 1st molar and the 2nd molar (Group 3) were sectioned and then scanned. After setting the axis of teeth, the cortical bone thickness was measured at the distance of 2 mm, 4mm, 6 mm, and 8 mm from alveolar crest. The mean thickness of cortical bone in the maxilla according to distance from alveolar crest was $1.30\;{\pm}\;0.63\; mm$ (2 mm), $1.49\;{\pm}\;0.62\; mm$ (4mm), $1.72\;{\pm}\;0.64\; mm$ (6mm), and $1.90\;{\pm}\;0.90\; mm$ (8 mm) at the buccal side and $1.33\;{\pm}\;0.47 \;mm$, $1.31\;{\pm}\;0.45\; mm$, $1.37\;{\pm}\;0.55\; mm$, and $1.39\;{\pm}\;0.58 \;mm$ at the palatal side. In the mandible, that was $3.14\;{\pm}\;1.71 \;mm$, $4.31\;{\pm}\;2.22 \;mm$, $4.23\;{\pm}\;1.94 \;mm$, and $4.30\;{\pm}\;1.57\; mm$ at the buccal side and $1.98\;{\pm}\;0.88 \;mm$, $2.79\;{\pm}\;1.01\; mm$, $3.35\;{\pm}\;1.27$ mm, and $3.93\;{\pm}\;1.38\; mm$ at the lingual side. The buccal cortical bone thickness in the maxilla was decreased from Group 1 to Group 3, while the thickness of palatal side was no change. In the mandible, it did not show a tendency at the buccal side and it was decreased from Group 1 to Group 3 without significant difference at the lingual side. Therefore, the buccal side of the Group 1 and Group 2 in both the maxilla and mandible seems to be the most appropriate site for a mini-implant placement with taking the stability and retention.
