A shape optimal design of synthetic intervertebral disc prosthesis is performed using a three-dimensional finite element method. Geometric parameters are introduced to model the cross-sectional geometry of the intervertebral disc. It is assumed that the total strain energy in the intact intervertebral disc is minimized under the normal load conditions, as often cited in other references. To calculate the stain energy density, both the nonlinear material properties and the large deformations are taken into account. The design variables of the annulus fiber angle and the area ratio of the nucleus pulposus are calculated as 31°and 30%, respectively, which complies well with the intact disc. Thus, the same optimization procedure is applied to the design of the synthetic intervertebral disc prosthesis whose material properties are different from the intact disc. For the given synthetic material properties, the values of 67°and 24% for the fiber angle and the area ratio are obtained.
Rock mass contains discontinuities such as faults and joints, and their mechanical properties and spatial distribution dominate the stability of rock mass. Because the deformation of rock mass occurs discontinuities in many cases. However in the case of poor quality rock mass under high stresses, the deformation along intact rock can also influence the structure's stability. In this study, two dimensional finite element program was developed with a rheological model to analyze the stability of the structure excavated in jointed rock mass. The “equivalent material” approach was used assuming intact rock, joints and rock bolts as visco-plastic materials. The program was verified by analysing an intact rock model, a jointed rock mass model and a reinforced jointed rock mass model. The displacement was examined in each model with changing the intact rock behaviour as elastic and visco-plastic. In the case of poor quality rock mass under high stresses, e assumption of visco-plastic behaviour of intact rock resulted in larger displacement than when assuming elastic behaviour for intact rock. Therefore it is recommended to add intact rock's visco-plastic behaviour to the existing model, which only assumes visco-plastic behaviour of joints and rock bolts.

The Grout-reinforcement technique which is widely used during the excavation of a shallow or an endangered tunnel can be classified into a couple of groups according to the properties and injection methods of the grout. The reinforcement design will, therefore, take a different approach based on the grouting method under consideration. However, the injection procedure is mainly performed by the experience of the foreman rather than engineering judgement , specifically the permeation grouting through the rock joints and its reinforcement effect Is not fully under-stood during the design stage, In this study, the anisotropic material properties of the grout-reinforced rock masses are derived from the concept of composite materials and the effect of intact rock, vertical grouting and permeation grouting is, therefore, fully accounted for. Through the parametric studies on the characteristics of rock joints, intact rock and grouting materials, various case studies have been considered. The results, illustrated via the design charts, can be directly used during the reinforcement design.

In this paper, the tensile failure behaviour of transversally bedding layers was numerically simulated by using particle flow code in two dimensions. Firstly, numerical model was calibrated by uniaxial, Brazilian and triaxial experimental results to ensure the conformity of the simulated numerical model's response. Secondly, 21 circular models with diameter of 54 mm were built. Each model contains two transversely bedding layers. The first bedding layer has low mechanical properties, less than mechanical properties of intact material, and second bedding layer has high mechanical properties, more than mechanical properties of intact material. The angle of first bedding layer, with weak mechanical properties, related to loading direction was $0^{\circ}$, $15^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $75^{\circ}$ and $90^{\circ}$ while the angle of second layer, with high mechanical properties, related to loading direction was $90^{\circ}$, $105^{\circ}$, $120^{\circ}$, $135^{\circ}$, $150^{\circ}$, $160^{\circ}$ and $180^{\circ}$. Is to be note that the angle between bedding layer was $90^{\circ}$ in all bedding configurations. Also, three different pairs of the thickness was chosen in models; i.e., 5 mm/10 mm, 10 mm/10 mm and 20 mm/10 mm. The result shows that In all configurations, shear cracks develop between the weaker bedding layers. Shear cracks angel related to normal load change from $0^{\circ}$ to $90^{\circ}$ with increment of $15^{\circ}$. Numbers of shear cracks are constant by increasing the bedding thickness. It's to be note that in some configuration, tensile cracks develop through the intact area of material model. There is not any failure in direction of bedding plane interface with higher strength.
In structures excavated in rock mass, load progressively increases to a level and remains constant during the construction. Rocks display different elastic properties such as Ei and ʋ under different loading conditions and this requires to use the true values of elastic properties for the design of safe structures in rock. Also, rocks will undergo horizontal and vertical deformations depending on the amount of load applied. However, under constant loads, values of Ei and ʋ will vary in time and induce variations in the behavior of the rock mass. In some empirical equations in which deformation modulus of the rock mass is taken into consideration, elastic parameters of intact rock become functions in the equation. Hence, the use of time dependent elastic properties determined under constant loading will yield more reliable results than when only constant elastic properties are used. As well known, rock material will play an important role in the deformation mechanism since the discontinuities will be closed due to the load. In this study, Ei and ʋ values of intact rocks were investigated under different constant loads for certain rocks with high deformation capabilities. The results indicated significant time dependent variations in elastic properties under constant loading conditions. Ei value obtained from deformability test was found to be higher than the Ei value obtained from the constant loading test. This implies that when static values of elastic properties are used, the material is defined as more elastic than the rock material itself. In fact, Ei and ʋ values embedded in empirical equations are not static. Hence, this workattempts to emerge a new understanding in designing of safer structures in rock mass by numerical methods. The use of time-dependent values of Ei and ʋ under different constant loads will yield more accurate results in numerical modeling analysis.
The biaxial failure mechanism of transversally bedding concrete layers was numerically simulated using a sophisticated two-dimensional discrete element method (DEM) implemented in the particle flow code (PFC2D). This numerical modelling code was first calibrated by uniaxial compression and Brazilian testing results to ensure the conformity of the simulated numerical model's response. Secondly, 21 rectangular models with dimension of $54mm{\times}108mm$ were built. Each model contains two transversely bedding layers. Rock and discontinuities are main factors consisting of a rock mass and the physical properties of each factor have direct effects on the mechanical stability of artificial structures in the rock mass. Because physical properties of the rock and discontinuities change a lot according to the size of test materials, a close attention is needed when the physical properties, obtained from laboratory tests, are used for the design of field structures. In this study, change of physical properties of intact materials due to the change of their size are studied. Six kinds of artificial materials including crystal, instead of an intact rock, are adopted for the study to guarantee the homogeneity of specimen materials even with relatively large size. Uniaxial strength and Young's modulus of each artificial material are checked out for a size effect and compared with the predicted values by Buckingham's theorem - dimensional analysis. A numerical analysis using PFC (Particle Flow Code) is also applied and primary factors influencing on the size effect are investigated.
Free tissue transplantation is commonly performed with the brilliant achievement in microsurgery and anticoagulants and antithrombotic agents have been prescribed in the procedures. However, there is no clean-cut indication as to which agents would be more effective in every steps and final consequences. Low molecular weight heparins inhibiting coagulation in plateletrich plasma and acting on the vascular endothelium have antithrombotic and fibrinolysis action. The experiment with rat groin free flap transplantation after 6-hour ischemia and injection of the low molecular weight heparin was performed and the results between the injection and non-injection group were analysed as follows, 1. Both of the 24-hour groups, vessel patency was not proportional to color change of the groin flap. 2. On the second day after anastomois, heparin-injection group showed intact intima, patent lumen without thrombus, and mild granulomatous inflammation around the suture material and control group with doubtful patency revealed intimal loss and thrombus formation. 3. On the 5th, 7th, and 9th postoperative day, heparin group was patent in anastomosis and showed acute inflammatory cells. 4. The 7th-week period, heparin-injection group showed intact flap color, patent lumen with intact intima and persistent foreign body granuloma.
Two case reports describing a new technique of creating a repositionable piezoelectric bony window osteotomy during apicoectomy in order to preserve bone and act as an autologous graft for the surgical site are described. Endodontic microsurgery of anterior teeth with an intact cortical plate and large periapical lesion generally involves removal of a significant amount of healthy bone in order to enucleate the diseased tissue and manage root ends. In the reported cases, apicoectomy was performed on the lateral incisors of two patients. A piezoelectric device was used to create and elevate a bony window at the surgical site, instead of drilling and destroying bone while making an osteotomy with conventional burs. Routine microsurgical procedures - lesion enucleation, root-end resection, and filling - were carried out through this window preparation. The bony window was repositioned to the original site and the soft tissue sutured. The cases were re-evaluated clinically and radiographically after a period of 12 - 24 months. At follow-up, radiographic healing was observed. No additional grafting material was needed despite the extent of the lesions. The indication for this procedure is when teeth present with an intact or near-intact buccal cortical plate and a large apical lesion to preserve the bone and use it as an autologous graft.

Although several artificial disc designs have been developed for the treatment of discogenic low back pain and used clinically, biomechanical change with its implantation seldom studied. To evaluate the effect of artificial disc implantation on the biomechanics of lumbar spinal unit, nonlinear three-dimensional finite element model of L1-L5, S1 was developed and strain and stress of vertebral body and surrounding spinal ligaments were predicted. Intact osteoligamentous L1-L5, S1 model was created with 1-mm CT scan of a volunteer and known material property of each element were applied. This model also includes the effect of local muscles which was modeled with pre-strained spring elements. The intact model was validated with reported biomechanical data. Two models implanted with artificial discs, SB Charite or Prodisc, at L4/5 via anterior approach were also developed. The implanted model predictions were compared with that of intact model. Angular motion of vertebral body, force on spinal ligaments, facet joint contact force with $2\sim12$ Nm flexion-extension moment.
