  • Spondylolysis, also known as stress injury of pars interarticularis, is a common cause of back pain in athletes, particularly children and young adults. Repeated minor traumas during flexion and extension of the spine are thought to result in bony failure due to excessive bone resorption. These lesions are common in the low back, with the majority found at the L5 vertebra. In the majority of cases of spondylolysis, non-operative treatments are recommended, such as NSAIDs, physiotherapy and bracing. Only if symptoms do not respond to conservative treatments should surgical intervention be considered. Recently, pars interarticularis injections for diagnostic and therapeutic purposes have been found to allow significant pain relief from spondylolysis for long periods. Here, the case of a 57-year-old man with spondylolysis, who suffered from back pain, which was not relieved by an epidural steroid injection, but in whom pars interarticularis injections of local anesthetic and steroid induced complete transient pain relief, following by moderate long-term relief, is presented.

  • Objective: To evaluate soft- and hard-tissue changes in the mandibular angle area after the administration of botulinum toxin type A (BoNT-A) injection to patients with masseteric hypertrophy by using three-dimensional cone-beam computed tomography (3D-CBCT). Methods: Twenty volunteers were randomly divided into two groups of 10 patients. Patients in group I received a single BoNT-A injection in both masseter muscles, while those in group II received two BoNT-A injections in each masseter muscle, with the second injection being administered 4 months after the first one. In both groups, 3D-CBCT was performed before the first injection and 6 months after the first injection. Results: Masseter muscle thicknesses and cross-sectional areas were significantly reduced in both groups, but the reductions were significantly more substantial in group II than in group I. The intergonial width of the mandibular angle area did not change significantly in either group. However, the bone volume of the mandibular gonial angle area was more significantly reduced in group II than in group I. Conclusions: The repeated administration of BoNT-A injections may induce bone volume changes in the mandibular angle area.

  • Modified rush ASIT protocol has been performed to identify the ideal schedule that allows the dose considered effective to be reached in the shortest possible time with the fewest adverse effects. Ten atopic dogs of this study includes fulfillment of Favrot's criteria. Offending allergens were identified by the use of IDST. During the induction period, the dogs were received a total of 15 injections. Ten injections were administrated every 30 minutes in a day with gradually increasing amounts and concentrations of allergens, and the last 5 injections were administered every 3 days. Disease severity was quantified by using the canine atopic dermatitis extent and severity index (CADESI). During induction period, reduction rate from baseline scores varied between 1% and 67% and the improvement of ${\geq}50%$ was recorded after induction period of therapy for CADESI-03 score in 6 of the 10 dogs. This study of ten dogs with atopic dermatitis provide evidence for the efficacy and safety of modified rush ASIT for clinical improvement.

  • The present experiments were undertaken to produce the standard antiserum for equine blood typing. The following results were obtained through ISO and Hetero Immunizations of the horses whose blood typing was analysed in the Laboratory of Racing Chemistry of Japan. 1. Of the 21 combinations of ISO-immune, 17 horses were produced antibody (about 80%) 2. Antibody titers were increased from early 1 week to late 5 weeks and any antibody titers were not be obtained in spite of the using of adjuvant and 10 repeated injections in the other 4 horses. 3. High antibody titers were obtained within the earliest period in the Dd antigen but were not increased over 32 times in spite of 8~10 repeated injections in the antigen. 4. Antibody were easily produced in the Ca antigen of ISO-Immune but production of antisera were tailed due to abscence of absorbed blood cell. 5. Antibody titers of 1,024 times were obtained through 5 injections in the Ca of HeteroImmune 6. Of the produced 15 antisera (16 system), 13 antigen (5 system) were absorbed.

  • Tachyphylaxis to local anesthetics has shown to be promote longer interanalgesic intervals between injections. Previous study demonstrated thermal hyperalgesia accelerates development of tachyphylaxis to sciatic nerve blockade in rats, while MK-801 prevents development of tachyphylaxis. Dextromethorphan is one of NMDA receptor antagonist similar to MK-801. A hypothesis that dextromethorphan would prevent the development of tachyphylaxis was tested in this study. A catheter was surgically implanted along the sciatic nerve a in rat. After recovery from surgery, the animal received repeated injections of 3% 2-chloroprocaine followed by motor block testing with or without hot-plate testing at $56^{\circ}C$. In other experiments, dextromethorphan was administrered by intraperiotneal injection prior to an injection of local anesthetic therough the implanted catheter. Sensory and motor testing was then carried out. Rats injected with 2-chloroprocaine and subjected to hot-plate testing, developed tachyphylaxis to motor and sensory blockade. However, animals pretreated with dextromethorphan did not develop tachyphylaxis over series of three injections. Dextromethorphan seems to prevent development of tachyphylaxis to sciatic nerve blockade in this rat model. Dextromethorphan, one of N-Methyl-D-aspartate receptor antagonist, can be applied to prolong the effect of local anesthetic.

  • Background Most women consider an oval-shaped face to be youthful and beautiful. In recent years, demand has grown for surgical procedures with a shorter downtime and fewer complications. These minimally invasive procedures include botulinum toxin type A (BoNTA) injection, filler injection, suction-assisted liposuction (SAL), laser-assisted lipolysis (LAL), thread lifting, and fat grafting. This study aims to introduce an effective method for creating an aesthetically pleasing lower face using a combination of minimally invasive procedures. Methods From March 2017 to March 2019, 94 patients simultaneously underwent LAL, SAL, and thread lifting. Ancillary procedures such as BoNTA injections, hyaluronic acid filler injections, and removal of the buccal fat pad (BFP) were selectively performed according to the patient's condition. Results Patients rated their postoperative satisfaction as very satisfied, satisfied, dissatisfied, or very dissatisfied. Approximately 83% of all respondents were satisfied with the results, whereas the remaining respondents had complaints regarding the outcomes. The most common reasons for dissatisfaction were a longer-than-expected recovery time and undercorrection, and the most severe complaint was skin depression as a result of overcorrection. Conclusions Our method of simultaneously performing LAL, SAL, and thread lifting, while adding BoNTA, filler injections, and BFP removal as needed, was capable of producing consistent and reliable aesthetic outcomes for the lower face.

  • In many cases of complete ear amputation, microvascular surgery is required for tissue perfusion and organ survival. However, microvascular reconstruction is not always feasible in the absence of suitable vessels. Here, we present the case of a 76-year-old man who underwent complete amputation of the left ear after a collapse at home because of cardiogenic syncope. He was treated with primary replantation and underwent a postoperative salvage course including continuous local hyperbaric oxygen therapy (HBOT), platelet-rich plasma (PRP) injections, and polydeoxyribonucleotide (PDRN) injections. The ear was almost completely salvaged, with a tiny eschar at the mid-scapha on both the anterior and posterior aspects. This case demonstrates the efficacy of local HBOT with PRP and PDRN injections.

  • Testing the volume of injection in container is to ensure that the nominal content of a container can be administered to the patient. The method to test the volume of injection in container is described in European Pharmacopeia (EP) and United States Pharmacopeia (USP). However there is no method to test the volume of injection in Korean Pharmacopeia (KP). So we surveyed the method practiced by 53 pharmaceutical companies in Korea by a questionnaire and tested commercially available injections by discharged volume. As the result, we agreed with the International Conference on Harmonization (ICH) test method for discharged volume. It was suggested that the ICH test method for discharged volume of injection in container would be adopted.

  • Purpose: The purpose of this study was to compare the effectiveness of training using an intravenous (IV) arm model versus a computer simulator for IV injections. Method: Study was a quasi-experimental study conducted with 106 nursing students. Participants were divided into two groups: the IV Arm Group using a mannequin arm model (control group) and the Computer Simulator Group using the Virtual IV demonstration (experimental group). Theoretical lectures and video presentations on IV injections were given to both groups. Each group went through the training practice using the IV arm or computer simulator. After the completion of training, questionnaires were given to the students to evaluate their learning attitudes and experiences, self-confidence in IV injection, and satisfaction with the training materials. Results: Student satisfaction with the training materials including the reality, usefulness, and educational effects showed notable differences between the two groups with the Computer Simulator group reporting more positive effects that the IV Arm group. However, there was no statistical difference between the two groups in the categories of learning attitude, learning experience, or self-confidence. Conclusion: While there was a differences in strengths and weaknesses of the two methods, both methods should be considered for practice and further study needs to be done on educational effectiveness.

  • This study described the effect of the multiple injections and diesel-ethanol on the NVH, combustion and emission characteristics of 4 cylinder common rail diesel engine. In order to investigate the influence of diesel-ethanol blended fuel in a light-duty common rail diesel engine, the injection strategy was varied with pilot injection, double pilot injections, and one main injection at various operating conditions. The results showed that diesel-ethanol blended fuel had longer ignition delay than that of the ultra low diesel fuel(ULSD). Also, in the case of multiple injections, the combustion pressure is increased smoothly near the TDC and the NVH are decreased. In the emission characteristics, diesel-ethanol blended fuel produced lower indicated specific nitrogen oxides(IS-NOX) and indicated specific Soot(IS-soot) emissions, however, indicated specific unburned hydrocarbon(IS-HC) and indicated specific carbon monoxide(IS-CO) emissions are slightly increased.
