The First Case of Novel Influenza A (H1N1) Fatality in Korea

  • Here we report the first fatality caused by H1N1 influenza virus infection with acute respiratory distress syndrome in Korea. A 55-year-old man presented at our emergency department with dyspnea, fever, diffuse myalgia and malaise. Bilateral lung air-space consolidation was detected on his initial chest radiograph combined with severe hypoxemia. He was supported by mechanical ventilation and treated with antibiotics. A nasopharyngeal aspirate was positive for influenza A rapid antigen and oseltamivir was started on day 3 of admission. The nasal swab sample was positive for influenza H1N1 virus by real-time reverse-transcriptase polymerase chain reaction. Despite aggressive treatment, he had refractory hypoxemia and uncontrolled septic shock. On day 5 of admission he went into cardiac arrest and expired.

Clinical Characteristics of Pandemic Influenza A (H1N1) 2009 Pediatric Infection in Busan and Gyeongsangnam-do: One Institution

  • Background: This study investigated the clinical characteristics and risk factors of the severity of pandemic influenza A (H1N1) 2009 infection in pediatric patients in Busan and Gyeongsangnam-do. Methods: Cases of influenza A (H1N1) 2009 in patients under the age of 18 years, confirmed by reverse transcription polymerase chain reaction, at Pusan National University Hospital and Pusan National University Yangsan Hospital from the last week of August 2009 through the last week of February 2010 were retrospectively analyzed. Results: Of the 3,777 confirmed cases of influenza A (H1N1) 2009, 2,200 (58.2%) were male and 1,577 (41.8%) were female. The average age of the patients was $8.4{\pm}4.8$ years. The total cases peaked during 44th to 46th week. Most of the patients were in the 5- to 9-year-old age group. Oseltamivir was administered to 2,959 (78.3%) of the patients. 221 patients (5.9%) were hospitalized, age an average of $6.7{\pm}4.5$ years. The average duration of hospitalization was $7.4{\pm}5.6$ days. One hundred cases (45.2%) had pneumonia. Risk factors for hospitalization included male gender, <2 years of age, and underlying disease. Children with asthma were at very high risk of hospitalization, over 20 times the non-asthmatic children (odds ratio [OR], 21.684; confidence interval [CI], 13.295~39.791). Likewise the children with neurologic deficits faced a 16 times higher risk (OR, 15.738; CI, 7.961~31.111). Ten of the patients (4.5%) were admitted to the intensive care unit, and eight (3.6%) required mechanical ventilation. Conclusion: Of the pediatric patients with pandemic influenza A (H1N1) 2009, most of the patients were in the 5- to 9-year-old age group. Risk factors for hospitalization included male gender, <2 years of age, and underlying disease. The most common complication was pneumonia. The very high risk of severe morbidity in children with asthma or neurologic disease shows the critical importance of targeted vaccine coverage, special awareness and swift care by both guardians and primary care providers.

인플루엔자 바이러스 감염 소아 환아에서 Oseltamivir 약제 효용성과 단기간.장기간 부작용 및 안전성에 관한 임상연구 (Oseltamivir efficacy, side effects, and safety in children with influenza)

  • 목 적 : Oseltamivir는 neuramidase inhibitor으로 인플루엔자의 치료제로 국내에서 사용하고 있지만, 약제사용으로 인한 부작용이나 효용성 및 내성에 대한 국내 연구는 미흡하여 이에 대해서 연구하고자 하였다. 방 법 : 2008년 1월부터 2008년 6월까지와 2008년 11월부터 2009년 1월까지의 두차례의 인플루엔자 유행기간 동안, 부산 성모 병원에서 독감 유사 증상 있어, 신속 항원 진단법으로 독감 진단받고 입원한 321명의 환아를 대상으로 하여, 환아들을 oseltamivir 치료군과 비치료군으로 무작위하여 전향적인 연구를 시행하였다. 치료군에서는 oseltamivir를 5일간 투여하였고, 비치료군에서는 증상 조절제만 사용하였다. 양군 모두에서 임상증상과 부작용을 관찰하였고 입원 기간 등을 조사하였고, 퇴원 4-8개월 후 전화 상담을 통하여 신경정신학적인 이상 유무를 확인하였다. 바이러스 배양 검사 및 oseltamivir에 대한 내성검사를 실시하였다. 결 과 : 두 기간 동안 본원 소아청소년과에 입원하였던 총 321명의 환아들을 분석하였고, oseltamivir 치료군은 186명, 비치료군은 135명이었다. Oseltamivir 치료군에서 비치료군에 비해 입원 당시 발병후 48시간이 경과한 환아들이 많았음에도 하기도 합병증이 현저히 적었고, 두 기간 동안 의미 있는 입원 기간 단축을 보였다(4.4일 vs. 5.0일) (P =0.000). 두 기간 동안 치료 군에서 경미한 소화기계 부작용을 보였으나 심각한 정신신경학적인 부작용을 보인 예는 없었다. 전반기 동안에는 양군 모두에서 influenza B (치료군 vs. 비치료군; 41.7% vs. 49.6%)가 가장 많이 동정 되었고, A/H3N2, A/H1N1 순으로 동정되었으며, 후반기 동안에는 치료군에서 A/H3N2가 배양된 한명의 환아를 제외하고는 양군 모두에서 A/H1N1이 동정되었다. 전반기동안 배양된 바이러스에 대한 oseltamivir 내성 검사상 치료 시작 전이나 치료 후 48-72시간 모두에서 oseltamivir에 대한 약제 내성은 발견되지 않았다. 결 론 : oseltamivir 치료군에서는 정신신경학적인 부작용을 포함한 다른 심각한 부작용 없이 입원 기간을 단축시키면서 하기도 합병증의 감소를 보이는 임상적인 효용성을 보여 소아 인플루엔자 치료에 있어 oseltamivir 치료의 효용성과 안전성을 보여주었다.

  • 최근의 유행한 신종 H1N1 S-OIV는 전세계적으로 퍼지고 있고, 이에 동반된 호흡기 질환등의 여러 합병증들이 보고되었다. 신증후군은 간염바이러스 등의 감염 후에 발현될 수 있는 것으로 알려져 있으나 S-OIV 감염 후에 발생한 예는 보고된 적이 없는 실정이다. 저자들은 S-OIV 감염으로 확진된 환아에서 발생한 신증후군이 발생하였으나 스테로이드 치료로 쉽게 치료된 경험을 하였기에 이를 보고하는 바이다.

2009년 국내 신종플루 유행시기의 Oseltamivir 부작용 평가 (Adverse Drug Reactions with Oseltamivir Treatment in the South Korea Outbreak of 2009 Pandemic Influenza A(H1N1))

  • The World Health Organization (WHO) declared the outbreak of H1N1 pandemic in 2009. South Korea also had outbreaks of H1N1 virus and used oseltamivir in large volume with increased reports of adverse drug reaction(ADR). The present study was aimed to investigate the ADR frequency, the factors related to ADR, and characteristics of oseltamivir's ADR. Participants for the study were patients randomly drawn from those who were prescribed oseltamivir for treatment from CHA Bundang Medical Center during October 1 and October 30. The information examined as factors related to ADR were collected by a subsequent cross-sectional telephone survey. The factors are the following; a) age; b) gender; c) patient medical history; d) diagnosis of H1N1 virus; e) adherence; f) whether taking other medication with oseltamivir or not; and g) the number of combined medications. We also asked ADR after taking oseltamivir. Total subjects were 86 patients. The average age is $22.6{\pm}18.48$ years old. The gender was 45.3% women and 54.7% men. Half (50%) of all respondents showed one or more ADR, 67.4% were positively diagnosed for H1N1 virus, and 54.7% were completed the full course of oseltamivir (i.e. twice daily x 5days). The most frequently reported ADR symptoms were: dizziness (15.1%), nausea (11.6%), lethargy (10.4%), diarrhea (10.4%), abdominal pain (8.1%), headache and vomiting (6.9%). ADR classifications by categories are gastro intestinal (44.2%), neuropsychiatric events (22.1%), systemic symptom (20.9%), skin events (5.8%), eye events (4.7%), and other cases (2.3%). The onset of ADR 'after taking 1~3 doses' was 69.7%. No increase in neuropsychiatric events was detected in children and adolescents. No factors examined for the study do have significant influence on the presence of ADR. This study showed that ADR of oseltamivir have occurred in half of the patients. The use of oseltamivir is essential for treatment and prophylaxis of influenza A(H1N1). But mass treatment should be properly monitored for ADR.

Genetic diversity of the H5N1 viruses in live bird markets, Indonesia

  • Background: The live bird market (LBM) plays an important role in the dynamic evolution of the avian influenza H5N1 virus. Objectives: The main objective of this study was to monitor the genetic diversity of the H5N1 viruses in LBMs in Indonesia. Methods: Therefore, the disease surveillance was conducted in the area of Banten, West Java, Central Java, East Java, and Jakarta Province, Indonesia from 2014 to 2019. Subsequently, the genetic characterization of the H5N1 viruses was performed by sequencing all 8 segments of the viral genome. Results: As a result, the H5N1 viruses were detected in most of LBMs in both bird' cloacal and environmental samples, in which about 35% of all samples were positive for influenza A and, subsequently, about 52% of these samples were positive for H5 subtyping. Based on the genetic analyses of 14 viruses isolated from LBMs, genetic diversities of the H5N1 viruses were identified including clades 2.1.3 and 2.3.2 as typical predominant groups as well as reassortant viruses between these 2 clades. Conclusions: As a consequence, zoonotic transmission to humans in the market could be occurred from the exposure of infected birds and/or contaminated environments. Moreover, new virus variants could emerge from the LBM environment. Therefore, improving pandemic preparedness raised great concerns related to the zoonotic aspect of new influenza variants because of its high adaptivity and efficiency for human infection.

Seroprevalence of swine influenza and porcine reproductive and respiratory syndrome in Korea

  • A total of 501 serum samples were selected from blood samples that were submitted to Department of Veterinary Pathology, Kangwon National University from all provinces in Korea from September 2001 to August 2002. Their sera were examined for antibodies to swine influenza virus subtype H1N1 (SlV H1N1) and porcine repro-ductive and respiratory syndrome virus (PRRSV) according to the age of pig, season, and herd size using enzyme-linked immunosorbent assay. The seroprevalence of SIV H1N1, PRRSV, and dual infection were 39.12%, 61.48%, and 25.95%, respectively. The seroprevalence of SIV H1N1 according to herd size was not significant differences (p>0.05). The results showed that the PRRSV infection spread widely in swine herds throughout the country.

수도권 일부 보건계열 대학생들의 신종인플루엔자에 대한 인식조사 (A Survey on Recognition of H1N1 Influenza in some Undergraduates for the Health Division in Metropolitan Area)

  • 본 연구는 수도권 소재 대학에 재학 중인 일부 보건계열 대학생들을 대상으로 신종인플루엔자에 대한 지식정도와 자기 효능감, 감염의 심각성, 행동의도를 조사해 보고, 성별에 따른 차이를 조사한 결과 다음과 같은 결론을 얻었다. 1. 조사대상자의 인구사회학적 특성으로는 여학생이 57.0%이었으며, 전공은 치위생과 학생이 56.8%로 가장 많았으며, 신종인플루엔자에 대한 교육경험은 78.1%가 받아본 적이 없다고 응답하였다. 2. 신종인플루엔자에 대한 지식은 '신종인플루엔자는 사람과 사람 사이에서 전염될 수 있다(94.6%)', '젊고 건강한 사람도 아이나 노인과 같이 신종인플루엔자에 감염될 수 있다(93.4%)', '열이 37.8도가 넘으면 신종인플루엔자 감염이 의심된다(93.2%),' '신종인플루엔자는 주로 감염된 사람의 기침이나 재채기를 통해 감염된다(91.3%)' 등의 항목에서 높은 정답률을 보였고, '신종인플루엔자는 조류인플루엔자와 동일한 것이다(16.3%)', '돼지고기를 잘못 먹으면 신종인플루엔자에 감염될 수 있다(20.5%)', '보통 독감백신주사를 맞으면 신종인플루엔자를 예방할 수 있다(25.6%)' 등의 항목은 낮은 정답률을 보였다. 신종인플루엔자에 대한 지식을 점수화하면 평균은 7.8점인 것으로 나타났다. 3. 성별에 따른 신종인플루엔자에 대한 자기효능감은 남학생이 여학생 보다 높게 나타나 유의미한 차이가 있었다.(P < 0.05). 4. 성별에 따른 신종인플루엔자에 대한 감염의 심각성은 여학생이 남학생보다 높게 나타나 유의미한 차이가 있었다(P < 0.01). 5. 성별에 따른 신종인플루엔자에 대한 행동의도는 '아는 사람이 신종인플루엔자에 걸리면 만나는 것을 고려해 볼 것이다'가 남학생 3.79점, 여학생 3.98점으로 여학생이 높게 나타나 유의미한 차이가 나타났고(P < 0.05), '신종인플루엔자가 발생한 지역은 여행이 망설여진다'는 남학생 3.93점, 여학생 4.07점으로 여학생의 행동의도가 남학생보다 높게 나타났다(P > 0.05).

Insect Cell Surface Expression of Hemagglutinin (HA) of Egyptian H5N1 Avian Influenza Virus Under Transcriptional Control of Whispovirus Immediate Early-1 Promoter

  • In the present study, whispovirus immediate early 1 promoter (ie-1) was used to initiate surface expression of the hemagglutinin (HA) protein of Egyptian H5N1 avian influenza virus (AIV) by using the baculovirus expression vector system. The HA gene and whispovirus ie-1 promoter sequence were synthesized as a fused expression cassette (ie1-HA) and successfully cloned into the pFastBac-1 transfer vector. The recombinant vector was transformed into DH10Bac competent cells, and the recombinant bacmid was generated via site-specific transposition. The recombinant bacmid was used for transfection of Spodoptera frugiperda (Sf-9) insect cells to construct the recombinant baculovirus and to induce expression of the HA protein of H5N1 AIV. The recombinant glycoprotein expressed in Sf-9 cells showed hemadsorption activity. Hemagglutination activity was also detected in both extra- and intracellular recombinant HAs. Both the HA and hemadsorption activities were inhibited by reference polyclonal anti-H5 sera. Significant expression of the recombinant protein was observed on the surface of infected insect cells by using immunofluorescence. SDS-PAGE analysis of the expressed protein revealed the presence of a visually distinguishable band of ~63 kDa in size, which was absent in the non-infected cell control. Western blot analysis confirmed that the distinct 63 kDa band corresponded to the recombinant HA glycoprotein of H5N1 AIV. This study reports the successful expression of the HA protein of H5N1 AIV. The expressed protein was displayed on the plasma membrane of infected insect cells under the control of whispovirus ie-1 promoter by using the baculovirus expression vector system.
