Cyclic fatigue resistance of M-Pro and RaCe Ni-Ti rotary endodontic instruments in artificial curved canals: a comparative in vitro study

  • Objectives: To compare the flexural cyclic fatigue resistance and the length of the fractured segments (FLs) of recently introduced M-Pro rotary files with that of RaCe rotary files in curved canals and to evaluate the fracture surface by scanning electron microscopy (SEM). Materials and Methods: Thirty-six endodontic files with the same tip size and taper (size 25, 0.06 taper) were used. The samples were classified into 2 groups (n = 18): the M-Pro group (M-Pro IMD) and the RaCe group (FKG). A custom-made simulated canal model was fabricated to evaluate the total number of cycles to failure and the FL. SEM was used to examine the fracture surfaces of the fragmented segments. The data were statistically analyzed and comparisons between the 2 groups for normally distributed numerical variables were carried out using the independent Student's t-test. A p value less than 0.05 was considered to indicate statistical significance. Results: The M-Pro group showed significantly higher resistance to flexural cyclic fatigue than the RaCe group (p < 0.05), but there was no significant difference in the FLs between the 2 groups (p ≥ 0.05). Conclusions: Thermal treatment of nickel-titanium instruments can improve the flexural cyclic fatigue resistance of rotary endodontic files, and the M-Pro rotary system seems to be a promising rotary endodontic file.

Patterns and Prognostic Significance of Cervical Lymph Node Metastasis and the Efficacy of Cervical Node Dissection in Esophageal Cancer

  • Background: The clinical value of 3-field lymph node dissection (3FLND) in esophageal squamous cell carcinoma (ESCC) remains controversial. This study aimed to identify the patterns and prognostic significance of cervical lymph node metastasis (CLNM) in ESCC. Methods: A retrospective review of 77 patients with E SCC who underwent esophagectomy and 3FLND between 2002 and 2016 was conducted. For each cervical node level, the efficacy index (EI), overall survival, recurrence rate, and complication rate were compared. Results: CLNM was identified in 34 patients (44.2%) who underwent 3FLND. Patients with CLNM had a significantly lower overall survival rate (22.7% vs. 58.2%) and a higher recurrence rate (45.9% vs. 16.3%) than patients without CLNM. CLNM was an independent predictor of recurrence in ESCC patients. Moreover, in patients with pathologic N3 tumors, the odds ratio of CLNM was 10.8 (95% confidence interval, 2.0 to 57.5; p=0.005). Level IV dissection had the highest EI, and level IV metastasis was significantly correlated with overall survival (p=0.012) and recurrence (p=0.001). Conclusion: CLNM was a significant prognostic factor for ESCC patients and was more common among patients with advanced nodal stages. Level IV exhibited the highest risk of metastasis, and dissection at level IV may be crucial when performing 3FLND, especially in advanced nodal stage disease.

임상간호사의 억압된 분노, 용서 및 삶의 질 간의 관계 (The Relationship among Anger-in, Forgiveness, and Quality of Life in Clinical Nurses)

  • Purpose: The study was to identify relationships among the Anger-in, Forgiveness, and Quality of Life in Clinical Nurses. Methods: Data were collected by questionnaires from 205 clinical nurses who worked at the 4 hospitals located in Busan. Data were collected from September 26 to October 10, 2013. Anger-in, Forgiveness, and Quality of Life were measured using a structured questionnaire. The instruments included Anger-in Scale, Forgiveness Scale, Quality of Life Scale. The collected data were analyzed using frequency, percentage, independent t-test, One-way ANOVA, $Scheff{\acute{e}}$ test and Pearson's correlation coefficients by SPSS/WIN 18.0 for windows. Results: There were significant correlations among anger-in and forgiveness(r=-.34, p<.001), anger-in and quality of life(r=-.33, p<.001), forgiveness and quality of life(r=.27, p<.001). There was a statistically significant difference in Anger-in of participants according to nurse's satisfaction of job. Forgiveness showed significant different according to nurse's perceived health status, belief of religion, turnover intension, and job satisfaction. Quality of life was significantly different according to nurse's age, perceived health status, work period, work pattern, night duty, turnover intention, motivation of choice in nursing and job satisfaction. Conclusion: These results suggested that reducing anger-in level and enhancing forgiveness level would increase quality of life among clinical nurses.

수여부와 블록 이식골의 동시 피질골 천공과 수여부 피질골 천공의 비교 (COMPARISON THE CORTICAL PERFORATION OF BOTH THE RECIPIENT BEDS AND GRAFTS WITH THE CORTICAL PERFORATION OF ONLY THE RECIPIENT BEDS)

  • Purpose: The aim of this study is to compare volume and revascularization of autogenous block bone grafts in simultaneously cortical perforation of recipient beds and grafts, and only cortical perforation of recipient beds. Materials and methods: Two block bone in 8mm diameter was harvested in both skull using trephine bur on 20 New Zealand white rabbits. Harvested block bone was grafted on both inferior border of mandible. On the left side(experimental side), cortical bone of recipient beds and graft were perforated, and on the right side(control side), the only recipient bed was perforated. The rabbits had been sacrificed and infused the India ink for the observation of revascularization at 20 day and 40 day after surgery. The specimens were processed for H-E staining and quantitative analysis(independent t-test, p<0.01) was made under an optical microscope. In additional, specimens were processed for the observation of revascularization. Results: After 20 days, more bone volume was observed in experimental group, but no significant difference between two groups(p=0.106). There were significantly more bone volume in the experimental group at 40 days after surgery(p<0.01). After 20 days, more discrete vascular sprouts were observed in experimental side, but no difference at 40 days after surgery. Conclusion: We conclude that the cortical perforation of both the recipient beds and grafts improve revascularization at early stage and overall graft persistence.

TIME-DEPENDENT DEFORMATION OF POLYMER-BASED PROVISIONAL CROWN AND FIXED PARTIAL DENTURE MATERIALS

  • Statement of problem. One of the common problems of provisional crown and fixed partial denture materials is that when they are subjected to constant loads for a long period of time, they exhibit a dimensional change (creep). Purpose. The aim of this study was to investigate the viscoelastic behaviour of polymer-based provisional crown and fixed partial denture materials with time at constant compressive load. Material and methods. Three dimethacrylate-based materials (Protemp 3 Garant, Temphase, Luxatemp) and one monomethacrylate-based material (Trim) were selected. Dimensional changes of the specimens were recorded by a LVDT to evaluate their viscoelastic behavior and creep strain. For all specimens, two loading procedures were used. At first, static compressive stress of 4 MPa was applied for 30 minutes and followed by 1 hour of strain recovery. Then, after 24 hours of water storage, the specimens were loaded again. The creep values between materials were statistically analyzed using one-way ANOVA and multiple comparison $Scheff\acute{e}$ test. Independent samples t-test was also used to identify the difference of creep strain between first and secondary loading conditions at the significance level of 0.05. Results. Following application of the first loading, Trim showed the highest maximum creep strain (32.7%) followed by Luxatemp, Protemp 3 Garant and Temphase, with values of 3.78%, 2.86% and 1.77%, respectively. Trim was significantly different from other materials (P<0.05), while there were no significant differences among Luxatemp, Protemp 3 Garant and Temphase (P>0.05). The highest recovery and permanent set of Trim, were significantly different from those of others (P<0.05). At the secondary loading of the dimethacrylate-based materials, creep deformation, recovery and permanent set decreased and the percentage of recovery increased, while in Trim, all values of the measurements increased. This result showed that the secondary loading at 24 hours produced a significant creep magnitude. Conclusion. The dimethacrylate-based provisional crown and fixed partial denture materials showed significantly higher creep resistance and lower deformation than the monomethacrylate-based material. Thus, monomethacrylate-based materials should not be used in long-term stress-bearing situations.

  • Nakdong River Plain and its adjoining sea arc unusually covered by very thick soft clay sediments which were caned Pusan clays, sometimes reaching 40-70m. Since early 1990s a large number of geotechnical investigations have been carried out for land reclamation works in the area, including Busan city and its neighboring cities. Nevertheless, geotechnical engineers have paid little attention to studying geological characteristics on the clays, except some researches related to mineralogy, geo-chemistry, benthic foraminiferal fauna etc. The purpose of reserach is the knowledge of the correlation between engineering properties and mineralogy of clay sediments. The correlation analysis carry out multiple regression that have independent variables (Engineering properties) and dependent variables (mineralogy, geochemistry). Engineering properties of clay are correlated with the mineral compositions and geochemical characteristics.   • Purpose: The purpose of this study was to measure changes in the H-reflex and V wave under loading conditions (e.g. prone and standing position) and to investigate whether postural change would affect the H-reflex and V wave in post stroke hemiplegic patients. Methods: Thirty persons with hemiplegia resulting from stroke (20 males, 10 females) participated in this study. Electromyography (EMG) was used to electrically stimulate and record the soleus H-reflexes and V waves under various loading conditions. The normality of the distribution of each variable (H latency, $H_{max}/M_{max}$ ratio, $V_{max}/M_{max}$ ratio) was tested using the Kolmogorov-Smirnov test. The means of normally distributed continuous data were assessed by independent t-test (${\alpha}$=0.05). Results: There were statistically significant differences in $H_{max}/M_{max}$ ratio (p<0.01), $V_{max}/M_{max}$ ratio (p<0.01), H latency (p<0.01) among the prone and standing position. Conclusion: We found that the H-reflex and V wave in standing position was more active to weight bearing load than prone position.

Mutational Analysis of Korean Patients with Phenylketonuria

  • Purpose Phenylketonuria is an inborn error of metabolism, which is inherited as an autosomal recessive trait. PKU is resulting from deficiency of phenylalanine hydroxylase. PAH gene spans about 90 kb on chromosome 12q and comprises 13 exons. In order to define the genetic basis of PKU and the frequencies and distribution of PAH mutations in the Korean population, we analyzed PAH gene in independent 80 patients with PKU. Methods All 13 exons including exon-intron boundaries and 2 kb of 5' upstream region of the PAH gene were analyzed by PCR-direct sequencing methods. Results PAH gene analysis revealed 39 different mutations including 10 novel mutations. The novel mutations consisted of 9 missense mutations (P69S, G103S, N207D, T278S, P281A, L293M, G332V, S391I and A447P) and a novel splice site variant (IVS10-3C>G). R243Q, IVS4-1G>A, and E6-96A>G were the most relevant mutations and they accounted in the whole for 38% of the mutant alleles identified in this study. We also observed that. $BH_4$ responsibility was. associated with genotype of R241C, R53H and R408Q. Conc1ustion Our present study with 80 participants extends the previous results to more comprehensive understanding of PAH allele distribution and frequency in Koreans. Although Korean mutation profile of PAH is similar to those of the nearest oriental populations (Japanese, Chinese, and Taiwanese), several different characteristic features are revealed. The characterization of the genotype-phenotype relationship was also performed. Our data would be very useful information for diagnosis, genetic counseling and planning of dietary and therapeutic strategies in Korean PAH patients.

Examination of Correlations Between Several Biochemical Components and Powdery Mildew Resistance of Flax Cultivars

  • A field trial was conducted in 2009/2010 and 2010/2011 growing seasons at Giza Agricultural Research Station to examine correlations between some biochemical componets and powdery mildews ($PM_s$) resistance in flax cultivars. Nine flax cultivars could be divided into five distinct groups, i.e., highly susceptible (Cortland and C.I. 2008), moderately susceptible (Giza 7, and Marshall), moderately resistant (Cass), resistant (Koto, Dakota and Wilden), and highly resistant (Ottowa 770B). The cultivars showed considerable variation in PM severity ranged from 8.05 on Ottowa 770B to 97.02% on Cortland. Total soluble proteins, total phenols, antioxidant enzymes (peroxidase and polyphenoloxidase), ascorbic acid, tocopherol, and malondialdehyde (MDA), were determined in uninfected leaves of the tested cultivars. Pearson's correlation coefficient was calculated to measure the degree of association between PM severity and each component. All components showed significant (P < 0.05) or highly significant (P < 0.01) negative correlation with PM severity except MDA, which showed positive correlation (P < 0.01). Linear regression analysis was used to evaluate the causal relationship between the biochemical components (independent variables) and PM severity (dependent variable). Coefficient of determination ($R^2$) values of the generated models ranged from 48.76 to 77.15%. Tocopherol, MDA, and proteins were the most important contributors to the total variation in PM severity as the $R^2$ values of their models were 71.78, 75.28, and 77.15%, respectively. The results of the present study suggest that tocopherol, MDA, and proteins in uninfected leaves can be used as biochemical markers to predict PM resistance in flax.

Effect of different veneering techniques on the fracture strength of metal and zirconia frameworks

  • PURPOSE. To determine whether the fracture strengths and failure types differed between metal and zirconia frameworks veneered with pressable or layering ceramics. MATERIALS AND METHODS. A phantom molar tooth was prepared and duplicated in 40 cobalt-chromium abutments. Twenty metal (IPS d.SIGN 15, Ivoclar, Vivadent, Schaan, Liechtenstein) and 20 zirconia (IPS e.max ZirCAD, Ivoclar) frameworks were fabricated on the abutments. Each framework group was randomly divided into 2 subgroups according to the veneering material: pressable and layering ceramics (n=10). Forty molar crowns were fabricated, cemented onto the corresponding abutments and then thermocycled ($5-55^{\circ}C$, 10,000 cycles). A load was applied in a universal testing machine until a fracture occurred on the crowns. In addition, failure types were examined using a stereomicroscope. Fracture load data were analyzed using one-way ANOVA and Tukey HSD post-hoc tests at a significance level of 0.05. RESULTS. The highest strength value was seen in metal-pressable (MP) group, whereas zirconia-pressable (ZP) group exhibited the lowest one. Moreover, group MP showed significantly higher fracture loads than group ZP (P=.015) and zirconia-layering (ZL) (P=.038) group. No significant difference in fracture strength was detected between groups MP and ML, and groups ZP and ZL (P>.05). Predominant fracture types were cohesive for metal groups and adhesive for zirconia groups. CONCLUSION. Fracture strength of a restoration with a metal or a zirconia framework was independent of the veneering techniques. However, the pressing technique over metal frameworks resisted significantly higher fracture loads than zirconia frameworks.
