Incidental Diagnosis of Multiple Myeloma during Conservative Treatment for Lumbar Disc Herniation; -A Case Report- (요추 추간판 탈출증 치료 중 진단된 다발성 골수종 1례)

  • Objectives: The aim of this study is to notify incidental diagnosis of multiple myeloma during conservative treatment for lumbar disc herniation, caused by right flank pain. Methods: A 45-year-old female came to our hospital for the control of pain caused by lumbar disc hemiation L5-S1. After using Cox technique, she occured right flank pain and found out multiple fracture by bone scan. For further evaluation, she was diagnosed with multiple myeloma with bone marrow biopsy. Results and Conclusions: This case report suggests that clinicians should consider the possibility of multiple myeloma in the case of multiple fracture without trauma.

The Research on the actual condition and Improvement Measure about Elementary School Equipments of Playground (초등학교 학교운동장 관련시설에 관한 실태조사 연구)

  • This research aims to take a close look at the following topics; the ground elevation of playground, equipments of playground, and equipments around playground, and to propose the improvement measure for favorable educational environment. In consideration of the needs mentioned, the following improvement measure for playground can be suggested. First, the elevation of playground should be changed. According to Environment psychology, current playground elevation should be lowered 2 stair-long height from a classroom site. This kind of elevation change can remove unnecessary stairs, and reduces inconvenience incurred by stairs and edges, which results in considerable educational finance saving. Second, the incidental equipment within a playground needs to be improved. This fact is found out at the surveyed schools; school gathering is held at classrooms (through broadcasting) or a multi-purpose auditorium in place of a playground. The results of this study make us conclude that the most important issue to be considered in the processes of selection and investigation for school playground elevation, incidental equipments and others.

  • The incidental finding of an enlarged mastoid foramen on the right posterior mastoid region of temporal bone is reported, together with a discussion of its clinical significance. A 67-year-old female underwent the pre-implant assessment of a maxillary left edentulous region. A cone-beam computed tomographic (CBCT) image was acquired and referred for consultation. Axial CBCT slices revealed a unilateral, well-defined, noncorticated, low-attenuation, transosseous defect posterior to the mastoid air cells in the right temporal bone. The borders of the osseous defect were smooth and continuous. No other radiographic signs suggestive of erosion or sclerosis were noted in the vicinity. The density within the defect was homogenous and consistent with a foramen and/or soft tissue. The patient's history and physical examination revealed no significant medical issues, and she was referred to a neuroradiologist for a second opinion. The diagnosis of an enlarged mastoid foramen was made and the patient was reassured.

  • A 48-years-old woman was visited to our hospital because of incidental finding of intrapulmonary foreign body. Chest X-ray showed a 4cm sized foreign body in left upper lung field without adjacent fibrosis. Chest CT showed a sewing needle shaped foreign body of metallic density, located in the 113ft upper lobe. The foreign bodies including the needle were removed surgically using a wedge resection. The extracted needle was divided into three 4 cm pieces. Patient was discharged without other respiratory symptoms after surgery. We report a case of wedge resection in a patient with intrapulmonary needle in the left upper lobe, with review of literatures.

Accuracy of incidental dynamic analysis of mobile elevating work platforms

  • This paper presents the results of a study into the dynamic behaviour of a support structure of a mobile elevating work platform. The vibrations of the mechanical system of the observed structure are examined analytically, numerically, and experimentally. Within the analytical examination, a simple mathematical model is developed to describe free and forced vibrations. The dynamic analysis of the mechanical system is conducted using a discrete dynamic model with a reduced number of vibrational degrees of freedom. On the basis of the expression for the system energy, and by applying Lagrange's equations of the second kind, differential equations are derived for system vibrations, frequencies are determined, and the laws of forced platform vibration are established. At the same time, a nonlinear FEM model is developed and the laws of free and forced vibration are determined. The experimental and numerical part of the study deal with the examination of the real structure in extreme conditions, taking into account: the lowest eigenfrequency, forced actions that could endanger the general stability, the maximal amplitudes, and the acceleration of the work platform. The obtained analytical and numerical results are compared with the experiments. The experimental verification points to the adverse behaviour of the platform in excitation cases - swaying. In such a situation, even a relatively small physical force can lead to unacceptably high amplitudes of displacement and acceleration - exceeding the usual work values.

  • Purpose: The purpose of this study is to provide an effective construction project cost estimation tool for preparing a business plan of healthcare facilities which can be practically used for development projects. Methods: In order to provide a tool for calculating construction project costs, this study analyzed the Building and Related Laws, the Building Technology Promotion Laws, the Ministry of Land, Infrastructure and Transport notifications and directives, the Ministry of Trade, Industry and Energy notifications, the detailed guidelines of the Ministry of Strategy and Finance, the Building Service Industry Promotion Acts, various certification standards, actual project budget calculation cases, etc. with advices from related experts. Results: 1) Construction cost is classified into construction costs, architectural design costs, supervision costs, incidental costs, and each sub-element. In particular, since there are many incidental cost items, essential items to be reviewed during planning are derived and costs are calculated according to appropriate consideration criteria. 2) Criteria for Payment calculation mainly applies the construction cost rate method or the actual cost fixed amount method in consideration of the characteristics and scope of work. Implications: There are many calculation factors that need to be applied to the construction project cost. Therefore, it is necessary to organize the calculation process more clearly.

A Case of Idiopathic Renal Hypouricemia with SLC22A12 Gene Mutation Showing General Weakness and Incidental Renal Stone

  • Idiopathic renal hypouricemia (iRHUC) is a rare hereditary disease caused by a defect in urate handling of renal tubules. Type 1 renal hypouricemia (RHUC1) is diagnosed with confirmation of a mutation in SLC22A12 gene which encodes a renal urate-anion exchanger (URAT1). The majority of iRHUC patients are asymptomatic, especially during childhood, and thus many cases go undiagnosed or they are diagnosed late in older age with complications of hematuria, renal stones, or acute kidney injury (AKI). We report a case of a 7-year-old boy with subtle symptoms such as general weakness and dizziness and revealed hypouricemia and incidental nephrolithiasis. Homozygous mutations were detected in the SLC22A12 (c.774G>A) by molecular analysis. The present case suggests that fractional excretion of uric acid (FEUA) screening could be better followed by the coincidental discovery of hypouricemia, to prevent conflicting complications of iRHUC, even with normal urine uric acid to creatinine ratio (UUA/UCr), and sequential genetic analysis if needed.

Earthquake Responses of Nuclear Facilities Subjected to Non-vertically Incidental and Incoherent Seismic Waves (비수직 입사 비상관 지진파에 의한 원전 시설물의 지진 응답)

  • Based on the random-vibration-theory methodology, dynamic responses of nuclear facilities subjected to obliquely incidental and incoherent earthquake ground motions are calculated. The spectral power density functions of the 6-degree-of-freedom motions of a rigid foundation due to the incoherent ground motions are obtained with the local wave scattering and wave passage effects taken into consideration. The spectral power density function for the pseudo-acceleration of equipment installed on a structural floor is derived. The spectral acceleration of the equipment or the in-structure response spectrum is then estimated using the peak factors of random vibration. The approach is applied to nuclear power plant structures installed on half-spaces, and the reduction of high-frequency earthquake responses due to obliquely incident incoherent earthquake ground motions is examined. The influences of local wave scattering and wave passage effects are investigated for three half-spaces with different shear-wave velocities. When the shear-wave velocity is sufficiently large like hard rock, the local wave scattering significantly affects the reduction of the earthquake responses. In the cases of rock or soft rock, the earthquake responses of structures are further affected by the incident angles of seismic waves or the wave passage effects.

Incidental finding of hemolymph nodes in a Holstein cow (Bos taurus taurus) with coccidiosis

  • This case report is about hemolymph nodes found in a dairy cow whose function is still not fully elucidated. A 4-month Holstein cow presented severe respiratory symptoms and hematochezia for a while with respiratory acidosis and metabolic alkalosis. Coccidiosis was diagnosed and treated immediately, but the cow died from respiratory acidosis and metabolic alkalosis. At necropsy, no abnormal appearance in thoracic and peritoneal organs was observed, but hemolymph nodes were observed being multifocally stuck on omasum serosa and the subcutaneous fascia of abdominal region, and the larger dark red lymph nodes were found along the omasum great curvature. Microscopically, lymphoid depletion and lymphadenitis in the lymph nodes were examined to point systemic infection, and in the hemolymph node, multifocally demarcated pale lesions with macrophage infiltration and fibrin deposition nearby subcapsular sinus. In subcapsular sinus of the hemolymph node, rod to linear gram-negative bacteria were found. Through this study, we might conclude that the hemolymph node is involved in pathogen phagocytosis.

  • Breast cancer is one of the most common cancers and causes several complications in females. Currently, MRI is a necessary method for preoperative studies in patients with breast cancer. A high frequency of breast MRI can lead to an increase in the number of incidental extramammary findings. Moreover, it can provide accurate preoperative workup; therefore, the prognosis of patients can be improved. Herein, we provide several extramammary findings, including the mediastinum, lung, upper abdomen, bone, and soft tissue, correlating with US, chest CT, liver MRI, PET-CT, and bone scan.
