관전압과 관전류량 변화에 대한 가상 그리드 소프트웨어(VGS) 화질평가 (Evaluation of Virtual Grid Software (VGS) Image Quality for Variation of kVp and mAs )

  • 본 연구의 목적은 가상 그리드 소프트웨어(VGS)의 유효성을 평가하기 방법으로 이동형 그리드를 사용하지 않은 상태에서 VGS를 사용할 때와 사용하지 않을 때(Without-VGS)로 구분하여 에너지와 피사체 두께에 변화를 주고 흉부 팬텀과 대퇴부 팬텀을 이용하여 영상을 획득하고, SNR과 CNR을 분석하여 VGS의 유효성을 알아보고자 하였다. 흉부 팬텀과 대퇴부 팬텀에서 관전류는 2.5 mAs로 고정하고, 관전압을 60 ~ 100 kVp에서 10kVp 씩 변화하여 X선 조사한 후 SNR과 CNR을 측정한 결과 흉부 팬텀에서 SNR은 Without-VGS 보다 VGS에서 약 1.09 ~ 8.86% 높게 나타났고, CNR은 Without-VGS 보다 VGS에서 4.18 ~ 14.56% 높게 나타났다. 그리고 대퇴부 팬텀에서는 SNR이 Without-VGS 보다 VGS에서 약 9.78 ~ 18.05% 높게 나타났고, CNR은 Without-VGS 보다 VGS에서 21.07 ~ 44.44% 높게 나타났다. 흉부 팬텀과 대퇴부 팬텀에서 관전압을 70 kVp로 고정하고, 관전류량은 2.5 ~ 16 mAs에서 각각 변화하여 X선 조사한 후 SNR과 CNR을 측정한 결과 흉부 팬텀에서 SNR은 Without-VGS 보다 VGS에서 약 1.49 ~ 11.11% 높게 나타났고, CNR은 Without-VGS 보다 VGS에서 4.76 ~ 13.40% 높게 나타났다. 그리고 대퇴부 팬텀에서는 SNR은 Without-VGS 보다 VGS에서 약 2.22 ~ 17.38% 높게 나타났고, CNR은 Without-VGS 보다 VGS에서 13.85 ~ 40.46% 높게 나타났다. 결론적으로 Without-VGS 보다 VGS를 사용할 때 SNR과 CNR이 높게 나타났다. 그러므로 이동형 X선 촬영장치로 검사를 해야 하는 경우 이동형 그리드를 사용하기 어려운 환경에서 VGS를 사용함으로써 좋은 영상의 화질을 얻을 수 있어 검사에 유용하게 사용될 것으로 판단되고, 이동형 X선 장치의 활용성을 증대시킬 수 있을 것으로 판단된다.

PET/CT 영상 획득 시 사전설정법 차이에 따른 영상 질 평가 (Evaluation of the Image Quality According to the Pre-set Method in PET/CT Image)

  • 영상장치를 이용한 검사 시 영상의 질은 검사결과와 밀접하게 연관되어 있으며, 이는 영상획득 조건들과 이에 대한 평가방법으로 인해 달라 질 수 있다. 본 연구에서는 영상획득 시 사전설정법(Pre-set method)차이에 따른 영상의 질을 평가하였다. 장비는 PET/CT Discovery STe16(GE Healthcare, Milwaukee, USA)을 사용하였으며, Chest PET Phantom (실험 1)과 94 NEMA Phantom (실험 2)을 이용하였다. 실험 1의 경우 3.5, 6.0, 8.6 kBq/mL, 실험 2의 경우는 3.3, 5.5, 7.7, 9.9, 12.1, 16.5 kBq/mL의 $^{18}F$-FDG를 채웠고, 열소와 배후방사능과의 방사능 농도는 4:1의 비율을 유지하였다. 각 실험은 CT 투과촬영 후 3D로 2분 30초/프레임으로 시간설정법(Time-set method)과 1억 계수의 계수설정법 (Count-set method)을 적용하여 방출촬영 하였다. 영상의 질에 대한 평가는 잡음 등가 계수율 (Noise Equivalent Count Rate, NECR)과 신호 대 잡음비 (Signal to Noise Ratio, SNR)를 이용하였다. 실험 1에서 NECR과 SNR은 방사능 농도의 증가에 따라 시간설정법에서 53.7, 66.9, 91.4 과 7.9, 10.0, 11.7로 평가 모두에서 증가함을 보였으나, 계수설정법은 53.8, 69.1, 97.8과 14.1, 14.7, 14.4로 SNR은 NECR과 다르게 증감현상을 보이지 않았다. 또 다른 모형실험인 실험 2에서도 NECR과 SNR은 방사능 농도가 증가에 따라 시간설정법에서 45.1, 70.6, 95.3 115.6 134.6 162.2 과 7.1, 8.8, 10.6, 11.5, 12.7, 14.0으로 증가함을 보였으나, 계수설정법에서는 42.1 67.3 92.1 112.2 130.7 158.7 과 15.2, 15.9, 15.6, 15.4, 15.5, 14.9로 실험 1에서와 마찬가지로 SNR에서는 증감현상을 보이지 않았다. 사전설정법 변화와 관계없이 단위 질량당 방사능량 증가는 영상의 질도 비례하여 향상된다. 그러나 동일한 단위 질량당 방사능량에서는 영상획득의 시간을 늘려 총계수값을 증가시켜도 NECR에 영향을 주지는 않는다. 이는 NECR을 이용한 영상 질 평가는 영상획득 시 얻은 총 계수 값보다는 단위 시간당의 계수율이 영향을 준다는 것을 의미한다고 할 수 있다. 실험 모두에서 계수설정법으로 얻은 경우에는 단위 질량당 방사능량의 증가에도 불구하고 거의 비슷한 SNR값을 보이게 된다. 따라서 좋은 질의 영상을 얻기 위해서는 총 계수값의 증가보다는 단위질량당 방사능량을 높여야 하며, SNR만을 이용한 영상의 질에 대한 평가는 적절하지 않을 수 있다는 점을 고려하여야 한다.

국내 치과방사선의 현황 및 이용 실태 (The Actual State and the Utilization for Dental Radiography in Korea)

  • 치과 X선 검사 시 환자선량의 권고량 가이드라인을 개발하기 위해서 우선 국내 치과방사 선의 현황 및 이용 실태를 설문조사하여 비교분석하였다. 응답을 받은 77곳의 치과병 의원 중에서 치과대학이 있는 치과대학병원을 A그룹(11곳), 치과대학이 없는 대학병원을 포함한 치과병원을 B그룹(30곳), 치과의원을 C그룹(36곳)으로 분류하였다. 그룹별로 분석한 결과는 다음과 같았다. 치과병 의원 한 곳당 평균 unit chairs수는 A그룹 140.2개, B그룹 15.3개, C그룹 5.8개, 평균치과의사수는 A그룹 112.6명, B그룹 7.3명, C그룹 1.7명, 평균방사선사수는 A그룹 3.1명, B그룹 0.5명, C그룹에는 한 명도 없었고, 평균치과위생사수는 A그룹 19.7명, B그룹 12.5명, C그룹 3.3명이었다. 설치된 치과용 X선장치의 평균보유수는 A그룹 14.64대, B그룹 3.21대, C그룹 2.19대로 나타났다. 이 중에서 구내촬영장치가 가장 많았고, 다음은 파노라마 장치, 세팔로 장치, CBCT 장치 순이었으며, 장치시스템의 비율은 세 그룹 모두 DR 시스템이 50% 이상으로 가장 많았다. 필름 시스템인 경우, 사용된 구내필름은 감광도가 높은 Insight 필름(Kodak, USA)이 대부분이었으며, 자동 현상기는 사용하는 곳이 적었으나, 필름유지기구는 사용하는 곳이 약간 많았다. PACS 이용률은 A그룹 90.9%, B그룹 83.3%, C그룹 16.7%이었고, 프로그램은 국내의 Infinitt에서 개발된 PiView STAR가 가장 많이 이용되고 있었다. 치과병 의원 한 곳당 2008년도의 연간 평균촬영건수는 A그룹이 B그룹보다 6.8배, C그룹보다 21.2배 더 많았으며, 구내 치근단 촬영과 파노라마 촬영이 대부분이었다. 구내 치근단 촬영 시 관전압과 관전류는 세 그룹 모두 비슷하였으나, 노출시간은 C그룹이 A그룹의 12배, B 그룹의 3.5배 정도 길었다. 즉, 대부분 치과위생사가 촬영하고 있는 C그룹에서는 방사선 노출량이 다른 그룹에 비해 훨씬 많았다. 그러나 파노라마촬영 시의 조건은 세 그룹에서 큰 차이가 없었다. 결론적으로, 촬영조건은 경험적 지식을 따르기보다는 권장량을 사용해야 하며, 필름시스템에서는 자동현상기와 필름유지기구를 사용하여 방사선의 노출량을 줄여준다. 또한 환자선량의 감소와 X선 영상의 질 향상을위해서 X선발생장치 및 관련기기의 체계적인 정도관리가 반드시 필요하다.

두경부암의 영상유도방사선치료에서 ExacTrac의 유용성 평가 (Evaluation of the Usefulness of Exactrac in Image-guided Radiation Therapy for Head and Neck Cancer)

  • 목 적: 현대 방사선치료기술에서 종양표적위치 및 정상장기에 정확한 선량을 전달하기 위해 여러 방법의 영상유도방사선치료(Image Guided Radiation Therapy, IGRT)가 사용되고 있으며 그 중 선형가속기에 장착된 CBCT(Cone Beam Computed Tomography, CBCT)와 이외 장치인 ExacTrac(ExacTrac X-ray System)이 있다. 두 시스템을 비교한 이전 연구들에서는 Offline-review 이용하여 후향적으로 팬텀 및 환자의 Set-up 오차를 분석하거나 X, Y, Z 축과 하나의 회전방향(Couch Rotation)으로만 연구되어졌다. 본 연구에서는 Head and Neck Cancer 환자를 대상으로 CBCT와 ExacTrac을 이용하여 한 치료중심센터에서 각각 6 DoF(Degree Of Freedom) IGRT를 시행한 후, 두 IGRT 장비에서 나타난 팬텀 및 환자의 Set-up 오차, 환자 Set-up에 걸리는 시간, 노출 방사선량의 비교를 통해 유용성을 평가하고자 한다. 대상 및 방법: Rando Phantom을 이용하여 환자 움직임을 배제한 상태의 Set-up 오차 평가와 Head and Neck Cancer 환자의 Set-up 오차 값 두 가지 경우로 나누어 획득하였다. 노출 방사선량 평가는 유리선량계로 하였다. 환자 Set-up 후 IGRT 시행하는데 소요되는 시간을 평가하기 위해 Head and Neck Cancer 환자 11명을 대상으로 하였다. 총 치료기간동안 환자 당 평균 10회의 CBCT와 ExacTrac 영상을 동시에 얻었고, 관심영역지정(Region Of Interest, ROI) 설정 후 6D 온라인 자동위치교정(Online Automatching) 값의 차를 6개의 축(Translation group: SI, AP, LR; Rotation group: Pitch, Roll, Rtn)으로 각각 계산하였다. 결 과: Phantom과 환자에서 Set-up 오차는 Translation group에서 1mm 미만, Rotation group에서 1.5° 미만의 차이가 보였으며, Rtn 값을 제외한 다른 모든 축의 RMS 값이 1mm, 1° 미만으로 나타났다. 각 시스템에서 최종적으로 Set-up 오차 교정까지 걸리는 시간은 CBCT를 이용한 IGRT에서는 평균 256±47.6sec, ExacTrac을 이용 시 평균 84±3.5sec로 각각 나타났다. 1회 치료 당 IGRT에 의한 방사선 노출선량은 Head and Neck 부위 7곳의 측정위치 중 Oral Mucosa에서 CBCT와 ExacTrac이 각각 2.468mGy, 0.066mGy로 상대적으로 ExacTrac에 비해 피폭선량이 37배 높게 측정되었다. 결 론: CBCT와 ExacTrac 두 시스템 간의 6D 온라인 자동위치교정을 통해 Set-up 오차는 두 시스템의 자체적인 Systematic error 뿐 아니라, 환자 움직임(Random error)를 포함한 Set-up 오차가 1mm, 1.02° 미만으로 나타났다. 이는 본원에서 Head and Neck IMRT 치료 시 PTV Margin이 3mm이라는 것을 고려했을 때, 이 오차범위는 합리적으로 사료된다. 하지만 치료기간 동안 환자체중변화로 인한 따른 표적, 손상위험장기의 변화를 고려했을 때 CBCT와 적절히 병용하여 사용하는 것이 좋을 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
