Prevalence and Related Risk Factors of Delirium in Intensive Care Units as Detected by the CAM-ICU (CAM-ICU로 평가한 중환자실의 섬망 발생률과 섬망 발생 위험요인)

  • Purpose: Screening of delirium using delirium assessment tools could promote delirium detection, however, there is lack of report about regular delirium assessment in Korea. This study was intended to describe the prevalence and related risk factors of delirium in intensive care unit (ICU). Methods: The Confusion Assessment Method for the ICU (CAM-ICU) data which were evaluated by nurses in ICUs was obtained through retrospective chart review. Data were analyzed using descriptive statistics, Chi-square test, t-test, Mann-Whitney U test, and stepwise logistic regression. Results: Delirium was evaluated in 125 patients. The incidence rate of delirium was 27.2% with a high prevalence of hypoactive delirium compared to hyperactive delirium (61.8 vs. 38.2%). Those with delirium were older, had hypertension, stayed longer in hospital, receiving ventilator support, had more number of catheters, had low serum protein and albumin level. Delirium incidence also varied according to diagnosis. Age, diagnosis of gastrointestinal disease, and application of ventilator were the significant risk factors for the incidence of delirium. Conclusion: Routine delirium screening is important for early detection of delirium. Identification of high-risk group and running delirium prevention programs could improve early recognition of delirium in ICU.

  • This paper describes the quick and precise control of the wafer temperature essential in rapid thermal processing(RTP). The bang-bang plus PID controller structure is introduced to satisfy rapid ramp-up rate and reduce overshoot and steady state error. The controller employs the PID action when the magnitude of the error between reference signal and the output temperature signal is smaller than some prescribed value. To find PID gains, the plant(autoregressive) model is first identified and Kappa-Tau tuning rule is used. The developed controller is applied to experimental RTP apparatus, and performances are evaluated.

Facial Shape Recognition Using Self Organized Feature Map(SOFM)

  • This study proposed a robust detection algorithm. It detects face more stably with respect to changes in light and rotation forthe identification of a face shape. The proposed algorithm uses face shape asinput information in a single camera environment and divides only face area through preprocessing process. However, it is not easy to accurately recognize the face area that is sensitive to lighting changes and has a large degree of freedom, and the error range is large. In this paper, we separated the background and face area using the brightness difference of the two images to increase the recognition rate. The brightness difference between the two images means the difference between the images taken under the bright light and the images taken under the dark light. After separating only the face region, the face shape is recognized by using the self-organization feature map (SOFM) algorithm. SOFM first selects the first top neuron through the learning process. Second, the highest neuron is renewed by competing again between the highest neuron and neighboring neurons through the competition process. Third, the final top neuron is selected by repeating the learning process and the competition process. In addition, the competition will go through a three-step learning process to ensure that the top neurons are updated well among neurons. By using these SOFM neural network algorithms, we intend to implement a stable and robust real-time face shape recognition system in face shape recognition.

Red Cell Alloimmunization and Autoimmunization in Multi-Transfused Thalassemia Patients in Sulaymaniyah Province-Iraq

  • Thalassemias are considered important health issues throughout Iraq, involving its Kurdistan region. This disorder, particularly its major form, needs lifelong regular transfusions. But this form of medical care is associated with various complications including red cell alloimmunization and autoimmunization. This study determined the frequency and associations of alloimmunization among multi-transfused patients with β-thalassemia major. The subjects were 204 patients who were registered at a thalassemia care center in Sulaymaniyah-Iraqi Kurdistan. The patients' records were analyzed, their red cells were phenotyped for ABO/RhD antigens using the gel card method, and irregular antibody screening/identification was performed using the standard tube method. Alloantibodies were detected in 5.8% of the patients, while DAT was positive in 4% of the patients, which indicated autoantibodies. The identified alloantibodies were anti-E (2.4%), anti-C (1.4%), anti-e (1%), and anti-K (1%). A patient's age at the start of transfusion (>2 years) (P=0.042) and a positive history of transfusion reactions (P=0.003) were correlated with a significantly higher rate of alloantibody formation. From the results of our study, we conclude that measures to decrease the development of alloantibodies may incorporate matching for Rhesus and Kell systems and early induction of blood transfusions.

Stress Identification and Analysis using Observed Heart Beat Data from Smart HRM Sensor Device

  • In this paper, we analyses heart beat data to identify subjects stress state (binary) using heart rate variability (HRV) features extracted from heart beat data of the subjects and implement supervised machine learning techniques to create the mental stress classifier. There are four steps need to be done: data acquisition, data processing (HRV analysis), features selection, and machine learning, before doing performance measurement. There are 56 features generated from the HRV Analysis module with several of them are selected (using own algorithm) after computing the Pearson Correlation Matrix (p-values). The results of the list of selected features compared with all features data are compared by its model error after training using several machine learning techniques: support vector machine, decision tree, and discriminant analysis. SVM model and decision tree model with using selected features shows close results compared to using all recording by only 1% difference. Meanwhile, the discriminant analysis differs about 5%. All the machine learning method used in this works have 90% maximum average accuracy.

  • Purpose: This study was done to identify essential nursing diagnoses using NANDA and their related factors and defining characteristics of patients who were cared in an emergency room. Methods: The research checklist developed by the researcher consisted of 44 nursing diagnoses with defining characteristics and related factors and was applied to 235 patients who were admitted to an emergency room from November 1 to December 31, 2012. Results: Forty-one of forty-four nursing diagnoses were identified. The most frequent nursing diagnoses were acute pain, risk for falls, and activity intolerance. The most frequent defining characteristic for the nursing diagnosis of 'acute pain' was verbal report of pain. The agreement rate with NANDA (2009)'s defining characteristics was 66.7%. Conclusion: Results indicate that identification of essential nursing diagnoses and their defining characteristics and related/risk factors is important for emergency patient nursing care to facilitate use of NANDA taxonomy in the emergency nursing practice and documentation systems.

  • Stress may be defined as any modification of environmental parameters that leads to a response by biological organisms. Stresses that affect biolpgical structures may be nonthermal, such as ultraviolet radiation, pH, or salinity, or thermal. Temperture is one of the major stresses that all living organism face. The major effects of cold(low emperature) are decrease of membrane fluidity and the stabilization of secondary structures of RNA and DNA in the cells, which may effect the efficiency of translation, transcription, and DNA replication. In this study, we focus on discovering the genes that are expressed by the cold(low temperature) stress in the silkworm. In cold(low temperature) stress test, we found 100% survive from cold stress at $0^{\circ}C$ up to 12h and $-5^{\circ}C$ up to 2h, and then, survive rate was rapidly decrease in silkworm. Thereafter two whole genes have selected by SSH(Suppression subtractive hybridization). (GenBank accession : GQ149511, GQ338156)

Contactless Palmprint Recognition Based on the KLT Feature Points (KLT 특징점에 기반한 비접촉 장문인식)

  • An effective solution to the variation on scale and rotation is required to recognize contactless palmprint. In this study, we firstly minimize the variation by extracting a region of interest(ROI) according to the size and orientation of hand and normalizing the ROI. This paper proposes a contactless palmprint recognition method based on KLT(Kanade-Lukas-Tomasi) feature points. To detect corresponding feature points, texture in local regions around KLT feature points are compared. Then, we recognize palmprint by measuring the similarity among displacement vectors which represent the size and direction of displacement of each pair of corresponding feature points. An experimental results using CASIA public database show that the proposed method is effective in contactless palmprint recognition. Especially, we can get the performance of exceeding 99% correct identification rate using multiple Gabor filters.

  • Metallothioneins(MTs) belong to the class of low molecular weight proteins. Recently, it has been suggested that MTs may playa direct role in cellular defense against oxidative stress by functioning as antioxidants. Oxidative damage to different cellular components makes a major contribution to many pathogenenesses. Several studies have demonstrated that MT is able to quench a wide range of reactive oxygen species at a higher efficiency than other well known antioxidants such as superoxide dismutate(SOD). The present study was designed to evaluate the effect of MT on the activities of the reactive oxygen species removal system. MT showed the scavenging of superoxide in the SOD assay system in the presence or absence of SOD. When MT was added to nicotinamide adenine dinucleotide phosphate(NADPH) oxidation system in presence of fixed amount of SOD increase the breakdown rate of superoxide. When MT was added to the system that form nitrite from hydroxylammonium chloride, the formation of nitrite was inhibit. We concluded that the function of MT as antioxidant might have an effect on the level of superoxide scavenging.

The Effect of Handwashing Improving Program and MRSA Carrier Screening Program on the MRSA Infection Rates in an Intensive Care Unit (손씻기 향상프로그램과 MRSA 보균자 색출프로그램이 MRSA감염 발생률에 미치는 영향)

  • Purpose: To assess the effect of handwashing improving program and MRSA carrier detection program on MRSA(methicillin resistant Staphylococcus aureus) infection rate in a intensive care unit. Method: The intervention was Nosocomial Infection(NI) control program consisted of hand washing improving program and identification and treatment of MRSA carrier. Data on the NI and MRSA infections were collected by an infection control nurse based on the definition of CDC. MRSA infection rates were calculated by the number of MRSA infection per 100 admissions or 1,000 patients-days. The difference of MRSA infection rates between pre and post intervention was tested by Chi-square at =.05. Result: MRSA infection rates 3.0% or 3.2 per 1,000 patient-days at the pre, 4.6% or 3.7 per 1,000 patient-days at the post, and the differences were not statistically significant (p=.411, p=.769 respectively). Conclusion: The handwashing improving program and MRSA carrier detection program was not effective in reducing the Nosocomial Infection(NI) or MRSA infection rates. It is recommended further studies with a longer intervention and follow-up period.
