재발성 심실성 빈맥이 동반된 승모판하 좌심실류의 외과적 치료 (Surgical Treatment of a Submitral Left Ventricular Aneurysm and the Patient Present with Recurrent Ventricular Tachycardia)

  • 승모판하 좌심실류는 흑인들에서 주로 발생하는 드문 질환이다. 저자들은 심실성 빈맥을 동반한 승모판하 좌심실류의 수술 치험을 보고하고자 한다. 환자는 68세 남자로 수술은 통상적인 저체온 심폐 바이패스 하에서 진행하였다. 심첨부를 들어올리고 좌심실 후방 기저부에 위치한 심실류에 직접 종절개를 넣고 먼저 심실류의 목에 해당하는 부위의 심내막을 돌아가며 고주파절제술(radiofrequency ablation)을 시행한 다음 승모판 및 판막하 구조물을 피해 첨포를 이용한 심실내 심실류봉합술(endoaneurysmorrhaphy)을 시행하였다. 수술 후 경과는 양호하였으며 현재 술 후 2년 정도 경과한 상태로 부정맥의 재발이나 승모판 기능이상은 관찰되지 않고 있다.

개심술 중 경정맥 산소포화도의 역할 (The Role of Jugular Venous Oxyhemoglobin Saturation Monitoring During Cardic Surgery)

  • 숭모판 치환술이 예정된 10명의 환자를 대상으로 심폐우회술 동안 발생할 수 있는 뇌산소 요구량과 소모량 사이의 불균형을 조사하기 위해 내경정맥 내의 산소포화도를 체외순환 5분전과 시작후 1분 이내, 저체온 상태가 안정되었을 때와 재가온하여 체온이 $34^{\circ}C$가 되었을 때, 그리고 체외순환 종료 후 15분 이내 등 5단계로 나누어 혈액을 채취하여 평균 동맥압, 체온, 동맥내 이산화탄소 분압, 혈색 소치, PH 등을 비교 분석하여 다음과 같은 결과를 얻었다. 1. 심폐우회술 중 이산화탄소 분압과 혈색소치 및 PH의 특이한 변화는 없었다. 2. 체외순환 직후 체온 및 평균 동맥압과 혈색 소치의 급격한 변화가 있었으나 임상적인 의미는 없었으며, $SjO_2$의 변화도 없었다. 3. 저체온 상태가 안정되었을때 $SjO_2$ 평균치가 72.4%이었으나 체온을 $34^{\circ}C$로 재가온하였을 때 56.1%로 감소하였다. 4. 재가온 시기에 $SjO_2$감소를 예방하기 위해 재가온 속도를 천천히 하고 이산화탄소를 증가시킴으로써 뇌혈류를 증가시키고 마취약제 등을 이용하여 산소에 대한 뇌대사율을 관류지수를 증가시켜야 하겠다.

외상 후 발생한 지연성 무명동맥류의 수술적 치료 -1예 보고 - (Surgical Treatment of Delayed Traumatic Anuerysm of the Innominate Artery - A case report-)

  • 무명동맥에 단독으로 발생하는 동맥류는 드문 질환으로 외상, 감염 또는 동맥경화증에 의해 발생하는 것으로 알려져 있다. 증례는 36세 남자 환자로 16년 전 교통사고의 병력 이외에는 특이한 과거력이 없었으며 신체검사에서 우연히 우상종격동 종양이 발견되어 본원으로 전원되었다. 컴퓨터단층촬영에서 흉골과 경계가 불분명한 5 cm크기의 진성 무명동맥류를 볼 수 있었으며 동맥류의 내부에는 혈전이 관찰되었다 수술은 중등도 이상의 저체온법을 이용하여 체외순환 하에서 Dacron Y-이식편을 사용하여 상행대동맥과 우측 경동맥 및 쇄골하동맥 사이에 우회도관을 연결하였으며 동맥류는 절제하였다. 환자는 수술 후 특별한 합병증 없이 회복되어 퇴원하였고 현재 추적 관찰 중이다.

Isolation of an Active Principle from Torilis Fructus and Its Pharmacological Activities

  • We reported that the hexane fraction of Torilis Fructus have an anti-inflammatory and analgesic effect. Therefore, in order to isolate the active compound, the hexan fraction of Torilis Fructus was chromatographed on silica gel column. The subfraction of hexane fraction was crystallized as colorless stout needles. The chemical structure of this compound was verified to be torilin through m.p., UV, IR, GC-MS, and NMR spectral data. In pharmacological tests, torilin exhibited strong anticarrageenan activity at the dose of 90 and 270 mg/kg, p.o. in rats, and it had inhibitory effect on the vascular permeability at the dose of 30 and 90 mg/kg, p.o. in mice. Torilin showed potent inhibition of leucocyte emigration in CMC-pouch at the dose of 3 and 9 mg/rat, s.c. Torilin have the analgesic effect at the dose of 30, 90 and 270 mg/kg, p.o. in both of the acetic acid- and phenyl-p-benzoquinone-induced writhing syndrome. It also increased the pain threshold at the dose of 30, 90 and 270 mg/kg, p.o. in the tail pressure method and the Randall-Selitto method. Torilin did not show a hypothermic action at the dose of 30 and 90 mg/kg, p.o. in mice. The acute toxicity of torilin was very weak: the LD$\_$50/ value was more than 5000 mg/kg, p.o. and 2000 mg/kg, Lp. in mice. From the above mentioned results, it was suggested that torilin had potent anti-inflammatory and analgesic activities in animals.

개심술 450례의 임상적 고찰 (Clinical Analysis of Open Heart Surgery -Review of 450 Cases)

  • 본원에서는 1985년 2월 부터 1996년 8월까지 450례의 개심술을 시행하였다. 450례중 선천성 심질환은 222례(49.3%), 후천성 심질환은 228례(50.7%)로 거의 비슷한 비율이었으며, 후반 기 5년간은 후천성 심질환이 더 많았다. 선천성 심질환은 비청색증심질환이 201례, 청색증 심질환이 21례를 차지 하였으며, 수술 사망율은 각각 5.5%와 38.1%를 차지하였다. 청색증 심질환은 대부분 활롯 4징증 이었다. 후천성 심질환 228례중 판막질환은 17례, 허혈성 심질환은 32례, 대동맥 질환은 12례, 심장종양은 6례 있었다. 판막질환은 승모판치환술이 87fll, 대동맥판막 치환술이 45례, 이중판막 치환술이 35례를 차지하였다. 허혈성 심장병의 평균문합순는 2.37개소 였으며, 사망율은 6.3%였다. 판막질환의 사망율은 10.8%, 대동맥질 환의 사망율은 16.7%였다. 전체 사망율은 9.6%이며, 선천성 심질환은 9.0%, 후천성 심질환은 10.1%였다.

너스바 제거 후 발생한 급성 대동맥 손상 -치험 1예- (Acute Aortic Injury after Nuss Bar Removal -A case report-)

  • 흉벽 기형의 일종인 누두흉의 수술방법인 Nuss 술식은 비교적 안전하고 교정 만족도가 높아 최근 많이 이용되는 방법이며 합병증으로는 기흉, 막대 편위, 창상 감염, 심낭염, 흉막 삼출, 혈흉, 심장 천공 등이 있다. 본원에서는 30세 남자 환자에서 Nuss 수술 3년 후 막대 편위로 인한 제거 수술 중 발생한 급성 흉부대동맥 손상을 경험하여 보고하는 바이다. 편위된 막대는 별 저항 없이 뽑혔으나 양측 수술 창을 통해 동맥혈이 뿜어져 나와 양손으로 출혈을 막고 신속하게 대퇴동-정맥 환류로 체외 순환을 시행하면서 정중 흉골절개를 가하고 초저온 순환 정지 하에 대동맥궁 기시부의 열상 부위를 봉합하였다. 환자는 수술 후 13일째 별다른 문제 없이 퇴원하였다.

유아기의 개심술14례 보고 (Open Heart Surgery During The First 12 Months Of Life)

  • Fourteen Infants with congenital cardiac anomalies underwent primary surgical Intervention within the first 12 months of life. There were eight patients with ventricular septal defect, two with total anomalous pulmonary venous return [TAPVR], and the remainders with tetralogy of Fallot, transposition of great arteries [d-TGA], Taussing-Bing malformation, and coronary A-V fistula. The age of the patients ranged from 5 to 12 months, with a mean age of 9.9 months. The mean weight was 6.7 Kg [3.8 to 9.5 KS]. Congestive heart failure persisting despite intensive medical treatment was present In 8 patients [56%], and was the most common indication for operation. Early operation was necessary in 5 of these patients [35%], because of failure to thrive and recurrent pulmonary infection. In one patient with TOF, frequent hypoxic spell prompted the necessity for early operation. In cases of VSD, TAP. VR, TOF, and coronary A-V fistula, Intracardiac repair was done with conventional cardiopulmonary bypass, chemical cold cardioplegia, and topical myocardial cooling. Deep hypothermic circulatory arrest with surface induced cooling, followed by core cooling and core rewarming, was employed .for better exposure in the cases of d-TGA and Taussing-Bing malformation. The results were however, not satisfactory. The overall mortality was 28 per cent. There were no deaths in the eight patients with VSD. The one with coronary A-V fistula survived. The other 5 cases all expired either on the table or immediately after operation. The non-fatal post-operative complications included low cardiac output, respiratory insufficiency, bleeding, and temporary A-V block. The causes of death were prolonged circulatory arrest time in d-TGA, complete A-V block and low cardiac output in TOF and Taussing-Bing malformation and prolonged bypass time and Inadequate correction in TAPVR.

심근경색에 의한 심정지 후 치료적 저체온증으로 호전된 쥐의 심폐소생술 모델 (Hypothermia Improves Outcomes of Cardiopulmonary Resuscitation After Cardiac Arrest In a Rat Model of Myocardial Infarction)

  • Therapeutic hypothermia(TH) improves neurological outcomes and reduces mortality among survivors of out-of-hospital cardiac arrest. Animal and human studies have shown that TH results in improved salvage of the myocardium, reduced infarct size, reduced left ventricular remodeling and better long-term left ventricular function in settings of regional myocardial ischemia. This study is to investigate the effect of TH on post-resuscitation myocardial dysfunction and survival time after cardiac arrest and resuscitation in a rat model of myocardial infarction (MI). Thoracotomies were performed in 10 Male Sprague-Dawley rats weighing 450-550 g. MI was induced by ligation of the left anterior descending coronary artery (LAD). Ninety min after LAD ligation, ventricular fibrillation induction and subsequent cardiopulmonary resuscitation was performed before defibrillation attempts. Animals were randomized to two groups: a) Acute MI-Normothermia b) Acute MI-Hypothermia ($32^{\circ}C$ for 4 h). Myocardial functions, including cardiac output, left ventricular ejection fraction, and myocardial performance index were measured echocardiographically together with duration of survival. Ejection fraction, cardiac output and myocardial performance index were $54.74{\pm}9.16$, $89.00{\pm}8.89$, $1.30{\pm}0.09$ respectively and significantly better in the TH group than those of the normothermic group at the first 4 h after resuscitation($32.20{\pm}1.85$,$41.60{\pm}8.62$,$1.77{\pm}0.19$)(p=0.00). The survival time of the hypothermic group ($31.8{\pm}14.8$ h) was greater than that of the normothermic group($12.3{\pm}6.5$ h, p<0.05). This study suggested that TH attenuated post resuscitation myocardial dysfunction in acute MI and would be a potential strategy in post resuscitation care.

Exploring Differences in Surgical Outcomes Depending on the Arterial Cannulation Strategy for Acute Type A Aortic Dissection: A Single-Center Study

  • Background: Type A aortic dissection (AD) and intramural hematoma (IMH) are critical medical conditions. Emergency surgery is typically performed under cardiopulmonary bypass immediately after diagnosis, which involves lowering the body temperature to induce total circulatory arrest. Selection of the arterial cannulation site is a critical consideration in cardiac surgery and becomes more challenging in patients with AD. This study explored the strengths and weaknesses of different cannulation methods by comparing each cannulation strategy and analyzing the reasons for patients' outcomes, especially mortality and cerebrovascular accidents (CVAs). Methods: This retrospective study reviewed the medical records of patients who underwent surgery for type A AD or IMH between 2008 and 2023, using the moderate hypothermic circulatory arrest approach at a single center. Results: Among the 146 patients reviewed, 32 underwent antegrade cannulation via axillary, innominate artery, aortic, or transapical cannulation, while 114 underwent retrograde cannulation via the femoral artery. The analysis of surgical outcomes revealed a significant difference in the total surgical time, with 356 minutes for antegrade and 443 minutes for retrograde cannulation (p<0.001). The mean length of stay in the intensive care unit was significantly longer in the retrograde group (5±16 days) than in the antegrade group (3±5 days, p=0.013). Nevertheless, no significant difference was found between the groups in the 30-day mortality or postoperative CVA rates (p=0.2 and p=0.7, respectively). Conclusion: Surgeons should consider an appropriate cannulation strategy for each patient instead of adhering strictly to a specific approach in AD surgery.
