Childhood hypertension (HTN) has become a significant public health issue because of the increased risk of cardiovascular disease in adulthood. However, childhood HTN is underrecognized and underdiagnosed in clinical practice. The European Society of Hypertension in 2016 and the American Academy of Pediatrics (AAP) in 2017 published updated guidelines for the screening, prevention, and management of pediatric HTN. There were notable differences between the two guidelines as well as many similarities. The updated AAP guidelines have clarified and simplified the recommendations for screening, diagnosis, and treatment of childhood HTN based on current evidence. This review highlights the important developments in both guidelines, focusing on recent advances in the classification and treatment of childhood HTN.
Rubinstein-Taybi syndrome (RTS) is characterized by peculiar facies, mental retardation, broad thumbs, and great toes. Approximately one-third of the affected individuals have a variety of congenital heart diseases. They can also have upper airway obstruction during sleep, due to hypotonia and the anatomy of the oropharynx and airway, which make these patients susceptible to obstructive sleep apnea (OSA). In our case, pulmonary hypertension was caused, successively, by congenital heart defects (a large patent ductus arteriosus and arch hypoplasia) and obstructive sleep apnea during early infancy. The congenital heart defects were surgically corrected, but persistent pulmonary hypertension was identified 2 months after the operation. This pulmonary hypertension was due to OSA, and it was relieved by nasal continuous positive airway pressure. This case is the first report of pulmonary hypertension from OSA in a young infant with RTS.
Purpose: The purpose of this study was to investigate the factors related to dyslipdemia and hypertension among male office workers. Methods: This study was a cross-sectional survey. The sample was 204 male office workers aged 30 to 62 years without cardiovascular disease and not taking medication for hypertension and dyslipidemia. Data were collected from November, 2011 to March, 2012. Dyslipidemia was examined by checking serum lipid profiles. Results: Dyslipidemia was related to career (adjusted OR 1.06, 95% CI 1.03~1.51), time spent at desk (adjusted OR 1.25, 95% CI 1.03~1.51), job stress of Q25-49 (adjusted OR 2.72, 95% CI 1.24~5.93), inactivity (adjusted OR 6.86, 95% CI 2.62~17.93), and snack intake frequency (adjusted OR 1.57, 95% CI 1.03~2.38). Hypertension was related to career (adjusted OR 1.07, 95% CI 1.01~1.14), heavy drinking (adjusted OR 5.00, 95% CI 1.25~20.04), and snack intake frequency (adjusted OR 2.10, 95% CI 1.33~3.34). Conclusion: Work and lifestyle-related factors were associated with dyslipidemia and hypertension in male office workers. These findings suggest that lifestyle intervention and improvement of working conditions are integral parts of cardiovascular disease prevention.
Objectives: This study thus set out to examine the health literacy and self-care behaviors of community-dwelling elderly patients with hypertension and investigate their relations and influential factors, thus providing basic data to develop a nursing intervention program to promote self-care behaviors among hypertension patients. Methods: The subjects include 180 hypertension patients using community health centers, public senior centers, and welfare centers in Jeju. Results: The subjects scored a mean of 49.8 (${\pm}12.4$) points on health literacy and 55.8 (${\pm}6.7$) points on self-care behaviors. In the results of hierarchical regression analysis, health literacy independently predicted self-care behaviors (${\beta}=.12$, p=.050) after controlling for general characteristics, hypertension-related knowledge and self-efficacy. Conclusions: These results suggest that interventions for improving health literacy are important to enhance elder's ability of self-care behaviors. It is necessary to understand the health literacy of elderly patients with hypertension before providing them with educational intervention and information and to develop appropriate educational materials and intervention programs.
Objectives: This study evaluated the association between work related characteristics and hypertension among white collar workers in Korea. Materials and methods: This study was based on the Third Korean Working Conditions Survey(KWCS) conducted in 2011 among workers 15 years of age or older. The total number of individuals included in the analysis was 10,365 white collar workers. Results: The prevalence rate of hypertension was 3.7% among men and 1.0% among women. In multiple logistic analysis the adjusted odds ratio showed statistically effective association to hypertension. For work related characteristics, 1.73 (95% confidence interval, 1.22-2.46) resulted for large-company workers compared with the small-company group, 2.14 (95% CI, 1.22-3.75) for the long working hours group (61 or above) compared with short working hours group(52 or below), 1.78(95% CI, 1.08-2.95) for night shift workers compared with day workers, and 1.54 (95% CI, 1.18-2.01) for high job stress workers compared with low job stress workers. Conclusions: This study showed that it is important for workers to manage their work environment in order to prevent hypertension by modifying their types of work.
$\beta$$_2$-Adrenergic receptors($\beta$$_2$-AR) contribute to cardiovascular regulation by influencing several functions and a several studies suggest that a decreased function of the $\beta$$_2$-AR may be involved in essential hypertension. We investigated the Arg16Gly mutation of $\beta$$_2$-AR gene, which show enhanced agonist-promoted downregulation of the receptor and yielded different results in terms of association with essential hypertension. We studied the relationship between genetic variation in the $\beta$$_2$-adrenergic receptor gene and hypertension in a Korean population using Nde I restriction fragment length polymorphism (RFLP) analysis. There were significant differences in allele and genotype frequencies between essential hypertensive and normotensive group (Odds ratio(CI) = 1.71 (1.09-2.70)). Therefore, our result suggests that the Nde I RELP of the $\beta$$_2$-adrenergic receptor gene may be useful as a genetic marker in hypertension diagnostics in Korean population.
Objectives : The aim of this study is to review clinical trials using Qigong exercise on patients with hypertension and to assess their methodology and results. Methods : Electronic literature searches for clinical trials (randomized trial, non-randomized trial, before-after study) of Qigong exercise were performed in 21 electronic databases (5 international databases and 16 Korean databases). English, Korean or Chinese articles were included. Laboratory or animal studies were excluded. Results : A total of 11 studies met the inclusion criteria. Five randomized controlled trials, Four non-randomized trials and two before-after studies were included. Seven studies used self-developed Qigong exercise. Two studies used Guolin Qigong exercise and two studies used Qigong exercise with an anti-hypertension drug. Of the nine randomized trials or non-randomized trials, four studies used an anti-hypertension drug control and three studies used waiting list controls. Compared to baseline, a change in blood pressure after the Qigong exercise treatment was significant in all studies. However, the results effect of blood pressure between Qigong exercise and controlled trials were not consistent. Conclusions : There are low-quality clinical trials of Qigong exercise for hypertension. To evaluate the effects of Qigong exercise, more rigorous trials are warranted.
Purpose: The purpose of this study was to identify the effects of an extreme heat Adaptation Program on the blood pressure, stress response, self-efficacy, and knowledge of management of hypertension and extreme heat of patients who suffered from hypertension. Methods: A quasi-experimental study with a non-equivalent control group pretest-posttest design was used. The data collection period was between July 2 and August 20, 2012. Thirty-seven patients participated in the study (18 in the experimental group and 19 in the control group). Data were analyzed using $X^2$-test, t-test, and Cronbach's alpha coefficients with SPSS/WIN 19.0. Results: Patients who participated in the program showed statistically significant improvements in systolic blood pressure (SBP), self-efficacy, and knowledge of management of hypertension and extreme heat. Conclusion: The results indicate that this extreme heat adaptation program can be utilized for patients suffering from hypertension in order to reduce their SBP and to increase self-efficacy and knowledge of management of hypertension and extreme heat. Therefore, it is recommended that this program be used for elderly patients suffering from chronic disease.
본 인제대학교 서울백병원 흉부외 과에서는 지난 8년간 술전 적극적 인 항고혈압제 투여에도 불구하 고 혈압 조절에 실패한9명의 신혈관성 고혈압 환자를 수술 치험하였다. 수술방법은9명중8명에서는 자가복재 정맥을 이용한 대동맥 신동맥간우회술을시행하였으며,나머지 1명은내장골동맥을이용한 신동맥 재건술을 시행하였다. 술후 9명의 환자중 사망한 예는 없었으며, 수술전후 비교한 혈압 수치의 선정 기준은, 술전에는 환자의 여러 차례 측정된 혈압중 대표적 인 혈압수치로 선정하였고, 술후혈압은 최종적으로 외래 추적 관찰한 혈 압수치를 선정 하였다. 술후 장기 추적한 걸과 9명의 환자 모두에서 혈압 호전을 보여 좋은 수술 결과를 얻었기 에, 중증 신혈관성 고혈압환자에 있어서 수술적 치료가 가장 효과적이고 유용한 치료법으로 생각 되는 바이다.
The objectives of this study were to identify the dietary patterns associated with hypertension among Korean males. Data from the 2001 Korean National Health and Nutrition Survey of 1,869 men aged 20-65 years were used for the analysis. As an initial analysis, a factor analysis was applied to identify major dietary patterns among the subjects. Then logistic regression analysis was conducted to identify the pattern related with hypertension. As a result of the initial analysis, three major dietary patterns were identified. Dietary pattern 1 (traditional) was heavily loaded with vegetables, fish and cereal. Dietary pattern 2 (Western) was loaded with fast foods, bread, meats and dairy products. Dietary pattern 3 (Drinker) was loaded with mostly pork, beer and soju (Korean liquor). From the second stage of the analysis, there was a tendency of positive association between traditional patterns and hypertension risks. However, the tendency did not meet statistical significance level (p<0.05). In summary, unlikely findings from European and American studies, vegetables rich traditional dietary patterns did not show any protective effect on hypertension in Korean males. The Korean dietary practice, which is consuming salted vegetables instead of fresh vegetables, might have played a role in these findings. However, the full explanation of the findings remained to be answered with further investigation since none of the dietary patterns identified showed any statistical significance.
