남녀 고혈압 노인의 위험음주 관련 요인 (Factors Associated with Hazardous Drinking in the Male and Female Elderly with Hypertension)

  • Purpose: This study examined socio-demographic and health status factors associated with hazardous drinking in male and female elderly with hypertension. Methods: The sample consisted of 2,322 seniors with hypertension and was obtained from the Seventh Korean National Health and Nutrition Examination Survey (VII-1, VII-2, VII-3). Complex sample analysis was performed using the independent t-test, the Chi-square test and multiple logistic regression in SPSS/WIN 24.0 program. Results: The factors associated with hazardous drinking in the male hypertensive seniors were age (OR=2.13, CI=1.37-3.32), weekly drink number (OR=0.11, CI=0.07-0.16) and weight gain control (OR=0.44, CI=0.21-0.94), and corresponding factors in female hypertensive seniors were age (OR=4.56, CI=2.37-8.80), income level (OR=1.88, CI=1.05-3.37), weekly drink number (OR=0.09, CI=0.04-0.17), current smoking (OR=6.88, CI=2.29-20.64), weight loss control (OR=1.99, CI=1.14-3.47), hypertension treatment (OR=35.62, CI=2.02-629.43), and hypertension drug (OR=0.06, CI=0.01-0.61). Conclusion: Risk drinking in elderly with hypertension was found to be related to personal characteristics and health status. Therefore, to manage hypertension effectively in the elderly, drinking patterns should be improved and active participation in the practice of healthy activities such as weight control, smoking cessation, and decreased alcohol intake, should be encouraged.

Acute cardiovascular complications in patients with diabetes and hypertension: management consideration for minor oral surgery

  • Objectives: Medically compromised patients often fear required dental surgical procedures that can increase the risk of medical emergency when combined with reduced tolerance for stress. A stress reduction protocol (SRP) helps doctors minimize treatment-related stress and improves patient management with minimum complications. Diabetes and co-morbid hypertension carry 4-fold risk of aggravation of cardiovascular emergencies and 7.2-fold risk of mortality. Diabetic neuropathy can result in difficult diagnosis of myocardial infarction and reduces chances of surviving a myocardial infarction compared with a non-diabetic person. The aim of the study was to assess the feasibility of a protocol for management of patients having both diabetes and hypertension who required minor oral surgery to minimize the rate of cardiovascular emergencies. Materials and Methods: A prospective study was conducted in 140 patients having both diabetes and hypertension who required minor oral surgical procedures. A systematic approachable protocol was designed for management of such patients. Results: Among 140 patients, 6 patients (4.3%) had cardiovascular complications, while 3 patients (1 with syncope and 2 with hypertension) did not require any intervention other than observation. Two patients were managed with aspirin and nitroglycerin, and 1 patient had possible myocardial infarction (overall incidence 0.7%) with chest pain, S-T segment elevation on electrocardiogram, and troponin level of 0.60 ng/mL. Conclusion: The proposed protocol helps to improve management of patients having both diabetes and hypertension. We recommend that patients with uncontrolled diabetes and uncontrolled hypertension and/or patients having history of cardiovascular complication should be treated in a medical facility with a readily available cardiology unit. This facilitates prompt response to emergency and instant implementation of treatment, helping to reduce morbidity and mortality.

  • 본 논문에서는 RFID tag의 충돌문제를 해결하여 고혈압 환자에게 정확한 의료서비스를 제공할 수 있는 hypertension management system을 제안한다. 제안한 시스템은 OR 로직을 사용하여 환자 주변의 RFID tag를 모바일 RFID 리더가 정확하게 인식할 수 있도록 하였다. 또한 모바일 환경에서 환자의 상태 및 의료서비스를 자동으로 제공하고, 관리하기 위해 멀티에이전트를 적용하였다.

The prevalence of hypertension in Haiti : A retrospective study

  • The purpose of this study was to explore the prevalence of hypertension in Haiti and to describe regional characteristics, and thus to speculate Haitian specific regional risk factors. This is a descriptive and comparative study. Subjects were 145 male/female Haitians, who aged between 30 and 59 and have lived in rural areas, had their hypertension indicators being screened. Data were analyzed with descriptive statistics, t-test and ANOVA. More than four fifth of the subjects (82.8%) showed pre-hypertensive or moderate to severe hypertensive condition. 65.5% of the total had abnormal pulse pressure ($PP{\geq}50$) and 13.8% of the sample showed tachycardia (Heart $Rate{\geq}100$). Female Haitian tended to have higher diastolic blood pressure and faster heart rate than male Haitian. The residents who lived in Part 5-1 area with less accessibility to drinking water tended to have higher systolic and diastolic pressure. Hydration status of Haitians was highly considered as an affecting factor on the distribution of hypertension as evidenced by different level of accessibility to drinking water.

  • The data mining is a new approach to extract useful information through effective analysis of huge data in numerous fields. We utilized this data mining technique to analyze medical record of 39,900 people. Whole data were separated by gender first and divided into three groups, including normal, stage 1 hypertension, and stage 2 hypertension. The data from each group were analyzed with data mining technique. Based on the result that we have extracted with this data mining technique, major risk factors for the hypertension are age, BMI score, family history.

  • Hypertension is characterized by multiple genetic and environmental factors. To establish the genetic basis of hypertension in Koreans, we investigated the genetic variations of two candidate genes (aldosterone synthase (CYP11B2), ${\gamma}$ subunit of the amiloride-sensitive epithelial sodium channel (ENaC) in the Korean patients with hypertension and normotensive controls. There were no significant differences in the genotype and allele frequencies between two groups, respectively. However, there was the significant difference between Korean and Caucasian populations in allele frequency of RFLPs in the two candidate genes. Therefore, these studies also need to be confirmed in other ethnic groups, although our results do not support a possible role of these genes on hypertension in Korean population

고혈압 환자의 맥상 연구 (A Study on the Pulse Wave Parameter in Hypertension Patients)

  • Objectives The purpose of this study was to determine whether a pulse analyzer was useful to characterize the variables of pulse wave of hypertension (HT), compared with those of normal blood pressure group (NP). Methods The subjects were 22,646 who worked at car manufacturing factory. The pulse wave variables and pulse types measured in Guan by DMP 1000. Results In the main and secondary pulse type, there were significant differences between HT and NP. Whereas, in the field of pulse wave variables, no specific differences were between HT and NP. Conclusion Although pulse wave variables were not statistically significant, pulse analyzer might have reliable diagnostic sensitivity between hypertension and non-hypertension population.

  • Purpose: This study was done to identify the effect of a group walking exercise program on weight, obesity, blood pressure and cholesterol in elderly patients with hypertension. Method: From the population of elderly people in Hwasung city, 80 people with hypertension were selected as participants in this study. Frequencies and paired t-test were used to analyze the data. Results: The group walking exercise program lowered weight, obesity, blood pressure, and cholesterol. And such a change appeared to be effective statistically. Conclusion: Therefore, it is important to develop a program and environment that promotes group walking exercise in elderly patients with hypertension.

Tissue-Specific Regulation of Angiotensinogen and Angiotensin II Receptor Gene Expression in Deoxycorticosterone Acetate-Salt Hypertensive Rats

  • Molecular regulation of the renin-angiotensin system (RAS) was investigated in deoxycorticosterone acetate (DOCA)-salt hypertension. The expression of renin, angiotensinogen and angiotensin II receptor genes in the kidney and liver was determined by Northern blot analysis in rats which were made DOCA-salt hypertensive over the period of 2 or 4 weeks. Along with the hypertension, renin mRNA was decreased in the remnant kidney. The expression of angiotensinogen gene was not significantly altered in the kidney, but was significantly decreased in the liver. The expression of angiotensin II receptor gene was increased in the kidney, while it remained unaltered in the liver. The duration of hypertension did not affect the altered gene expression. It is suggested that the components of RAS are transcriptionally regulated in DOCA-salt hypertension in a tissue-specific manner.

일 도시지역 노인의 고혈압과 우울 간의 관계 (A Study on the Relationship between Hypertension and Depression among the Elderly in Urban Areas)

  • Purpose: This study was designed to identify the correlation among perceived health status, health behavior, activities of daily living and depression among the elderly with hypertension. Methods: The subjects consisted of 238 person aged over 65 with hypertension who live in house located in BukGu Daegu city. The Data was collected through interview with questionnaire from July, to August 10, 2010. Results: There was a significant relation between perceived health status & ADL (r=.328, p=.000) and between depression & ADL (r=-.293, p=.000). There was s significant relation between depression & perceived health status (r=-.199, p=.002). The general characteristics which significantly affected depression was sex (t=2.49, p=.013), age (F=5.882, p=.001) education (F=6.550, p=.000), occupation (t=-6.990, p=.000). Conclusion: The findings of this study provide useful information for constructing an intervention program and care for the elderly with hypertension.
