Hypertension knowledge and Treatment compliance of Hypertensive Patients -Comparative analysis between groups treated at Primary Health Care Post in "H" Gun (country) and at other private Clinics·Hospitals. - (고혈압 환자의 고혈압지식 및 치료지시이행 -H군 관내 보건진료소와 병·의원 이용그룹 간 비교분석-)

  • Purpose: To determine the knowledge and treatment compliance with hypertension, directions for the aspects of implementation, and related factors to patients with hypertension using Primary Health Cares and Private Clinics Hospitals. Method: General characteristics and High blood pressure-related properties(17 questions), Hypertension knowledge measures(20 questions), and Treatment Compliance(22 questions) were used. Result: 1. The patients of Private Clinics Hospitals had statistically significantly higher drinking habits than Primary Health Care centers. The patients of Private Clinics Hospitals had statistically significantly higher rate of no family history of hypertension than Primary Health Care centers. 2. Primary Health Care centers had statistically significantly higher knowledge of hypertension than Private Clinics Hospitals. Primary Health Care centers had statistically significantly higher treatment compliance than Private Clinics Hospitals. Conclusion: Both Primary Health Care centers and Private Clinics Hospitals are high medication compliance but low lifestyle compliance with hypertension. We need to recognize the importance of lifestyle compliance, to apply proper programs and to provide therapists' aggressive intervention.

The Health Care Status and Healthy Life Practices of Hypertensive Patients (고혈압 환자의 건강행태와 생활습관 실천정도)

  • Purpose: The purpose of this study was to examine the health care status and healthy life practices among hypertensive patients in our South Korea in a bid to provide some information on the efficient management of hypertension. Methods: The data of the fifth Korea National Health and Nutrition Examination Survey of 2011 (KNHANES V-2) was utilized to analyze the state of hypertension management of 1,932 adults who were at the Western age of 19 and up and suffered from hypertension in order to identify what factors were related to their health care status and healthy life practices. IBM SPSS/WIN Statistics 20 software was employed, and frequency analysis and a ${\chi}^2$-test were carried out. Results: According to the results of the analysis, subjective health status, depression, and suicidal thoughts were significantly related to variable income, while health status and healthy life practices were not significant in town. Furthermore Depression, alcohol dependency, and walking were significant in hypertension patients who had experienced hypertension education programs. Conclusion: This study will provide database on hypertension management. Furthermore, multidisciplinary approaches should be taken for efficient and effective care for hypertension patients.

A Comparison of Controlled and Uncontrolled Hypertension Groups Regarding Comprehensive Diagnosis of Qi Blood Water and Quality of Life (고혈압 환자에서 혈압 조절 여부에 따른 기혈수(氣血水) 변증(辨證)과 삶의 질 비교)

  • This study is about a comparison of controlled and uncontrolled hypertension groups regarding comprehensive diagnosis of Qi blood water and quality of life. We surveyed "controlled and uncontrolled hypertension patients" using questionnaires for comprehensive diagnosis of Qi blood water, SF-36 and HTN QoL (Measurement Scale for the quality of life in hypertensive patients). There was no difference in comprehensive diagnosis of Qi blood water between the controlled and uncontrolled hypertension groups. Within the controlled hypertension group, the patients diagnosed with a Qi deficiency, Qi stagnation, Qi counterflow, blood deficiency, and water retention received lower total scores in SF-36 and HTN QoL than in undiagnosed patients. Within the uncontrolled hypertension group, the patients diagnosed with Qi deficiency, blood deficiency, and water retention got lower total scores in SF-36 and HTN QoL than in undiagnosed patients. These results were statistically significant. These results are insufficient that we and use comprehensive diagnosis of Qi blood water for a diagnosis tool of hypertension. But if we have better studies that make up for weak points, these results will help to make a diagnosis tool for hypertension.

Effect of Hypertension on Childhood-onset Systemic Lupus Erythematous in a Tertiary Medical Center in Korea

  • Purpose: The purpose of this study was to evaluate the prevalence, clinical characteristics, and long-term clinical effects of hypertension in Korean childhood-onset systemic lupus erythematous (SLE) patients. Methods: The medical records of SLE patients, diagnosed by 2019 SLE European League Against Rheumatism/American College of Rheumatology (EULAR/ACR) classification criteria, who visited Samsung Medical Center from January 2009 to May 2019 were reviewed. Disease activity and long-term damage were evaluated using the Modified Systemic Lupus Erythematosus Disease Activity Index 2000 (SLEDAI-2K) and the Pediatric Systemic Lupus International Collaborating Clinics/ACR Damage Index (Ped-SDI), respectively. The sex-, age- and height-blood pressure standards recommended by the American Academy of Pediatrics 2017 guideline was used to define hypertension. Results: A total of 32 patients were enrolled in this study. The median follow-up duration was 7.3 years and females were predominant. The median ages at SLE and hypertension diagnoses were 14.2 and 14.3 years, respectively. The biopsy-proven lupus nephritis was detected in 90.6% and 37.5% were class IV. During the follow-up, 12 patients (37.5%) had hypertension. Among them, 2 patients had 3 episodes of posterior reversible encephalopathy syndrome and 5 patients had left ventricular hypertrophy (LVH). Univariate analysis showed baseline hypertension was significantly correlated with a lower estimated glomerular filtration rate, higher body mass index and SLEDAI at baseline. The development of hypertension during the follow-up was significantly correlated with obesity, LVH, and higher Ped-SDI. Conclusion: Our study revealed that hypertension in pediatric SLE is associated with obesity and renal function at SLE diagnosis and could affect long-term damage.

Effects of an Extreme Heat Adaptation Program in Hypertensive Patients (고혈압환자의 폭염 적응력 증진을 위한 프로그램 효과)

  • Purpose: The purpose of this study was to identify the effects of an extreme heat Adaptation Program on the blood pressure, stress response, self-efficacy, and knowledge of management of hypertension and extreme heat of patients who suffered from hypertension. Methods: A quasi-experimental study with a non-equivalent control group pretest-posttest design was used. The data collection period was between July 2 and August 20, 2012. Thirty-seven patients participated in the study (18 in the experimental group and 19 in the control group). Data were analyzed using $X^2$-test, t-test, and Cronbach's alpha coefficients with SPSS/WIN 19.0. Results: Patients who participated in the program showed statistically significant improvements in systolic blood pressure (SBP), self-efficacy, and knowledge of management of hypertension and extreme heat. Conclusion: The results indicate that this extreme heat adaptation program can be utilized for patients suffering from hypertension in order to reduce their SBP and to increase self-efficacy and knowledge of management of hypertension and extreme heat. Therefore, it is recommended that this program be used for elderly patients suffering from chronic disease.

Long-Term Result of Surgical Treatment for Renovascular Hypertension (신혈관성 고혈압의 외과적 치료 결과)

  • During the past eight years, we have encountered 9 patients, aged between 2 and 61 years, with renovascular hypertension. The renovascular hypertension In this series included Takayasu's disease with renal artery stenosis, arteriosclerosis of renal artery, fibromuscular dysplas a of renal artery Aortd-renal bypass was performed In 8 patients, iliac-to-renal bypass in 1 patient. 9 patients have been followed form 2 months to 5.1 years. Postoperatively, all patients'hypertension was improved. Only 2 patients need to take small dose of antihypertensive medication after discharge. These data indicated the good results of renovascular reconstruction for the patients with renovascular hypertension.

  • Objective: A case report on the improvement of hypertension patients by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 59-year-old Korean male with hypertension having bad habits as alcohol, smoking and overweight. Results: Weight loss and blood pressure improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of patients with chronic symptoms to hypertension.

Late Results of Operation for Patent Ductus Arteriosus with Preoperative Pulmonary Hypertension (폐동맥고혈압을 동반한 동맥관개존증의 장기관찰 성적)

  • This study concerns the late results observed at follow-up[average: 32.5 months] of 23 patients, in whom Patent Ductus Arteriosus with pulmonary hypertension among 55 patients. They were operated from May 1984 to July 1987 in Chonnam University Medical School. There was predominance of woman [2.5:1]. No operative death occurred. All of patients subside preoperative symptoms, but 1 patient diagnosed recannalization of duct at follow-up. Systolic murmur was found over the pulmonary area in 5 patients [22 %], but it may be not related to systolic pulmonary arterial pressure[SPAP]. Also, SPAP were normalized in 74 % of patients and had mild hypertension in 26 % of patients. Although the patients had Patent Ductus Arteriosus with pulmonary hypertension, successful surgical correction was carried out safely in all instance but one, by ligation and facilitated by hypotensive anesthesia.

Effects of Chronic Disease Education for Hypertension, Diabetes Patients's Knowledge (만성질환 교육이 고혈압, 당뇨병 환자의 지식수준에 미치는 영향)

  • Objectives: This study is based on Public Care Service for Chronic Diseases in Incheon Metropolitan City featuring Health-Point Service. It is objective to evaluate whether the service could improve self-management ability of hypertension and diabetes patients. Methods: The knowledge about diabetes and hypertension, which can be improved by educational intervention, is considered as one of the most important factor for self-management ability. While the education service have been conducted, 194 patients who have been diagnosed as hypertension and diabetes within 3 months were included. They completed questionnaires two times within 4 months period, which give information about the knowledge, service registration and education experiences. For further inspection, 396 patients who had been diagnosed formerly were studied by cross-sectional analysis. Results: Regardless of service registration or educational intervention, there wasn't statistically meaningful difference between the initial and the follow-up assessments on the knowledge. However, when investigating formerly diagnosed patients, educated patients had better knowledge on hypertension compared with non-educated patients and registered patients, educated patients had the better knowledge on diabetes compared with non-registered patients, non-educated patients. Additionally, analyzing each questions, educated patients got higher grades in questions about definition of hypertension, diabetes managements which they can practice in daily lives. Conclusions: It is probable that long-term service registration and education experiences could improve the knowledge and self-management ability.

Accessible Strategy of the Registration & Management of Hypertension and Diabetes Mellitus Patients through the Public-Private Partnership: Policy Implications Fron the Hongcheon-gun Case (민관협력을 통한 고혈압.당뇨병 등록.관리사업의 접근 전략: 홍천군 사례의 정책적 함의)

  • Objectives: This study aimed to share with experiences of a demonstration program based on a community for prevention and management of hypertension and diabetes mellitus, and to supply the evidence of accessible strategies within the community through the public-private partnershipin the near future. Methods: This study case was "the program of registration and management of hypertension and diabetes mellitus patients" which was conducted in Hogncheon-gun in Gangwon-province, 2012. Results: The infrastructure of this center was constructed with the public-private sector partnership according to the basic model of demonstration program since November, 2012. So, the total registered rate of hypertension and diabetes mellitus patients were 26.6% in comparison with suspected patients (to the result of 2011 Korea National Health and Nutrition Examination Survey), 37.8% in comparison with the real number of outpatient (to the claims data of 2011 Branch Honcheon-gun, National Health Insurance Corporation), and 107.8% in comparison with the project goal, sequentially. To the patients who were not treated for 30 and 60 days among the registered patients, a recall service was conducted. Through this intervention, it was monitored that this program has enhanced the consecutive treatment rate of the registered patients. Conclusions: To improve the continuous management of hypertension and diabetes mellitus patients, we are gotten to know that the community need the joint participation and mutual cooperation with public-private sector partnership.
