Purpose: Aplasia Cutis Congenita (ACC) is a rare disease characterized by the focal defect of the skin at birth, frequently involving scalp, but it may affect any region of the body. There are no etiology known but some conditions such as intrauterine vascular ischemia, amniotic adherences and viral infections are associated. The ideal treatment for the ACC is not known. Superficial and relatively small sized defects (< $3{\times}5\;cm$) may heal spontaneously and large defects related with risks of infection and bleeding may require aggressive surgical treatment. Hyalomatrix$^{(R)}$ is a bilayer of an esterified hyaluronan scaffold beneath a silicone membrane. It has been used as a temporary dermal substitute to cover deep thickness skin defect and has physiological functions derive from the structural role in extracellular matrix and interaction with cell surface receptor. This material has been used for the wound bed pre-treatment for skin graft to follow and especially in uncooperative patient, like a newborn, this could be a efficient and aseptic way of promoting granulation without daily irritative wound care. For this reason, using Hyalomatrix$^{(R)}$ for the treatment of ACC was preferred in this paper. Methods: We report a case of a newborn with ACC of the vertex scalp and non-ossified partial skull defect. The large sized skin and skull defect ($6{\times}6\;cm$) was found with intact dura mater. No other complications such as bleeding or abnormal neurologic sign were accompanied. Escharectomy was performed and Hyalomatrix$^{(R)}$ was applied for the protection and the induction of acute wound healing for 3 months before the split-thickness skin graft. During the 3 months period, the dressing was renewed in aseptic technique for every 3 weeks. The skin graft was achieved on the healthy granulation bed. Results: The operative procedure was uneventful without necessity of blood transfusion. Postoperative physical examination revealed no additional abnormalities. Regular wound management was performed in out-patient clinic and the grafted skin was taken completely. No other problems developed during follow-up. Conclusion: Hyalomatrix$^{(R)}$ provides protective and favorable environment for wound healing. The combination of the use of Hyalomatrix$^{(R)}$ and the skin graft will be a good alternative for the ACC patients with relatively large defect on vertex.
우리나라토종호두나무인가래나무(Juglans mandshurica Maxim.)의 수액의 유리당 함량은 약 3%로 다른 나무 수액에 비해 높게 나타났으며 미량 원소 중에는 Ca이 30 mg/kg의 농도로 포함되어 있는 것으로 확인되었다. 가래나무 수액 $200{\mu}g/ml$에서 elastase 저해 활성이 약 30% 정도를 보이고 있고 HaCaT 세포를 이용한 피부보습 효과에서는 자외선을 조사한 HaCaT세포의 aquaporin 3 유전자와 hyaluronan synthase 2 유전자 발현을 촉진시켜 hyaluronic acid의 생합성을 증가시킨 것으로 확인되었다. 본 연구를 통해 가래나무 수액의 피부 노화 방지 효능을 최초로 규명하였고 이는 가래나무 수액의 활용도를 높일 수 있는 가능성을 제시한 결과라 사료된다. 다만 물질 분석과 in-vivo 시험, 임상 연구 등 심도 있는 추가 연구가 필요할 것으로 판단된다.
레몬그라스와 자소엽 추출물은 다양한 생리 효과를 나타내는 것으로 알려져 있지만, 피부보습과 피부장벽에 미치는 영향은 아직까지 연구되지 않았다. 본 연구에서는 레몬그라스와 자소엽 추출물의 피부보습과 피부장벽에 미치는 영향과 페놀성 화합물을 분석하였다. 피부각질형성세포에서 각 추출물이 피부보습에 미치는 영향을 조사한 결과, 두 추출물 모두 물보다 에탄올 추출물에서 히알루론산 생성이 많았다. HPLC를 이용한 19종 페놀성 화합물 분석 결과는 레몬그라스 에탄올 추출물(CCE)에서 chlorogenic acid와 p-coumaric acid가 검출되었으며 자소엽 에탄올 추출물(PFE)에서 rosmarinic acid와 caffeic acid가 검출되었다. 피부보습에 관련된 HAS1, HAS2, HAS3 및 AQP3와 피부장벽에 관련된 filaggrin, loricrin 발현은 PFE보다 CCE에서 높았다. 또한, CCE, PFE 모두 피부보습과 표피분화 조절에 관여하는 $PPAR-{\alpha}$ 단백질의 발현이 농도 의존적으로 증가하였으며 CCE의 주요성분인 chlorogenic acid와 p-coumaric acid가 $PPAR-{\alpha}$ 발현을 증가시켰다. 결론적으로 피부보습과 피부장벽보호 효과에 있어서 CCE가 PFE보다 우수한 효과를 나타내었고, 두 추출물은 피부보습과 피부장벽개선에 대한 기능성 소재로써 활용될 수 있을 것이라 생각된다.

Purpose: The aim of this study was to conduct a histologic evaluation of irradiated calvarial defects in rats 4 weeks after applying fibroblast growth factor-2 (FGF-2) with hyaluronan or biphasic calcium phosphate (BCP) block in the presence or absence of adjunctive hyperbaric oxygen (HBO) therapy. Methods: Twenty rats were divided into HBO and non-HBO (NHBO) groups, each of which was divided into FGF-2 and BCP-block subgroups according to the grafted material. Localized radiation with a single 12-Gy dose was applied to the calvaria of rats to simulate radiotherapy. Four weeks after applying this radiation, 2 symmetrical circular defects with a diameter of 6 mm were created in the parietal bones of each animal. The right-side defect was filled with the materials mentioned above and the left-side defect was not filled (as a control). All defects were covered with a resorbable barrier membrane. During 4 weeks of healing, 1 hour of HBO therapy was applied to the rats in the HBO groups 5 times a week. The rats were then killed, and the calvarial specimens were harvested for radiographic and histologic analyses. Results: New bone formation was greatest in the FGF-2 subgroup, and improvement was not found in the BCP subgroup. HBO seemed to have a minimal effect on new bone formation. There was tendency for more angiogenesis in the HBO groups than the NHBO groups, but the group with HBO and FGF-2 did not show significantly better outcomes than the HBO-only group or the NHBO group with FGF-2. Conclusions: HBO exerted beneficial effects on angiogenesis in calvarial defects of irradiated rats over a 4-week healing period, but it appeared to have minimal effects on bone regeneration. FGF-2 seemed to enhance new bone formation and angiogenesis, but its efficacy appeared to be reduced when HBO was applied.
Background: Ergothioneine (EGT) is a natural amino acid derivative in various animal organs and is a bioactive compound recognized as a food and medicine. Objectives: This study examined the effects of EGT supplementation during the in vitro maturation (IVM) period on porcine oocyte maturation and subsequent embryonic development competence after in vitro fertilization (IVF). Methods: Each EGT concentration (0, 10, 50, and 100 μM) was supplemented in the maturation medium during IVM. After IVM, nuclear maturation, intracellular glutathione (GSH), and reactive oxygen species (ROS) levels of oocytes were investigated. In addition, the genes related to cumulus function and antioxidant pathways in oocytes or cumulus cells were investigated. Finally, this study examined whether EGT could affect embryonic development after IVF. Results: After IVM, the EGT supplementation group showed significantly higher intracellular GSH levels and significantly lower intracellular ROS levels than the control group. Moreover, the expression levels of hyaluronan synthase 2 and Connexin 43 were significantly higher in the 10 μM EGT group than in the control group. The expression levels of nuclear factor erythroid 2-related factor 2 (Nrf2) and NAD(P)H quinone dehydrogenase 1 (NQO1) were significantly higher in the oocytes of the 10 μM EGT group than in the control group. In the assessment of subsequent embryonic development after IVF, the 10 μM EGT treatment group improved the cleavage and blastocyst rate significantly than the control group. Conclusions: Supplementation of EGT improved oocyte maturation and embryonic development by reducing oxidative stress in IVM oocytes.
본 연구에서는 식용 어류로부터 얻은 콜라겐을 효소 처리하여 글라이신-프롤린-하이드록시프롤린(Gly-Pro-Hyp, GPH)과 같은 트리펩타이드의 함량이 높은 콜라겐 가수분해물을 얻었고, 이 저분자 콜라겐 가수분해물(이하 콜라겐 트리펩타이드)의 피부 보습효과를 생체 외 실험(in vitro)과 생체 내 실험(in vivo)을 설계하여 증명하고자 하였다. 사람의 피부 섬유아세포 HDF 세포를 자외선 조사를 통해 인위적으로 손상된 세포를 만든 후, 콜라겐 트리펩타이드가 이를 회복시키는 정도를 몇 가지 보습인자의 활성과 발현량으로 확인하였다. Ceramide kinase의 효소활성은 콜라겐 트리펩타이드의 농도에 의존적으로 증가하였고, hyaluronic acid의 농도 역시 유의적으로 증가하였다. 더불어 히알루론산을 합성하는 유전자인 HAS2의 mRNA와 단백질 발현량이 시료의 처리양에 비례하여 증가하였고, 히알루론산을 분해하는 효소인 HYAL1의 수준은 콜라겐 트리펩다이드의 농도에 의존적으로 감소하였다. 상기의 효과는 1형 콜라겐을 합성하는 Collagen 1A 유전자의 단백질 발현량과 피부염증과 종양발생 시 증가한다고 알려진 CD44의 발현량의 확인으로 증명되었다. 유기용매의 도포로 피부건조를 유도한 동물모델을 사용하여 콜라겐 트리펩타이드의 피부에의 유효성을 평가하기 위해 경피수분손실량(TEWL)와 수분함유량을 직접적으로 측정한 바, 콜라겐 트리펩타이드 34 mg/kg bw의 고용량 처리군에서 가장 긍정적인 변화가 확인되었으나 17 mg/kg bw의 콜라겐 트리펩타이드 처리군에서도 수분함량에서 유의적인 변화가 관찰되었다. 이는 피부 건조로 유도된 가려움증에 대하여 동물이 자신의 몸을 긁는 행위를 계수한 결과와 같은 양상을 보였다. 또한 피부가 건조할 시 유도될 수 있는 염증인자인 $TNF-{\alpha}$와 IL-6의 단백질 발현량을 동물 피부 조직을 적출하여 확인하였고, 염색법을 통하여 피부 진피 내 탄력섬유의 변화를 가시적으로 제시하였다. 이 피부 조직에서의 Collagen 1A와 AQP3의 단백질 발현량 및 세라마이드 키나이제, HAS2, 그리고 HYAL1 효소활성 결과는 상기의 누적된 콜라겐 트리펩타이드의 보습 효과를 더불어 증명하여 주었다. 이상의 결과로부터 본 연구의 저자는 콜라겐 트리펩타이드가 피부 보습에 효능을 갖는 소재로서 활용가치가 높음을 알 수 있었으며, 현재 시장에서 유통되고 있는 분자량 1000 Da 이상의 콜라겐 가수분해물보다도 저분자화 되며 트리펩타이드를 다량 함유하는 상기의 소재가 증명된 유효성과 증가된 체내 흡수도에 근거하여 보다 확장된 가능성을 보여줄 수 있을 것이라 판단된다.
본 연구에서는 이너뷰티용 소재로서의 활용가능성을 확인하기 위하여 생물전환 가시오가피-진피 혼합추출물(B-MEAC)의 항산화 활성, 엘라스타제 억제활성 및 피부보습 효과 등을 평가하였다. B-MEAC의 항산화 활성을 측정한 결과 DPPH radical을 농도의존적으로 소거하여 SC50이 156.1±0.82 ㎍/ml로 나타났다. 또한, 엘라스타제 억제활성을 측정한 결과 B-MEAC 처리 농도 의존적으로 엘라스타제 활성을 억제하는 것으로 확인되었다(p<0.001). 경구투여한 B-MEAC가 마우스 피부 보습에 미치는 영향을 확인하기 위하여 피부 수분량과 경피수분손실량을 측정한 결과 건조유도 된 대조군에 비하여 피부 수분량은 증가하고(p<0.001), 경피수분손실량은 감소하는 것으로 나타났다(p<0.01). 또한, 건조유도 된 마우스 피부의 교원섬유 변화에 미치는 영향을 Masson's trichrome 염색을 통해 관찰한 결과 B-MEAC를 투여한 군에서 콜라겐의 양이 대조군에 비해 상대적으로 증가하여 푸른색으로 보다 진하게 염색되었다. 또한, 건조유도 된 마우스 피부의 보습 기능에 미치는 영향을 확인하기 위하여 보습 관련 인자들의 발현을 Western blot 기법으로 확인한 결과 B-MEAC를 투여한 군에서 대조군에 비하여 MMP-1 단백질의 발현량은 감소하였고, COL1A1, HAS2, filaggrin, AQP3 단백질 발현량은 회복되는 것으로 나타났다(p<0.001). 따라서 B-MEAC의 보습효과는 엘라스타제 활성 억제, MMP-1 발현 감소, 콜라겐 생성 촉진, 피부 보습 관련 단백질인 HAS2 및 AQP3 발현 증가, 피부 장벽기능의 중요인자인 filaggrin 발현 증가를 통한 수분량 증가와 TEWL 감소에 의해 나타나는 것으로 이너뷰티 소재로서의 개발 가능성을 제시한다.
