In an effort to use plant biotechnology for the production of biopharmaceuticals, pollens collected from lily (Lilium longiflorum) were grown in vitro and transformed with a PCR-amplified 1.7 kb cDNA encoding human tissue plasminogen activator (tPA) using Agrobacterium via a vacuum infiltration process. Western blotting showed that transgenic lily pollen tubes selected on kanamycin for 16 hrs expressed a tPA protein with a size similar to the human standard, suggesting their possible use as a disposable host for rapid foreign protein production.

There has been an increase in the public concern about possible health risks by electromagnetic exposure from mobile phones. Recently, increase in the use among young children of portable telephones public concern regarding potential health hazards due to a hot spot appearing inside the infant head, has been growing. Since the biological hazards due to RF exposure are caused mainly by a temperature-rise in tissue, the effect of localized SAR for portable telephones should also be related to the temperature-rise in the human head. In this paper, it was measured that in the actual use of portable telephone the temperature of the local tissue in the human-head change. As a result, it should be noted that the mean temperature of human-body and localized tissue is rising from beginning call. However the temperature variation of localized tissue is recovered rapidly as normal temperature, although the mean temperature of human-body rising continuously at ending call of portable telephone.

In order to investigate the effects of GamiTakliSodocyum(GTS) on wound healing, migration of epidermis, formation of granulation tissue and number of capillary within the granulation tissue were measured in diabetic mice by local application and NZW rabbits by local application and prescription of medicine in vivo, and proliferation of human epidermal keratinocytes and human dermal fibroblasts and composition of extracellular matrix were measured in vitro. The results were summerized as follows. 1. $2\%,\;10\%$ GTS remarkably increased migration of epidermis in diabetic mice by local application. 2. $2\%,\;10\%$ GTS remarkably increased formation of granulation tissue, number of neovascularization within the granulation tissue in diabetic mice by local application. 3. 5\%,\;10\%$ GTS remarkably increased migration of epidermis in NZW rabbits by local application. 4. $5\%,\;10\%$ GTS remarkably increased fonnation of granulation tissue, number of neovascularization within the granulation tissue in NZW rabbits by local application. 5. $5\%,\;10\%$ GTS increased migration of epidennis in NZW rabbits by prescription of medicine. 6. $5\%,\;10\%$ GTS increased formation of granulation tissue, number of neovascularization within the granulation tissue in NZW rabbits by prescription of medicine. 7. GTS didn't show effect on the proliferation of human epidermal keratinocytes. 8. GTS increased the proliferation of cultured human dermal fibroblasts. 9. GTS increased the expression of procoliagen ${\alpha}1(I) mRNA in cultured human dermal fibroblasts. 10. GTS increased the expression of fibronectin mRNA in cultured human dennal fibroblasts according to dosage of GTS using northern blot hybridization but didn't increase, using RT-PCR. From the above results, it is conclude that GTS might use on wound healing.
Background In prosthesis-based breast reconstruction patients, the drain tends to be kept in place longer than in patients who undergo only mastectomy. Postoperative arm exercise also increases the drainage volume. However, to preserve shoulder function, early exercise is recommended. In this study, we investigated the effect of early exercise on the total drainage volume and drain duration in these patients. Methods We designed a prospective randomized trial involving 56 patients who underwent immediate breast reconstruction following mastectomy using tissue expanders. In each group, the patients were randomized either to perform early arm exercises using specific shoulder movement guidelines 2 days after surgery or to restrict arm movement above the shoulder height until drain removal. The drain duration and the total amount of drainage were the primary endpoints. Results There were no significant differences in age, height, weight, body mass index, or mastectomy specimen weight between the two groups. The total amount of drainage was 1,497 mL in the early exercise group and 1,336 mL in the exercise restriction group. The duration until complete removal of the drains was 19.71 days in the early exercise group and 17.11 days in the exercise restriction group. Conclusions Exercise restriction after breast reconstruction did not lead to a significant difference in the drainage volume or the average time until drain removal. Thus, early exercise is recommended for improved shoulder mobility postoperatively. More long-term studies are needed to determine the effect of early exercise on shoulder mobility in prosthesis-based breast reconstruction patients.

Background Face transplantation has naturally evolved from reconstructive procedures. However, few institutions perform face transplantations, because it is time-consuming and it is necessary to justify non-vital organ transplantation. We investigated the process of organ donation from brain-dead patients and the possibility of incorporating face transplantation into the donation process. Methods A retrospective review was performed of 1,074 brain-dead patients from January 2015 to December 2016 in Korea. We analyzed the time intervals from admission to brain death decisions (first, second, and final), the causes of brain death, and the state of the transplanted organs. Results The patient base (n=1,074) was composed of 747 males and 327 females. The average period between admission to the first brain death decision was 8.5 days (${\pm}15.3$). The average time intervals between the first brain death decision and medical confirmation using electroencephalography and between the first brain death decision and the final determination of brain death were 16 hours 58 minutes (${\pm}14hours$ 50 minutes) and 22 hours 57 minutes (${\pm}16hours$ 16 minutes), respectively. The most common cause of brain death was cerebral hemorrhage/stroke (42.3%), followed by hypoxia (30.1%), and head trauma (25.2%). Conclusions When face transplantation is performed, the transplantation team has 22 hours 57 minutes on average to prepare after the first brain death decision. The cause of brain death was head trauma in approximately one-fourth of cases. Although head trauma does not always imply facial trauma, surgeons should be aware that the facial tissue may be compromised in such cases.

Cultures of primary human alveolar bone-derived cells were established from alveolar bone chips obtained from normal individuals undergoing tooth extraction. These cells were expanded in vitro until passage 3 and used for the in vivo assays. Cells were loaded into transplantation vehicles, and transplanted subcutaneously into immunodeficient mice to study the capacities of human alveolar bone-derived cells to form bone in vivo. Transplants were harvested 12 weeks after transplantation and evaluated histologically. Of 10 human alveolar bone-derived cell transplants, two formed a bone-like tissue that featured osteocytes and mineral. Eight of the ten formed no osseous tissue. These results show that cells from normal human alveolar bone are capable of forming bone-like tissue when transplanted into immunodeficient mice.
Purpose: The reconstruction of a soft tissue defect of the heel pad can be challenging. One vital issue is the restoration of the ability of the heel to bear the load of the body weight. Many surgeons prefer to use local flaps or free tissue transfer rather than a skin graft. In this study, we evaluated the criteria for choosing a proper flap for heel pad reconstruction. Methods: In this study, 23 cases of heel pad reconstruction were performed by using the flap technique. The etiologies of the heel defects included pressure sores, trauma, or wide excision of a malignant tumor. During the operation, the location, size and depth of the heel pad defect determined which flap was chosen. When the defect size was relatively small and the defect depth was limited to the subcutaneous layer, a local flap was used. A free flap was selected when the defect was so large and deep that almost entire heel pad had to be replaced. Results: There was only one complication of poor graft acceptance, involving partial flap necrosis. This patient experienced complete recovery after debridement of the necrotic tissue and a split thickness skin graft. None of the other transferred tissues had complications. During the follow-up period, the patients were reported satisfactory with both aesthetic and functional results. Conclusion: The heel pad reconstructive method is determined by the size and soft-tissue requirements of the defect. The proper choice of the donor flap allows to achieve satisfactory surgical outcomes in aesthetic and functional viewpoints with fewer complications.

We developed a three dimensional cardiac tissue model based on human cardiac cell and mono-domain approximation for action potential propagation. The human myocyte model proposed by ten Tusscher et al. (TNNP model) (2004) for cell electrophysiology and a mono-domain method for electric wave propagation are used to simulate the cardiac tissue propagation mechanism using a finite element method. To delineate non-homogeneity across cardiac tissue layer, we used three types of cardiac cell models. Ansiotropic effect of action potential propagation is also considered in this study. In this 3D anisotropic cardiac tissue with three cell layers, we generated a reentrant wave using S1-S2 protocol. Computational results showed that the reentrant wave was affected by the anisotropic properties of the cells. To test the reentrant wave under pathological state, we simulated a hypertopic model with non-excitable fibroblasts in stochastic manner. Compared with normal tissue, the hypertropic tissue result showed another center of reentrant wave, indicating that the wave pattern can be more easily changed from regular with a concentric focus to irregular multi-focused reentrant waves in case of patients with hypertrophy.

Background Reconstruction after removal of a malignant tumor in the head and neck region is crucial for restoring tissue integrity, function, and aesthetics. We retrospectively analyzed patients who underwent intraoral reconstruction surgery using radial forearm free flaps (RFFF) and anterolateral thigh free flaps (ALT) at a single institution to provide more information supporting the choice of a reconstruction method after removal of head and neck cancer. Methods The charts of 708 patients who underwent head and neck reconstruction between 1998 and 2018 at the Department of Plastic and Reconstructive Surgery at our institution were retrospectively reviewed. Patients' age, sex, and history of radiation therapy, diabetes mellitus, and smoking were retrieved. The primary cancer site, types of defects, and complications were investigated. Results Overall, 473 and 95 patients underwent reconstruction surgery with RFFF and ALT, respectively. RFFF was more often used in patients with cancers of the pharynx, larynx, esophagus, or tonsil, while ALT was more frequently used in patients with cancers of the mouth floor with tonsil or tongue involvement. The proportion of patients undergoing ALT increased gradually. Flap failure and donor site morbidities did not show significant differences between the two groups. Conclusions RFFF and ALT flaps resulted in similar outcomes in terms of flap survival and donor site morbidity. ALT can be an option for head and neck reconstruction surgery in patients with large and complex defects or for young patients who want to hide their donor site scars.
Background Oncoplastic surgery is being increasingly performed in Korean women; however, unlike Westerners, Korean women usually have small to moderate-sized breasts. To achieve better outcomes in reconstructed breasts, several factors should be considered to determine the optimal surgical method. Methods A total of 108 patients who underwent oncoplastic surgery from January 2013 to December 2016 were retrospectively investigated. We used various methods, including glandular tissue reshaping, latissimus dorsi (LD) flap transposition, and reduction oncoplasty, to restore the breast volume and symmetry. Results The mean weight of the tumor specimens was 40.46 g, and the ratio of the tumor specimen weight to breast volume was 0.12 g/mL in the patients who underwent glandular tissue reshaping (n=59). The corresponding values were 101.47 g and 0.14 g/mL, respectively, in the patients who underwent reduction oncoplasty (n=17), and 82.54 g and 0.20 g/mL, respectively, in those treated with an LD flap (n=32). Glandular tissue reshaping was mostly performed in the upper outer quadrant, and LD flap transposition was mostly performed in the lower inner quadrant. No major complications were noted. Most patients were satisfied with the aesthetic results. Conclusions We report satisfactory outcomes of oncoplastic surgical procedures in Korean patients. The results regarding specimen weight and the tumor-to-breast ratio of Asian patients will be a helpful reference point for determining the most appropriate oncoplastic surgical technique.
