Effect of the non-Medical Services on Attitude to Hospital Utilization (의료기술외적 병원써비스가 병원이용태도에 미치는 영향)

  • Effects of various factors related to the process of social action of hospital services on the selecting a hospital were investigated by using of questionnaires answered by 1,319 patients in a certain University Hospital, in Chungchongnam-Do, from November, 1986 to November, 1987. The results summarized were as follows ; 1. Each of the examined factors, that is, sincerity of an attendant physician in medical treatment and explaining the condition of a disease, confidence to doctor and nurse, doctor's and nurse's response to calling by patient, kindness of the hospital staffs and food handlers, sanitariness of hospital, cleanness of clothes and bed sheets, hospital foods, and rest surroundings, are not solely decisive to attitude of hospital utilization, but the factors effects compositely on hospital utilization 2. Hospital services of the social action process are more effective to hospital utilization in cases of the hospitalization caused by the traffic facility than for the medical skill and facilities.

The Effect of Hospital Service Coordinator Education Curriculum on the Education Satisfaction and the Quality of Medical Service (병원서비스코디네이터 교육과정이 교육만족과 의료서비스 품질에 미치는 영향)

  • The increase of the supply of medical service and the increase of hospitals have intensified the competition of hospitals, and the advancement towards internationalization in the opening of medical industry has triggered the infinite competition of medical profession. In addition, the high expectation of customers and quality improvement in the medical care in accordance with the improvement of overall income, and the change of active role of medical consumers according to the popularization and the improvement of rights awareness reflect the customer needs and choice in the medical service. Customers wanted to receive the kind and pleasant service under the up-to-date medical service. Therefore, as a solution, hospital coordinators were emerged for the purpose of smooth treatment and customer satisfaction by generalizing all service of hospital. Accordingly, this thesis attempted to investigate the effect of hospital coordinator education curriculum on the education satisfaction and the quality of medical service. In order to solve the purpose of this study, I, author reviewed the existing literatures, established hypothesis, and verified hypothesis by using the variety of statistics techniques such as reliability, validity, frequency analysis, and regression analysis. The verification of hypothesis is as followings: First, among education training factors of hospital coordinators, the quality of instructor significantly affects the satisfaction of hospital coordinator education training. Second, among training factors of hospital coordinator, the attitude of trainee significantly affects the training satisfaction of hospital coordinator. Third, among education training factors of hospital coordinator, education course significantly affects the training satisfaction of hospital coordinator education. As the qualities of instructor are better equipped, the satisfaction of education becomes higher. It indicates that the education method of instructors is important as an index to represent the qualities of instructor such as the appropriateness of education method, preparation, passion, visual materials, the adequacy of education procession, and specialized knowledge, and it has important effect on the satisfaction of education. In order to enhance the satisfaction of hospital coordinator education, the creation of education environment, making trainee concentrate on the education, is required by appropriately allocating programs, arousing interest in education, based on the attitude of trainee, discussion, and preliminary programs, preparation, ahead of enforcement of education. Fourth, the satisfaction of hospital coordinator education training significantly affects the reliability among the qualities of medical service. Fifth, satisfaction of hospital coordinator education training significantly affects hospitality I kindness among the qualities of medical service. If the education satisfaction of trainee is high, it is effective in the practical application such as dealing with complaints, the duty performance for the patients, and so on in offering the medical service, related to reliability and furthermore, we can find the positive change in the attitude change of medical professions related to the reliability of hospital coordinator. In addition, in the process of offering medical services such as the kind explanation on the duty, rapid response to the customers inquiry, and tidy uniform, practical effect was verified. Sixth, the education training factor of hospital coordinator significantly affects the reliability among the quality of medical service. Seventh, the education training factors of hospital coordinator significantly affect hospitality/kindness. In the education of hospital coordinator, the methods to attract the interest of trainee by emphasizing reliability should be sought and for gaining the practical effect of hospital coordinator education, the sufficient preparation and investigation on the education curriculum should be prerequisite and under this condition, intensified discussion on the instructor and education course is needed. In the design of education course, more education hours and subjects should be allocated in the part of hospitality in order to improve the practical application of hospitality. Therefore, it is meaningful in a sense that this study newly approached the components of hospital coordinator education and the need to modify the quality components of medical service in accordance with the study subjects was raised. This study also finds its meaning in that it provides basic materials for the study of future hospital coordinator education by suggesting the system development model of hospital coordinator education through preliminary study of education training. In addition, this study is meaningful in the aspect that it suggested the direction of education training by showing how the hospital coordinator education training would applied to the hospital coordinator course of the Continuing Education Center at Pusan and Kyungnam National University to some extent. Since all investigation of this study was approached from the side of hospital coordinator, the thoughts of patients who are beneficiaries of medical service, and care givers cannot be identified. Therefore, the satisfaction of patients and care givers through the experience of medical service, which is the essential prerequisite of medical service, should be importantly considered and investigated. Accordingly, The study of comparing and analyzing the views of both patients and care givers should be carried out in the future.

  • One of the most important things for hospital managements is understanding the patients' needs. The aim of this study is to analyse the priorities of the factors influencing patients' choice for hospital with analytic hierarchy process (AHP), which can derive the relative importances of factors. With understanding the factors that influencing patients' choice for hospital, the proper methods to improve hospital reputation and health care quality could be determined. Among 12 factors, speciality of hospital, number of departments, staff kindness, doctor-patient communication, and patients' visit time were relatively more important and higher priority. Whereas, public transport accessibilities, hospital equipments, and parking lots were relatively less important and lower priority. These results could be used to improve hospital reputation and health care quality effectively. Therefore, the hospitals in Gwangju should consider the factors that influencing patients' choice and improve policies for hospital marketing strategies for better hospital reputations and health care quality.

A Study on the Effect of the Environmental Improvement in the Diagnostic Radiography Room on Patients (진단방사선과 검사실의 환경개선이 환자에게 미치는 영향에 관한 연구)

  • Background : This study was attempted to provide us with basic data on how to environmental improvement with patients for examination, and to offer them better treatment. This study was performed to compare the patients, perception between before and after improvement in the diagnostic radiography room. Methods : The data was collected by interviewing 75 patients who underwent the radiography under the diagnostic radiology at Seoul National University Hospital in Korea. The interview ran from August 9 to October 18, 1999. Data were analyzed by percentage and paired t-test. SD(Semantic Differential) method was composed of adjective 13 words. Results : Patients were attending the elementary schools in the Seoul residents. There was no significant difference in kindness unkindness dimension and were significant differences in other dimensions. The mean score of response level to present room was 3.67 and that of improvement room was 2.16. Conclusions : The results of this study suggest a radiography room plan which is considering emotional aspect of children.

A Study for Enhancing the Image of Medical Services in a Hospital (일 병원의 의료 이미지 향상을 위한 조사 연구)

  • The purpose of this study was to investigate client's satisfaction and to promote the image of medical services in the hostpital. It is important to identify how client is satisfy with the medical services for the image of hospital. The study was done on population group of 334 patients and their families who had been admitted to the Shospital in Seoul from July 15 to July 31. 1995. Data was collected by questionnaires on all steps of the research. Subjects were instructed to rate the 5 point likert type scale on items. 328 questionnaires returned and were used for this analysis. The results were summarised as follows: 1. The charcteristics of the subjects were as follows; the age group of 20-39 years was 61.0%. For academic background. 51.2% were graduated from high school and 28.7% were graduated from College. 2. The average scores for the services rendered were as follows; a) Satisfaction on nursing services 3.75 b) Satisfaction on kindness of hospital workers 3.55 c) Satisfaction on medical services 3.36 d) Satisfaction on environmental elements 3.19 e) Satisfaction on diet for the patients 3.18 3. The lowest score of client's satisfaction on each area shown in this study is as follows; a) Doctors explain result of tests and patient's home management 3.23 b) Nurses are eager to resolve client's problem 3.47 c) In environmental elements. toilets. roomtemperture. diet. store 2.81-2.91

The Assessment of Foodservice Satisfaction by Orthopedic Patients according to their Involvement (정형외과 환자의 관여도에 따른 병원급식 만족도 분석)

  • The purposes of this study were to evaluate the quality of hospital food services in view of patients in orthopedic wards and accomplish the quality improvement in hospital foodservice operations. Quantitative questionnaires for patients containing foodservice satisfaction and demographic information were developed. A survey of 8 general hospitals was undertaken and detailed information was collected from 290 patients in orthopedic wards. The collected data were processed using the SAS PC 6.12 for descriptive analysis, t-test. In demographic information of patients, 32.6% was over 50 years old and 31% was hospitalized over 30 days. 80% of patients was taking normal diet. 47.7% and 47.9% of patients showed moderate appetite and moderate pain respectively. The overall satisfaction score for patients was 3.24 out of 5, showing slightly higher level than the average score(3.00). According to foodservice involvement scores of patients, they were divided into two groups which were high involved group and low involved group. Two groups showed significant differences in taste of meals, variety of menu, punctuality of meal times, temperature of meals and portion size. The foodservice involvement factor which affected significantly patient foodservice satisfaction was 'kindness of foodservice staff'.

Comparison of Nursing Needs as Perceived by Admitted Patients and Nurses in a General Hospital (입원환자와 간호사의 간호요구도 비교연구)

  • Purpose: This study was done to compare the perceived nursing needs as reported by admitted patients and nurses in a general hospital. Method: The participants were 194 patients and 183 nurses in a general hospital in Seoul. The instruments were constructed based on a prior study review and revised after a pilot study with 3 patients. The questionnaire for nursing needs included questions on physical, therapeutic, emotional, educational and socioeconomic domains. Data were collected between February 29 and March 26 for patients and between February 19 and 26, 2002 for nurses. The data were analyzed with frequencies, percentiles, means and t-test. Results: The results were as follow: 1. The total score for perceived nursing needs by nurses was higher than that reported by patients except for socioeconomic needs. 2. There were significant differences between patients and nurses only in the emotional domain (t=3.50, p=.001). 3. The highest score was for therapeutic needs and the next was for educational needs. 4. Relatively higher scored items were for comfort care, prevention of nosocomial infections, immediate treatment, monitoring health condition, kindness, and explanation of tests and treatments. Conclusion: Nurses generally understand patients' nursing needs but have to be more interested in patients' needs in order to prevent nosocomial infections, to provide care in a good relationship with the doctors and to encourage the patients.

The Relationship of National University Hospital Inpatient's Perceived Quality, Satisfaction, and Customer Loyalty (국립대학병원 입원환자가 느끼는 의료서비스 질, 만족도, 고객 충성 도간의 관련성 분석)

  • The purpose of this study is to identify the nature of the inpatient service quality of national university hospital, and based on that, to examine the relationship of hospital inpatient's perceived quality, overall satisfaction, customer loyalty(intention of revisiting, intention of oral transmitting). To carry out these objectives, first we analyzed the dimensions of inpatient care service quality using SERVQUAL scale. The SERVQUAL scale is based on the gap theory, that is, the difference of patients' expectations and the actually received medical care service in hospital. On the basis of this theory, we measured the inpatient's perceived service quality, overall patient satisfaction and customer loyalty. Data were collected by self-administered questionnaires at a 809 bed national university hospital. These questionnaires measuring the service quality were distributed to 400 inpatients. The data samples are 347 cases in final. The response rate was 86.8%. Firstly, to categorize inpatient service quality in hospital, the factor analysis was performed on 48 items. The reliability and validity of these items was evaluated. Finally to explore the relationship of service quality, overall satisfaction, and customer loyalty, the multiple regression and logistic regression analysis are used. This study shows firstly, the dimension of inpatient service quality was categorized into 7 dimensions, that is, kindness, medical service, nurse caring, environment, facilities, appropriateness and access. Secondly, the reliability and validity of inpatient service quality items was satisfied. Thirdly, as a result of multiple regression analysis, the effect of inpatient's perceived service quality, especially, nurse caring(P<0.01), environment (P<0.01), facilities, appropriateness and access variables(P<0.05), on overall satisfaction was statistically significant. Lastly, in case of the effect on customer loyalty as a intension of oral transmitting, medical service(P<0.05), environment(P<0.01) and overall satisfaction(P<0.01) are statistically significant. Also, in case of intension of revisiting, medical service, environment, access, and overall satisfaction variables are significant factors. In conclusion, to maintain the satisfaction and customer loyalty on national university hospitals, the efforts to improve the inpatient service quality, especially, environment, medical service, and access factors might be needed.

Comparison of patient's satisfaction on the pharmacy services between those using the pharmacy nearby hospital and those using the pharmacy in resident areas (의약분업이후 병원문전약국과 동네약국 이용자의 만족도 비교)

  • The purpose of the study was to compare patient's satisfaction on the pharmacy services after introducing the new system of separation of dispensing from prescribing medicines, between those filling their prescriptions from the pharmacy nearby hospitals and those from the pharmacy in their resident areas. To measure patient's satisfaction, a questionnaire survey was conducted with 354 outpatients who received prescriptions from any of the three university hospitals located in In-Cheon city. Study results showed that geographic accessibility to pharmacy was a main attribute to select pharmacy. Size of the pharmacy and availability of prescription drugs are the second major reasons for pharmacy selection for the patients from the pharmacy nearby hospital, whereas patronage is the second major reason for those from the pharmacy in resident areas. Overall satisfaction was higher among the patients from the pharmacy in resident areas than those from the pharmacy nearby hospitals, mainly due to better facilities(waiting area, public telephone, etc), kindness, cleanliness, shorter waiting time, and pharmacist's concern about patient's health. On the other hand, the patients of the pharmacy nearby hospitals showed low satisfaction because of long waiting time and the lack of pharmacist's knowledge and information about patients' health status and medication history. Patients visiting the hospital that has pharmacy-hospital cooperation system showed higher satisfaction as compared to those visiting the hospital without such system. This study provided an empirical evidence that it would be more advantageous for patients to receive pharmacy services from pharmacies located in their living areas than from pharmacies nearby hospitals. This implies that there is a strong need for adequate strategies to enhance the role of pharmacies in resident areas under the new system of separation of dispensing from prescribing medications.

Factors Affecting Health Services Satisfaction of Cancer Patients in University Hospitals (대학병원 암환자의 의료서비스 만족도에 영향을 미치는 요인)

  • The purpose of this study is to analyze the factors that satisfy health services of cancer patients in university hospitals. 420 cancer patients who had visited hospitals in the capital and regional area were selected for this study. The data was collected through questionnaire survey along with a face-to-face interview from March. 21 to March. 30, 2011. This major findings of this research are as follows. First, satisfaction of cancer patients regarding health services is determined by the quality, kindness and detailed explanation of medical team. Second, health services satisfaction of cancer patients is strongly related to good convenience facilities and up-to-date medical equipments. Third, satisfaction of cancer patients regarding health services was statistically proved to have no significant relation to easy accessibility of hospital. Therefore, it is very important for university hospitals to improve quality of medical staff such as ability, kindness and detailed explanation of doctor, offer good convenience facilities and the most advanced equipments. It is expected that the outcomes of this study will contribute to the satisfaction improvement for cancer patients in university hospitals.
