산업분류와 만성질환 유무와의 관계 (The Relationship between Industrial Classification and Chronic Disease)

  • Purposes: The industry has specialized and fragmented than in the past. As a factor of economic growth and industrialization, the number of people employed in primary industry decreased and the number of people employed in secondary and third industry continuously increased. In modern times, incidence of chronic disease is increasing according to industrial development. So, the purpose of this study was to analyze the chronic disease according to Clark's industrial classification. Methodology: Data were derived from the 2012 Korea Health Panel. The sample was made up of 7,132 adult participants aged 20 or over selected Korea Health Panel by probability sampling from Korea. Binary logistic regression analysis was conducted to examine the main factors associated with chronic disease. Findings: The significant factors associated with chronic disease were gender, age, marital status, household member, education level, insurance type, disability, BMI, and industrial classification. Female, elderly, divorced(including bereavement, missing and separation), one-person households, less than high school graduation, medical aid, disability, obese and primary industry were confirmed chronic disease increases. Practical Implications: The study finds that primary industry's prevalence of chronic disease was higher than secondary and third industry. Therefore, this study aims to management and effort of the worker who engaged in the primary industry. Policy development is required to address inequality or popularization of the differences in these factors by conducting a study to define the working conditions and socio-economic factors between industry.

우리나라 종합병원의 하방 경직적 원가행태 분석 (Sticky Cost Behavior Analysis of General Hospitals in Korea)

  • The purpose of this empirical study is to investigate whether costs are 'sticky' -that is, whether costs increase more when revenues rises than they decrease when revenues falls by an equivalent amount by using the financial data fromf korean general hospital Financial data used in this study were obtained from the Database of Korean Health Industry Development Institute and analyzed using multiple regression model in dummy variables. The main results of this study are as follows: First, we found, for 69 Korean general hospitals for 3 years(2000~2002), that total hospital costs, hospital labor costs, hospital administrative costs were sticky, these costs provided strong support for the sticky costs hypothesis 1, but hospital material costs were shown to be proportional to sales revenues. Second, this results provided strong support for the hypotheses that the' degree of stickiness was lower in sales revenues declining that were preceded by revenue-declining periods (hypothesis 2-1), and that stickiness was less pronounced in a second successive year of revenue decline(hypothesis 2-2). Third, this results provided strong support for the hypothesis(hypothesis 3) that stickiness was greater hospitals that employ relatively more people to support their sales revenues(hypothesis 4) that stickiness was greater for hospital that used relatively more assets to support their sales revenues. After all, a managerial implication of this study was that sticky cost, for the general hospital, could be recognized and controlled.

병원유형별 의료기관인증이 병원운영효과에 미치는 영향 분석 - 인증 전과 후 비교 - (Analysis of the Impact of Medical Institution Certification by Hospital Type on Hospital Operation Effect to Increase Wellness - Comparison Before and After Certification -)

  • 본 연구는 의료기관 인증 제도를 받은 요양병원 내부 구성원과 양방병원의 내부구성원 297명을 대상으로 설문조사를 실시한 후 SPSS 26.0을 이용하여 인증을 받은 전과 후 차이를 비교 분석하여 구성원 만족도, 직무스트레스, 의료서비스향상이 병원운영 효과에 미치는 영향을 파악하고자 하였다. 요양병원의 병원운영효과는 구성원 만족도와 의료 서비스향상이 높을수록, 병원운영효과는 높아짐을 볼 수 있으며, 양방병원의 경우, 구성원 만족도와 의료 서비스 향상이 높을수록, 직무스트레스는 낮을수록 병원운영효과는 높아짐을 볼 수 있다. 의료기관인증제도의 기본가치 체계가 환자 안전과 의료의 질 향상인 만큼 요양병원은 의무적 인증으로 인증제의 효과가 양방병원보다 높게 나타났을 것으로 사료된다.

경도 인지장애 환자의 병원선택 요인 (Hospital Selection Factors of Patients with Mild Cognitive Impairment)

  • 본 연구는 경도 인지장애 환자들이 병원을 선택하는 기준에 대해 알아보고자 실시하였다. 연구는 2018년 5월 1일부터 9월 30일까지 연구참여에 동의한 36명을 대상으로 설문을 실시하였다. 자료수집은 재활병원과 요양병원에 입원한 환자를 대상으로 하였으며 K-MMSE를 이용하여 인지 선별평가를 시행하고, 병원선택요인 도구를 본 연구에 맞게 수정 보완하여 사용하였다. 연구의 결과를 종합해보면 거주지 또는 직장과의 거리가 가깝고 교통용이, 간편한 행정적 절차, 병원의 유명도, 새로운 서비스, 물리치료와 작업치료 등과 관련된 서비스 등이 경도 인지장애 환자의 병원 선택에 영향을 미쳤다(p<.05). 본 연구의 결과를 토대로 증가하는 경도 인지장애 환자의 병원 내원을 대비하여 임상가들과 환자를 유치하는 의료기관의 마케팅 전략에도 변화가 필요할 것이라 사료된다.

네트워크 시스템 병원의 경영성과 : 비교 연구 (The Financial Performance of Hospitals Belonging to Multi-hospital System : A Comparative Study)

  • The purpose of this study is to analyze the performance difference between multi-hospitals and free-standing hospitals. Scholars in industrial economics argue that, due to economies of scale and integration, multi-hospital system may have a better performance compared to freestanding hospitals. The study overturned the hypothesis based on a theory. By analyzing 425 acute-care hospitals in Korea, this research shows that multi-hospital systems and market factors, which have been perceived to be strengths to hospitals, are negatively related to hospitals' financial performance. Specifically, the results showed a better performance of freestanding hospitals compared to multi-hospital systems. Higher labor and administrative cost by multi-hospital system may be the reason for the difference, and it means they are not more effective at cost control. Managers in multi-hospital system, therefore, should pay attention on cost-reducing issues to regain managerial efficiency of organizations.

강원도형 녹색건강산업 특화분야의 발전 방안 (A Study on Development Strategy of Green Health Industry Specialized fields in Gangwon-do Privince)

  • As Gangwon-do Province needs an environmental-friendly alternative tourism according to low carbon green growth, this study analyzes specialized fields in green health industry, selects seven specialized fields in green health industry that is fit to Gangwon-do Province, and suggests a development strategy by conducting recognition surveys on preference and impotence of the specialized fields. Surveys and interviews are conducted of 106 students at Green Health Industry Education Center which is operating by Kwandong University and 84 persons who work at medical institutions and tourism related companies in Ganwondo. The surveys and interviews were completed from November 5 to November 19 in 2010, determined factors through a factor analysis when they have many questions, and analyzed by using SPSS 14.0. Out of the seven specialized fields, rest & recuperation field is highly recognized. In order to have competitive edge compared to other self-governing provinces, Ganwon-do Province should prioritize rest and recuperation field. A healthcare program in the green health industry specific to Ganwon-do Province includes hot springs and spa programs as a priority. Gangwon-do's hot springs are deemed to be competitive resources as global medical tourists prefer spa and sauna healthcare programs, while the province promotes its spring resources and builds medical tourism infrastructures. Gangwon-do Province can promote a medical tourism industry that is well suitable for the strength and characteristics of the province. It might pursue oriental medicine therapy tourism, which is related to a recuperating medical service that uses both oriental and western medicine. It can also run such programs as forest bathing (oxygen road), spring and spa, and sea water treatment, which are the specialized fields in green health industry with respect to recreation and healthcare.

Development of Korean CARcinogen EXposure: Assessment of the Exposure Intensity of Carcinogens by Industry

  • Background: Occupational cancer is a global health issue. The Korean CARcinogen EXposure (K-CAREX), a database of CARcinogen EXposure, was developed for the Korean labor force to estimate the number of workers exposed to carcinogens by industry. The present study aimed to estimate the intensity of exposure to carcinogens by industry, in order to supply complementary information about CARcinogen EXposure intensity to the K-CAREX. Methods: We used nationwide workplace monitoring data from 2014 to 2016 and selected target carcinogens based on the K-CAREX list. We computed the 95th percentile levels of measurements for each industry by carcinogens. Based on the 95th percentile level relative to the occupational exposure limit, we classified the CARcinogen EXposure intensity into five exposure ratings (1-5) for each industry. Results: The exposure ratings were estimated for 21 carcinogenic agents in each of the 228 minor industry groups. For example, 3,058 samples were measured for benzene in the manufacturing industry of basic chemicals. This industry was assigned a benzene exposure rating of 3. Conclusions: We evaluated the CARcinogen EXposure ratings across industries in Korean workers. The results will provide information on the exposure intensity to carcinogens for integration into the K-CAREX. Furthermore, it will aid in prioritizing control efforts and identifying industries of concern.

Four-week Repeated Dose Toxicity Test for Myelophil in SD Rats

  • Aim : To evaluate the pharmaceutical safety of the herbal formula Myelophil, composed of Astragali Radix and Salviae Radix, via systemic subacute toxicological study using SD rats. Methods : Forty male and 40 female SD rats were fed with Myelophil (5000, 2500 or 1250 mg/10 mL/kg) or distilled water for four weeks. Adverse effects were examined intensively by comparing the differences between normal and drug-administered groups using clinical signs, necropsies, histopathologic findings, hematology, urinalysis, and blood biochemical analysis. Results : No altered clinical symptoms including body weight, diarrhea, anorexia, death, and abnormal necropsy of major organs were observed in male or female rats. No drug-induced abnormalities in histopathological finding, hematological values, urinalysis, and blood biochemical values were found at any doses of Myelophil. Conclusion : Myelophil should be very safe when used in a clinical application with a wide therapeutic index.

병원의 규모와 범위의 경제 (Economies of Scale and Scope in Hospitals)

  • This study investigates economies of scale, cost complementarity and economies of scope for healthcare organizations using econometric approaches. The economies of scale appear to exist in each service provided by a hospital such as inpatient treatment services, outpatient treatment services, and other patient treatment services, respectively. When we test all services in aggregate level, it also indicates that the healthcare industry on average exhibits the economies of scale of 6 percent, which implies that scaling up hospital sizes will bring substantial cost savings to them Evidence shows that cost complementarity exists between outpatient services and other services for patients and, i.e., these other services for patients experience the reduction in marginal costs as the outputs of the outpatient services increase. For the economies of scope, they are present in most service areas; aggregate level services, outpatient services, and other services for patients, respectively. Inpatient treatment services, however, do not show any evidence of the economies of scope. Results show that the economies of scope are achieved by the general hospital type that provides all service areas such as inpatient treatments, outpatient treatments, and other services for patients. The existence of the economies of scope provides the rationale for extending the existing line of business in a hospital into more diverse areas of services where its benefit comes in the form of cost savings. In sum, it overall provides evidence that the M&As in this industry are encouraged to achieve cost reductions from the economies of scale and scope by changing the size and the output mix.
