The study on the dynamic characteristics of steel structure system for vibration estimation in hospital building (철골조 고층 병원건물의 진동예측을 위한 동적특성 및 설계인자에 관한 연구)

  • The primary objective of this study is to provide simple analytical tools to estimate dynamic characteristics of steel framed floor system in hospital building for vibration serviceability due to human activities, bogie, medical equipment. and so on. In order to evaluate the dynamic characteristics and vibration levels according to steel framed floor system, we had executed impact test and measurement on steel structure floor system in various hospital buildings. But perhaps most importantly, how to make the most of deduced design factor for design of hospital building. therefore we presented the access method such as the three-dimensional F.E. numerical analysis on the basis of the design drawing, and the properties of all floors for estimation of vibration level in hospital building.

Factors Affecting Delayed Hospital Arrival Times in Acute Ischemic Stroke Patients

  • The purpose of this study was to investigate the factors of hospital arrival delays of acute ischemic stroke patients. The study subjects were 126 cerebral infarction patients G Metropolitan City university hospital emergency center. General characteristics, disease-related characteristics and stroke-related were collected by self-reported questionnaires. Hospital arrival times by subjects' characteristics were tested by $x^2$ test and logistic regression analysis. Of 126 cerebral infarction patients, Their average hours taken to move to a hospital was 12.7 hours with the fastest case being 0.5 hour and the most delayed case being 127.8 hours. 61.1%(77 persons) of the stroke patients under this experiment said to have taken 3 hours or less. In logistic regression analyses, Coming to the hospital directly without passing through other hospitals was found to have higher probability of arriving less than 3 hours(${\beta}$=2.960, p=.009), And if LAPSS was tested positive, such cases are more likely to arrive within 3 hours(${\beta}$=2.219, p=.049). For acute ischemic stroke and caregivers need training to be conducted promptly admitted to hospitals for education and treatment hospital stroke screening will help to improve the treatment of stroke patients

Clinical Characteristics Analysis of 58 Patients with Breast and Gynecologic Cancer in Oriental Hospital (한방병원에 내원한 유방암 및 부인암 환자 58명에 대한 임상적 특성 분석)

  • This study was aimed to obtain epidemiological information of cancer patients treated with Oriental medicine. 58 breast and gynecological cancer patients treated in Cancer Center of Daeguhanny Oriental Hospital from August 2012 to August 2013 were reviewed. Careful investigations were done by categorizing these patients by their origin, stage, treatment, conventional treatment type, chief complaint, etc. In tumor origin, breast cancer showed the largest proportion in total patients(74.1%) and inpatients(81.8%). 63.8% of the patients' tumors were stage III and IV. 48.3% of patients visited Oriental hospital for combination treatment with conventional medicine. 91.4% of the patients have under 5 years of cancer duration. Their cheif complaints are general weakness, postoperative pain and abdominal discomfort in general. This study presented the characteristics of breast and gynecological cancer patients treated by Oriental medical therapies, and thus would be valuable for futher studies of Oriental medical cancer treatments.

  • Purpose: This study is to propose direction for healing environment design in hospital architecture considering the characteristics of elderly patients. The primary goal of a public hospital is providing cheap health care and quality service chance to the underprivileged and elderly in the dead zone. Compared to the rapid increase of the aged population and chronic diseases, Ministry of Health & Welfare is currently planning model of health promotion hospital in development plans of local based public hospital. Due to the increased elderly medical expenses, elderly patients' high utilization is considered. Methods: The literature on the design factors of healing environments were investigated. based on this, analyzed drawings of surveyed hospitals ward in architectural characteristics and observe and evaluate directly healing environment design through field surveys. Results: The design of hospital environment affects patient's therapeutic effect. There is no any official formula for hospital design but environment that architect create could be a big part of the healing process. To increase the quality of the environment, apply the characteristic factors and harmonize well as a human-centered healing environment. Implications: Healing environment design for the elderly is first necessary condition due to high proportion of elderly patients.

A Study on the Preference of Hospital Names in Korea (우리나라 병원의 명칭 선호도에 관한 연구)

  • This study purposed to analyze the preference of names in Korean hospitals. Names were analyzed in terms of hospital's characteristics such as hospital type, location, ownership. Also, multivariate regression analysis was applied to assess how the amount of information measured from their names was varied by the hospitals' internal characteristics. Yearbook of Korean Hospital Association in 2008 was used to collect the study data. The full name of hospitals was divided into meaningful words to investigate the preferred words. The amount of information in hospital names was measured based on the number of words extracted. The study results showed that hospitals preferred different words depending on their characteristics. The amount of information contained in hospital names tends to decrease between before and after year 2000. Korean hospitals increased the uses of Hangeul and foreign words for their names in recent years. These changes were considered as the responses of Korean hospitals to increase competitive advantages by establishing unique brand power.

Factors Associated with Length of Stay in Elderly Inpatients in a General Hospital in Seoul (서울소재 3차 의료기관에 재원 중인 노인 환자의 재원기간에 영향을 미치는 관련 요인)

  • To identify characteristics of elderly inpatients who had long term hospitalization in a general hospital, this study categorized into two groups who were hospitalized for less than 30 days and a group hospitalized for more than 30 days. To compare the groups the independent variables were the sociodemographical characteristics, the medical care utility characteristics, and the disease characteristics and finally analyzed factors affecting the hospitalization period. The data of 18,727 inpatients who were older than 65 years of age by the year 2005 were used and the data were analyzed using SPSS for Windows 12.0. With the results, it is necessary to provide intensive and positive management to elderly inpatients who belong to the more than 30 days group and also necessary to share roles and functions of hospital by medical network with local hospitals and clinics in order to manage long-term elderly inpatients and offer continuous post-management to discharged patients by encouraging them to use a long-term care center or by implementing an early discharge program. This study should provide many studies on how to manage the period of hospitalization efficiently on long-stay elderly inpatients in the future.

Effects of community emergency medical resources on survival in out-of-hospital cardiac arrest (지역사회 응급의료 자원이 병원 밖 심장정지 환자의 생존에 미치는 영향)

  • Purpose: This paper is to determine whether automatic defibrillators (AEDs) deployed across communities make a contribution to prevent death in patients with acute cardiac arrest out-of-hospital. Methods: A total of 30,179 cases of cardiac arrest investigation data from the Korea Centers for Disease Control and Prevention was matched to those on emergency medical statistics drawn from annual report for the 2018 Central Emergency Medical Center, and statistics from the National Statistical Office in 2018. Results: Multiple logistic regression analyses revealed that availability of emergency medical resources across associated with different survival rates at emergency room after taking variability of the patient's personal characteristics and episodic situational characteristics held constant. The survival rate was 1.71 times higher for patients living in communities with more than 105 AEDs avaiable per 100,000 inhabitants than for those living in communities with less than 55 AEDs. Conclusion: The survival-related factors of patients with acute cardiac arrest that occurred out-of-hospital were found to be associated with patients' and episodic situational characteristics. The hospital stage were found to be associated with patients characteristics and episodic situational characteristics, The variability of AED available in a community has an impact on survival rate after emergency room treatment.

Characteristic Changes in First-Visit Patients with Peripheral Facial Palsy before and during COVID-19 Pandemic: Focused on a Korean Medicine Hospital

  • This study aimed to analyze whether the coronavirus disease 2019 (COVID-19) pandemic affected the characteristics of first-visit patients with peripheral facial palsy (PFP) and observe changes in their characteristics. This study analyzed the electronic medical records of 2,310 first-visit patients with PFP who visited the Facial Palsy Center, Kyung Hee University Korean Medicine Hospital from January 1, 2019, to December 31, 2021, in terms of demographic characteristics, disease phase, residence locations, hospital visit route, and patient care. During COVID-19, the proportion of acute patients increased by 5.3%, the proportion of visits by residents in Seoul increased by 3.8%, and the proportion of patients receiving only outpatient treatments increased by 12.8%. Significant relationships were present between the presence of the COVID-19 pandemic and the number of patients by disease phase (p = 0.043), residence locations (p = 0.003), and patient care (p = 0.003). Thus, several differences in the characteristics of first-visit patients with PFP visiting a Korean medicine hospital during the COVID-19 pandemic in terms of demographic characteristics, disease phase, residence locations, and patient care.

The Effect of Job Characteristics and Job Satisfaction on Organizational Commitment of Hospital Foodservice Employees in Busan Area (부산지역 병원급식 조리종사자의 직무특성 및 직무만족이 조직몰입에 미치는 영향)

  • Purpose: This study was conducted to provide basic data for human resources management of hospital foodservice employees by determining their job characteristics, job satisfaction, and organizational commitment according to recent hospital environment changes. Methods: Our survey was administered to 248 hospital foodservice employees in Busan area from September 1 to September 25, 2014. A total of 158 questionnaires were used for final analysis. Results: The mean score of job characteristics showed significantly difference by work experience (p<0.001), annual salary (p<0.01), and cooking certification (p<0.05). Job satisfaction had significantly difference by the age (p<0.05), annual salary (p<0.001), and turnover intention (p<0.001). Organizational commitment showed significantly (p<0.001) difference by age, work experience, and annual salary. There was positive correlation (p<0.001) between organizational commitment and job characteristics, job satisfaction. Organizational commitment had significantly positive correlation with skill variety (p<0.001), feedback (p<0.01), and task significance (p<0.001) of job characteristics, with work (p<0.001), pay (p<0.001), and co-workers (p<0.001) of job satisfaction. Job characteristics (${\beta}=0.249$, p<0.001) and job satisfaction (${\beta}=0.380$, p<0.001) had positive influences on the organizational commitment(p<0.001). In sub factors of job satisfaction, work (${\beta}=0.291$, p<0.001) and pay (${\beta}=0.252$, p<0.01) had positive influences on organizational commitment. Conclusion: To develop the organizational commitment, hospital managers need to reinforce responsibility and fulfillment by job enrichment and to consider increasing salaries to get a higher satisfaction from foodservice employees.

  • The objective of this study is to introduce the spatial characteristics and the better arrangement of waiting areas in general hospital on comprehensing the patient's behavior as patient call-system and spatial characteristics is characteristics is changed. That is for improvement in the circumstance of waiting area more important than before, and for the patient satisfaction and patient's affirmative judgement of hospital quality by offering the convenience. Through this study, the way of improvement for waiting area can be suggested such as the beeper system, the restructuring of program information, the multiple aim space without hospital program, the reform of the seat's arrayal, the waiting area with conveniency.
