Relationship between Hospital Case Mix and Costs and Incomes of Tehran Heart Center

  • Purpose - Clarifying one of the biggest public Hospital Costs and incomes according to patients' case mix. It leads to prepare financial information about pubic medical tariffs and hospital operational costs. Research design, data, and methodology - This study calculates the costs both, with and without taking into account capital costs. This holds for comparison of hoteling based on case mix in all medical procedures. The checklists were reviewed and filled by reviewing accounting documents of the hospital, warehouse exclusion list, and daily books of laundry and CSR. Data was analyzed descriptively by using Excel. Results - In both cases, the hospital is losing in terms of hoteling. Because the buildings and equipment are new, this loss is not tangible. However, this will be revealed when costs of reconstruction and replacement of equipment. The loss rate per day of hospitalization was 569318 Rials for Coronary Care Unit (CCU), 528171 Rials for Post Intensive Care Unit (Post ICU), 474570 Rials for ICU, 233183 Rials for Post CCU and 204803 for Surgical ward. Conclusions - Income of hoteling was lower than its costs. ANOVA showed a strong relationship between case mix and hospital costs as well as case mix and its income. This suggests that optimal case mix can minimize the costs and maximize income.

  • Healthcare design is always challenged to come up with the rapid changes in healthcare system and industry as well as complicated planning requirements. This thesis aims to grab good instructions from Louis I Kahn who designed more than 15 healthcare projects during his entire life, from the winning project of the student competition in 1920 to Ayub hospital in Dhaka, Bangladesh after his death. His many hospital projects are deeply related to his famous projects such as Yale Art gallery, Philips Exeter Library, and Sher-E-Bangla Nagar national capital of assembly hall. But, it is not easy to find the researches about his hospital projects among many books and theses about him. Thus, this thesis investigates them and their contextual and historical relationships in order to find the way to apply his knowledge and philosophy about healthcare design to the modern hospital projects. We expect that it will promote more following researches for his healthcare projects.

  • This paper presents a basic energy performance data of micro gas turbine, Renewable Energy(BIPV and Solar Collector System, geothermal system) and a hybrid energy system(geothermal system and microturbine) installed in Hospital Building. The efficiency of solar collector and BIPV system was 30%, 10% individually, and lower than micro gas turbines. Micro gas turbines are small gas turbines that bum gaseous and liquid fuels to produce a high-energy exhaust gas and to generate the electrical power. Recently, the size range for micro gas turbines is form 30 to 500kW and power-only generation or in combined heat and power(CHP) systems. Finally, in energy performance aspect, Micro gas turbine system and hybrid energy system were high-efficiency system in hospital building. Hybrid energy system also give us a powerful alternative energy system economically.

  • A measurement of indoor microorganism using Reuter Centrifugal Air Sampler(RCS) was undertaken during October 1991-February 1999 and 6-Stage Cascade Air Sampler was undertaken during May 2001-June 2001 in Seoul. The results were as follows : 1. The average of total microorganism collected on the agar strip GK-A media were, in the order, subway station, hospital, underground shopping center, department store, book store, theater, sports facility, educational institution, office building in sites. 2. The average of staphylcocci collected on the agar strip GK-S media, in the order, were subway station, underground shopping center, hospital and department store, department store, theater, office building, sports facility and educational institution in sites. 3.   • Energy demand characteristics of hotel, hospital, and office building are compared to provide guidelines for combining building in community energy system design. The annual, monthly, and daily energy demand patterns for electricity, heating, hot water and cooling are qualitatively compared and important features are delineated based on the energy demand models. Key statistical values such as the mean, the maximum are also provided. Important features of the hourly demand patterns are summarized for weekdays and weekends. Substantial variations in both magnitudes and patterns are observed among the 3 building types and smart grouping or combination of building type and size is essential for a successive energy supply.

  • Purpose: Numerous researches about healthcare buildings have been performed however, they were mainly focused on convenience or healing condition for medical treatments. The hospitals consume energy and generate $CO_2$ as twice as the residential or commercial buildings do(Lim, et al., 2010a:154). The public regional hospitals are especially in serious conditions. They are more than 20 years old in average. Energy efficiency and environmental friendliness in the public regional hospitals are far behind ones in private sectors. Even though the ministry of health & welfare is supporting renovation of the builidngs and enhancement of the facilities every year, it is not integrated including sustainibility. In this study, we investigates current conditions of the regional public hospital in envrionment-friendly standpoint, especially focused on Chungcheong Province area Hospitals. Methods: we investigates current conditions of the regional public hospital in envrionment-friendly standpoint, especially focused on Chungcheong Province area Hospitals. The study was executed by qualitative and quantitative evaluations with site inspection, drawing analysis and interviews. Results: Through this study, we found that the Chungnam regional public hospitals can be environmentally improved by management plans and programs. Implications: Based on this analysis, Korean Green Building Certification for healthcare facilities will be developed in near future.

Seismic Retrofitting Effects of General Hospital Using Self-Centering Energy Dissipative Bracing System (자기복구형 에너지소산 가새시스템을 적용한 종합병원의 내진보강효과)

  • 2016 Gyeongju and 2017 Pohang earthquakes led Koreans to acknowledge that the Korean peninsula is not an earthquake-free zone anymore. Among various buildings crucial to after-shock recovery, general hospital buildings, especially existing old ones, are very significant so seismic retrofitting of those must be an important issue. Self-centering energy dissipative(SCED) brace is one of retrofitting methods, which consists of tendon with restoring force and friction device capable of dissipating seismic energy. The strength of the SCED brace is that the tendon forces a structure to go back to the original position, which means residual drift can be negligible. The residual drift is a very important parameter to determine usableness of general hospitals after shock. To the contrary, buckling-restrained braces(BRB) are also a very effective way to retrofit because they can resist both compressive and tensile, but residual drift may exist when the steel core yields. On this background, the seismic retrofitting effect of general hospitals reinforced with SCED braces was investigated and compared to that of the BRD in this study. As a result, although the floor acceleration cannot be reduced, the story drift and residual drift, and the shear demand of walls significantly decreased. Consequently, seismic retrofitting by SCED braces are very effective for domestic low-rise general hospitals.

Analysis of The Management of Three Tertiary General Hospital(2011 to 2013)

  • Purpose. For more effective hospital management, it analyzes the trend through general characteristics, balance sheet, income statement, and financial ratio analysis, grasps the causes of the problems, and analyzes management of the hospital in order to use the result as baseline data for development of the hospital in the future. Methods. The collected data of 3 years from 2011 to 2013 about 3 tertiary hospitals in metropolitan cities from Alio (provider of public institution information; www.alio.go.kr), Health Insurance Review & Assessment Service (www.hira.or.kr), and the website of the Ministry of Health and Welfare (www.mw.go.kr) were analyzed and general characteristics, balance sheet, income statement, and financial ratio, analysis are used as data. Results & Conclusions. From the result of data analysis from 2011 to 2013, general characteristics, balance sheet, income statement, financial ratio analysis, and pie charts could lead to conclusions as follows. In the result of comprehensive analysis, the 3 tertiary hospitals showed increase of fixed expense due to extension of the buildings and so did the scale of fund and asset. Although medical revenue increased, the margin of increase for medical expense was greater than that of medical revenue, which consequently led to loss. In prediction for the 3 tertiary hospitals based on characteristics so far, it is expected to see improved revenue structure after building extension is completed, but it is necessary to exert management effort to maintain its optimal level by enhancement in stability of management and inventory turnover through management of inventories.

Analysis on the Impact of Draft Curtains on Hospital Building Fires

  • Draft curtains are an important tool to reduce the spread of fire smoke in buildings. This paper mainly studies the influence of draft curtains on smoke spread in a hospital outpatient building with a spacious hall. The outpatient building of a county hospital in China is taken as the research object, and a fire simulation model is established based on PyroSim software. Four scenarios are designed: no draft curtains in the hall, draft curtains with a length of 1.5m in the hall, draft curtains with a length of 2.0m in the hall, and draft curtains with a length of 2.5m in the hall. At the same time, considering the worst situation, only the main emergency exit is opened, while the remaining emergency exits and windows are all closed, and the fire-fighting facilities are not activated. The detection point is set at the open emergency exit, 1.8m above the ground. Through the simulation of four scenarios, the effect of draft curtains on smoke spread is discussed from four aspects: smoke layer height, temperature, visibility, and CO concentration. The results show that draft curtains have a significant impact on the spread of smoke in a hospital outpatient building with a spacious hall. The longer the draft curtains are, the better the smoke-blocking effect. It should be noted that in the actual design, the length of the draft curtains should be considered without affecting the evacuation of personnel.

Energy Performance Evaluation of Building Micro-grid System Including Micro-turbine in Hospital Buildings (마이크로터빈이 포함된 빌딩마이크로그리드시스템의 병원건물의 에너지성능평가)

  • Distributed generation(DG) of combined cooling, heat. and power(CCHP)has been gaining momentum in recent year as efficient, secure alternative for meeting increasing energy demands. This paper presents the energy performance of microturbine CCHP system equipped with an absorption chiller by modelling it in hospital building. The orders of study were as following. 1)The list and schedule of energy consumption equipment in hospital were examined such as heating and cooling machine, light etc. 2) Annual report of energy usage and monitoring data were examined as heating, cooling, DHW, lighting, etc. 3) The weather data in 2007 was used for simulation and was arranged by meteorological office data in Daejeon. 4) Reference simulation model was built by comparison of real energy consumption and simulation result by TRNSYS and ESP-r. The energy consumption pattern of building were analyzed by simulation model and energy reduction rate were calculated over the cogeneration. As a result of this study, power generation efficiency of turbine was about 30% after installing micro gas turbine and lighting energy as well as total electricity consumption can be reduced by 40%. If electricity energy and waste heat in turbine are used, 56% of heating energy and 67% of cooling energy can be reduced respectively, and total system efficiency can be increased up to 70%.
