Home health care system in Korea has been classified into three types of home care programs based on different laws and regulations; for example, home health care nursing(HHCN) is based on medical laws, visiting health care nursing (VHCN) is based on long-term health care insurance, and visiting health care(VHC) is based on the regional health care act. HHCN in Korea has taken on an important role under the mandate of the national health care system since 2000. VHCN will commence its role under the long term health care insurance system in 2008. The strengthening of VHC commanded health promotion and prevention for vulnerable families in the community in 2007. This is an important turning point for increasing quality management for home health care program; it suggests certain possibilities for building a foundation for further changes in the service delivery structure. Accordingly, the home health care policy makers in Korea have a major function and role that consists of developing an agenda and alternatives for policy making in a systematic manner and clearly presenting implementation strategies for elderly health care system.
Purpose: The study was conducted in order to identify factors influencing depression and quality of life in elderly customized home visiting health services. Methods: A total of 442 people participated as the subjects of this study. Data were collected during the period from June to September in 2010 and the measurement tool used for this study was the customized home visiting health service recording sheet recommended by the Ministry of Health. Data were analyzed using t-test, one-way ANOVA, Pearson correlation coefficients, and stepwise multiple regression with SPSS/WIN 17.0. Results: Depression was correlated negatively with quality of life. According to the results of this research, factors influencing depression in elderly customized home visiting health services were quality of life, customized home visiting health service period, and IADL. Factors influencing quality of life in elderly customized home visiting health services were depression, joint exercise capacity, age, connection to volunteerism, IADL and ADL. Conclusion: The results of this study can be leveraged as complementary information for the effective management of customized home visiting health service subjects. Moreover, the results can be used as a reference for future studies.
Objective : The purpose of this paper was to suggest a model of the home-visiting occupational therapy for the child with sensory integrative dysfunction through parent's satisfaction of volunteer activity to home-visiting occupational therapy. Method : A questionnaire survey were conducted to investigate the satisfaction on after of volunteer activity to home-visiting occupational therapy for the child with sensory integrative dysfunction. The 12 parents agreed on this service that students of department of occupational therapy visited at home 2 or 3times during the 16weeks. Results : Parents responded that satisfaction was seen orderly agreed(66%), very agreed (34%) in the convenient portion of service process, orderly agreed(50%), very agreed(42%), moderate agreed(8%) in the therapeutic technique and professionalism, very agreed(58%), agreed(42%) in the portion of general satisfaction. Correlation was not between satisfaction of convenient portion and related questions, but in the student attitude and the therapeutic technique, professionalism and related question was related to all question except a family treatment access method portion. Conclusions : It will be able to present the model of the home-visiting service in various occupational therapy field above the results and home-visiting occupational therapy service system must be introduced with the hygienic medical treatment delivery system through the deepening research.
Purpose: The purpose of this study was to explore and understand customized home visiting nurses' experiences in telephone visiting. Methods: Qualitative data were collected from several discussions of 4 focus groups consisting of 20 customized home visiting nurses. All interviews were recorded and transcribed according to thematic content analysis processes. Results: The five main themes on customized home visiting nurses' experiences of telephone visiting were 'greeting', 'counseling', 'using strategies', 'maintaining attitude for enhancing relations' and 'enduring difficulties'. The category of 'counseling' was composed of confirming, educating, supporting, adjusting, and indicating. And, the category of 'using strategies' included complimenting, scolding, persuading, compensating, and ignoring. Conclusion: Developing a standardized manual on telephone visiting is needed to guide an effective counseling. In order for a customized home visiting nurse to be a good telephone counselor, training of motivational interviewing is recommended.
Purpose: This study aimed to investigate the role of hospital-based home health nursing in community care by examining the institutional progress of hospital-based home health nursing and the current status of home health nursing in Korea. Methods: Korean research data, national statistical data, government press releases, and related laws were investigated to clarify the role of hospital-based home health nursing in community care. Results: Korean visiting medical care services, including hospital-based home health nursing, was not found to be sufficient nationwide. The supply of home health nursing did not increase due to the nature of the visiting services that required transportation time, poor profitability due to insufficient insurance fees, and increase in acute beds. Conclusion: The nature of the Korean medical environment and visiting medical care makes it challenging to establish a visiting medical supply system for community care. Therefore, hospital-based home health nursing is an important infrastructure for visiting medical care, and will be a valuable resource to link discharged patients returning to the community when moving health care services. Hence, laws and institutional supplementation to expand the role of home health nursing agencies nationwide are needed along with addressing the limitations in the supply of home health nurses.
Purpose: The study was aimed at qualitatively enhancing and promoting a home visiting nursing program established in Korea on July 1, 2008, as part of the Long-Term Care Insurance for the Elderly program. Methods: Structural, procedural and consequential aspects of home visiting nursing care wereclassified on the horizontal axis by applying the standard notions for the evaluation of medical care (Donabedian, 1998). At the same time, the home visiting nursing care service support system and the service provision system weredivided on the vertical axis with reference to the accreditation standards for home visiting nursing care organizations suggested by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO, 2008). The data were collected from June 4, 2008 to October 27, 2008, and were analyzed using SPSS ver. 15.0. Results: Twenty-two (proposed) standards, centered on the standard elements under the conceptual framework of the study, were developed, and comprised structural aspects (n=10), procedural aspects (n=6) and consequential aspects (n=6). Those criteria and indicators underwent two content validity surveys among groups of home visiting nursing care research and training experts. The research produced 22 proposed standards, 50 proposed criteria and 166 proposed indicators. Conclusion: The home visiting nursing care standards developed pursuant to the Long-Term Care Insurance for the Elderly Act and the applicability of these standards need to be verified by home visiting nurses. These proposed standards should prove useful in developing an assessment tool to encourage the qualitative enhancement of visiting nursing care in Korea.

Objectives: The study was investigated to assess efficacy for home visiting treatment using Traditional Korean Medicine (TKM). Methods: The traditional medical doctor of Um-Sung health center has treated the 109 patients and they also answered the questionnaires of home visiting treatment using TKM before and after. The questionnaires were composed of flexibility, pain, health behavior, perception of health, satisfaction, cognition of TKM, etc. The questionnaires were analyzed by use of paired t-test and chi-square test. Results: They have significantly improved the flexibility (p<.001), pain (p<.001), health behavior (p<.001), and the perception (p<.001) of health after home visiting treatment. It was also brought to improve about the perception of TKM, and showed a good satisfaction and the improvement of health. Conclusion: Home visiting treatment using TKM is helpful to improve the health based on these results. We need to systemize the research for the activation of home visiting treatment using TKM.
Purpose: This descriptive study examined the management status of the home health care visiting vehicles and the nurse bags, by the home health care center of hospitals (at the hospital level or higher) in Korea, and identified the relevant factors. Methods: Of 120 managers or home health care nurses from medical institutions at hospital level or higher that provide home nursing, 93 individuals participated in the study in July 2021. Results: Hospitals that followed standard guidelines were more likely to perform internal disinfection of home health care visiting vehicles, and distinguished between clean and contaminated areas inside the visiting vehicles. Further, hospitals that followed standard guidelines were more likely to use more barrier surfaces to protect the surfaces of nurse bags to prevent infection. In addition, hospitals supporting the washing cost of the interior of home health care visiting vehicles were more likely to conduct the washing, and hospitals supporting nurse bags were more likely to use barrier surfaces to protect the bags' surfaces. Conclusion: This study only investigated home health care centers at hospital level or higher. Therefore, to generalize the results of the study, it is necessary to conduct a qualitative study involving additional investigation of home health care visiting vehicles and nurse bags and interviews with nurses from all domestic home health care centers.
Purpose: This study was aimed to investigate predictors of job satisfaction of home visiting nurses at the 16 public health centers in Busan. Method: There are two groups of independent factors: non-work related (age. educational level. working duration as nurses in hospitals. certificate). and work related (working duration as nurses in public health centers. working duration as home visiting nurse. position. number of households visited per week, workload, cooperation among staff, support by supervisors, supply of vehicles, supply of materials). The participants were 88 home visiting nurses from 16 public health centers in Busan. Data were collected with self-administrated questionnaires and analysed using an univariate logistic regression and multiple logistic regression analysis. Result: support by supervisors (good vs not-good, OR=3.70. p=0.025), and supply of materials (good vs not-good, OR=3.33, p=0.038) had significants effects on job satisfaction. Conclusion: The results were similar to those of other studies on the predictors of job satisfaction of clinical nurse at hospitals, and were helpful in developing nursing interventions to increase job satisfaction among home visiting nurses in public health centers. Purpose: The aim of this study was to establish basic data for developing education program of tailor made visiting home health care. Method: For this study, the subject was recruited in one directorial area among 14 nation-wide areas. The data collection was done using self-report questionnaire developed by Korea Health Industry Development Institute. Ninety one questionnaires were analyzed and response rare was 88.3%. Result: The major results were as follows. Aspect of education operations, the satisfaction of education space was revealed high. However, the satisfaction of education time, hour, and schedule were low. The overall satisfaction of education contents were revealed high. The most useful curriculums were found Chronic disease management, Infant & toddler health management, and Case management of tailor made visiting home health care. However the most unuseful curriculums were Nutritional management according to subject's health problem, Health promotion for poor family, and Understanding of visiting service in community health center. Conclusion: With these results, it was identified that the most needed contents of tailor made visiting home health care education program. So these results will be used to develop the more effective education program to activate tailor made visiting home health care service in community health center.
