A Study on the trabecular change of Femur according to $17{\beta}-Estradiol$ Dosage in Ovariectomized Rat (난소 절제된 백서에서 에스트로젠 투여용량에 따른 대퇴골주 변화에 대한 연구)

  • Osteoporosis is the consequence of an imbalance between osteoclastic and osteoblastic activity, coupled with an increased rate of bone turnover observed with menopause. Estrogen is generally considered to maintain bone mass through suppression of bone resorption. The purpose of this study was to evaluate the rat femoral trabecular change not only in the deficiency of estrogen but also in the administration of estrogen following ovariectomy(OVX). 30 female Sprague-Dawley rats were subjected to bilateral OVX or sham surgery(control). Groups of OVX were divided into 4 groups. The first group was injected daily with vehicle alone for 20 days after 20 weeks following OVX. The additional groups of OVX was injected daily with low, medium, or high doses of $17{\beta}-estradiol$(10, 25 or $50{\mu}g/kg$ BW, respectively). All rats were sacrified 23 weeks after OVX, and their femur were processed for H&E, MT stain and histomorphometry. The results were as follows; 1. In the histomorphometric analysis, the trabecular bone volume/tissue volume, trabecular thickness and trabecular seperation were respectively $31.2{\pm}8.3%$, $54.3{\pm}4.8{\mu}m$ and $280.7{\pm}16.4{\mu}m$ in vehicle treated OVX group and $48.6{\pm}7.3%$, $90.4{\pm}4.5{\mu}m$ and $126.3{\pm}5{\mu}m$ in sham operation group, and they showed statistical significance compare to control group. 2. The trabecular bone volume/tissue volume, trabecular thickness and trabecular separation were respectively $44.4{\pm}4.3%$, $109.5{\pm}12.3{\mu}m$ and $94.9{\pm}8.5{\mu}m$ in low doses of $17{\beta}-estradiol$ injected group and they showed statistical significance compare to OVX group. 3. The trabecular bone volume/tissue volume, trabecular thickness and trabecular separation were respectively $44.4{\pm}4.3%$, $109.5{\pm}12.3{\mu}m$ and $94.9{\pm}8.5{\mu}m$ in medium doses of $17{\beta}-estradiol$ injected group and they showed statistical significance compare to OVX group, but they didn't show statistical significance compare to low doses of $17{\beta}-estradiol$ injected group. 4. The trabecular bone volume/tissue volume, trabecular thickness and trabecular separation were respectively $46.4{\pm}4.5%$, $154.4{\pm}13.2{\mu}m$ and $113.7{\pm}12.8{\mu}m$ in high doses of $17{\beta}-estradiol$ injected group and they also showed statistical significance compare to OVX group, but they didn't show statistical significance compare to other experimental groups. From the above results, metaphyseal bone formation was markedly reduced in OVX rate but treatment of OVX rats with $17{\beta}-estradiol$ resulted in normalization of femur trabecular bone volume. But they didn't show statistical significance the effect of bone formation according to the dose dependency.

A STUDY ON THE HEAT GENERATION OF BONE DRILLING BURS ACCORDING TO IRRIGATION DURING IMPLANTATION OF DENIAL IMPLANT (치과 임플랜트 식립시 관주에 따른 골 천공 기구의 열 발생에 관한 연구)

  • There are a lot of elements affecting the success of implant prosthesis. The quality of surgical procedure is considered as one of the key factors. To avoid the excess heat generation is an important element of successful osseointegration and it can be achieved by using a gentle surgical technique with a sharp instrument in bone drilling. This study was performed to measure and analyze comparatively the heat transmitted to sur-rounding bone at a distance of 0.5mm from the periphery of the drill hole in each drilling stage. The results were as follows. In standard system, the temperature of surrounding bone tissue ranged from $29.2^{\circ}C\;to\;48.3^{\circ}C$ with irrigation and from $34.6^{\circ}C\;to\;84.3^{\circ}C$ without irrigation. And in wide system, the temperature of surrounding bone tissue ranged from $29.5^{\circ}C\;to\;52.5^{\circ}C$ with irrigation and from $34.8^{\circ}C\;to\;87.8^{\circ}C$ without irrigation. And the temperature ranges exceeded the threshold without irrigation, while showing less than the threshold by the cooling effect of irrigation. In comparing standard system with wide system, although there was no significant difference, ${\phi}4.3mm$ pilot and ${\phi}4.3mm$ twist drill of wide system showed high value and wide system showed slightly high elevation of temperature in all depth in fixture installation. In the finite element analysis, the calculated value by the Fourier's cooling law were applied to the bone drilling surface. And through analysis using different irrigation temperatures at $28^{\circ}C,\;15^{\circ}C\;and\;5^{\circ}C$, and according to the time. The result was that the cooling water at least below $15^{\circ}C$ was required to maintain the temperature of surrounding bone less than threshold in bone drilling, the cooling water below $5^{\circ}C$ was required to gain more sufficient cooling effect, and cooling over 5 seconds was needed after bone drilling for sufficient effect.

  • Oral implants must fulfill certain criteria arising from special demands of function, which include biocompatibility, adequate mechanical strength, optimum soft and hard tissue integration, and transmission of functional forces to bone within physiological limits. And one of the critical elements influencing the long-term uncompromise functioning of oral implants is load distribution at the implant- bone interface, Factors that affect the load transfer at the bone-implant interface include the type of loading, material properties of the implant and prosthesis, implant geometry, surface structure, quality and quantity of the surrounding bone, and nature of the bone-implant interface. To understand the biomechanical behavior of dental implants, validation of stress and strain measurements is required. The finite element analysis (FEA) has been applied to the dental implant field to predict stress distribution patterns in the implant-bone interface by comparison of various implant designs. This method offers the advantage of solving complex structural problems by dividing them into smaller and simpler interrelated sections by using mathematical techniques. The purpose of this study was to evaluate the stresses induced around the implants in bone using FEA, A 3D FEA computer software (SOLIDWORKS 2004, DASSO SYSTEM, France) was used for the analysis of clinical simulations. Two types (external and internal) of implants of 4.1 mm diameter, 12.0 mm length were buried in 4 types of bone modeled. Vertical and oblique forces of lOON were applied on the center of the abutment, and the values of von Mises equivalent stress at the implant-bone interface were computed. The results showed that von Mises stresses at the marginal. bone were higher under oblique load than under vertical load, and the stresses were higher at the lingual marginal bone than at the buccal marginal bone under oblique load. Under vertical and oblique load, the stress in type I, II, III bone was found to be the highest at the marginal bone and the lowest at the bone around apical portions of implant. Higher stresses occurred at the top of the crestal region and lower stresses occurred near the tip of the implant with greater thickness of the cortical shell while high stresses surrounded the fixture apex for type N. The stresses in the crestal region were higher in Model 2 than in Model 1, the stresses near the tip of the implant were higher in Model 1 than Model 2, and Model 2 showed more effective stress distribution than Model.

The Change of Bone Mineral Density by Bisphosphonates Therapy with Calcium-Antagonists in Osteoporosis

  • Imbalance in calcium and phosphorous metabolism due to aging or menopause leads to osteoporosis. In contrast to patients with normal blood pressure, hypertensive patients have a higher loss of calcium in the urine with its attendant risk of osteoporosis. The high blood pressure is associated with the risk of bone loss and abnormalities in calcium metabolism leading to calcium loss. So we retrospectively investigated the changes of bone mineral density (BMD) which drugs can have clinical influences over osteoporosis treatments of patients with calcium-antagonists as common antihypertensive drugs and with bisphosphonates which causes a most effective inhibition of osteoclasts resorption. As a result over 70 years of age group and within bisphosphonates group, alendronate 70 mg once-weekly group showed significant increase of BMD in lumbar area. Combination group of cilnidipine and $maxmarvil^{(R)}$ showed very significant decrease of BMD. In conclusion, it is desirable that combination therapy with calcium-antagonists is used carefully in the treatment of osteoporosis with high blood pressure.

Effect of Bluefin Tuna Bone on Calcium Metabolism of the Rat (참다랑어 골분이 흰쥐의 칼슘대사에 미치는 영향)

  • This study was conducted to examine the effect of bluefin tuna bone on the bone metabolism of the rats. Weaned 6-week old male rats were fed low-calcium diets for 2 weeks after the adjustment period. Rats were divided into 6 groups and were fed experimental diets for six weeks. Experimental groups were \circled1 Normal calcium: CC (0.5% CaCO$_3$; control) \circled2 TB (bluefin tuna bone powder) \circled3 CT (citrated bluefin tuna bone powder) \circled4 BB (bovine bone powder) \circled5 CL (calcium lactate) \circled6 Low calcium LC (0.15% CaCO$_3$). Low-calcium diet group (LC) showed the lowest calcium retention. There was no differences in calcium excretion in stool and calcium absorption among various calcium sources. Serum calcitonin levels were high in TB, CT and BB group compared to those in CC, CL LC group. Parathyroid hormone and osteocalcin levels showed no differences among experimental groups. Deoxypyridinoline (DPD) levels were significantly higher in LC group than in other groups. Wet weight of the femur were significantly high in TB and CT group, and dry weight of femur showed no differences among normal calcium groups. Bone density of femur in LC group was significantly lower than those of normal calcium feeding group, and TB group showed highest bone density among experimental groups. There was no differences in bone metabolism among various calcium sources. Therefore, it is pointed out that the amount of calcium intake is very important because there was significant differences between normal calcium diet and low calcium diet. According to the results of femur weight, ash, calcium and bone density, it is suggested that bluefin tuna bone have alternative effects to bovine bone powder on the maintenance of bone health.

The Role of Bone Scans in Routine Preoperative Evaluations of Non-Small Cell Lung Cancer Patients. (비소세포 폐암의 병기에 있어 통상적인 골 스캔의 역할)

  • The objective of this study was to assess the usefulness of bone scans in routine preoperative examinations of patients with newly diagnosed non-small cell lung carcinoma. Material and Method: We reviewed the medical records of 258 patients who were newly diagnosed with non-small cell lung cancer in our hospital between January 2000 and December 2000. More than half of the patients (132) were deemed to be inoperable due to their advanced stage based on the CT scans. The remaining 126 patients were considered potentially operable. For these patients, clinical evaluation including the presence of bone pain, serum alkaline phosphatase, and calcium levels was used as clinical predictors of bone metastasis. All patients received bone scans. Bone X-rays, MRI or bone biopsy were performed to confirm the presence of bone metastasis. The usefulness of the bone scan was evaluated by comparing its power of predicting bone metastasis to that of the clinical information. Result: In all patients, the positive and negative predictive values of bone scans for the bone metastasis were 44%, and 99%, respectively. Those of the clinical information were 38% , and 94%. However, in potentially operable patients, the negative predictive value of the clinical information was as high as 99%. Conclusion: If newly diagnosed non-small cell lung cancer patients are presented as potentially operable on the basis of CT scan with no clinical evidence of distant metastases, curative resection could be considered without performing routine bone scans because of the low probability of bone metastasis. However, if there are positive clinical findings, further evaluations, including bone scan should be followed as metastasis will be documented in more than 30% of patients.

The Effect of Porcine Cancellous Bone for Bone Regeneration and Application of Platelet-rich Fibrin in Rabbit Calvarial Defect (가토 두개골 결손부에 이식된 저골(猪骨)과 혈소판 풍부 섬유소의 골형성 효과)

  • Purpose: The purpose of this study was to evaluate the effectiveness of the platelet-rich fibrin (PRF) used in combination with the porcine cancellous bone as a scaffold, in promoting bone regeneration in the bone defects ofthe rabbit calvaria. Methods: Ten rabbits were used in the study. Three round-shaped defects (diameter 8.0 mm) were created in the rabbit calvaria and were filled with nothing (control group), porcine cancellousbone (Experimental Group 1, porcine bone) and PRF-mixed porcine cancellous bone (Experimental Group 2). TS-GBB is a xenogenic bone-substitute product comprised of a high heat-treated mineralized porcine cancellous bone. Animals were sacrificed at 6 weeks and 12 weeks for the histological and radiographic evaluations. Results: In the micro computed tomography and histological results, the experimental groups 1 and 2 showed more bone formation, remodeling, and calcification than the control group. The new bone formation ratio showed theGroup 2 to be larger than Group 1 at6 and 12 weeks. However, there was no significant difference between the experimental groups 1 and 2 in the new bone formation area, at the 6 and 12 weeks (P>0.05). Conclusion: The PRF-mixed group showed more bone formation than the porcine cancellousbonegroup (TS-GBB), butthere was a no significant difference. The PRF may not lead to enhanced bone healing when grafted with the porcine cancellous bone.

Circulating Tumor Cells are Associated with Bone Metastasis of Lung Cancer

  • Lung cancer (LC) is the leading cause of cancer mortality worldwide, predominantly due to the difficulty of early diagnosis and its high metastatic potential. Recently, increasing evidence suggests that circulating tumour cells (CTCs) are responsible for cancer metastatic relapse, and CTCs have attracted interest in cancer metastasis detection and quantification. In present study, we collected blood samples from 67 patients with bone metastasis, and 30 patients without such metastasis, and searched for CTCs. Then the association of CTC numbers with bone metastasis and other clinico-pothological variants was analyzed. Results demonstrated that when 5 or 1 was taken as a threshhold for the CTC number, there were significantly higher positivity of CTCs in the bone metastasis group than in the non-metastasis group. While the increase in CTC number was not significantly associated with any other clinicopathological factor, including age, gender, pathological type, intrapulmonary metastasis and lymph node metastasis, the CTC number in patients with positivity of the last above mentioned variants was obviously higher than in patients with negativity of the two variants. Taken together, the CTC number appears to be significantly associated with the bone metastasis from lung cancer.

  • A torn anterior cruciate ligament(ACL) is the most common serious ligamentous injury to the knee joint. The incidence of ACL tears seems to be increasing, at least partly as a result of the increasing participation of individuals of all ages in high-risk sports. The most commonly used graft source for ACL reconstruction is the autogenous bone-patellar tendon-bone graft unit. Despite a good success record. postoperative complications, such as infection, patellar contracture/patellar baja, patellar fracture, rupture of the patellar tendon, graft failure without reinjury, can occur following ACL surgery. The purpose of this paper is to provide guidelines regarding the key points of the reconstructive procedure in a sequence and how to prevent or minimize the complications that can follow ACL reconstructive surgery. We want this knowledge can help orthopaedic surgeons to understand the reasons for previous and current successes and failures of reconstruction of the ACL, and it can help them to plan the care of patients who have an injury of the ligament.

Evaluation of the Effectiveness of the Shielding Device and the Organ Dose of Subject During Bone Mineral Density (골밀도검사에서 피검자의 장기선량 측정 및 차폐기구의 효용성 평가)

  • Bone mineral density is a examination to measure the amount of bone in patients with metabolic bone disease. It is a low dose, but may cause unnecessary exposure to the gonads and other organs located in the periphery when examining the lumbar and proximal femurs. Therefore, the purpose of this study was to evaluated the exposure dose for each organ exposed during the bone mineral density through simulation, and analyzed the applicability of the subject to radiation shielding devices using 3D printing materials. As a result, the highest dose was shown at 11.47 uSv in the breast during lumbar examination and 8.98 uSv in the testis during proximal femur examination. Also, the farther away from the examination site, the lower the effect of the scattering-ray. The shielding effect of using 3D printing shielding device showed high results in proportion to the effective atomic number and specific gravity of the printing material. Among the printing materials, ABS + W showed an effect of at least 78.72 to 96.3 9% compared to the existing lead material.
