Time relationship between the change of blood glucose concentration and the change of hemoglobin A1 concentration in experimentally induced diabetic dogs (실험적으로 당뇨병을 유발시킨 개에 있어서 혈당량과 Hemoglobin A1 농도의 변화의 시간적인 상관관계)

  • Fifteen mongrel dogs (14 male and 1 female) were injected intravenously with 30mg of streptozotocin and 50mg of alloxan monohydrate per kilogram of body weight to induce diabetes mellitus. Before treatment with streptozotocin and alloxan fasting serum glucose concentration was determined every other day or thrice a week (Monday, Wednesday, Friday) for 3 months. Among 15 dogs 4 dogs developed diabetes mellitus and survived more than 9 weeks without injection of insulin. After treatment fasting serum glucose and hemoglobin $A_1$ concentrations of the 4 dogs were determined every other day or thrice a week. Fasting serum glucose concentration increased acutely from 24 hours after treatment and then showed severe fluctuation. Hemoglobin $A_1$ concentration increased gradually until 7~9 weeks after treatment and then showed very slow increase afterwards. Correlation of hemoglobin $A_1$ to fasting glucose concentration was relatively weak(r = 0.10~0.80). Hemoglobin $A_1$ and fasting glucose concentration of preceding 7 week showed very high correlation (r = 0.98~0.99). It was indicated that hemoglobin $A_1$ concentration in chemically induced diabetic dogs reflects mean glucose concentration of preceding 7~9 weeks.

Pharmacokinetics of PEG-Hemoglobin SB1, a Hemoglobin-Based Oxygen Carrier, after Its Intravenous Administration in Beagle Dogs

  • The purpose of the present study was to investigate the pharmacokinetics of PEG-hemoglobin SB 1, a modified bovine hemoglobin with polyethylene glycol, after its single and multiple administration in beagle dogs. For this purpose, the analytical method of free hemoglobin in the plasma was developed and validated. Excellent linearity ($r^2$=0.999) was observed in the calibration curve data, with the limit of quantification of 0.005 g/dL. The precision and the deviation of the theoretical values for accuracy were always within $\pm$15% in both the between-and the within-day results. The method was tested by measuring the plasma concentrations following intravenous administration to beagle dogs and was shown to be suitable for pharmacokinetic studies. In a single dose study, the plasma half-life (t$_{1}$2/) increased and the total body clearance (Cl$_{t}$) decreased with the dose (i.e., 0.017 to 0.75 gHb/kg as PEG-hemoglobin SB1) in both sexes. The volume of distribution at steady-state (Vd$_{ss}$ ) showed no difference with the dose. In contrast, the values of t$_{1}$2/, CL$_{t}$ and the area under the plasma concentration-time curve (AUC) after the multiple dose were significantly different from those of the single dose administration. The values of t$_{1}$2/ in the multiple administration were about two times higher-than that of the single dose. As a result, t$_{1}$2/ of hemoglobin after the administration of PEG-hemoglobin SB1 was about 15-30 h, indicating the PEG modification of the hemoglobin lead to a prolongation of plasma concentration of the protein. Therefore, these observations suggested that the PEG modification of hemoglobin is potentially applicable in the hemoglobin-based therapeutics.tics.

  • Two males and one female in the group of 5020 Korean school children and university students in Seoul and Taejon were found to have a show hemoglobin to normal hemoglobin A. In all three subjects the show component migrated at a rate characterstic of the G hemoglobin. By urea-starch-gel electrophoresis in alkaline pH and Chernoff method ws demonstrated that another 3 cases of abnormal hemoglobin also were beta-chain variants. This was reconfirmed by hybridization experiment with canine hemoglobin. And the results of family test of 3 case of abnormal hemoglobin were heterozygous carrier.

Relationship between the Self-Reported Oral Health Status and Hemoglobin A1c Level among Diabetic Patients (일부 당뇨병 환자에서 당화혈색소 수치와 주관적 구강건강상태와의 연관성)

  • Objective: This study was to investigate the relationship between hemoglobin A1c level and self-reported oral health status. Methods: The subjects of this study were 150 diabetic patients (60 male patients and 90 female patients) who went to the internal and family medicine departments of clinic. This study used the questionnaire and NYCOCARD$\square$ READERII to quantify hemoglobin A1c. Results: The proportion of well controlled diabetes subjects was 31.3%. When hemoglobin A1c levels became higher, subjects perceived that there were more decay teeth, bleeding, swollen gums, mobility teeth, and oral disease symptoms. In comparison with the well glycemic control group, poor glycemic control group perceived that the overall oral health status was worse, and had more bleeding teeth, swollen gums and mobility teeth. Conclusion: These results show that hemoglobin A1c level is related to the self-reported oral health status. Therefore, dental professionals should emphasize more the necessity of maintaining the hemoglobin A1c of normal range and monitoring it periodically, and the practice of thorough oral hygiene care in order to promote diabetic patients' oral health.

  • Electrochemical behaviors of bovine hemoglobin (Hb) at an acetylene black paste electrode based on the enhancement effect of sodium dodecyl sulfate (SDS) were investigated. In the optimal conditions, a very weak reduction peak was observed at an acetylene black paste electrode for hemoglobin in the absence of SDS. However, the reduction peak current increased remarkably after the addition of 4.0 × 10?4 mol L?1 SDS, suggesting that SDS exhibits obvious enhancement effect to the determination of hemoglobin. All the experimental parameters, such as pH value, concentration of SDS, accumulation time and accumulation potential were optimized for hemoglobin analysis. The proposed method possesses high sensitivity (detection limit is 3.0 × 10?9 mol L?1), wide linearity (6.0 × 10?9 to 6.0 × 10?7 mol L?1), rapid response and low cost. Finally, the method was successfully employed to determine hemoglobin in a spiked sample.

  • The effect of protease (subtilisin Carlsberg) on the removal of hemoglobin as protein soil was studied. The relation between the renloval and the hydrolysis of hemoglobin by subtilisin Carlsberg was discussed. The soiled babric was prepared by spotting of hemoglobin solution evenly on the cotton fabric and was denatured by steaming. The soiled fabric was washed by using Terg-0-Tometer at various conditions. The removal efficiency was evaluated by analysis of protein on the fabrics before and after washing by means of copper-Folin method. 1. The removal of hemoglobin was increased in proportion to increasing of the enzyme concentration up to a certain point, but it began to decrease above the point. 2. The hemoglobin was removed effectively by adding of subtilisin Carlsberg, and more effectively removed by adding of AOS in the enzyme solution. 3. The removal of hemoglobin deviated from the first order reaction in detergency. 4. The renloval of hemoglobin was highest at $50^{\circ}C$ in detergency, Even at low temperature the removal efficiency of enzyme was relatively higher compared with the hydrolysis of hemoglobin by the enzyme. However the removal of hemoglobin was apparently decreased with the increase of temperature over $60^{\circ}C$. 5. The removal of hemoglobin was relatively high at pH 7.0~8.0 and increased continuously with the increase of pH in detergency 6. In detergency, the removal mechanism of hemoglobin by subtilisin Carlsberg could be explained as follows: Fisrt of all, the enzyme hydrolyzed hemoglobin substrates partially by forming E-S complex at the surface of hemoglobin on the cotton fiber, and decomposed cooperative binding of hemoglobin. Subsequently, the fragments of hemoglobin were easily removed by washing. According as the enzyme penetrated to inner part of hemoglobin gradually, the hemoglobin on the cotton fiber was effectively removed by the repetition of these process. The removal of hemoglobin was more effectively increased by adding both the enzyme and AOS in the washing solution. Therefore, it was regarded that AOS molecules were adsorbed at the hydrophobic surface of denatured hemoglobin, subsequently, decomposed more effectively cooperative binding of hemoglobin, and the fragments of hemoglobin were removed more efficiently by means of the interfacial reaction of AOS.

  • The Effect of protease (subtilisin Carlsberg) on the removal of hemoglobin as protein soil was studied. The hydrolysis characteristics of subtilisin Carlsberg was examined by electrophoretic techniques. The fragmentation patterns of hemoglobin were analyzed by SDS-PAGE. The hydrolysis efficiency was evaluated by analysis of protein bands shown on gels before and after hydrolysis by using densitometer. 1. The hydrolysis of hemoglobin by subtilisin Carlsberg was increased markedly with the increase of the enzyme concentration. 2. The hydrolysis of hemoglobin by subtilisin Carlsberg was effectively increased in proportion to increasing of the hemoglobin concentration up to a certain point, but it began to decrease above the point. 3. The hydrolysis of hemoglobin by subtilisin Carlsberg followed the first order kinetics, yielding a rate constant of $4.05\time10^{-4}S^{-1}s$. 4. The hydrolysis of hemoglobin by subtilisin Carlsberg was highest at $50^{\circ}C$ and was decreased markedly at $80^{\circ}C$. 5. The hydrolysis of hemoglobin was comparatively low at pH 7.0~8.0, and highest at pH 11.0.

Novel Detection Technology for Glycated Hemoglobin using Gold Nanoparticles (금 나노입자를 이용한 새로운 당화혈색소의 검출 기술)

  • We report a novel detection technology for glycated hemoglobin (HbA1c) that is measured primarily to identify the three-month average plasma glucose concentration. In enzymatic measuring of glycated hemoglobin, the generated hydrogen peroxide was then used as a reducing agent of gold (III) for the synthesis of gold (0). Gold nanoparticles obtained from this novel approach were measured by optical and piezoelectric methods. In optical method, we have developed polymer based film-type sensor cartridge filled with all the reagents for glycated hemoglobin analysis and the cartridge worked very well having the detection limit of 0.53% of glycated hemoglobin. On the other hand, quartz crystal microbalance (QCM) sensors also have been developed to determine the abilities of surface modified QCM sensors at various levels of the concentration of glycated hemoglobin to bind gold nanoparticles and limit of detection was 0.90%. Finally, despite of relatively lower sensitivities of QCM sensor and film-type optical sensor than well-plate based optical detection, these two sensors were available to measure the glycated hemoglobin level for diabetes patients and normal person.

Prognostic Significance of CYFRA21-1, CEA and Hemoglobin in Patients with Esophageal Squamous Cancer Undergoing Concurrent Chemoradiotherapy

  • Purpose: To evaluate the prognostic value of serum CYFRA21-1, CEA and hemoglobin levels regarding long-term survival of patients with esophageal squamous cell carcinoma (ESCC) treated with concurrent chemoradiotherapy (CRT). Methods: Age, gender, Karnofsky Performance Status (KPS), tumor location, tumor length, T stage, N stage and serum hemoglobin, and CYFRA21-1 and CEA levels before concurrent CRT were retrospectively investigated and related to outcome in 113 patients receiving 5-fluorouracil and cisplatin combined with radiotherapy for ESCC. The Kaplan-Meier method was used to analyze prognosis, the log-rank to compare groups, the Cox proportional hazards model for multivariate analysis, and ROC curve analysis for assessment of predictive performance of biologic markers. Results: The median survival time was 20.1 months and the 1-, 2-, 3-, 5- year overall survival rates were 66.4%, 43.4%, 31.9% and 15.0%, respectively. Univariate analysis showed that factors associated with prognosis were KPS, tumor length, T-stage, N-stage, hemoglobin, CYFRA21-1 and CEA level. Multivariate analysis showed T-stage, N-stage, hemoglobin, CYFRA21-1 and CEA level were independent predictors of prognosis. By ROC curve, CYFRA21-1 and hemoglobin showed better predictive performance for OS than CEA (AUC= 0.791, 0.704, 0.545; P=0.000, 0.000, 0.409). Conclusions: Of all clinicopathological and molecular factors, T stage, N stage, hemoglobin, CYFRA21-1 and CEA level were independent predictors of prognosis for patients with ESCC treated with concurrent CRT. Among biomarkers, CYFRA21-1 and hemoglobin may have a better predictive potential than CEA for long-term outcomes.

Factors Associated with Hemoglobin A1c among Patient Aged 40 years over with Diabetes Mellitus: 2012 Korea Health and Nutrition Examination Survey (40대 이상 당뇨환자의 당화혈색소 조절 상태에 영향을 미치는 요인: 2012 국민건강영양조사결과를 바탕으로)

  • Purposes: This study was done to identify factors associated with uncontrolled hemoglobin A1c among patient with diabetes mellitus. Method: The sample was 412 Koreans with diagnosed diabetes aged 40 years or older who participated in the Fifth Korea Health and Nutrition Examination survey, which was a nationwide, cross-sectional survey. Data was analyzed using logistic regression. Uncontrolled hemoglobin A1c rate was 55%. Results: There was a difference between controlled and uncontrolled group in hemoglobin A1c as follows: duration of diabetes, BMI(Body Mass Index), hypertriglyceridemia, vitamin D, sleep duration. After adjusting for confounding factor, longer duration of diabetes (>7 years vs. ${\leq}7$)(Adjusted OR=2.277, 95% CI [1.277-4.060]), presence of hypertriglyceridemia (Adjusted OR=4.019, 95% CI [1.871-8.634]), lower vitamin D level (<20ng/mL vs. ${\geq}20$)(Adjusted OR=2.487, 95% CI[1.411-4.381]), longer sleep duration (6-8 hours vs >8 hours)(Adjusted OR=6.831, 95% CI [1.877-24.855]) were significantly associated with increased odds of uncontrolled hemoglobin A1c. Conclusions: Results show that duration of diabetes, hypertriglyceridemia, vitamin D and sleep duration are significantly related to hemoglobin A1c. Therefore, considering these factors it would be helpful to develop strategies to improve blood glucose control in patients with diabetes.
