A Study on Iron and Protein Nutriture of Preschool and School Children in a Korean Rural Area (일부(一部) 농촌지역(農村地域) 성장기(成長期) 아동(兒童)의 단백질(蛋白質) 및 철분(鐵分) 영양상태(營養狀態)에 관(關)한 연구(硏究))

  • A general survey of 82 perschool children and 473 school children (from 1st to 3rd grade) residing in the Dongnae area of Shindong Myun, choonseoung Gun, Kangwon Province was conducted from June 12 to 21, 1973, to obtain information concerning nutritional status, especially the protein and iron nutrition of preschool and school children in a rural area of Korea. A sample of 58 children 2 to 9 years old was selected from these 555 children for hematological, biochemical and parasitological tests. The results of these tests are presented below, 1) Both the mean body height and mean body weight of these children were higher and heavier than the averages in the Korean Physical Standards. About 39 per cent were above the 90 th percentile of the Korean Physical Standards of height and 35 per cent were above the 90th percentile of the Korean Physical Standards of weight. 2) The rate of infestation by helminthes was 67.6 per cent and there were no cases of hookworm infestation Among the ascaris-infected children, 61.6 per cent fell within an Ascaris E.P.G. count range of $501{\sim}2,000$. This was considered a rather mild intensify of ascaris infection. Nutrient loss due to helminthes was also estimated to be low. 3) The mean hemoglobin level was 11.3gm% $({\pm}0.94)$, and 68.4 per cent of the children were anemic (less than 11gm% among those 5 years old or less, less than l2gm% among those 6 years old or more). The average hematocrit value was 36.1% $({\pm}2.36)$, and only 3.5 per cent showed more than 34% on the M.C.H.C. test. These results showed that the incidence of anemia, in these rural children was high, and that in general it displayed hypochromic characteristics. 4) The average serum iron level was $78.2{\mu}g%$\;({\pm}29.8). The Proportion of the children showing less than $50{\mu}g% in the serum iron level was 5.6 per cent, while 41.8 percent revealed $450{\mu}g% or more in the TIBC determination. Transferrin saturation was below 15% in 71.4 percent of the preschool children, and in 28.6 percent of the school children. Those who had a serum iron level of $50{\mu}g% or less and a TIBC of more than $450{\mu}g% constituted 11.9 percent of the children examined. From these results, it was concluded that especially preschool children were low in iron nutrition, and that most of the anemia might be iron-deficiency anemia. 5) The mean values of total serum protein and albumin were 6.98gm% and 4.99gm% respectively. Especially in serum albumin, all children stowed more than 4 gm%. 6) The mean urinary urea nitrogen Per creatinine ratio was 11.5 $({\pm}4.2)$, and 21.8 percent had a ratio of more than 15. Considering these results, it was suggested that protein nutrition in these children was relatively good.

  • The Purpose of this study was to investigate physical conditions and related factors of 5th grade, in Korea. The number of students surveyed was 10.751 . School health records of 63 grade schools were used. The school health record of 5th grade contains results of physical examination from 1st grade to 5th grade and results of physical fitness assessment for 5th grade. A questionnaire was also administered to the student. 1) The proportion of mother who has a job was 29% of students surveyed. Since some of mothers were working at odd hour, it is hard to take care of student's meal and nutritional intake. The proportion of students who didn't eat breakfast everyday was 5.8% . Students who took medicine for health were 26% of total students surveyed. 2) Male students were taller than female students until 4 th grade. At 5 th grade, the mean height was 137.1cm for male students and 137.6cm for female students . At 5 th grade, the mean weight was 31.5kg for male students and 31.2kg for female students. The 10, 25, 50, 75.90 percentile of the total distribution was examined . By rising in grade, the height velocity was decreased for male student and increased for female students . The weight velocity was increased sharply for male and female students and the increasing velocity was larger in female than male students. Using heights and weights from 1st grade, BMI, RI, and RW were calculated. 3) The proportion of students who drank milk everyday was 49%, The \intake frequency of cookies, eggs, ice cream, chocolate were higher than the intake freguency of cheese, ham and soy bean milk

Biomechanical Effectiveness and Anthropometric Design Aspects of 3-dimensional Contoured Pillow

  • Objective: The aim of this study was to evaluate the effectiveness of 3 dimensional contoured pillow through analyzing contact pressure and to suggest its design guidelines through analyzing the posture of head and neck area. Background: The quality of sleep is a very important factor closely related with human's health. To improve the quality of sleep, the verification of design factors affecting the posture of the head and neck is needed, and a pillow design that can induce proper posture is required. Method: This study measured the contact pressures of the two reference groups (bead and cotton pillows) as a method to evaluate the design effectiveness of the contoured pillow. This study proposed 3-dimensional design guidelines by drawing anthropometry (head length) affecting cervical curvature angle (CCA) through the measurement of the participants' cervical curvature angles. Results: In the design effectiveness evaluation, the contact pressure of cervical region was higher than that of a reference group (cotton pillow), and contract area increased, and contract pressure decreased in the shape distributing the occipital region's body pressure. This study proposed pillow's cervical supporting height by percentile of a head length [head length (%tile) (cervical supporting height) affecting the posture of the head and neck: 14.6cm (5%tile) (6.2cm), 15.5cm (25%tile) (6.7cm), 16.4cm (50%tile) (7.1cm), and 19.3cm (75%tile) (8.9cm)]. Conclusion: This study confirmed the contoured pillow's design effectiveness maintaining cervical angle comfortable to sleep with the shape supporting the cervical vertebrae and by reducing the contact load of the occipital region. Also, this study proposed pillow design guidelines based on the 3-dimensional contoured pillow design effectiveness, through which the study laid the foundation for pillow design in a systematic method. Application: The results of this study are expected to be utilized as the basis data by which the optimum pillow type and pillow design according to main percentile can be standardized.

Effects on School Lunch Service Programme of Elementary School in Rural Area (농촌지역(農村地域) 국민학교(國民學校) 급식아동(給食兒童)과 성장발달(成長發達)과 식생활(食生活) 습관(習慣))

  • The purpose of this study is to know the effects on school lunch service programme of elementary school in rural area, by using the group consisting of the sixth year students in the schools that have provided them with the lunch for six years or longer(male student:312, & female student:324), while using the comparing group consisting of the sixth year students in the schools that have not provided them with the school lunch under their similar living condition(male student: 306 & female student:322). In addition, this study was carried out by examining all continued information about their height and weight shown in the developmetal record for six years from the 1st to 6th year, and by checking their eating habits on the basis of questionnaires. The result of this study is summarized as follows. As the result of comparing the values of their height and weight grown for 6 years, it was shown that the height of the male group provided with school lunch is 27.8 cm while the male group without lunch is 27.1 cm. And the female group provided with school lunch indicated the growing value of 29.9 cm while the group without lunch did 28.4 cm. Then, it appeared that both male and female groups provided with school lunch show higher growing values of 0.7 cm, respectively, and 1.5 cm than these groups without lunch. Also, the weight of the group without lunch was 14.8 kg. Moreover, the weight of the female group provided with school lunch was 16.9 kg while the group without lunch was 17.2 kg. Then, it was shown that the male group provided with school lunch indicates heavier growing value of 0.9 kg than the group without lunch while the female group without lunch does heavier value of 0.3 kg than the group provided with school lunch. It's figure showed that although this distribution according to percentile in the 1st year is similar to the standard regular curve it is positioned in the upper group(more thatn 70%) divided centering around 50% in the 6th year, of which distribution of children provided with school lunch was higher. When comparing the values of physical status in the 6th year, it was also shown that male children with school lunch are better than these children without lunch in jumping, throwing, chinning and lifting while female children are better than these children without lunch only in jumping, which were a significant difference. In addition, the group provided with lunch showed distribution of the higher physical grade. The result of analysis on their breakfast indicated that the children with every morning breakfast account for 67.6% of the group provided with school lunch while the group without lunch for 57.8%. Regarding the reason that they do not have the breakfast, the group with school lunch answered "Because of habits"(50.7%) while the group without lunch did "Because they have no appetite"(58.9%). When comparing the degree of preference for hot or salty food, it was apparent that these children with school lunch generally tend to prefer less hot or sailty food. With respect to the frequency and place of their eating between meals, it was shown that about 70.0% of both groups has the eating between meals, more than one time a day. Then, the group with school lunch had the eating between meals at home(45.2%) while the group without lunch did it in the process of returning to home(48.4%). Regarding the degree of their preference for a certain food, it was shown that more children of the group with school lunch do not prefer a food to others. Also, their eating attitude indicated that such children as eating the food with chat after completely swallowing food and with T.V watching are larger and lower among the group with school lunch, which showed a remarkable defference from the group without lunch. With respect to their sanitary habits such as hand washing and toothing, these children who always wash their hand before eating, accounted for 84.4 % of the group provided with school lunch while the group without lunch did for 63.6%, of which the female group with school lunch indicated a remarkable difference. The actual condition of their nutrition education showed that these children who answered "Received this education" accounted for 78.0% of the group with school lunch while the group without lunch accounted for 57.5%.

The characteristics of mild head injuries in preschool-age children fall: a retrospective observational study (학령전기 소아에서 추락으로 인한 경증 두부손상의 특징에 대한 후향적 관찰 연구)

  • Objective: Falling is a common cause of head injury in preschool aged children. We investigated the characteristics of mild head injuries caused by falling and the association between body weight and occurrence of traumatic brain injuries (TBI). Methods: This retrospective observational study was conducted on head-injured preschool-aged children that visited the emergency department from January 2012 to December 2015. Characteristics such as age, sex, weight, free fall height, floor type, and presence of TBI, as defined as cerebral hemorrhage or skull fracture, were investigated. We calculated body weight percentiles by calibrating age and weight and categorized them into four quartile ranges. We grouped all included cases into two groups according to the presence of TBI. The characteristics of the two groups were compared by using chi-square test, and the association with TBI was investigated by using binomial logistic regression. Results: A total of 701 children were included, and TBI was observed in 95 children. Children with TBI were younger. The proportion of children with TBI was higher in the third and fourth quartiles of the body weight group as well as according to soft floor and fall from high height (${\geq}1m$). The odds of soft floor being associated with TBI was higher than the odds for hard floor (odds ratio, 2.734; 95% confidence interval [CI], 1.597-4.680). The odds of high height (${\geq}1m$) being associated with TBI was higher than that for low height (odds ratio, 2.306; 95% CI, 1.155-4.601), and the odds ratio for the weight percentile group was 1.228 (95% CI, 1.005-1.499). Conclusion: Prevalence of TBI after falling in preschool-aged children might be associated with high fall-height and body weight quartiles.

A Comparative Study on BMD of Lumbar Spine and Proximal Femur in Post-Menopausal Women Using Dual Energy X-ray Absorptiometry (이중에너지 X선 흡수계측법을 이용한 폐경기 여성의 요추 및 근위 대퇴부의 골밀도 비교 연구)

  • Osteoporosis, which causes mainly fracture of the spine, proximal femur and distal radius by minimal trauma, is a major public health problem and its prevalence is steadily increasing in Korea according to the development of public health care. There are reliable methods for diagnosis based on bone densitometry. Early detection and intervention are important for reducing the incidence of fractures. A consensus definition of osteoporosis, based on bone density measurement, has been developed by the World Health Organization(WHO). In this study, bone mineral density(RMD) was measured by dual energy x-ray absorptiometry(DEXA) at the proximal femur and lumbar spine in 132 post-menopausal women. The purpose of this study is to find influential factors on the BMD of the proximal femur and the lumbar spine and to analyze correlation between BMD and the problematic factors. We obtained the following results : 1. Mean BMD score, T-score and Z-score of the proximal femur were $0.81(g/cm^2)$, -2.45(S.D.) and -2.09(S.D.) respectively and in the lumbar spine were $0.83(g/cm^2)$, -2.02(S.D.), -2.43(S.D.) respectively. 2. In correlation analysis between BMD and many factors, correlation coefficients were -0.467, 0.212, -0.321 and 0.241 in age, height, duration after menopause respectively. BMI and the residuals were comparatively small. 3. Correlation coefficients to age matched BMD, in height and body weight were 0.222 and 0.241, in age and duration after menopause were -0.268, -0.282. 4. The fracture threshold of proximal femur BMD to the 90th percentile was $0.845(g/cm^2)$. 5. At the result of multiple regression analysis, age, body weight, $BMI(kg/m^2)$ and duration after menopause described as significant variables.

Synoptic Characteristics of Cold Days over South Korea and Their Relationship with Large-Scale Climate Variability (한반도 혹한 발생시 종관장 특성과 대규모 기후 변동성 간의 연관성)

  • This study explores the synoptic characteristics of cold days over South Korea and their relationship with large-scale climate variability. The cold day, which is different from cold surge, is defined when daily-mean surface air temperature, averaged over 11 KMA stations, is colder than 1-percentile temperature in each year by considering its long-term trend over 1960~2012. Such event is detected by quantile regression and the related synoptic patterns are identified in reanalysis data. Composite geopotential height anomalies at 500 hPa show that cold days are often preceded by positive anomalies in high latitudes and negative anomalies in midlatitudes on the west of Korea. While the formers are quasi-stationary and quasi-barotropic, and often qualified as blocking highs, the latters are associated with transient cyclones. At cold days, the north-south dipole in geopotential height anomalies becomes west-east dipole in the lower troposphere as high-latitude anticyclone expands equatorward to the Northern China and mid-latitude cyclone moves eastward and rapidly develops over the East Sea. The resulting northerlies cause cold days in Korea. By performing composite analyses of large-scale climate indices, it is further found that the occurrence of these cold days are preferable when the Arctic Oscillation is in its negative phase and/or East Asian monsoon circulation and Siberian high are anomalously strong.

Growth and Development of Infants Fed Soy- Based Formulas over 3 Months (액상 대두 유아식을 3개월 이상 섭취한 영유아의 성장과 발달)

  • The objective of this study was conducted to investigate growth and development status of infants fed soy-based formulas over 3 months. The height and weight were measured and Z-scores were calculated by using standard of the same age groups. Their mothers were interviewed using questionnaires including general and environmental characteristics, total food intakes, soy based formula intakes and Ewha infant development screening test. Main results were as follows: 1) Nutrient intake levels of subjects were similar to or more than the level of Korean Recommended Dietary Allowances except for intakes of vitamin E (79.89% RDA), and the average status of nutrient intakes of infants were fairly good. 2) Z-scores of height for age (HAZ) and Z-scores of weight for age (WAZ), Kaup index, WLI and Ewha Infant Developmental Screening Test score of subjects were in the normal growth range. 3) There were no significant differences among soy based formula intake percentile groups in HAZ, WAZ, Kaup index, WLI and Ewha Infant Developmental Screening Test score. 4) Total energy intake was positively correlated with HAZ (p < 0.00, WAZ (p <0.00, and WLI (p < 0.05) in infants less than 12 month. Also, soy based formula energy intake was positively correlated with HAZ (p < 0.05) in infants less than 12 month. However, energy and soy based formula intake levels of infants over 12 month were not significant among variables. Considering results of this study, infants fed soy-based formulas over 3 months showed normal growth and development status. Further studies are needed to evaluate longterm growth and development in infants fed soy based formulas.

The Effect of NutriPlus Program among 1-5 Year Children in Daejeon Area - The Improvement in Nutritional Status of Children and Nutrition Knowledge and Attitude of Parents - (대전지역 유아대상 영양플러스 사업 효과 - 유아 영양개선과 보호자 영양지식 및 태도에 미친 영향 -)

  • This study investigated the effect of nutritional improvement of 1-5 year children participated in NutriPlus program. The program was carried out at Public health Center, Daedeok-gu, Daejeon from March 2009 to April 2011. The subjects were selected among applicants for low-income family financing of the government and included 90 children and their parents. they were divided into 2 groups (less than 6 months and more than 6 months of participation in the study). We analyzed the effects of NutriPlus program and nutrition education in both study groups. The results of this study were summarized as follows: The number of children with less than 10th percentile in height and weight and weight for height was decreased. The anemia prevalence rate was reduced from 64.4% to 8.9%. NAR value of subjects increased for energy, protein, calcium, iron, vitamin A, riboflavin, niacin and vitamin C. The education program improved knowledge and attitude of the parents. In nutrition knowledge according to the income level, there were greater changes in the group of less than 100% compared to the subsistence. In nutrition attitude according to the education level, there were greater changes in the group of middle school graduates than in high school graduates or college graduates. There were no difference between less than 6 months group and more than 6 months group in improvement of hemoglobin level, anthropometric data and nutrients intakes. Based on these results, we conclude that providing supplementary food to children and nutrition education brought positive effects on growth of the children.

Long-term Outcomes of Augmentation Cystoplasty in a Pediatric Population With Refractory Bladder Dysfunction: A 12-Year Follow-up Experience at Single Center

  • Purpose: Augmentation cystoplasty (AC) is a surgical procedure used in adults and children with refractory bladder dysfunction, including a small bladder capacity and inadequate bladder compliance, and in whom conservative and medical treatment has failed. This study was aimed to determine the long-term outcomes of AC in children. Methods: A retrospective analysis was conducted of 42 patients (31 males; mean age, $14.2{\pm}6.2years$) who underwent AC for neurogenic and nonneurogenic bladder dysfunction, with a median $12.0{\pm}1.5years$ of follow-up. All patients underwent AC using the ileum with or without continent reconstruction. Pre-AC, concurrent, and post-AC procedures and complications were analyzed. Patients who underwent ureterocystoplasty, were lost to follow-up, or had less than 10 years of follow-up were excluded. The primary outcomes were the complication and continence rates, the post-AC linear rate of height and weight gain, and renal function. The Student t-test was used to evaluate between-group differences and the paired t-test was used to evaluate longitudinal changes in measured variables. Results: Renal function was stable or improved in 32 of 42 patients (76.2%), with a post-AC continence rate of 88.1%. Thirty patients (71.4%) required 72 procedures post-AC. There was no statistically significant difference in the mean percentile of height (P=0. 212) or weight (P=0.142) of patients in the pre- and post-AC periods. No cases of bladder perforation or malignancy were detected. Conclusions: We consider AC to be a safe and effective procedure that does not negatively affect future physical growth, while achieving a good rate of stable renal function. Patients need long-term follow-up to address long-term complications.
